Cytoreductive Surgery and Hyperthermic Chemotherapy for Intrathoracic Pseudomyxoma Peritonei.

Dissemination of pseudomyxoma peritonei into the thoracic cavity is rare and carries a poor prognosis. Optimal treatment has not been defined. In our institution, patients have received cytoreductive surgery with hyperthermic intrathoracic chemotherapy, which demonstrated a good prognosis. A 51-year-old woman was referred to our hospital with right intrathoracic dissemination of pseudomyxoma peritonei. We performed parietal and mediastinal pleurectomy and simple resection of all visible tumors on the diaphragm and visceral pleura, followed by hyperthermic intrathoracic chemotherapy with 20 mg

Patient Survival after Surgical Management in Intrathoracic Pseudomyxoma peritonei.

BACKGROUND: Pseudomyxoma peritonei (PMP) is a rare disease, characterized by mucinous ascites and deposits diffusely present on the peritoneal surfaces. However, extension of PMP to the thoracic cavity is extremely rare. Although there are a few case reports on the long-term postoperative prognosis of intrathoracic lesions, whether surgical resection of these lesions can improve patient prognosis remains unclear. METHODS: We reviewed 17 patients with PMP who underwent resection of intrathoracic lesions after abdominal cytoreductive surgery and examined their clinical outcome after surgery. R

Carbonic anhydrase II: a novel biomarker for pseudomyxoma peritonei.

Altered expression of carbonic anhydrase (CA) II is associated with human carcinogenesis. We analysed CA II protein expression in 89 patients with pseudomyxoma peritonei (PMP) and correlated its association against survival. We determined the expression of CA II by immunohistochemistry and then scored the staining results. The correlations of CA II expression with Peritoneal Cancer Index (PCI) and tumour grade were examined. The effect of CA II and tumour grade on survival was investigated. Positive CA II expression was found in 58 patients (65%) and absent in 31 patients (35%). High-grade (HG

Endometriosis with myxoid change. A case simulating pseudomyxoma peritonei.

After a diagnosis of endocervical adenocarcinoma was made on examination of a curettage specimen, a 46-year-old woman underwent laparotomy for the purpose of radical hysterectomy. After frozen section of a biopsy specimen obtained from a nodule 2 cm in diameter on the posterior surface of the cervix was interpreted as adenocarcinoma, consistent with pseudomyxoma peritonei, the planned hysterectomy was abandoned; only biopsies of the pelvic and para-aortic lymph nodes and a bilateral salpingo-oophorectomy were performed. Permanent sections of the nodule revealed pools of almost acellular mucin,

Pseudomyxoma peritonei. A localized, benign variant of appendiceal origin.

A case of a huge, self-contained cyst of pseudomyxoma peritonei arising from the rupture of a benign cystadenoma of the appendix is reported. Curative surgical removal involved both small bowel and right colon resection. The pathologic spectrum of the appendiceal mucoceles, from mucosal hyperplasia to benign cystadenoma and malignant cystadenocarcinoma, is presented. The variants of pseudomyxoma peritonei (benign-malignant, localized-diffuse) are discussed and correlated with the appendiceal lesions. Clinicopathologic relationships and treatment options also are covered.

Origin and survival outcomes of Pseudomyxoma peritonei-A retrospective study.

BACKGROUND: Pseudomyxoma peritonei (PMP) is an unusual clinical condition typically presenting with widespread mucinous neoplastic lesions within the peritoneum resulting in gelatin material-rich ascites. It was first described by Werth in 1884. Ever since, its clinical presentation, definition, site of origin, and prognosis have been a subject of debate. However, many histopathologic, immunohistochemical, and genetic studies have attempted to locate the primary lesion in the appendix in both genders. OBJECTIVES: To analyze the histological origin and survival outcomes of pseudomyxoma periton

Acute Chylopericardium With Tamponade and Cardiac Arrest With Pseudomyxoma Peritonei.

