The Role of Pharmacy Benefit Managers in Access to Biologics for Dermatologic Conditions.

In this commentary, we discuss the role of pharmacy benefit managers (PBMs) on access to biologics for patients with psoriasis. We highlight structural and system level barriers to biologics access, as well as how PBMs work as intermediaries between insurers, pharmacies, and drug manufactures to influence prescription formularies and generate health savings. We also discuss how controversial PBM practices such as step therapy, prior authorizations, and spread pricing may limit access to biologics and potentially increase cost for patients. Finally, we highlight how dermatologists and national

Financial constraints on genetic counseling and further risk-management decisions among U.S. women at elevated breast cancer risk.

Clinical guidelines recommend that women at high risk of breast cancer should consider various risk-management options, which remain widely underutilized. We conducted semi-structured, qualitative interviews with 50 high-risk women to understand how financial constraints affect use of genetic counseling, genetic testing, and further risk-management decisions. Inductive analyses revealed three categories of health-related financial constraint: (a) lack of insurance, (b) underinsurance, and (c) other financial constraints (e.g., medical debt, raising children, managing comorbidities). Various br

Cost-Effectiveness of Pre-exposure Prophylaxis Among Men Who Have Sex With Men in China: A Systematic Review.

OBJECTIVE: Men who have sex with men in China meet the definition of the population at "substantial risk" of contracting human immunodeficiency virus (HIV) according to the World Health Organization; therefore, initiating pre-exposure prophylaxis (PrEP) is recommended for this population. Lack of convincing evidence on cost-effectiveness has resulted in the lack of large-scale PrEP implementation at a national level. The objective of this review is to assess the cost-effectiveness of pre-exposure prophylaxis implementation among men who have sex with men in China. METHODS: The following datab

What should infectious diseases clinicians know about pharmacy benefit managers and their impact on our patients?

Pharmacy Benefit Managers (PBMs) are contracted by health plans, employers, and government programs to manage pharmacy and prescription drug benefits. They maintain drug formularies, process and pay pharmacy claims, establish and manage pharmacy networks, and negotiate prices with manufacturers and pharmacies. Despite PBMs' role in cost management, their lack of transparency and complex administrative processes can delay treatment and increase out-of-pocket costs, affecting vulnerable populations like those in pharmacy deserts. These factors influence healthcare delivery for persons living wit

Cough Syrup Tragedies and the Commercial Determinants of Health.

Child deaths related to consumption of adulterated cough syrups in India and other low- and middle-income countries highlight an ongoing public health crisis. Recent tragedies linked to diethylene glycol adulteration of cough syrups expose weaknesses in drug regulation, supply chain oversight, and market transparency, and need to be viewed through the lens of commercial determinants of health (CDoH)-the conditions, actions, and omissions by commercial actors that affect health. These incidents reflect intersections of high demand for cough syrups, irrational fixed-dose combinations, widespread

Development and Use of Gene Therapy Orphan Drugs-Ethical Needs for a Broader Cooperation Between the Pharmaceutical Industry and Society.

Gene therapy orphan medicinal products constitute a unique group of new drugs which in case of hereditary diseases are usually administered only once at an early age, in the hope to provide sufficient gene product to last for the entire life of the patients. The combination of an exceptionally large single payment and the life-long clinical follow-up needed for understanding the long-term benefits and safety of gene therapy, represent new types of scientific, financial, social and ethical challenges for the pharmaceutical industry, regulators and society. With special consideration of the uniq

[The incorporation of nusinersen by the Brazilian Unified National Health System: critical thoughts on the institutionalization of health technology assessment in Brazil].

In April 2019, a ruling was signed for the incorporation of the drug nusinersen by the Brazilian Unified National Health System (SUS). Nusinersen is the most expensive drug ever incorporated by the SUS and is used to treat type I 5q spinal muscular atrophy. The incorporation has been described as a milestone in decision-making on new technologies in the SUS, enabled through a risk-sharing agreement. The article discusses the process involved in the incorporation of nusinersen, highlighting the context, timing, and technical issues, in addition to possible consequences for the institutionalizat

Advanced clinical medicine requires advanced clinical ethics.

Many advances have occurred in clinical medicine in the last decades. Solid organ transplants, corrective surgery for congenital malformations, improved cytostatic regimes for children with cancer, and respiratory care for premature infants are but a few examples of the changing face of medical practice. Such changes have added years to life. But along the way many patients have paid a price, both in terms of loss of life and of added suffering. Even today, some survivors are faced with a life of impairment and suffering. Follow-up studies of extremely low-birth-weight infants show that the sm

Human rights and access to hepatitis C treatment for people who inject drugs.

BACKGROUND: People who inject drugs (PWID) achieve adherence to and outcomes from hepatitis C virus (HCV) treatment comparable to other patients. Nonetheless, this population has been excluded from treatment by regulation or practice. Approval of safer and more effective oral HCV medicines should offer greater treatment options for PWID, although high medicine prices have led to continued treatment rationing and exclusion in developed countries. In middle-income countries (MICS), treatment is largely unavailable and unaffordable for most PWID. METHODS: Human rights analysis, with its emphasis

Putting our best foot forward: Clinical, treatment-based and ethical considerations of nusinersen therapy in Canada for spinal muscular atrophy.

