Impact of antihypertensive treatment on resiliency to clinical pain.

OBJECTIVES: Although progress has been made in the regulation of hypertension over the past decades, the USA and some other countries have faced a significant rise in incidence of chronic pain management cases during the same period. Studies of the relationship between pain and blood pressure (BP) regulations propose that these two processes may be interconnected. Studies of effects of antihypertensive treatment on pain in general and its chronification have never been reviewed. METHODS: A narrative review of respective studies with analysis of credibility of the findings was conducted. RESU

Baroreflex Amplification and Carotid Body Modulation for the Treatment of Resistant Hypertension.

PURPOSE OF REVIEW: Patients with true resistant hypertension (RH) are characterized by having high sympathetic activity and therefore potentially benefit from treatments such as baroreflex amplification (baroreflex activation therapy (BAT) or endovascular baroreflex amplification therapy (EVBA)) or carotid body (CB) modulation. This review aims at providing an up-to-date overview of the available evidence regarding these two therapies. RECENT FINDINGS: In recent years, increasing evidence has confirmed the potential of baroreflex amplification, either electrically (Barostim neo) or mechanical

A basic posture control mechanism: the stabilization of the centre of gravity.

Holding the body's centre of gravity steady represents the crucial variable for the stabilization of posture in upright stance in man. The visual, vestibular and muscle proprioceptive systems have all been shown to contribute to sway stabilization. Nevertheless, earlier work has suggested that an additional receptor system is needed to signal the position of the body's centre of gravity relative to the support surface, i.e., the feet. This proposed receptor system should be 'gravity' dependent. To evaluate the properties of this receptor system, an experiment was designed under simulated 'micr

Contributions of the sympathetic nervous system, vasopressin and baroreceptor function in brain angiotensin II excess-induced hypertension in the dog.

To determine the principal effects of brain angiotensin(Ang) II on the sympathetic nervous system, vasopressin (AVP), and the high and low pressure baroreceptor systems, we observed the hemodynamic and neurohumoral characteristics induced by the acute(1-hr) and chronic(1-wk) infusion of Ang II into the brain ventricle in conscious dogs, and then evaluated the hemodynamic responses to sole-innervated carotid artery occlusion(COR) after Ang II infusion and again after vagotomy in anesthetized dogs. Both acute(50ng/kg/min) and chronic(15ng/kg/min) infusion of Ang II caused a significant rise in a

Electroconvulsive shock activates endogenous opioid systems: behavioral and biochemical correlates.

From the evidence reviewed above, there is little doubt that ECS activates endogenous opioids and modifies their receptors. Thus, this form of SIA is accompanied by many other corollaries of opioid-like actions, including catalepsy, similar EEG patterns, common autonomic effects, and increases in opioid receptor binding sites. Investigations have further indicated that the amnestic effects of ECS can also be attenuated by naloxone, and that pituitary-derived opioids may play an important role as a predominant source of opioids that contribute to these opioid-like effects following ECS. It is h

Effect of cardiovascular variables on hyperpnea during recovery from diving in ducks.

Control of hyperpnea during recovery from diving in unanesthetized White Pekin ducks, Anas platyrhynchos, was examined. Postdive minute ventilation (VE) increased five times regardless of the length of the preceding dive (1-4 min), although longer dives resulted in slower return of VE towards predive levels. Manipulation of arterial blood gases showed that both hypoxia and hypercapnia contributed to hyperpnea on emergence. Although chronic bilateral carotid body denervation depressed VE before and after diving, VE still increased four times after 1-min dives. Postdive hyperpnea was accompanied

Denervation of arterial chemoreceptors and baroreceptors in fetal lambs in utero.

Arterial baroreceptors and chemoreceptors were denervated in eight fetal lambs in utero. Carotid sinus and carotid body denervation was accomplished by stripping the carotid artery rostral and caudal to the origin of the occipital and lingual arteries. Aortic bodies were denervated by section of the superior laryngeal nerve and aortic nerve in the neck. The anatomy relevant to the technique is presented. The effectiveness of the denervation was tested by injection of 20 micrograms phenylephrine. The slowing of the fetal heart rate normally present in the intact lambs was abolished in the dener

The effect of opiates on arterial baroreceptor reflex function in the rabbit.

