Children with congenital heart disease: probability of natural survival.

The age distribution of death in all children with congenital heart disease (CHD), who died in a 27-year period in Central Bohemia (population of 1.2 million), and the data on the incidence of CHD in children born in Bohemia (population of 6.3 million) in 1980 were used to calculate the probability of survival of a child born with CHD. Eighty-six percent of these children survived to the first month of life--mostly those with pulmonary stenosis (PS, 99%), aortic stenosis (AS, 95%), ventricular septal defect (VSD, 92%), and atrioventricular septal defect (AVSD, 91%). Seventy-one percent of pati

Measurement of single channel open probability with voltage ramps.

In patches from membranes containing many active channels it is difficult to measure the voltage dependence of activation. We have developed a technique using ramp potentials to measure activation rapidly over a wide range of potentials. If the rate of change of holding potential is small compared to the activation or deactivation time constant of the channels voltage ramps can be used to measure N*p(open), where N is the number of channels per patch and p(open) is the open probability of the single channel. This provides a continuous reading of N*p(open) and allows an efficient measurement of

Inverse probability weighting-based mediation analysis for microbiome data.

MOTIVATION: Mediation analysis is an important tool for elucidating causal mechanisms in biomedical research and has recently gained increasing attention in microbiome studies. Motivated by a microbiome study of acute myeloid leukemia (AML) patients, we investigate whether the effect of induction chemotherapy intensity on infection status is mediated through gut microbial taxa. This problem poses substantial methodological challenges due to the high dimensionality, zero inflation, and dependence of microbial mediators, as well as the presence of an exposure-induced mediator-outcome confounder,

Empirical transition probability modeling of continuous glucose monitoring data.

INTRODUCTION: Continuous glucose monitoring (CGM) produces rich time series data. Standard metrics such as time-in-range summarize glycemia but miss the sequence of changes. We aimed to characterize glucose dynamics using empirical transition probability techniques. MATERIALS AND METHODS: Using publicly available data for 244 participants who enrolled in the Insulin-Only Bionic Pancreas (IOBP2) randomized clinical trial, we mapped each CGM reading to one of five states: < 54, 54-69, 70-180, 181-250, or >250 mg/dL. We counted state changes between consecutive readings collected every 5 minutes

A 4-zone model to determine fentanyl overdose probability.

Determining the cause of death using postmortem fentanyl concentrations in femoral blood remains a major challenge in forensic toxicology. This is due to substantial concentration overlap between overdose deaths and incidental non-drug-related deaths. Existing literature largely reports descriptive statistics such as ranges and medians. These statistics do not provide an objective framework for evaluating the evidentiary value of a specific fentanyl concentration. Using distribution analysis, nonparametric tests, and likelihood ratios, we developed a four-zone statistical model to quantify ove

A marginal structural model for normal tissue complication probability.

The goal of radiation therapy for cancer is to deliver prescribed radiation dose to the tumor while minimizing dose to the surrounding healthy tissues. To evaluate treatment plans, the dose distribution to healthy organs is commonly summarized as dose-volume histograms (DVHs). Normal tissue complication probability (NTCP) modeling has centered around making patient-level risk predictions with features extracted from the DVHs, but few have considered adapting a causal framework to evaluate the safety of alternative treatment plans. We propose causal estimands for NTCP based on deterministic and

Inverse probability weighting for causal inference in hierarchical data.

OBJECTIVE: The aim of this study was to explore the impact of model misspecification, balance, and extreme weights on average treatment effect (ATE) estimation in hierarchical data with unmeasured cluster-level confounders using the multilevel propensity score model and inverse probability weight (IPW). METHODS: We simulated 48 hierarchical data scenarios with unmeasured cluster-level confounders, fitting nine ATE estimation strategies. These strategies were combined with IPW, which used both marginal stabilized weights and cluster-mean stabilized weights. Extreme weights were handled by trun

SARS-CoV-2: Receptor and Co-receptor Tropism Probability.

The recent pandemic which arose from China, is caused by a pathogenic virus named "severe acute respiratory syndrome-related coronavirus-2 (SARS-CoV-2)". Its rapid global expansion has inflicted an extreme public health concern. The attachment of receptor-binding domains (RBD) of the spike proteins (S) to the host cell's membrane, with or without the help of other cellular components such as proteases and especially co-receptors, is required for the first stage of its pathogenesis. In addition to humans, angiotensin-converting enzyme 2 (ACE2) is found on a wide range of vertebrate host's cellu

Using Probability Estimates to Evaluate a Patient With Weakness.

