The effect of prophylactic cruroplasty on the incidence of hiatal herniation after robot-assisted minimally invasive esophagectomy (RAMIE).

BACKGROUND: Hiatal herniation (HH) following esophagectomy can cause severe complications and affect functional outcomes. This study assessed whether prophylactic cruroplasty during robot-assisted minimally invasive esophagectomy (RAMIE) reduces HH incidence. METHODS: Patients undergoing curative RAMIE with cervical anastomosis for esophageal cancer at a high-volume center were included. Cruroplasty became routine in 2017 unless the hiatus was narrow. The primary outcome was HH incidence with versus without cruroplasty. Median follow-up was calculated using the reverse Kaplan-Meier method. Ka

Prophylactic salpingectomy for the prevention of ovarian cancer: Who should we target?

Ovarian cancer is the most fatal gynecologic malignancy (50% 5-year survival) due to a typically advanced stage at diagnosis and a high rate of recurrence. Chemoprevention options are limited, and few interventions have been shown to reduce cancer risk or mortality. Emerging data support the model that fallopian tubes are the site of origin for a proportion of high-grade serous cancers. This implies that a subset of cancers may be prevented by removing the fallopian tubes while leaving the ovaries intact. Accordingly, there has been shift in clinical practice for average risk women; some now r

Potential adverse effects of prophylactic bilateral salpingo-oophorectomy on skin aging in premenopausal women undergoing hysterectomy for benign conditions.

OBJECTIVE: This study aimed to assess the effects of hysterectomy and bilateral salpingo-oophorectomy (BSO)--compared with the effects of hysterectomy alone--on skin aging in premenopausal women undergoing hysterectomy for benign conditions. METHODS: One hundred thirty-five premenopausal women who underwent hysterectomy with BSO were compared with a control group of women who underwent hysterectomy alone based on skin parameters (including wrinkling, laxity/sagging, and texture/dryness) and Skindex-29 questionnaire scores. The inclusion criteria were as follows: aged between 40 and 50 years,

[Preventive antibiotic therapy and postoperative infection in orthopedic surgery. 1983 total hip prostheses].

In a previous paper the authors studied the role of aerobio-contamination in the rate of post-operative infections. In the present study, the results of 1,172 total hip prostheses performed between 1979/1982 with prophylactic antibiotics were compared with those obtained in 811 cases performed before 1979 without antibiotics. In the 1979/1982 series, there was 0,5 p. 100 of post-operative infection and in the pre- 1979 series, 3,3 p. 100. During the two periods the peri-operative environment was identical and the surgical technique was the same. The rate of infection in more than 8,000 other s

Prophylactic antibiotics for preventing infection after continence surgery in women with stress urinary incontinence.

BACKGROUND: Surgical options for treating stress urinary incontinence (SUI) are usually explored after conservative interventions have failed. Surgeries fall into two categories: traditional techniques (open surgery) and minimally invasive procedures, such as laparoscopic procedures, midurethral sling and injections with urethral bulking agents. Postsurgery infections, such as infections of the surgical site or urinary tract, are common complications. To minimise the risk of postoperative bacterial infections, prophylactic antibiotics may be given before or during surgery.  OBJECTIVES: To asse

Antibiotic prophylaxis in oral and maxillofacial surgery: a systematic review.

Surgical site infections are a complication of oral and maxillofacial procedures, with the potential for significant morbidity and mortality. Use of preoperative, perioperative, and postoperative antibiotic prophylaxis to reduce the incidence of surgical site infections must be balanced with considerations of a patients' risk of antibiotic-related adverse events. This review aimed to provide evidence-based recommendations for antibiotic prophylaxis. Searches were conducted using MEDLINE, the Cochrane Library, EMBASE, and PUBMED for maxillofacial procedures including: treatment of dental absces

Prophylactic antibiotics in orthopaedic surgery.

The use of prophylactic antibiotics in orthopaedic surgery has been proven effective in reducing surgical site infections after hip and knee arthroplasty, spine procedures, and open reduction and internal fixation of fractures. To maximize the beneficial effect of prophylactic antibiotics, while minimizing any adverse effects, the correct antimicrobial agent must be selected, the drug must be administered just before incision, and the duration of administration should not exceed 24 hours.

Postoperative prophylactic antibiotics and surgical site infection rates in breast surgery patients.

BACKGROUND: A single preoperative prophylactic dose of an intravenous antibiotic with antistaphylococcal activity is standard of care for breast and axillary surgical procedures. Some surgeons also prescribe postoperative prophylaxis for all patients with drains to prevent infection despite its lack of proven efficacy. METHODS: A retrospective chart review of patients with breast and/or axillary surgical procedures between July 2004 and June 2006 were included. Data were collected on patient demographics, procedure types, and use of prophylactic antibiotics. Surgical site infection (SSI) was

Management of Cholelithiasis in Children With Associated Diseases: Should Prophylactic Cholecystectomy Be Recommended?-A Retrospective Analysis.

