[Unexplained cyanosis in two newborns: neonatal methemoglobinemia after maternal perineal infiltration of prilocaine].

BACKGROUND: Methemoglobinemia is a rare cause of neonatal cyanosis in the newborn. It is considered a medical emergency. Failure of recognition or appropriated treatment could result in serious disease and neonatal death. Neonatal methemoglobinemia can be caused by both hereditary and acquired factors. CASE DESCRIPTION: We present two cases of newborns who developed severe cyanosis a few hours after birth due to methemoglobinemia. This was thought to be related to the local maternal perineal infiltration of prilocaine during childbirth. Though rare, prilocaine is the most potent agent to indu

Tolerability of 2.5% Lidocaine/Prilocaine Hydrogel in Children Undergoing Cryotherapy for Molluscum Contagiosum.

The tolerability of a 2.5% lidocaine/prilocaine hydrogel (Nanorap, Biolab Indústria Farmacêutica Ltd., Sao Paulo, Brazil) was evaluated in 20 children ages 2 to 11 years undergoing cryotherapy for molluscum contagiosum (MC). The product was well tolerated, with only two children presenting with eczema at the application site. These adverse reactions were considered unlikely to be related to the test product, because a patch test was negative in one of the individuals and the other event occurred in only one of the two treated areas. Nanorap is an efficacious and well-tolerated option for topic

[Acute cyanosis after transcatheter balloon valvuloplasty: toxic methemoglobinemia due to local prilocaine use].

Methemoglobinemia is characterized by varying degrees of cyanosis due to increased hemoglobin concentrations containing oxidized iron. Prilocaine is a widely used local anesthetic and can cause methemoglobinemia in infants even in therapeutic doses. We present two female infants (younger than 2 months) who developed severe cyanosis after transcatheter pulmonary balloon valvuloplasty and were diagnosed with toxic methemoglobinemia. Both infants were anesthetized with local prilocaine application before balloon valvuloplasty. Methemoglobin levels of the patients were measured as 49.6% and 37.7%,

Petechial and purpuric eruption induced by lidocaine/prilocaine cream: a rare side effect.

Lidocaine/prilocaine cream is a topical anesthetic commonly used in pediatric and dermatologic practice to obtain local anesthesia. Common side effects include: transient skin blanching, erythema, urticaria, allergic contact dermatitis, irritant contact dermatitis, and hyperpigmentation. The authors report a petechial and purpuric reaction after the application of lidocaine/prilocaine cream. This is a rare side effect, since to our knowledge only few case reports have been documented in literature.

Dermatoscope for the diagnosis of erythema with purpura induced by lidocaine/prilocaine cream: A case report.

BACKGROUND: Lidocaine/prilocaine (EMLA) cream is a local anesthetic that is applied to the skin or mucosa during painful therapeutic procedures with few reported side effects. CASE SUMMARY: Here, we report the use of dermatoscopy to identify a case of erythema with purpura, a rare side effect, after the application of 5% EMLA cream. CONCLUSION: We conclude that erythema with purpura is caused by irritation and toxicity associated with EMLA, but the specific mechanism by which the toxic substance affects skin blood vessels is unclear. In response to this situation and for cosmetic needs, we r

Improving the therapeutic efficacy of prilocaine by PLGA microparticles: Preparation, characterization and in vivo evaluation.

A delivery system based on poly(lactic-co-glycolic acid) polymer (PLGA) microparticles has been developed for parenteral administration of the local anesthetic prilocaine in its free base form. Both drug-free and drug-loaded microparticles, prepared by a double-emulsion-evaporation method, were characterized for mean size by Laser Diffraction Analysis, while their morphology was investigated by scanning electron microscopy. The preparation technique allowed obtainment of homogeneous microparticles of about 25 µm diameter, suitable for subcutaneous administration. The encapsulation efficiency,

A systematic review of lidocaine-prilocaine cream (EMLA) in the treatment of acute pain in neonates.

OBJECTIVE: Neonates routinely undergo painful cutaneous procedures as part of their medical treatment. Lidocaine-prilocaine 5% cream (EMLA) is a topical anesthetic that may be useful for diminishing the pain from these procedures. EMLA is routinely used in children and adults. There is substantial apprehension about its use in neonates because of concerns that it may cause methemoglobinemia. The objective of this review was to determine the efficacy and safety of EMLA as an analgesic for procedural pain treatment in neonates and provide evidence-based recommendations for clinical practice. [Small skin surgery in children: skin analgesia with a cream made of lidocaine and prilocaine].

The problems arising daily in the management of many skin diseases as the removal of warts, molluscum contagiosum, etc. are often complicated simply by the fear of the pain of some treatment modalities (i.e., cryosurgery, curettage, etc.). A choice between no analgesia and analgesia by the classic percutaneous infiltration with local anaesthetics is sometimes difficult. A new lidocaine/prilocaine cream provides us an effective local anaesthesia for such treatments.

Prilocaine induced methaemoglobinaemia in a medically compromised patient. Was this an inevitable consequence of the dose administered?

Patient M, a 45-year old female, was admitted for extractions with local anaesthetic, sedation and monitoring. This was to be carried out on an in patient basis due to the patient's extensive medical history which included rheumatic heart disease, previous deep vein thrombosis and severe ulcerative colitis. Patient M also gave a history of allergy to penicillin and lignocaine. The procedure was completed without incident, local anaesthesia having been achieved using Citanest with Octapressin (prilocaine 30 mg/ml and felypressin 0.03 unit/ml 2 ml cartridge). However, on recovery Patient M's oxy

Intrathecal mepivacaine and prilocaine are less neurotoxic than lidocaine in a rat intrathecal model.

