Perineal proctectomy, posterior rectopexy, and postanal levator repair for the treatment of rectal prolapse.

This report describes a one-stage perineal operation for the treatment of complete rectal prolapse aimed at restoration of anal continence in addition to resection of the prolapsed segment of rectum. This procedure was performed on 25 patients, most of whom were elderly or debilitated, over the past three years without local or systemic complications. Significant improvement in continence was seen in 88 percent of the patients within four weeks of operation, and in the remainder of the patients, within three months postoperatively.

Bio-Thiersch as an Adjunct to Perineal Proctectomy Reduces Rates of Recurrent Rectal Prolapse.

BACKGROUND: The rates of recurrent prolapse after perineal proctectomy vary widely in the literature, with incidences ranging between 0% and 50%. The Thiersch procedure, first described in 1891 for the treatment of rectal prolapse, involves encircling the anus with a foreign material with the goal of confining the prolapsing rectum above the anus. The Bio-Thiersch procedure uses biological mesh for anal encirclement and can be used as an adjunct to perineal proctectomy for rectal prolapse to reduce recurrence. OBJECTIVE: The aim of this study was to evaluate the Bio-Thiersch procedure as an a

Evaluation of pilot experience with robotic-assisted proctectomy and coloanal anastomosis for rectal cancer.

BACKGROUND: Robotic-assisted proctectomy with coloanal anastomosis (RPCA) is an innovative technique of pelvic dissection for low rectal cancer. Our objective was to evaluate our pilot experience with this procedure compared with open proctectomy with coloanal anastomosis (OPCA). METHODS: We performed a retrospective 5-year review of all consecutive cases of RPCA and OPCA performed at our institute. We focused on tumour characteristics, quality of surgery, analgesic requirements, average length of hospital stay (LOS), complications and long-term outcomes. RESULTS: Three patients underwent RP

[Indications and results of mucosal proctectomy with colo-anal anastomosis in villous disease of the rectum].

The objective of this study was to define the indication for proctectomy and colo-anal anastomosis in large rectal villous adenomas. The study population consisted of 20 patients (12 men and 8 women; mean age 63.6) who underwent rectal excision and colo-anal anastomosis from 1990 to 1997. The average size of tumors was 59.8 mm; 18 tumors were located in the lower third of the rectal ampulla; 8 patients had prior treatment (surgical or medical) before proctectomy. There were 13 straight colo-anal anastomoses and 7 constructed with colonic J pouch. Eighty percent of the anastomoses were defuncti

Fulminant Food Aspiration-Induced Pneumonia Leading to Tension Pyopneumothorax after Proctectomy: An Autopsy Case.

INTRODUCTION: Postoperative aspiration pneumonia is an uncommon but severe pulmonary complication, particularly in older adults with cognitive impairment. We report an autopsy case of fulminant aspiration pneumonia, caused by food aspiration following colorectal cancer surgery, that progressed to tension pyopneumothorax. CASE PRESENTATION: A male aged ≥75 years with dementia underwent laparoscopic high anterior resection for rectal cancer and resumed oral intake after passing a water-swallowing test. Shortly thereafter, he developed rapidly progressive pneumonia, hypoxemia, and septic shock,

Secondary proctectomy and ileal pouch-anal anastomosis after ileorectal anastomosis for familial adenomatous polyposis.

Conversion of ileorectal anastomosis to ileal pouch-anal anastomosis (IPAA) was indicated in 29 patients with familial adenomatous polyposis but technically impossible in three because of unsuspected pelvic desmoid tumours. There was no operative mortality and overall postoperative morbidity and late complication rates were no different from those in a group of 120 patients who underwent IPAA as a first operative procedure. Unsuspected adenocarcinoma (one invasive, three in situ) was found in four patients. The functional results in patients with IPAA did not differ from those of ileorectal an

Mucosal proctectomy and ileoanal pull-through technique and functional results in 23 consecutive patients.

Mucosal proctectomy with ileoanal pull-through in the treatment of ulcerative colitis and familial polyposis provides a technique for the preservation of the anal sphincters and relatively normal mechanisms of continence. Five patients had straight ileoanal anastomosis while 18 had the construction of a J-pouch. A two-team approach was used for simultaneous abdominal and perineal procedures to facilitate a shortened operating time. A loop ileostomy was routinely used in the postoperative period and was closed an average of 4.5 months (range: 2-16 months) later without complication. Prolonged p

New technique of transanal proctectomy with completely robotic total mesorrectal excision for rectal cancer.

Anterior resection with total mesorectal excision is the standard method of rectal cancer resection. However, this procedure remains technically difficult in mid and low rectal cancer. A robotic transanal proctectomy with total mesorectal excision and laparoscopic assistance is reported in a 57 year old male with BMI 32 kg/m2 and rectal adenocarcinoma T2N1M0 at 5 cm from the dentate line. Operating time was 420 min. Postoperative hospital stay was 6 days and no complications were observed. Pathological report showed a 33 cm specimen with ypT2N0 adenocarcinoma at 2 cm from the distal margin, co

Transanal endoscopic proctectomy and nerve injury risk: bottom to top surgical anatomy, key points.

