Thalidomide maintenance treatment increases progression-free but not overall survival in elderly patients with myeloma.

BACKGROUND: Thalidomide maintenance therapy after stem cell transplantation resulted in increased progression-free survival and overall survival in a few trials, but its role in non-transplant eligible patients with multiple myeloma remains unclear. This study assessed the impact of thalidomide-interferon in comparison to interferon maintenance therapy in elderly patients with multiple myeloma. DESIGN AND METHODS: Of 289 elderly patients with multiple myeloma who were randomized to thalidomide-dexamethasone or melphalan-prednisolone induction therapy, 137 finally completed 9 cycles of inducti

Long-Term Progression-Free Survival in Patients Treated With RVD Induction and Risk-Adapted Maintenance.

BACKGROUND: A subset of patients with newly diagnosed multiple myeloma (NDMM) achieves prolonged remission following modern induction and maintenance, but predictors of long-term progression-free survival (PFS) and the durability of remission beyond a decade remain incompletely defined. METHODS: We retrospectively analyzed 1000 consecutive NDMM patients treated with lenalidomide, bortezomib, and dexamethasone (RVD) induction followed by risk-adapted maintenance from 2007 to 2016. Patients diagnosed after October 2015 were excluded to ensure ≥10 years of follow-up. Long-term progression-free s

BOIN12-PFS: A Bayesian Optimal Interval Phase I/II Design Incorporating Progression-Free Survival Endpoint.

Modern oncology drug development is increasingly focused on identifying the optimal biological dose (OBD) rather than just the maximum tolerated dose (MTD). Early-phase oncology trials have conventionally relied on short-term endpoints, such as overall response rate (ORR), to evaluate efficacy. However, there is a growing perception that incorporating longer-term endpoints, such as progression-free survival (PFS), into early-phase dose-finding trials can provide more informative guidance for dose optimization. This study proposes a new design, BOIN12-PFS, to identify the optimal dose based on

Recurrent pyuria is associated with shorter progression-free survival in dogs with bladder urothelial carcinoma.

OBJECTIVE: To evaluate the association between pyuria and clinical outcomes in dogs. METHODS: Data were collected from 2010 through 2025. Pyuria was defined as ≥ 5 WBCs/hpf in urine sediment. Dogs were stratified according to pyuria frequency (0, 1 to 2, or ≥ 3 episodes) and tumor size (< 3 or ≥ 3 cm). Progression-free survival (PFS) and overall survival were analyzed using Kaplan-Meier curves, log-rank tests, and Cox proportional hazards models. RESULTS: 51 dogs diagnosed with bladder urothelial carcinoma (UC) were included. When stratified by pyuria frequency, dogs with ≥ 3 episodes had a

Results of TcMEP guided vestibular schwannoma surgery; long term follow-up and progression free survival.

In patients with large vestibular schwannomas (VS), surgery is the primary treatment despite risks such as facial nerve dysfunction. Intra-operative monitoring with transcranial motor evoked potentials (TcMEP) helps predict facial nerve outcome but may lead to subtotal resections and later recurrence. This study compares TcMEP thresholds, facial nerve outcome, recurrence rate and progression-free survival in Koos grade 4 vestibular schwannomas. Fifty-five surgically treated patients with Koos grade 4 VS (2015-2024) were included in this retrospective study. All underwent facial nerve TcMEP mon

Stereotactic ablative radiotherapy-driven immunosuppression is associated with poorer progression-free survival in cancer patients.

BACKGROUND: The landscape of cancer treatment has evolved rapidly within the last 50 years, and whilst radiotherapy, chemotherapy, and surgery remain the mainstay treatment options, there has been a shift towards using immunotherapy alone or in combination with other treatment modalities. There is an emerging paradigm that radiotherapy is immunogenic, driving stimulation of antigen-specific T cells capable of recognising tumour cells at distal sites to the treatment location. METHODS: Whole blood samples were collected from patients with primary and oligometastatic cancer before, during, and

Elevated baseline C-reactive protein levels predict poor progression-free survival in sporadic vestibular schwannoma.

BACKGROUND: Recent investigations showed emerging evidence of the role of inflammation in the growth of sporadic vestibular schwannoma (VS). The present retrospective study investigated the impact of systemic inflammation on tumor progression using serum C-reactive protein (CRP) levels in a series of 87 surgically treated sporadic VS patients. METHODS: The optimal cut-off value for CRP was defined as 3.14 mg/dl according to the receiver operating characteristic curve (AUC: 0.70, 95% CI 0.47-0.92). Patient cohort was dichotomized into normal (n = 66; < 3.14 mg/dl) and high baseline (n = 21; ≥

Radiographic progression-free survival as a surrogate endpoint for overall survival in first-line ARPi naïve metastatic castration-resistant prostate cancer.

BACKGROUND: Overall survival (OS) is the gold standard endpoint in oncology trials but requires long follow-up and may be confounded by post-protocol treatments. Radiographic progression-free survival (rPFS) is used as an earlier endpoint in metastatic castration-resistant prostate cancer (mCRPC). This study evaluated the validity of rPFS as a surrogate for OS in first-line, asymptomatic/mildly symptomatic, androgen receptor pathway inhibitor (ARPi) naïve, mCRPC using methods recommended by Germany's Institute for Quality and Efficiency in Health Care (IQWiG). Intracranial progression-free survival correlates with overall survival and varies by tumor subtype in patients with breast cancer and brain metastases.