In this case report, we review how probabilistic reasoning can be implemented in retrospect to refine the diagnostic process. A 67-year-old female with a history of polymyalgia rheumatica (PMR) and a recent dental procedure presented with weakness, falls, and chills ongoing for two weeks. She reported pain in her shoulders and lower back. On presentation, she was febrile, and labs were notable for leukocytosis with neutrophilic predominance and an elevated erythrocyte sedimentation rate (ESR). Chest radiograph revealed a left lower lung opacity, which was not seen on a repeat film. She was tre

A novel computer task to assess outcome probability bias.

This research describes the development of a novel computer task to assess outcome probability bias for social anxiety - the tendency to make unrealistically high evaluations of the probability of experiencing a negative outcome when anticipating a social encounter - that improves upon existing measures by using images and by assessing the construct at automatic and controlled levels of processing. The first study evaluated the images selected for the task and the extent to which the task elicited automatic responding, and the second study evaluated the task's reliability and validity. Across

Probability assessment of intracerebral hemorrhage in prehospital emergency patients.

BACKGROUND: Routing of patients with intracerebral hemorrhage (ICH) and acute ischemic stroke (AIS) to the most appropriate hospital is challenging for emergency medical services particularly when specific treatment options are only provided by specialized hospitals and determination of the exact diagnosis is difficult. We aimed to develop a prehospital score - called prehospital-intracerebral hemorrhage score (ph-ICH score) - to assist in discriminating between both conditions. METHODS: The ph-ICH score was developed with data from patients treated aboard a mobile stroke unit in Berlin, Germ

Aversive Learning Increases Release Probability of Olfactory Sensory Neurons.

Predicting danger from previously associated sensory stimuli is essential for survival. Contributions from altered peripheral sensory inputs are implicated in this process, but the underlying mechanisms remain elusive. Here, we use the mammalian olfactory system to investigate such mechanisms. Primary olfactory sensory neurons (OSNs) project their axons directly to the olfactory bulb (OB) glomeruli, where their synaptic release is subject to local and cortical influence and neuromodulation. Pairing optogenetic activation of a single glomerulus with foot shock in mice induces freezing to light

Utility of inverse probability weighting in molecular pathological epidemiology.

As one of causal inference methodologies, the inverse probability weighting (IPW) method has been utilized to address confounding and account for missing data when subjects with missing data cannot be included in a primary analysis. The transdisciplinary field of molecular pathological epidemiology (MPE) integrates molecular pathological and epidemiological methods, and takes advantages of improved understanding of pathogenesis to generate stronger biological evidence of causality and optimize strategies for precision medicine and prevention. Disease subtyping based on biomarker analysis of bi

A neural implementation of Wald's sequential probability ratio test.

Difficult decisions often require evaluation of samples of evidence acquired sequentially. A sensible strategy is to accumulate evidence, weighted by its reliability, until sufficient support is attained. An optimal statistical approach would accumulate evidence in units of logarithms of likelihood ratios (logLR) to a desired level. Studies of perceptual decisions suggest that the brain approximates an analogous procedure, but a direct test of accumulation, in units of logLR, to a threshold in units of cumulative logLR is lacking. We trained rhesus monkeys to make decisions based on a sequence

Neural correlates of the divergence of instrumental probability distributions.

Flexible action selection requires knowledge about how alternative actions impact the environment: a "cognitive map" of instrumental contingencies. Reinforcement learning theories formalize this map as a set of stochastic relationships between actions and states, such that for any given action considered in a current state, a probability distribution is specified over possible outcome states. Here, we show that activity in the human inferior parietal lobule correlates with the divergence of such outcome distributions-a measure that reflects whether discrimination between alternative actions in

Disruptive effects of contingent food on high-probability behavior.

The delivery of food contingent on 10 s of consecutive toy engagement resulted in a decrease in engagement and a corresponding increase in other responses that had been previously reinforced with food. Similar effects were not observed when tokens exchangeable for the same food were delivered, suggesting that engagement was disrupted by the contingent provision of the food, which may have functioned as a discriminative stimulus that occasioned competing responses.

Diagnosing pulmonary embolism: a comparison of clinical probability scores.

Pulmonary embolism (PE) is a major cause of community and in-hospital mortality. This study aimed to compare the performance of the British Thoracic Society (BTS) score to the Wells' score in diagnosing PE. Data from two separate prospective diagnostic PE studies were analysed. All patients underwent gold standard investigation to determine the presence or absence of PE, together with a 3-month follow-up. The posttest prevalence of PE was compared using both scores and the receiver operating characteristic (ROC) curves. Seven hundred and seventy-nine patients were consented and investigated fo
