Heavy users of an emergency department: psycho-social and medical characteristics, other health care contacts and the effect of a hospital social worker intervention.

A small proportion of the patients coming to emergency departments of general hospitals account for a substantial share of the department's resources by making repeated visits. Repeater behaviour is a complex product of many, sometimes concurrent factors. This study has focused on the medical and psycho-social factors. A group of patients with repeated visits to an emergency department of a middle-sized Stockholm hospital was studied in 1980. The repeater group had a heavier load of psycho-social problems than the source population. The repeater behaviour profile included: feeling of lonelines

Patient rights and law: tobacco smoking in psychiatric wards and the Israeli Prevention of Smoking Act.

In August 2001, the Israeli Ministry of Health issued its Limitation of Smoking in Public Places Order, categorically forbidding smoking in hospitals. This forced the mental health system to cope with the issue of smoking inside psychiatric hospitals. The main problem was smoking by compulsorily hospitalized psychiatric patients in closed wards. An attempt by a psychiatric hospital to implement the tobacco smoking restraint instruction by banning the sale of cigarettes inside the hospital led to the development of a black market and cases of patient exploitation in return for cigarettes. Associations between violence, self-harm and acute psychiatric service use: Implications for inpatient care.

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: Service users who behave violently may suffer. This is because violence can lead to unnecessary medication, seclusion on the ward and strained relationships with other people. It can also affect a service user's self-esteem and lead to feelings of shame. Service users who behave violently can also make life frightening and unpredictable for members of staff, and other service users on the ward. It is important to gain a sound understanding of violence in order to help reduce it. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: Previous research has shown t

Association between neutrophil to lymphocyte ratio and mood polarity in adolescents admitted to an inpatient psychiatric ward.

Inflammatory processes are associated with mood disorders, but data on pediatric patients are scarce. The aim of this study was to investigate a possible association between elevated neutrophil/lymphocyte ratio (NLR) - a marker of inflammation and mood polarity (manic/depressed) in adolescents, admitted between 2010 and 2015 due to a mood disorder episode and to an adolescent inpatient ward. Electronic medical records of 305 patients (aged 10-19 years, 60.6% males) admitted during the study period due to a mood disorder episode were reviewed. Of these, 63 were diagnosed with manic episodes and

Do patient and ward-related characteristics influence the use of coercive measures? Results from the EUNOMIA international study.

PURPOSE: This study aims to identify whether selected patient and ward-related factors are associated with the use of coercive measures. Data were collected as part of the EUNOMIA international collaborative study on the use of coercive measures in ten European countries. METHODS: Involuntarily admitted patients (N = 2,027) were divided into two groups. The first group (N = 770) included patients that had been subject to at least one of these coercive measures during hospitalization: restraint, and/or seclusion, and/or forced medication; the other group (N = 1,257) included patients who had n

Patient receipt of smoking cessation care in four Australian acute psychiatric facilities.

This study aimed to report the receipt of smoking care, and associated clinical and smoking characteristics among smokers admitted to four public psychiatric inpatient facilities in New South Wales, Australia. Between October 2012 and July 2014, adult smokers (N = 236) were surveyed during admission to and 1 month following discharge from the facilities. Measures of smoking care receipt were reported descriptively, and logistic regression analyses were used to explore characteristics associated with care receipt. The majority of participants were offered (78%) and used (78%) nicotine replaceme

Childhood Visual Hallucinations: A Case Report of Idiopathic Occipital Epilepsy.

Although various types of hallucinations are reported at high rates during childhood, the research focusing on visual hallucinations is relatively scarce.1 Visual hallucinations have often been linked to biological causes and have been considered uncommon in psychiatric samples. However, recent studies suggest that almost 30% of visual hallucinations occur in the context of psychosis.1 The risk of psychopathology is often a cause of concern when children report visual hallucinations, especially when associated with poor functioning. A detailed history must be taken to determine whether reporte

Seclusion: the quest to determine effectiveness.

