OBJECTIVE: To review our recent experience with protothecosis in patients with cancer at The University of Texas MD Anderson Cancer Center, and compare these cases with others reported in the literature.
METHODS: We report on three patients with protothecosis and cancer who were seen at The University of Texas MD Anderson Cancer Center from January 1979 to May 2002, and reviewed all cases of protothecosis in patients with cancer reported in the literature since 1966.
RESULTS: Overall, 13 cases of protothecosis complicating cancer were evaluated. The median age of the patients was 41 years (r

BACKGROUND: Protothecosis is a rare infectious disease caused by unicellular, achlorophyllous, microalgae of the genus Prototheca, ubiquitously distributed in nature. The algae are emerging pathogens, whose incidence is increasing in both human and animal populations and serious systemic infections related to this pathogen have been increasingly described in humans in recent years. After mastitis in dairy cows, canine protothecosis is the second most prevalent form of the protothecal disease in animals. Here, we report the first case of chronic cutaneous protothecosis due to P. wickerhamii in

Protothecosis, an infectious disease caused by the green algae Prototheca zopfii and P. wickerhamii, occurs sporadically in domestic animals and humans. Diagnosis of CNS protothecosis is based on neurologic signs that indicate multifocal nervous system lesions and that follow a period of chronic diarrhea and weight loss, cytologic observation of algae in fecal culture or histopathology, and detection of the agent by PCR assay of infected tissues. Here, we report a case of a paraparetic dog with CNS protothecosis that was diagnosed definitively antemortem using CSF cytology, PCR, and DNA sequen

Protothecosis is a severe, emerging opportunistic infection caused by the saprophytic, achlorophyllous microalgae of the genus Prototheca. Though uncommon, human and animal cases are increasing worldwide, making awareness of this fungal-like pathogen important in both human and veterinary medicine. We report a fatal case of disseminated protothecosis caused by P. wickherhamii in a 10-year-old, spayed, female, mixed-breed collie dog. Clinical signs included uveitis, ulcerated cutaneous nodules on the limbs and face, pododermatitis with nail loss and, lymphadenomegaly in the prescapular and popl

A 10-year-old spayed mongrel dog was referred with repeated intercurrent hematochezia and anal bleeding. The dog was vigorous and had a normal appetite, and the fecal test showed no abnormal signs. Despite treatment primarily with sulfasalazine, the condition did not improve and unilateral blindness developed. A Prototheca zopfii infection was identified by further examination with bowel culture on Sabouraud's agar without cyclohexane and antibiotics. Subsequent to a vision loss in the other eye, the dog died showing signs of neurological disorder.

BACKGROUND: Protothecosis is an extremely rare infectious disease in humans caused by achlorophyllous algae within the genus Prototheca, which is present in many natural environments. To date, very few cases of ocular involvement have been reported.
CASE REPORT: We present the case of an immunocompetent patient with progressive loss of visual acuity in the right eye and with a history of cataract surgery in the same eye 5 years before. Ocular examination revealed numerous whitish retrolental deposits in the anterior chamber. A vitrectomy was performed and samples of vitreous humour, vitreous

Recent phylogenetic studies based on ribosomal RNA sequences have confirmed that the organisms traditionally treated as fungi include those that have evolved from several different lines (multiphyletic organisms), as has been suspected. Even organisms causing disease in humans represent at least two evolutional lines. Pythium insidiosum and Prototheca species are both believed to have evolved from one line, while the rest of the pathogens have evolved from another line. P. insidiosum is more closely related to red algae and diatoms than to fungi. Prototheca species, as has been previously post

A patient is described from whom repeated olecranon bursa fluid cultures yielded Prototheca wickerhamii. Following intrabursal instillation of amphotericin B, the cultures became negative. There was no progression of infection or adverse consequences of this treatment. Review of the literature regarding this unusual infection reveals seven reported cases of protothecal olecranon bursitis. Five cases from which sufficient information was provided were diagnosed by histologic examination of excised bursa that showed the characteristic sporangia.