Cholelithiasis is increasing in the pediatric population, and there are currently no guidelines for the management of asymptomatic patients with both cholelithiasis and a predisposing condition. This study seeks to highlight situations where prophylactic cholecystectomy may be desirable. We retrospectively reviewed the medical records of children who underwent elective laparoscopic cholecystectomy between October 2011 and September 2022. Thirty-two patients were included in the study. Five different groups of patients were identified based on associated pathologies. Twenty-six patients were sy

Aortic regurgitation in Marfan syndrome patients who underwent prophylactic surgery: A single-center experience.

BACKGROUND: Marfan syndrome is an inherited connective tissue disease that causes aortic root dilatation and dissection and requires surgical intervention. Apart from emergent surgery for aortic dissection or aortic aneurysmal rupture, prophylactic surgical intervention can also be administered, depending on the severity of aortic root dilatation. The direct relationship between surgical intervention and aortic regurgitation was seldom mentioned in previous studies. METHODS: A retrospective cohort study was designed to determine the clinical presentations of prophylactic surgery in patients w

Preoperative Prophylactic Balloon-Assisted Occlusion of the Internal Iliac Arteries in the Management of Placenta Increta/Percreta.

Background and Objectives: Preoperative prophylactic balloon-assisted occlusion (PBAO) of the internal iliac arteries minimizes blood loss and facilitates surgery performance, through reductions in the rate of uterine perfusion, which allow for better control in hysterectomy performance, with decreased rates of bleeding and surgical complications. We aimed to investigate the maternal and fetal outcomes associated with PBAO use in women with placenta increta or percreta. Material and Methods: The records of 42 consecutive patients with a diagnosis of placenta increta or percreta were retrospect

Contraception options for patients with known hereditary risk for ovarian cancer.

For patients with a known hereditary risk of ovarian cancer, primary prevention typically is accomplished through prophylactic bilateral salpingo-oophorectomy. However, some patients may wish to preserve fertility or delay menopause and its associated comorbidities. This article discusses appropriate methods of contraception for these patients.

Prophylactic salpingectomy and prophylactic salpingoophorectomy for adnexal high-grade serous epithelial carcinoma: A reappraisal.

At present, there is no effective screening of ovarian cancer. Primary prevention may be the only strategy to decrease the mortality from ovarian cancer, not only in women at high risk but also at low risk. Several recent studies have identified the distal fimbriae end of the fallopian tubes as primary precursor of High-grade serous carcinoma. Serous tubal intraepithelial carcinomas and occult invasive serous carcinomas have been identified in 2-17% of the fallopian tubes of BRCA1/2 positive women undergoing risk-reducing salpingo-oophorectomy. Removal of the fallopian tubes with ovarian prese

Antibiotic prophylaxis in surgery.

Scientific studies conducted during the last 10 years have resulted in a great improvement of our approach to the appropriate use of prophylactic antibiotics in the surgical patient. Errors of the past including faulty timing of the initial dosage as well as prolonged duration of prophylaxis have largely been remedied. Present studies are designed to define the patients within the various subsets of diseases or surgical procedures who are at greatest risk of infection. It is these patients who can be expected to benefit most from the efficacious use of prophylactic antibiotics as well as other

[Preventive operations in intestinal abnormalities].

In intestinal anomalies, preventive surgical procedures include gastrostomies and enterostomies as well as some special operations such as ligature of the cardia and associated appendectomies. General indications for enterostomies are seen in life-threatening circumstances, i.e. prematurity, surfactant deficiency, major associated malformations and complications. Preventive surgical procedures are necessary in certain cases of esophageal atresia, complicated small bowel atresia, meconium ileus, anorectal malformations, Hirschsprung disease, neuronal intestinal dysplasia and necrotizing enteroc

Is Prophylactic Immunosuppressive Therapy for Patients with a History of Postsurgical Pyoderma Gangrenosum Necessary?

Postsurgical pyoderma gangrenosum (PSPG) is a rare but serious surgical complication with a predilection for the breast and abdomen. Immunosuppression is the mainstay of treatment of PSPG. In addition, it has become a common practice for clinicians to prophylactically treat patients with a history of PSPG with corticosteroids or immunomodulators during subsequent operative procedures to prevent recurrence. Although many practitioners have reported successful outcomes with these measures, currently no protocol exists for prophylactic perioperative therapy. Here, we present the clinical course a