A 51-year-old woman with pseudomyxoma peritonei developed cardiac arrest 5 days after surgery. Acute echocardiography demonstrated pericardial tamponade. Emergency pericardiocentesis evacuated milky fluid and circulation was re-established. Analysis of the pericardial fluid suggested chylopericardium. In conclusion, this case demonstrates that chylopericardium may be life-threatening and underlines the importance of acute echocardiography in critical management of patients with unexplained shock.

The Biological Synthesis and the Function of Mucin 2 in Pseudomyxoma Peritonei.

Excessive mucus secretion is the most prominent feature of pseudomyxoma peritonei (PMP), which often leads to significant increase in abdominal circumference, intractable abdominal pain, progressive intestinal obstruction, abdominal organ adhesions, and cachexia. Excessive mucus secretion is also the main cause of death. Cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) is the recommended treatment for PMP. However, recurrence is frequently observed even after CRS and HIPEC, presenting similar clinical manifestations. Mucin 2 (MUC2) is the main type of

Pseudomyxoma peritonei extending to the lower extremity: a case report.

Pseudomyxoma peritonei is characterized by mucinous ascites originating from a mucin-producing neoplasm; however, even the definition is still under debate. Tumor deposits extend and ultimately engulf the entire cavity, causing death from cachexia due to limited intestinal movement. Here, we report a unique case of an 80-year-old woman with pseudomyxoma peritonei, which extended to the lower extremity mimicking infectious condition. The patient survived for a long time without bowel obstruction despite having the histologic subtype that has an unfavorable prognosis. The extremity lesion was tr

Pseudomyxoma peritonei: a clinical case of this poorly understood condition.

BACKGROUND: Pseudomyxoma peritonei is an uncommon condition with an estimated incidence of one to two per million (worldwide) per year. It is characterized by the peritoneal deposition of mucinous tumors, most commonly of the appendix, and occasionally from the ovary, coupled by mucinous ascites. CASE PRESENTATION: We report the case of a 76-year-old woman who presented with increased abdominal girth and dyspnea for 2 weeks. She was diagnosed as a case of pseudomyxoma peritonei. She was submitted to right oophorectomy, omentectomy, and pseudomyxoma debulking. The histology was compatible with

Laparoscopic evacuation of mucinous ascites for palliation of pseudomyxoma peritonei.

INTRODUCTION: Accumulation of mucinous ascites causes significant morbidity for patients with unresectable pseudomyxoma peritonei (PMP). The success of paracentesis for mucin evacuation is limited due to the presence of adhesions, disease burden, and the viscous nature of mucinous ascites. We sought to review our experience with laparoscopic evacuation of mucinous ascites for palliation of PMP. METHODS: Records were reviewed for patients who underwent laparoscopy for carcinomatosis secondary to appendix or colon cancer from July 2007 to January 2014. Of 123 patients, 10 were identified who un

Clinical determinants of treatment failure in patients with pseudomyxoma peritonei.

Pseudomyxoma peritonei is a mucinous cancer of low biologic aggressiveness that disseminates widely throughout the abdominopelvic cavity prior to diagnosis. Complete control of the disease process on peritoneal surfaces should translate into long-term disease-free survival. In a series of 120 patients with pseudomyxoma peritonei, 46 were defined as treatment failures after cytoreductive surgery and regional chemotherapy. Clinical features that correlated significantly with treatment failure were tumor site (colon vs. appendix), histopathology grade (grade II vs. grade I), preoperative cancer v

Pseudomyxoma peritonei and malignant mucocele of the appendix. A case report.

A case of pseudomyxoma peritonei originating in a malignant mucocele of the appendix, a very rare clinical entity, is presented. The cytologic findings in the gelatinous ascitic fluid of epithelial clusters with minimal nuclear atypia suggested a diagnosis of pseudomyxoma peritonei. Both routine stains and special stains for mucus assisted in making the cytologic diagnosis. Histologic examination of the laparotomy specimen confirmed the presence of a well-differentiated mucinous cystadenocarcinoma, with an apparent appendiceal origin. The origins and relationships of appendiceal mucocele and p

Pseudomyxoma peritonei presenting with pyloric obstruction: a case report.