Here is presented, a rare case of disseminated protothecosis in a 10-year-old boy with combined immunodeficiency, hitherto unreported from India. Even though it is difficult to diagnose clinically, observation of the sporangiospores within the sporangium in culture gives the accurate laboratory identification of Prototheca spp. In this patient, failure to eradicate the infection with amphotericin B and recurrence with olecranon bursitis along with skin lesions and splenomegaly was observed. Disseminated protothecosis in a child with combined immunodeficiency and failure to eradicate the infect

Infectious endocarditis (IE) is an inflammatory disease of cardiac valves and endocardium of different origin. Subacute IE is a specific form of sepsis associated with the presence of the site of infection in the heart responsible for recurrent septicemia, embolism, and progressive changes in the immune system leading to nephritis, vasculitis, synovitis, and polyserositis. This form develops in response to a low-virulent pathogen (e.g.fungi) or as a result of inefficient antibacterial therapy. A patient is described presenting with fibrous body, aortic and mitral valve infection by the alga Pr

This is a report on the clinical history and pathological lesions of a dog suffering from disseminated protothecosis due to Prototheca zopfi. Clinically, the dog was presented with bilateral conjunctivitis followed by blindness, deafness and posterior paresis. Pathological lesions were most severe in the eyes and consisted of subacute panophthalmitis with secondary posterior subcapsular cataract, posterior synechia, retinal detachment and microscopic evidence of glaucoma. The kidney, liver, brain, spleen and lungs were also affected. This is believed to be the first published account of protot

Protothecosis is an infection caused by achloric algae of the genus Prototheca. These organisms have been isolated from water, sewage, soil, and the slime flux of trees, and are a known cause of disease in other mammals. Infection in humans occurs after traumatic inoculation, producing localized olecranon bursal or, rarely, systemic disease. Only two previous cases of disseminated cutaneous disease have been reported in patients with defective neutrophil function. We describe a rare case of widespread cutaneous dissemination occurring after an arthropod bite in an immunocompromised patient.

Protothecosis is a rare infection caused by achlorophyllic algae called Prototheca. Approximately 117 cases have been described in the literature world wide, the majority caused by the species P. wickerhamii. Cutaneous infection is the most common and cases of tenosynovitis are very rare. A local or systemic immunosuppressive factor is seen in half of the cases of protothecosis. We report a case of protothecal tenosynovitis in a middle-aged, immunocompetent woman that developed after she received sclerosing therapy of varicose veins. Administration of itraconazole with surgical débridement pro

A case of olecranon bursitis due to the achlorophyllic algae Prototheca species in an otherwise healthy 74-year-old man prompted our review of reported cases occurring in the southeastern United States. Most cases have involved the olecranon bursa, have occurred in otherwise healthy individuals, and were cured by simple excision. Intensive therapy with amphotericin B has been used successfully in a few cases that required systemic antibiotic therapy. The histologic features of this infection are predominantly those of granulomatous inflammation, with relatively easy demonstration of organisms

A 3 1/2-year-old intact male German Shepherd Dog was admitted with a history of protracted haemorrhagic diarrhoea and tenesmus. Clinical evaluation revealed thickening of the rectal mucosa and presence of fresh blood and mucus in the faeces. Severe mucosal granularity and multiple nodules, in addition to a rectal stricture 7 to 9 cm from the anus, were found on colonoscopy. Histopathological evaluation of biopsy specimens identified organisms with morphologic features compatible with those reported for Prototheca spp. Although surgical excision of the rectal stricture was performed and ketocon

Prototheca species are ubiquitous, aerobic, unicellular algae closely related to the green algae Chlorella. Their involvement in human disease--in both immunocompetent and immunocompromised patients--has been reported with increasing frequency. The wide array of presentations has included cutaneous, subcutaneous, mucosal, bursal, catheter-related, and (in rare instances) systemic disease. We report a case of protothecosis complicating prolonged endotracheal intubation presenting as a nasopharyngeal ulceration with a soft-tissue mass, and we review the presentation, treatment, and outcome of th

A review of protothecosis in dogs revealed that this malady usually begins in the gastrointestinal tract and progresses to systemic involvement. Clinical signs generally include bloody diarrhea or blood-stained feces as well as blindness, ataxia, and polyuria. Histologically, myriads of protothecal organisms in different stages of development are found in the granulomatous lesions. Two main species have been culturally identified: Prototheca zopfii and P wickerhamii. In the absence of cultural studies, species identification can be accomplished readily by immunofluorescence. The present case i
