Morphological and functional skin changes in postthrombotic syndrome.

After a period of deep venous thrombosis a lot of patients will develop chronic venous insufficiency. In time different skin alteration will occur. Several hypothesis have been proposed to explain the pathogenesis behind these skin changes. In this paper we will briefly discuss the most important theories. We concentrate on microcirculation and the different skin changes.

Clinical study of late-stage postthrombotic lower extremities.

Through clinical and venographic study of 353 diseased limbs, the authors advocate a new classification for the postthrombotic syndrome of the deep veins of the lower extremity. The postthrombotic lower extremities may be divided into two types: (1) lesions of the whole lower extremity, consisting of the completely obstructed, partially recanalized, and completely recanalized types; (2) local lesions, such as segmental obstruction of the iliac, iliofemoral, superficial femoral, femoropopliteal veins, and the tibioperoneal venous trunk and the venous plexus in calf muscles. Each type has its ch

Occult antithrombin III deficiency: a potentially lethal complication of the postphlebitic limb.

Chronic venous insufficiency is a frequent sequel to lower extremity venous thrombosis. A relatively uncommon, but potentially lethal, cause of the thrombosis is congenital antithrombin III deficiency. Recognition and treatment of this occult deficiency is critical. The following report describes a family treated by the authors for this problem. In one generation of nine siblings, three males had documentation of the disease with functional antithrombin III levels in the range of 50% to 60%. Before evaluation for the deficiency one female sibling died at the age of 20 years as a consequence of

Performance and Head-to-Head Comparison of Three Clinical Models to Predict Occurrence of Postthrombotic Syndrome: A Validation Study.

OBJECTIVE: The SOX-PTS, Amin, and Méan models are three different clinical prediction scores stratifying the risk for postthrombotic syndrome (PTS) development in patients with acute deep vein thrombosis (DVT) of the lower limbs. Herein, we aimed to assess and compare these scores in the same cohort of patients. METHODS: We retrospectively applied the three scores in a cohort of 181 patients (196 limbs) who participated in the SAVER pilot trial for an acute DVT. Patients were stratified into PTS risk groups using positivity thresholds for high-risk patients as proposed in the derivation studi

Residual Venous Obstruction as an Indicator of Clinical Outcomes following Deep Vein Thrombosis: A Management Study.

BACKGROUND: Residual venous obstruction (RVO) is considered a risk factor of recurrence and possibly other clinical outcomes following deep vein thrombosis (DVT). Current guidelines do not support an RVO-tailored duration of anticoagulant therapy; contemporary data of such management strategies are scarce. We aimed to evaluate an RVO-based management strategy and to assess associations of RVO with recurrence, post-thrombotic syndrome (PTS), arterial events and cancer. To gain further insight, D-dimer levels were measured 1 month after stopping anticoagulant therapy. METHODS: Consecutive patie

[The trophic venous ulcer. The physiopathological, microbiological and pharmacological aspects].

The ulcus cruris is an ulcerative lesions caused by chronic venous stasis and chronic venous hypertension. Every pathologic obstacle to venous return can cause ulcus cruris. Ulcus could be brought back to 3 situations: postphlebitic syndrome, chronic venous insufficiency and primitive varicosities. The common denominator is always the increase in venous pressure, the appearance of reflux and venous stasis. Venous ulcers are possibly characterized by tissue degeneration and by bacterial superinfection. A particular bacterial flora is found in these injuries, it is qualitatively different from t

Thrombolytic agents: balancing cost, efficacy, and side effects.

Balancing the benefits, risks, and cost of thrombolytic treatment is a complex issue which depends considerably upon the variable threat of the thrombus to organ physiology and patient survival. For example, after deep vein thrombosis (DVT), the major risk is long-term disability due to the postphlebitic syndrome, while in pulmonary embolism (PE) patients, the risks concern short-term mortality and impaired pulmonary physiology. Thus, for treating DVT or PE, the question is whether thrombolytic therapy would be valuable in addition to other antithrombotic approaches. Clearly, the best indicati

Developing and optimizing a machine learning predictive model for post-thrombotic syndrome in a longitudinal cohort of patients with proximal deep venous thrombosis.

BACKGROUND: Post-thrombotic syndrome (PTS) is the most common chronic complication of deep venous thrombosis (DVT). Risk measurement and stratification of PTS are crucial for patients with DVT. This study aimed to develop predictive models of PTS using machine learning for patients with proximal DVT. METHODS: Herein, hospital inpatients from a DVT registry electronic health record database were randomly divided into a derivation and a validation set, and four predictive models were constructed using logistic regression, simple decision tree, eXtreme Gradient Boosting (XGBoost), and random for

Effect of physical activity after recent deep venous thrombosis: a cohort study.