BACKGROUND AND OBJECTIVES: Histologic evidence of the comparative neurotoxicity of lidocaine, mepivacaine, and prilocaine is incomplete. We compared the intrathecal neurotoxicity in rats among these 3 drugs based on morphologic and neurofunctional findings. METHODS: Rats (n=169) randomly received 0.12 microL/g of 0%, 2%, 5%, 7.5%, 10%, or 20% lidocaine, mepivacaine, or prilocaine or 25% glucose dissolved in distilled water via a chronically implanted intrathecal catheter. The effect of the agents on neurofunction was evaluated by movement of the hind limb (behavior test) and by sensory thresh

Topical lidocaine-prilocaine spray for the treatment of premature ejaculation: a proof of concept study.

Premature ejaculation (PE) is the most common sexual dysfunction in men. Since heightened sensitivity of the glans is implicated in PE, it seems reasonable that reducing this sensitivity could have a delaying effect on intravaginal ejaculation latency time (IVELT) without adversely affecting the sensation of ejaculation. We have developed a local anesthetic combination, in a metered-dose aerosol delivery system, that delivers lidocaine and prilocaine in base form without organic solvents. The drug can be easily removed prior to vaginal penetration, circumventing the need for a condom that most

[The blood level and a pharmacokinetic model of prilocaine during a continuous brachial plexus blockade].

Continuous brachial plexus blockade achieved by repeated injections through an axillary catheter is used increasingly often for microsurgical procedures and for postoperative pain relief. Repetitive administration, especially of long-acting agents, can cause problems with local anesthetic toxicity. Based upon a pharmacokinetic analysis of prilocaine serum concentrations after single-dose axillary plexus blockade in 14 patients, a pharmacokinetic model was established from which to predict serum concentrations after successive doses. METHODS. Each of 14 patients (ASA I-II, age 42 +/- 20 years,

Intravenous regional anaesthesia below the knee. A cross-over study with prilocaine in volunteers.

The quality of anaesthesia provided by three dosages of prilocaine, 40 ml 0.5%, 20 ml 1%, or 40 ml 1%, administered in random order, for intravenous regional anaesthesia of the lower extremity below the knee was compared in six volunteers. Limited sensory blockade was achieved in the groups with 40 ml 0.5% and 20 ml 1% during the 30-minute test period. One subject was not fully anaesthetised with any of the dosages used. Recovery was rapid (mean 12-13 minutes) in these two groups. In two subjects given 40 ml of 1%, complete recovery took 90 minutes. At 30 minutes, the degree of flexion remaini

Bilateral forearm intravenous regional anesthesia with prilocaine for botulinum toxin treatment of palmar hyperhidrosis.

BACKGROUND: Treatment of palmar hyperhidrosis with botulinum toxin (BTX) requires effective anesthesia, but previous methods have not provided enough pain relief or have resulted in a prolonged impaired hand function. OBJECTIVE: This is a study of bilateral forearm intravenous regional anesthesia using prilocaine for BTX treatment of palmar hyperhidrosis. METHODS: In all, 166 patients (100 female and 66 male) were treated bilaterally with intracutaneous BTX type A injections using intravenous regional anesthesia with prilocaine (5 mg/mL). In a subgroup of patients, forearm nerves were studie

Analgesic and anti-inflammatory effects of lignocaine-prilocaine (EMLA) cream in human burn injury.

Pain relief may be improved by reducing sensitization of nociceptive pathways caused by tissue injury. Such a reduction depends mainly on inhibition of local inflammatory changes and the relation between duration of nociceptive block and nociceptive input. In this study we examined if prolonged topical treatment with local anaesthetics could reduce late hyperalgesia and local inflammation after burn injury in healthy volunteers. The effects of EMLA treatment for 8 h after burn on hyperalgesia, inflammation and wound healing were compared with the contralateral placebo-treated leg for 48 h afte

Prilocaine leakage during tourniquet inflation in intravenous regional anaesthesia: the influence of fracture manipulation.

Recent evidence has shown that there may be leakage of anaesthetic drugs into the systemic circulation during the tourniquet inflation phase of intravenous regional anaesthesia. This, together with increased pressure in forearm vessels during manipulation of fractures, prompted the measurement of arterial levels of local anaesthetic to assess potential toxicity. Prilocaine levels were measured in nine patients undergoing manipulation of Colles' fractures and six patients undergoing other upper limb procedures. Eleven patients had measurable concentrations of prilocaine in the arterial circulat

Spinal Anesthesia for Ambulatory Surgery Using Hyperbaric Prilocaine vs Hyperbaric Bupivacaine: A Prospective Study.

Background Spinal anesthesia is a suitable technique for ambulatory surgery, offering benefits such as rapid recovery and minimal complications. The choice of local anesthetic is a key factor in achieving a rapid turnover in an ambulatory surgery unit. This study compares the safety and effectiveness of hyperbaric prilocaine and hyperbaric bupivacaine in an ambulatory setting. Methods A total of 114 patients undergoing ambulatory surgery were randomly allocated to receive either hyperbaric prilocaine (20mg/mL) or hyperbaric bupivacaine (5mg/mL).
