PURPOSE: Inflammatory breast cancer (IBC) poses a radiotherapeutic challenge due to dermal lymphatic involvement, which often necessitates larger target volumes and chest wall boosts, making advanced planning techniques attractive to reduce exposure to nearby organs. We report our experience with intensity modulated proton therapy (IMPT) for the treatment of IBC.
METHODS: Between 2016 and 2020, all IBC patients treated with adjuvant IMPT at our institution were identified. Overall survival (OS) and distant metastasis-free survival (DMFS) were estimated using the Kaplan-Meier method. Adverse e

Background/Objectives: Proton therapy provides superior dose conformity and better normal tissue sparing than conventional radiotherapy but is limited by its relatively low relative biological effectiveness (RBE). Nanoparticles have emerged as promising proton radiosensitizers capable of enhancing physical dose deposition and biological responses. Methods: A narrative literature review was conducted using PubMed, Scopus, Web of Science, and Google Scholar. Results: Gold nanoparticles were the most extensively studied radiosensitizers, while platinum, gadolinium, hafnium oxide, titanium dioxide

PURPOSE: Periorbital tumor location presents a significant challenge with 3-dimensional conformal radiation therapy or intensity modulated radiation therapy due to high tumor dose needed in the setting of close proximity to orbital structures with lower tolerance. Proton beam therapy (PBT) is felt to be an effective modality in such cases due to its sharp dose gradient.
MATERIALS AND METHODS: We reviewed our institutional PBT registry and identified 17 patients with tumor epicenters within 2 cm of the eye and optic apparatus treated with passive scatter PBT with comparison volumetric arc ther

PURPOSE: To report the results in patients reirradiated with proton therapy for recurrent or progressive chordoma, with or without salvage surgery.
METHODS AND MATERIALS: A retrospective review of 16 consecutive patients treated from 2005 to 2012 was performed. All patients had received at least 1 prior course of radiation therapy to the same area, and all but 1 patient had at least 1 surgical resection for disease before receiving reirradiation. At the time of recurrence or progression, half of the patients underwent additional salvage surgery before receiving reirradiation. The median prior

Off-line PET imaging after proton therapy is limited by transport delay and by the sensitivity and short axial field of view of conventional scanners, which hinder imaging at ultralow activity and characterization of whole-body biologic washout. We evaluated whether a near-room total-body PET/CT system could enable clinically practical imaging of proton-induced activity after treatment and its whole-body biologic washout. Methods: We conducted a series of phantom studies to evaluate the performance of total-body PET at ultralow activity with Monte Carlo (MC)-simulated activity distributions as

In-room imaging is a prerequisite for adaptive proton therapy. The use of onboard cone-beam computed tomography (CBCT) imaging, which is routinely acquired for patient position verification, can enable daily dose reconstructions and plan adaptation decisions. Image quality deficiencies though, hamper dose calculation accuracy and make corrections of CBCTs a necessity. This study compared three methods to correct CBCTs and create synthetic CTs that are suitable for proton dose calculations. CBCTs, planning CTs and repeated CTs (rCT) from 33 H&N cancer patients were used to compare a deep convol

Airway chondronecrosis is a rare but serious manifestation of late radiation-induced toxicity. We report the case of a 67-year-old woman with epidermal growth factor receptor mutation-positive lung adenocarcinoma who developed bronchial chondronecrosis 8 years after proton-beam therapy (PBT) to the right upper lobe nodule. For 2 years, she was treated for cough-variant asthma owing to a persistent cough. Chest computed tomography (CT) showed only a subtle irregularity of the right main bronchial wall. Bronchoscopy revealed yellow necrotic deposits in the right main bronchi. Although a conventi

IMPORTANCE: Proton therapy reduces radiation exposure to the heart, lungs, and contralateral breast compared with conventional photon-based breast radiotherapy. However, its clinical benefit-particularly relative to photon deep inspiration breath-hold (DIBH)-remains uncertain, and its high cost raises questions about cost-effectiveness.
OBJECTIVE: To evaluate the cost-effectiveness of proton therapy compared with photon-based radiotherapy, including DIBH and free breathing (FB), among patients with breast cancer in Belgium.
DESIGN, SETTING, AND PARTICIPANTS: This economic evaluation using st

IMPORTANCE: Osteoradionecrosis (ORN) is a potentially debilitating late complication of radiotherapy (RT) for head and neck cancer. While proton therapy offers superior dose conformality, its impact on ORN risk remains uncertain, particularly in patients with oropharyngeal squamous cell carcinoma (OPSCC).
OBJECTIVE: To characterize the incidence, severity, and predictors of ORN in a large institutional cohort of patients with OPSCC treated with curative-intent RT, and to compare outcomes between proton therapy and intensity-modulated radiation therapy (IMRT).