1. The decision whether to use seclusion as an intervention often is based on emotional argument and anecdotal evidence. Little research examining effectiveness as an intervention exists. 2. This study compares two units, one of which uses seclusion, around the parameters of resolution time of emergencies, levels of PRN medication used, and the nurses' perception of the effectiveness of used interventions. 3. Results of the study indicate that around some parameters, seclusion may be more effective than alternate interventions. More research is needed.

Implementing Group Clinical Supervision for Mental Health Nurses in Acute Adult Inpatient Wards. A Qualitative Descriptive Study.

Clinical supervision for health care workers is well established across many disciplines; however, supervision for nurses has proven more difficult to implement and sustain. The purpose of this study was to evaluate the experience of establishing regular, ongoing group clinical supervision for mental health nurses in two acute adult inpatient wards. The study took a qualitative descriptive approach to data collection, gathering and triangulating data from nurses who took part in the supervision sessions, as well as supervision facilitators and ward managers. Data were collected through a brief

The Peregrinating Psychiatric Patient in the Emergency Department.

Many emergency department (ED) psychiatric patients present after traveling. Although such travel, or peregrination, has long been associated with factitious disorder, other diagnoses are more common among travelers, including psychotic disorders, personality disorders, and substance abuse. Travelers' intense psychopathology, disrupted social networks, lack of collateral informants, and unawareness of local resources complicate treatment. These patients can consume disproportionate time and resources from emergency providers. We review the literature on the emergency psychiatric treatment of p

The psychiatric ward environment and nursing observations at night: A qualitative study.

WHAT IS KNOWN ON THE SUBJECT?: Nursing observations at night are conducted on psychiatric wards to ensure the safety and well-being of patients as well as to reduce the risk of suicide or severe harm. To our knowledge, no studies have examined the lived experience of the psychiatric ward environment and of nursing observations at night. WHAT DOES THE PAPER ADD TO EXISTING KNOWLEDGE?: The main complaint from patients was the constant interruption of their sleep, as most of the patients in this study were observed 2-4 times an hour. Their sleep was interrupted by (a) the light from torches shone

Inpatient suicide on mental health units in Veterans Affairs (VA) hospitals: avoiding environmental hazards.

INTRODUCTION: One thousand five hundred suicides take place on inpatient psychiatry units in the United States each year, over 70% by hanging. Understanding the methods and the environmental components of inpatient suicide may help to reduce its incidence. METHODS: All Root Cause Analysis reports of suicide or suicide attempts in inpatient mental health units in Veterans Affairs (VA) hospitals between December 1999 and December 2011 were reviewed. We coded the method of suicide, anchor point and lanyard for cases of hanging, and implement for cutting, and brought together all other reports of

[Velo-cardio-facial syndrome at the psychiatry consulting-liaison service in a general hospital].

Psychotic syndromes secondary to genomic disorders have low prevalence and may easily go unnoticed in the daily clinical practice. The velo-cardio-facial syndrome or DiGeorge syndrome (VCFS/DGS) is the genomic disorder most frequently associated to an interstitial deletion of the 22q11 region, with an incidence of one per every 4,000 newborns. Clinical manifestations constitute a constellation of cardiac, facial, urogenital and psychiatric disorders, among which schizophrenia or schizophreniform disorder stand out with an incidence of about 30% over the lifetime. In the following, we present t

[Schizophrenia and schizophrenia spectrum disorders in general hospital].

The prevalence and clinical presentation of schizophrenia and schizophrenia spectrum disorders in general hospitals were studied. The disease incidence rate was estimated at 3.1% that was 3 times higher than that in general population. Slow-progredient (latent) schizophrenia with comorbid functional visceral disorders ("organoneurotic" schizophrenia) occurs most frequently. Once manifested, the latter was associated with somatoform disorders during the disease course and developed concomitantly with neurotic and overvalued hypochondria symptoms. The authors suggest that several variants of com