The transanal approach for rectal resection is a promising approach, because it increases the circumferential radial margin, especially for difficult cases. Meanwhile, functional sequelae are frequent after rectal cancer surgery and are often due to neurological lesions. There is little literature describing surgical anatomy from bottom to top. We combined our surgical experience with our fetal and adult anatomical research to provide a bottom-up surgical description focusing on neurological anatomy (see Video, Supplemental Digital Content 1, http://links.lww.com/DCR/A148).

Low risk of pelvic sepsis after intersphincteric proctectomy in patients with low rectal cancer.

INTRODUCTION: Pelvic sepsis after Hartmann's procedure for low rectal cancer is a frequent complication. It has been reported at a frequency of 12.2-17.2% and has even reached 33% when the transection level of the rectum is ≤ 2 cm from the anal verge. The aim of this study was to examine whether intersphincteric proctectomy reduces the frequency of pelvic sepsis in patients operated with an extended Hartmann's procedure for rectal cancer. METHODS: Patients undergoing elective extended Hartmann's procedure with an intersphincteric proctectomy from 2010 until 2014 were reviewed retrospectively.

[Primary healing of the sacral cavity after proctectomy - a solvable surgical problem (author's transl)].

Primary healing of the perineal wound and the sacral cavity after a.p. resection of the rectum is an old surgical problem. In order to achieve primary healing we are using a lengthened omental graft to fill up the sacral space. Since January 1977 to July 1977 we used this method in 17 patients. 15 patients had very low rectal cancers. 2 patients had to undergo proctocolectomy because of ulcerative colitis. In 14 cases we achieved primary healing, among these the colitis patients. Overall we had three abscess formations in the sacral cavity. One of these patients got in addition an obstruction

A risk score model of 30-day readmission in ulcerative colitis after colectomy or proctectomy.

INTRODUCTION: The Center for Medicare and Medicaid Services established 30-day readmission rate as a key metric in measuring high-value, cost-conscious care; therefore, our aim is to develop a risk score for 30-day readmission in ulcerative colitis (UC) patients undergoing colectomy or proctectomy. METHODS: This study used data from the American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) participant user file (2011-2015). Patients with UC undergoing colectomy or proctectomy were identified using ICD-9, 10, and CPT codes. Stepwise multivariate analyses were u

Risk of cancer and secondary proctectomy after colectomy and ileorectal anastomosis in familial adenomatous polyposis.

PURPOSE: The aim of our retrospective study was to review the outcome of patients undergoing colectomy with ileorectal anastomosis (IRA) due to familial adenomatous polyposis (FAP) in Finland during the last 50 years. METHODS: The cumulative risk of rectal cancer and the rate of anus preservation were analyzed. A total of 140 FAP patients with previous colectomy combined with ileorectal anastomosis were included. Kaplan-Meier analysis was performed to evaluate cumulative risks. RESULTS: Secondary proctectomy was performed for 39 (28 %) of 140 patients. The cumulative risk of secondary procte

On the edge: pathology specimen quality in robotic and laparoscopic proctectomy, a propensity score-matched study.

BACKGROUND: Although robotic surgery has seen increasing adoption over time, its benefits over conventional laparoscopy for rectal cancer resection remain unclear. This study compares composite pathologic specimen quality and overall survival between robotic and laparoscopic proctectomy in a national sample of patients with rectal cancer. METHODS: Using the National Cancer Database (2010-2021), we conducted a retrospective cohort study of adults with stage I to III rectal adenocarcinoma who underwent elective laparoscopic or robotic proctectomy. The primary outcome was an optimal composite pa

Evaluation of the National Accreditation Program for Rectal Cancer and Association with Oncologic Outcomes After Proctectomy.

BACKGROUND: Circumferential resection margin (CRM) positivity is a validated predictor of local recurrence and survival in rectal cancer and reflects multidisciplinary care quality. The National Accreditation Program for Rectal Cancer, launched in 2017, standardizes rectal cancer care, but national oncologic outcome data remain limited. STUDY DESIGN: We performed a retrospective National Cancer Database analysis of clinical stage I to III rectal adenocarcinoma (2015 to 2022) treated with curative-intent proctectomy using a stacked difference-in-differences design. Risk factors for ischemia/necrosis of the colonic stump after proctectomy and delayed coloanal anastomosis.

INTRODUCTION: Delayed coloanal anastomosis (DCAA) is a two-stage procedure. DCAA has been increasingly reused in recent years in the management of rectal cancer. Such increased use of DCAA has highlighted the complications associated with this procedure. We aimed to evaluate the risk and risk factors of ischemia/necrosis of the colonic stump between the two stages of DCAA. PATIENTS AND METHODS: All patients who underwent a proctectomy with a DCAA were included in this retrospective single-centre study from November 2012 to June 2022. Two groups of patients were defined: patients with a well v
