Mania in AIDS: clinical significance and theoretical considerations.

Cases of mania associated with acquired immune deficiency syndrome (AIDS) are reviewed in an attempt to elucidate patterns that may be helpful in guiding treatment, determining prognosis, and understanding pathophysiology. Fourteen well-described cases in the English language literature were critically reviewed. Data was collected regarding chronological appearance of signs and symptoms, specific psychiatric symptoms, associated neurologic and cognitive function, objective testing of brain structure and function, and outcome. When mania or hypomania occur in the setting of a human immunodefici

Depression, HIV dementia, delirium, posttraumatic stress disorder (or all of the above).

A 29-year-old single Puerto Rican woman with AIDS was admitted to the Medical Service for pneumonia, seen by the Psychiatric Consultation Service, an eventually transferred to the Inpatient Psychiatric Unit with several possible psychiatric diagnoses including major depression, HIV dementia, delirium, and posttraumatic stress disorder. These possibly coexisting and interacting syndromes are discussed by three psychiatrists, one of whom is also a board-certified neurologist. This case illustrates the combined contribution of organic and psychological factor to complex behavioral disorders, whic

Common psychiatric problems in homosexual men and women consulting family physicians.

This paper describes the commoner psychiatric difficulties of homosexual patients seen in a family practice. It is written primarily to aid general practitioners in deciding who should be referred to a psychiatrist. Recent developments in the study of homosexuality indicate that homosexual persons are no more likely than heterosexuals to suffer psychiatric problems. Practical suggestions are made for management and counseling, and some of the misconceptions about the gay community are dispelled.

Secondary mania due to AIDS and cryptococcal meningitis in a 78-year-old patient.

We report a 78-year-old man without past psychiatric history who experienced his first manic episode successfully treated with quetiapine and lorazepam, but was ultimately found to have AIDS and Cryptococcus neoformans meningitis. Our presented case highlights the importance of comprehensive differential diagnoses to rule out secondary causes of psychiatric symptoms presenting for the first time in elderly patients.

Psychiatric predictors of pseudoepileptic seizures in patients with refractory seizures.

This study compared psychiatric factors and diagnoses in patients with pseudoepileptic seizures and genuine seizures. A semistructured interview identified psychiatric diagnosis and the presence of psychiatric factors associated with conversion phenomena. Twenty-five subjects with medically refractory seizures were interviewed and followed prospectively until a definitive diagnosis of pseudoepileptic or genuine seizures was made by neurologists using video-EEG telemetry. Four factors help to identify patients with pseudoepileptic seizures: a psychiatric diagnosis of either somatization disorde

Psychologic aspects of acne.

Psychologic factors may play a significant role in acne in at least three ways. First, as many patients readily report, emotional stress can exacerbate acne. Second, it is common for patients to develop psychiatric problems as a consequence of the conditions, such as those related to low self-esteem, social phobias, or depression. Finally, primary psychiatric illnesses such as obsessive-compulsive disorder and psychosis may be based on a complaint that is focused on acne. In this review we hope to facilitate an understanding of how emotional factors and acne connect, and aid the dermatologist

Biochemical and brain heterogeneity characterizes psychiatric and non-psychiatric illness.

Biological heterogeneity presents a formidable challenge in understanding psychiatric illness, yet it remains underexplored at scale or in a pan-disease setting. We characterized biochemical and brain heterogeneity in 445,374 individuals, considering 9 psychiatric disorders and 30 non-psychiatric medical conditions. Psychiatric and non-psychiatric illnesses were characterized by greater biological heterogeneity than predicted by healthy variation. Notably, heterogeneity in psychiatric disorders was comparable to non-psychiatric illnesses and often diverged from diagnostic classification. Circu

Correlates of psychiatric distress among wives of hemophilic men with and without HIV infection.

OBJECTIVE: The authors' objectives were 1) to examine symptoms of depression, anxiety, and anger-hostility among the wives of men with hemophilia, a major risk group for AIDS, and 2) to identify psychosocial characteristics of the women and/or their husbands that were associated with elevated distress in the women. METHOD: Thirty-six women married to men with hemophilia were studied; the husbands of 17 of these women were HIV-seropositive. The men were drawn from the population of adults with hemophilia residing in a 24-county region of western Pennsylvania. Measures of wives' psychiatric sym

Major depressive disorder with delusion of having AIDS: a case report.

Acquired immune deficiency syndrome (AIDS) has become a topic of increasing public concern. Several conditions have been described in patients who are afraid that they have acquired AIDS, including depression, somatization disorders, hypochondriasis, adjustment disorders and various psychoses. This paper presents a case study of a young man with persistent fear of having AIDS. In spite of negative HIV test results, the patient still insisted that he suffered from AIDS. The psychiatric diagnosis was major depressive disorder with delusion of having AIDS. The patient was treated with fluoxetine

Psychiatric and psychosocial correlates of sexual risk behavior among adults with severe mental illness.

Persons with severe mental illness (SMI) are disproportionately affected by HIV/AIDS. This study examined multivariate correlates of sexual risk among 152 adults with SMI receiving outpatient psychiatric treatment. Structured interviews assessed psychiatric, psychosocial, and behavioral risk factors. The majority was sexually active (65%), and many reported unprotected intercourse (73%), multiple partners (45%), and sex trading (21%) in the past year. Logistic regression models found that sexual behaviors were differentially associated with non-psychotic disorder, psychiatric symptoms, substan

Acquired immune deficiency syndrome as a novel content in delusion symptomatology.