PURPOSE: Proton therapy provides superior dose conformity compared to photon therapy, but its treatment planning is challenged by sensitivity to anatomical changes, setup/range uncertainties, and computational complexity. This narrative review evaluates the role of artificial intelligence (AI) in improving proton therapy treatment planning, with emphasis on proton-specific requirements for dose-calculation accuracy, robustness, and adaptive decision-making.
MATERIALS AND METHODS: Recent studies on AI applications across the proton therapy planning workflow, including imaging, registration, se

Proton-activated ion channels mediate ion transport in response to extracellular acidification, enabling cellular adaptation to acidic microenvironments. Despite their biological importance, mimicking proton-activated functionality in artificial ion channels remains a significant challenge. Here, we present a novel class of proton-activated artificial ion channels built from self-assembled peptide chains integrated into a pH-responsive 2,2'-bipyridine scaffold. Protonation induces a conformational switch in the channel-forming units, promoting one-dimensional self-assembly and subsequent hydro

Proton beam radiation has a unique depth-dose distribution with lower surface dose, peaked energy loss at the end of the traveling range, and negligible exit dose compared to photon radiation that allows better sparing of normal tissues and potential dose escalation to the tumor. With advanced proton beam delivery and imaging guidance systems and years of successful proton treatment experience, clinicians are moving forward to implement proton beam in stereotactic body radiation therapy (SBRT). Major challenges in implementing stereotactic body proton therapy (SBPT) still exist such as the pro

BACKGROUND: Thailand's first Proton Beam Therapy (PBT) center became operational in August 2021. Given the high costs and specialized expertise required, evaluating the clinical and economic implications of PBT and planning for its expansion are essential for Thailand's healthcare system.
METHODS: This study projected national PBT demand using data from the Thai PBT center, the Thai Association of Radiation Oncology (THASTRO), national cancer registry reports (Volumes VIII-X), and GLOBOCAN (2022-2040). The number of cancer patients requiring PBT was estimated based on: (1) Thailand's Comptrol

AIM: This study presents an analysis (efficacy and toxicity) of outcomes in patients with esthesioneuroblastoma after pencil beam proton therapy with a fixed beamline in the upright position.
BACKGROUND: Esthesioneuroblastoma (ENB) is an extremely rare tumor of sinonasal area located in critical proximity to vital structures. Proton therapy (PT) is often considered the optimal radiation treatment for head-and-neck tumors, although of limited availability. Upright PT delivered using fixed pencil beamline and rotating chair is a fairly promising option.
METHODS: This is a single-center experie

OBJECTIVE: Proton beam therapy (PBT) is an increasingly used treatment modality for pediatric patients with brain tumors. Moyamoya syndrome (MMS) is well recognized as a complication of traditional photon radiotherapy, however its association with PBT is less well described. The authors discuss their initial experience with the neurosurgical management of MMS secondary to PBT in a large-volume pediatric neurovascular service.
METHODS: The authors performed a retrospective case review of consecutive children referred for neurosurgical management of MMS after PBT between 2009 and 2022. Patient

Extraskeletal osteosarcoma (ESOS) arising from the mediastinum is a rare malignant tumor and associated with a poor prognosis. We present the case of a 73-year-old man with a hoarseness. Imaging studies revealed a large calcified tumor of the median mediastinum. Surgery was performed, but complete resection was impossible and approximately two thirds of the tumor was excised. The tumor was diagnosed pathologically as ESOS. Proton beam therapy has been performed on the remaining lesion, and the patient is alive without tumor regrowth after 29 months. This case report documents a mediastinum ESO