PURPOSE: To determine whether increased physical activity 1 month after deep vein thrombosis (DVT) led to worsening of venous symptoms and signs within the subsequent 3 months. METHODS: By a multicenter prospective cohort study of patients with acute DVT, we used validated questionnaires at baseline, 1 month, and 4 months post-DVT for each exposure, using the Godin Questionnaire to measure physical activity, the VEINES-QOL to measure disease severity, and the postthrombotic syndrome (PTS) scale to measure symptoms and signs usually attributed to sequelae of DVT. RESULTS: Of 301 patients foll

[The value of measuring reflux by color duplex in post-thrombotic syndrome].

Until now, the diagnosis of the postthrombotic syndrome is based upon venography. This study compares the diagnostic value of the CW-Doppler with the sonographic signs in the deep venous system observed in B-mode sonography and with the quantitative measurement of venous reflux by color Doppler sonography. 84 venous sections of 21 patients with phlebographically diagnosed PTS and substantial clinical signs were examined. CW- Doppler and B-mode sonography did not show significant differences compared to phlebographic findings. Color doppler sonography, however, showed signs which significantly

[Relation between changes in the microcirculation in the capillaries supplying the toenails and the degree of chronic venous insufficiency].

Chronic venous insufficiency (CVI) is characterised by stasis which may lead to an imbalance in the cutaneous microcirculation resulting in a wide spectrum of complications. The use of capillaroscopy on the toenail folds of patients with CVI allows the morphological and microhemorrheological aspects of microcirculatory disorders to be studied. The aim of this study was to clarify the correlation between the extent of abnormalities of the nutritional capillary and the degree of severity of CVI according to Widmer's classification. Capillaroscopy of the toenail fold was used to study 100 patient

[Practical proposals for overcoming problems with phlebography].

Venography is considered the gold standard in the diagnosis of venous disease. In the everyday practice, however, it is not used according to this attribute. The article scrutinizes the reasons for the discrepancy, emphasizes the possibilities wrongly renounced to and gives advice to a better indication for and execution of venography. Arguments not to use venography regularly to diagnose or exclude deep venous thrombosis, include claims that it would be superfluous, not readily available, invasive and expensive. Such considerations do not take into account that the clinical diagnosis it not b

[Malleolar and plantar light reflection rheography].

It is reported about the plantar LRR. This modification of the malleolar LRR distinguishes venous healthy from venous patients like the malleolar procedure. By the plantar procedure LRR becomes possible in cases where malleolar LRR is not performable like in larger leg ulcers and lymphedemas. The evaluation of plantar LRR has to regard changed normal values i.e. a prolonged refilling time and an increased delta R.

Clinical and noninvasive assessment of venous disease as related to pulmonary embolism.

Proper management of patients with venous disease requires recognition of the various syndromes and an accurate objective diagnosis. Unfortunately many physicians are unfamiliar with the different venous disorders and are unaware of the fallibility of the clinical diagnosis of these syndromes. This article reviews the six common venous conditions that collectively are more common than coronary or peripheral arterial disease. Acute and recurrent deep vein thrombosis, postthrombotic syndrome, superficial thrombophlebitis, varicose veins, and pulmonary embolism may all confront the physician rega

Are temporary inferior vena cava filters really temporary?

BACKGROUND: Despite significant risk for venous thromboembolism, severely injured trauma patients often are not candidates for prophylaxis or treatment with anticoagulation. Long-term inferior vena cava (IVC) filters are associated with increased risk of postphlebitic syndrome. Retrievable IVC filters potentially offer a better solution, but only if the filter is removed; our hypothesis is that the most of them are not. METHODS: This retrospective study queried a level I trauma registry for IVC filter insertion from September 1997 through June 2004. RESULTS: One IVC filter was placed before

Pulmonary embolism secondary to venous thrombosis of the arm.

Pulmonary embolism is described as an infrequent complication of axillary and subclavian vein thrombosis. We have reported our recent clinical experience with 14 patients admitted to the Harbor-UCLA Medical Center who had a clinical diagnosis of axillary and subclavian vein thrombosis documented by phlebography of the thrombosed arm. The causes of thrombosis were effort (three patients), trauma (three patients), drug abuse (four patients), underlying neoplastic disease (three patients), and congenital venous malformation (one patient). Pulmonary emboli were diagnosed by arteriogram, ventilatio