The case histories of three female patients are presented (two of them diagnosed as paranoid schizophrenia, one as endogenous depression in the course of involution), who manifested the delusion of AIDS disease in the psychopathological image of their primary illness. This novel content of delusional symptomatology appears in the thinking of psychiatric patients as a distorted reflexion of a new objective reality of the world, the AIDS disease, becoming recently one of the global problems of the human population.

Unusual intravesical foreign bodies: a report of two cases.

The introduction of a foreign body into the urethra is an exceptionally rare occurrence. It is often secondary, either to erotic curiosity or to psychiatric disorders. The symptomatology is multiple and the diagnosis is aided by radiologic imaging. The extraction is most often done by endoscopic. The management of these patients must be done systematically and will need psychiatric assessment. We report the case of two patients who inserted a foreign body into their urinary bladder. One was 22-year-old and the second was 20-year-old and all with no history of psychiatric disorders. The first a

Quality of parent communication about sex and its relationship to risky sexual behavior among youth in psychiatric care: a pilot study.

BACKGROUND: The number of HIV infections among adolescents is increasing, and youth in psychiatric care are at particular risk because of their high rates of risky sexual behavior. METHODS: As part of a larger longitudinal study examining AIDS-risk behavior among adolescents in psychiatric care, this pilot study investigated the relationship between parent communication about sex and sexual risk-taking among treatment-seeking adolescents. Adolescents reported their risky sexual behavior (e.g., inconsistent condom use, sex with multiple partners), and parents reported how frequently they bring

Treatment of tobacco use disorders in smokers with serious mental illness: toward clinical best practices.

Addiction to tobacco-derived nicotine remains highly prevalent in the United States, with 18% using daily, and 53% of those with serious mental illness using daily. While smokers with serious mental illness have been excluded from most large nicotine-dependence treatment studies, a growing evidence base is available to guide clinicians in assisting their patients with psychiatric illness to quit smoking. The aim of this review is to present the evidence on safety and efficacy of smoking cessation interventions for those with serious mental illness. Smokers with schizophrenia spectrum disorders

18F-fluorodeoxyglucose PET in Major Psychiatric Disorders.

18F-fluorodeoxyglucose (FDG)-PET offers a systems-level view of brain dysfunction in psychiatry, with findings more informative at the circuit and network level than as fixed disease-specific patterns. In schizophrenia, addictions, and mood disorders, reproducible abnormalities involve prefrontal, limbic, striatal, and multimodal associative networks, although their expression depends on clinical state, stage, treatment, and comorbidity. This review summarizes principal FDG-PET findings in major psychiatric disorders and integrates them with selected molecular imaging data. Although FDG-PET is

Influence of medications and diagnoses on fall risk in psychiatric inpatients.

PURPOSE: The influence of medications and diagnoses on fall risk in psychiatric inpatients was evaluated. METHODS: In this retrospective case-control study, psychiatric inpatients age 18 years or older with a documented fall that was reported served as study cases. These patients were matched to control patients from the same hospital (1:1) by admission year, sex, and age. Psychiatric diagnoses evaluated included major depressive disorder, schizophrenia or schizoaffective disorder, bipolar disorder, Alzheimer's disease and dementia, anxiety or neurosis, delirium, personality disorder, and obs

Cryptococcal meningoencephalitis.

A patient was admitted to hospital with an apparent psychiatric disturbance. When she became stuporous the cerebrospinal fluid was cultured but proved sterile. The latex test showed that serum was positive for cryptococcal antigens, and cryptococcal meningoencephalitis was diagnosed. Amphotericin B was given but when she developed a toxic reaction it was replaced by flucytosine. She responded well to flucytosine alone and no side effects appeared on continued treatment. Cryptococcal meningitis may present as a psychiatric disturbance, and serological tests are invaluable aids to diagnosis.

Effectiveness of dual focus mutual aid for co-occurring substance use and mental health disorders: a review and synthesis of the "Double Trouble" in Recovery evaluation.

Over 5 million adults in the United States have a co-occurring substance use disorder and serious psychological distress. Mutual aid (self-help) can usefully complement treatment, but people with co-occurring substance use and psychiatric disorders often encounter a lack of empathy and acceptance in traditional mutual aid groups. Double Trouble in Recovery (DTR) is a dual focus fellowship whose mission is to bring the benefits of mutual aid to persons recovering from co-occurring disorders. An evaluation of DTR was conducted by interviewing 310 persons attending 24 DTR meetings in New York Cit

The Catastrophic Effects of Psychogenic Polydipsia: A Case Report.

Patients with hyponatremia are at risk of severe complications including seizures, coma, and death. Psychiatric patients are particularly susceptible to death from hyponatremia due to the association between psychiatric conditions and psychogenic polydipsia, characterized by water intoxication. We report a case of a schizophrenic patient who presented with altered mental status, leading to a differential diagnosis narrowed through clinical investigations to include hypovolemic hyponatremia, syndrome of inappropriate antidiuretic hormone secretion (SIADH), and psychogenic polydipsia.
