The immunoreactive antigens in heat-extracted (autoclaved) preparations of an arthritogenic strain of Erysipelothrix rhusiopathiae (isolate VRS 229, serotype 1a) have been identified by gel diffusion precipitin (GDP) tests and a novel application of the enzyme linked immunosorbent assay (ELISA) procedure. The agglutination of avian red cells, particularly pigeon and budgerigar cells, by sera from patients who had been exposed to birds and had respiratory symptoms, as well as from healthy bird fanciers, has been used to detect the presence of avian specific antibody. Precipitating antibodies to avian antigens play a controversial role in the diagnosis of bird breeder's disease (BBD). In order to establish the sensitivity, specificity and accuracy of precipitins in our laboratory, we conducted a prospective study including 128 sera received in 1988 for determination, by immunoelectrophoresis, of avian precipitins. Accurate information was obtained for 90 patients; definitive clinical diagnosis was given by the patient's own physician. Specific sensitization and respiratory effects associated with the inhalation of sugar cane dust were evaluated in a group of 51 Nicaraguan workers exposed to bagasse. A specific immunodiffusion test for blastomycosis has been developed. The test permitted the detection of approximately 80% of 113 proven cases of blastomycosis. Two diagnostically important precipitins designated A and B were frequently recognized in patients with blastomycosis. Routine use of reference sera containing the A and B precipitins in immunodiffusion tests would permit the specific diagnosis of blastomycosis without the need for parallel tests with coccidioidin and histoplasmin.