The contribution of exogenous and endogenous opioid peptides to the central modulation of the baroreceptor reflex was investigated in rabbits. Baroreceptor sensitivity was assessed in pentobarbitone anaesthetised animals by measuring heart period in response to rises in arterial pressure after bolus intravenous injections of phenylephrine and falls induced by intravenous sodium nitroprusside and controlled haemorrhage. The slope of the linear relationship between arterial pressure and heart period was used as an index of baroreflex sensitivity. Thirty minutes after the intracisternal administr

Prolonged hypotension following elective stenting of an internal carotid artery stenosis.

We report a case of a 72-year-old man who had undergone elective stenting of an asymptomatic high grade stenosis of the proximal left internal carotid artery. The indication was progression of the stenosis over a 3 year period despite receiving maximal medical therapy, and patient preference for stenting over endarterectomy. He became profoundly hypotensive in the immediate post-stent period and required immediate and prolonged vasopressor support over the next week. The etiological hypothesis is baroreceptor hyperactivity related to procedure induced unilateral stretching of the carotid body.

Psychophysiology of arterial baroreceptors and the etiology of hypertension.

Arterial baroreceptors are sensitive to blood pressure dependent blood vessel dilation. They play a key role in the short term regulation of blood pressure. Their impact on psychological and psychophysiological aspects is of increasing interest. The review focuses on experimental techniques for the controlled baroreceptor manipulation. Results from the application of these techniques show that baroreceptor activation influences the cardiovascular system as well as central nervous functioning: Behavioral and electrophysiological measures of arousal, low level reflexes and pain responses are mod

[Variability of blood pressure and baroreceptor function; clinical and scientific relevance].

The human arterial blood pressure shows not only exogenic alterations, but also spontaneous fluctuations. This blood pressure variability has both scientific and clinical relevance. A well-known phenomenon is the so-called "white coat hypertension" which may severely affect diagnosis and therapy of essential hypertension. Furthermore, several studies have provided evidence that the degree of end-organ damage in hypertensive patients is closely related to the circadian blood pressure variability. Therefore, it is tempting to suggest that an optimal antihypertensive treatment should take an impr

Vascular corrosion replicas of chemo-baroreceptors in fish: the carotid labyrinth in Ictaluridae and Clariidae.

Scanning electron microscopy of vascular corrosion replicas and light microscopy revealed a pair of highly vascularized tissues, the carotid labyrinths, in the dorsal head region of the channel catfish, Ictalurus punctatus, the black bullhead, I. melas, and the walking catfish, Clarias batrachus. The labyrinth consists of a myriad of arterioles that arise from the common carotid artery immediately distal to the origin of the common carotid from the efferent branchial (epibranchial) artery of the first gill arch. The arterioles anastomose with each other to form: (1) the internal carotid artery

Action of the morphinometic agent, fentanyl, on the nucleas tractus solitarii and the nucleus ambiguus cardiovascular neurons.

Using extracellular recordings, the discharges of blood pressure dependent neurons were detected in the nucleus tractus solitarii (NTS) and in the nucleus ambiguus (NA) of anesthetized dogs. Type A blood pressure dependent neurons (sympathoinhibitory or vagoexcitatory neurons) were detected when their firing rates were increased during i.v. noradrenaline and when their firing rates were reduced or abolished during bilateral carotid occlusion. Type B blood pressure dependent neurons (sympathoexcitatory neurons or vagoinhibitory neurons) were identified when their firing rates were reduced or wh

Haemodynamic and baroreceptor responses to beta-adrenoceptor blocking agents in rabbits with cardiopulmonary bypass.

Blood pressure and peripheral resistance were reduced, baroreceptor activity was enhanced after i.v. infusion of beta-adrenoceptor blocking agents in rabbits with intact circulation or with total cardiopulmonary bypass. The latter preparation allowed cardiac effects of the drugs to be excluded. In rabbits with an intact circulation, post-ganglionic sympathetic activity was reduced. These results suggest a direct vascular action of the drugs. The increased baroreceptor firing is not a cardiac-mediated effect and may partly induce the observed reduction in sympathetic activity.