Spinal muscular atrophy (SMA) is the most common genetic cause of infant mortality. SMA is a spectral disorder and is categorised based on symptom onset and severity. The median life expectancy for infants with SMA presenting before 6 months of age is less than 2 years without respiratory support. To date, there is no cure for SMA. In June 2017, nusinersen was approved in Canada as the first disease-modifying drug for SMA because of its demonstrated benefits on motor function and survival in clinical trials. However, with a price tag of almost 1 million dollars for the first year of therapy, c

Corruption risks in COVID-19 vaccine deployment: lessons learned for future pandemic preparedness.

BACKGROUND: During the COVID-19 pandemic corruption risks were amplified in health systems globally, increasing health inequities within and between countries. During the pandemic, the deployment of COVID-19 vaccines, particularly concerning their procurement and distribution, had corruption risks given the large amounts of public and private funding allocated to them, the need for speed, the involvement of a high number of stakeholders, and often insufficient oversight. To explore this issue further, we conducted a descriptive, qualitative study of corruption risks in the COVID-19 vaccine dep

The Genomic Equity Accessibility Framework (GEAF): A dimensionally consistent framework for modeling affordability and equity in genomic and regenerative therapies.

BACKGROUND: Advances in CRISPR-based gene editing and stem cell-derived embryo models have created substantial biomedical opportunity alongside significant ethical and distributive concerns. Personalized genomic and regenerative therapies approved in the United States as of this writing carry list prices spanning approximately $2.2 million (Casgevy) to $4.25 million (Lenmeldy), with several other approved products priced between these figures (e.g., Lyfgenia, $3.1 million; Hemgenix, $3.5 million). This raises the question of how affordability, accessibility, ethical compliance, and distributio

Developing a medical-device formulary for council of health insurance beneficiaries in Saudi Arabia: stakeholder challenges and policy recommendations.

Ensuring equitable access to dispensed-to-patient (DTP) medical devices is an important policy priority for health insurance systems. This mixed-method study examined stakeholder perspectives on barriers and policy priorities for developing a DTP medical-device formulary for health insurance beneficiaries in Saudi Arabia. An exploratory online survey was distributed to 58 purposively selected stakeholders, and 17 responded (29.3%). Survey findings were summarized descriptively as n/N (%) with 95% confidence intervals. A subsequent stakeholder consultation workshop with 42 stakeholders was used

A single-source precursor route to anisotropic halogen-doped zinc oxide particles as a promising candidate for new transparent conducting oxide materials.

Numerous applications in optoelectronics require electrically conducting materials with high optical transparency over the entire visible light range. A solid solution of indium oxide and substantial amounts of tin oxide for electronic doping (ITO) is currently the most prominent example for the class of so-called TCOs (transparent conducting oxides). Due to the limited, natural occurrence of indium and its steadily increasing price, it is highly desired to identify materials alternatives containing highly abundant chemical elements. The doping of other metal oxides (e.g., zinc oxide, ZnO) is

Case studies of health economic analyses informing pharmaceutical health technology assessments for essential medicine selection and public-sector guidelines in South Africa.

BACKGROUND: Constrained resources under universal health coverage (UHC) necessitate a balance between medication costs and essential health system requirements. Policymakers practice priority-setting, as either implicit or explicit rationing, embedded in evidence-informed decision-making processes to guide funding decisions. Health technology assessment (HTA) is a method that may assist explicit evidence-informed priority setting. South Africa developed an official HTA methods guide in 2022, however before this, commissioning and performing economic evaluations was not standardized. METHODS:

Future of supplementary private hospitalization insurance in Switzerland: a policy brief.

BACKGROUND: Voluntary hospitalization insurance (VHI) in Switzerland has lost part of its traditional role as mandatory health insurance has expanded, hospital financing reforms have increased public co-financing of inpatient care, and patterns of hospitalization have changed. Nevertheless, VHI remains strategically important for hospitals, insurers, and some providers. ANALYSIS: This policy brief draws on a structured review of academic and grey literature, regulatory and policy documents, market information, and ten semi-structured interviews with key stakeholders. The analysis identifies f

Perceptions of diabetes patients and their caregivers regarding access to medicine in a severely constrained health system: A qualitative study in Harare, Zimbabwe.

Nearly half of all sub-Saharan African countries lack operational Diabetes Mellitus policies. This represents an opportunity to build reliable evidence to underpin such policies when they are eventually developed. Representing the interests of those with the experience of living with the condition in national diabetes policies is important, particularly the interests regarding medicine access, a key pillar in diabetes management. One way to achieve this representation is to publish patient perceptions. Patient perspectives are especially valuable in the context of diabetes in Sub-Saharan Afric

Why guideline-directed medical therapy depends on more than guidelines: microeconomics of medication use in heart failure.

Despite substantial mortality benefits, fewer than 25% of individuals with heart failure with reduced ejection fraction (HFrEF) receive all four pillars of guideline-directed medical therapy (GDMT), with underuse driven in part by economic and policy factors. Three clinician- and policy-relevant considerations may improve GDMT optimization. First, reduce patient cost sharing for GDMT at the public policy level. Evidence from national surveys demonstrates medication cost is a significant driver of nonadherence, particularly among economically vulnerable populations. Economic analyses have repea