BACKGROUND: Brain metastases in patients with breast cancer (BCBM) are common. While the Breast Graded Prognostic Assessment (Breast GPA) predicts overall survival (OS), its ability to predict intracranial progression-free survival (icPFS) has not been established. METHODS: We conducted a retrospective, multi-institutional cohort study to evaluate the association between Breast GPA and icPFS among BCBM patients treated initially with stereotactic radiosurgery (SRS) or whole-brain radiation therapy (WBRT). icPFS was defined as the time from initial local BCBM treatment to the first local salva

Exploring the relationship between Overall Survival (OS), Progression Free Survival (PFS) and Objective Response Rate (ORR) in patients with advanced melanoma.

PURPOSE: From 2011 to 2016, 13 randomized clinical trials with active controls were submitted to U.S. Food and Drug Administration (FDA) for the treatment of advanced melanoma. While regular approval is generally granted due to a substantial improvement in overall survival (OS), or a large, clinically meaningful improvement in progression-free survival (PFS), accelerated approval can be granted based on incremental change in a surrogate end point reasonably likely to predict clinical benefit, such as objective response (ORR) of large magnitude and long duration. However, the relationship betwe

Sacituzumab govitecan in breast cancer patients - an evaluation of progression-free survival, overall survival, and hypersensitivity reactions in real-world patients.

IntroductionSacituzumab govitecan is approved for the treatment of triple-negative advanced/metastatic breast cancer and hormone-receptor positive, HER2-negative advanced/metastatic breast cancer. Clinical trials show favorable improvements in progression-free survival (PFS) and overall survival (OS). However, they report a large percentage of patients experiencing hypersensitivity reactions. It has been observed that use of sacituzumab govitecan at [redacted institution] may have a lower rate of hypersensitivity reactions than seen in clinical trials.MethodsThis study aimed to determine the r

Gene Expression Clustering of Lung Adenocarcinoma Tumors as a Prognosis Tool to Infer Progression-Free Survival.

INTRODUCTION: Lung cancer is the leading cause of cancer-related death. Studies have reported distinct tumor type based on gene expression signatures and attempted to use this information to predict survival, with limited replicability due to tumor heterogeneity, technical artefacts, and cohort size. METHODS: RNA expression profiling was performed on 515 early stage resected lung adenocarcinomas from a single institution. We then used this data set to explore the potential of gene expression to stratify patients into distinct classes predictive of survival using unsupervised clustering and Co

Low baseline cGAS expression is associated with longer progression-free survival in patients with pleural mesothelioma.

INTRODUCTION: The cyclic GMP-AMP synthase (cGAS)/STING pathway, a central DNA-sensing mechanism, is known to activate anti-tumor immune response but may also promote tumor progression when chronically activated. Given the role of asbestos-induced chronic inflammation in pleural mesothelioma (PM), we investigated cGAS/STING expression and its association with treatment outcomes. METHODS: We analyzed tissue microarrays from 190 PM patients using multiparameter immunofluorescence single-cell imaging. cGAS and STING expression were quantified in Calretinin+ tumor cells, Calretinin-CD8-DC-LAMP- ce

Prognostic factors for progression free survival in patients with medullary thyroid cancer: a multicenter cohort study.

Many patients with medullary thyroid cancer (MTC) experience recurrent or persistent disease after surgery due to its aggressive nature. However, the prognostic factors for progression-free survival (PFS) have been poorly investigated. This study aimed to explore prognostic factors associated with PFS in patients with MTC. Patients with MTC were enrolled from 15 medical centers in Shandong Province, China, between January 2010 and December 2021. Univariate and multivariate Cox regression analyses were used to explore the prognostic factors for PFS in patients with MTC. Receiver operating chara

A Nomogram for Predicting Progression-Free Survival in Primary Extramedullary Multiple Myeloma Using Routine Laboratory Variables.

INTRODUCTION: Extramedullary involvement in multiple myeloma represents aggressive disease, with clinical stratification of primary extramedullary disease (EMD) remaining a challenge. In this study, we aimed to develop a credible nomogram utilizing routine laboratory variables to predict individual survival for primary EMD patients. METHODS: We retrospectively analyzed a cohort of 60 primary EMD patients, from January 2006 to December 2022. Independent risk factors were identified and subsequently incorporated to generate a nomogram using the Cox proportional hazard regression model. Then, we

Nomogram Predicting Progression-Free Survival in Locally Advanced Papillary Thyroid Cancer with Recurrent Laryngeal Nerve Invasion.

OBJECTIVES: The recurrence rate of T4a papillary thyroid cancer (PTC) is relatively high, but research on the prognosis of T4a PTC is rarely investigated. This study aims to analyze the prognosis of T4a PTC patients with recurrent laryngeal nerve (RLN) invasion. METHODS: Univariable and multivariable Cox proportional hazard models were employed to identify prognostic factors for the progression-free-survival (PFS) of PTC patients. A nomogram was constructed based on essential prognostic factors to predict the risk of disease progression in T4a PTC patients with RLN invasion. RESULTS: A total