Pseudomyxoma peritonei (PMP) is a rare malignancy characterized by mucinous ascites. Gastric outlet obstruction (GOO) due to extrinsic compression is exceptionally uncommon. We present a 76-year-old male with high-grade appendiceal mucinous neoplasm presented with progressive abdominal pain, nausea, and vomiting. Imaging demonstrated extensive peritoneal disease causing pyloroduodenal compression. Five sequential self-expanding metal stents were placed, providing only transient relief due to persistent compression and tumour overgrowth. With worsening nutritional status, a multidisciplinary te

Surgical management of pseudomyxoma peritonei: low-volume center experience.

INTRODUCTION: Pseudomyxoma peritonei (PMP) is a rare disease characterized by diffuse peritoneal invasion of multifocal mucinous deposits. Gold standard treatment involves cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC). We aimed to compare the outcomes of operated PMP patients in a low-volume center, with those of high-volume centers and using other regimens. MATERIALS AND METHODS: Retrospective analysis was conducted for PMP patients treated by CRS + HIPEC (using the Elias high-dose Oxaliplatin HIPEC regimen) over a 15-year period (January 2007 - December

[Clinical Features and Management of Pseudomyxoma Peritonei Appendiceal Origin].

Pseudomyxoma peritonei(PMP)is a rare clinical entity that typically arises from the rupture of a low-grade appendiceal mucinous neoplasm(LAMN), resulting in accumulation of gelatinous mucinous material throughout the peritoneal cavity. Despite its low-grade histological features, PMP can be clinically aggressive due to continuous mucin production, progressive peritoneal dissemination, and organ compression. Patients often present with abdominal distension, palpable mass, or gastrointestinal symptoms, and the disease may occasionally be discovered during surgery for presumed appendicitis or ova

Precision Oncology and Systemic Targeted Therapy in Pseudomyxoma Peritonei.

PURPOSE: Pseudomyxoma peritonei (PMP) is a rare and poorly understood malignant condition characterized by the accumulation of intra-abdominal mucin produced from peritoneal metastases. Currently, cytoreductive surgery remains the mainstay of treatment but disease recurrence and death after relapse frequently occur in patients with PMP. New therapeutic strategies are therefore urgently needed for these patients. EXPERIMENTAL DESIGN: A total of 120 PMP samples from 50 patients were processed to generate a collection of 50 patient-derived organoid (PDO) and xenograft (PDX) models. Whole exome s

Appendiceal pseudomyxoma peritonei: predictors of recurrence and iterative surgery.

AIM: Appendiceal pseudomyxoma peritonei (PMP) is a rare entity, with recurrence rates up to 26% despite optimal cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Evidence specific to PMP originating from non-infiltrative appendiceal mucinous neoplasms (low grade - LAMN and high grade - HAMN) is lacking. The aim of this study was to identify patterns of recurrence and predictive factors for patients appropriate for iterative surgery. METHOD: A bi-institutional retrospective analysis was performed on patients undergoing complete cytoreduction and HIPEC for PMP d

Pseudomyxoma Peritonei of Extra-Appendiceal Origin: A Comparative Study.

BACKGROUND: Pseudomyxoma peritonei (PMP) usually originates from appendiceal neoplasms and, less commonly, from extra-appendiceal lesions. To date, the clinical and therapeutic implications of extra-appendiceal origin are largely unknown. METHODS: A prospective database of 225 PMP patients uniformly treated by cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) was reviewed to identify cases with extra-appendiceal primaries. Histologically, negative appendix defined extra-appendiceal origin. Clinical, pathological, and immunohistochemical features (cytokeratin [C
