Audiologic Assessment of Tinnitus: Integrating Contemporary Clinical Practice and International Guideline Recommendations.

Audiologic assessment of tinnitus has evolved from a predominantly psychoacoustic evaluation to a comprehensive, patient-centered process that integrates auditory, emotional, cognitive, functional, and quality-of-life factors. Contemporary international guidelines emphasize a detailed case history, a comprehensive hearing assessment, the use of validated outcome measures such as the Tinnitus Functional Index, and screening for hyperacusis, psychological distress, sleep disturbance, and other comorbidities. While psychoacoustic measures are valuable for characterizing tinnitus and planning trea

Tinnitus is associated with reduced sound level tolerance in adolescents with normal audiograms and otoacoustic emissions.

Recent neuroscience research suggests that tinnitus may reflect synaptic loss in the cochlea that does not express in the audiogram but leads to neural changes in auditory pathways that reduce sound level tolerance (SLT). Adolescents (N = 170) completed a questionnaire addressing their prior experience with tinnitus, potentially risky listening habits, and sensitivity to ordinary sounds, followed by psychoacoustic measurements in a sound booth. Among all adolescents 54.7% reported by questionnaire that they had previously experienced tinnitus, while 28.8% heard tinnitus in the booth. Psychoaco

Typewriter tinnitus revisited: The typical symptoms and the initial response to carbamazepine are the most reliable diagnostic clues.

Although neurovascular compression of the cochlear nerve (NVC-C) presenting as typewriter tinnitus is a discrete disease category, verified diagnostic criteria are lacking. We sought to refine the diagnostic criteria for NVC-C by reference to a relatively large case series. The medical records of 22 NVC-C patients were retrospectively reviewed. Psychoacoustic characteristics, the results of diagnostic work-up (including audiovestibular neurophysiological tests and radiological evaluations), and the initial treatment response to carbamazepine were investigated. All subjects described their tinn

Underwater hearing in sea ducks with applications for reducing gillnet bycatch through acoustic deterrence.

As diving foragers, sea ducks are vulnerable to underwater anthropogenic activity, including ships, underwater construction, seismic surveys and gillnet fisheries. Bycatch in gillnets is a contributing source of mortality for sea ducks, killing hundreds of thousands of individuals annually. We researched underwater hearing in sea duck species to increase knowledge of underwater avian acoustic sensitivity and to assist with possible development of gillnet bycatch mitigation strategies that include auditory deterrent devices. We used both psychoacoustic and electrophysiological techniques to inv

Effects of overpressure on hearing function in Menière's disease.

The relationship between ambient pressure changes and hearing function was investigated in 14 patients with well-defined sensorineural hearing losses caused by Meniere's disease. The period of exposure to pressure changes varied individually between one and 4 months. The evaluation of hearing function was made by means of pure-tone audiometry, speech discrimination tests and psychoacoustic measurements of the inner ear frequency selectivity. Significant changes were found in the hearing parameters in relation to the period of pressure application. The use of psychoacoustic tuning curves allowe

Mitigating the toilet flush noise: A psychometric analysis of noise assessment and design of labyrinthine acoustic Meta-absorber for noise mitigation.

Modern apartments in major cities are usually made of lightweight structures with inadequate acoustic properties, leading to a growing number of noise complaints by residents of inadequate sound insulation in high-rise buildings. When a toilet is flushed, loud and irregular sounds like humming, gurgling, or bubbling, "explosive" fury from vacuum-assisted toilets are generated. Though these toilet flushing sounds are not very harmful, they can still be perceived as nuisances by users nearby. The toilet flushing noise has posed a serious concern among apartment occupants and adjacent residents,

Modulation of auditory percepts by transcutaneous electrical stimulation.

Transcutaneous, electrical stimulation with electrodes placed on the mastoid processes represents a specific way to elicit vestibular reflexes in humans without active or passive subject movements, for which the term galvanic vestibular stimulation was coined. It has been suggested that galvanic vestibular stimulation mainly affects the vestibular periphery, but whether vestibular hair cells, vestibular afferents, or a combination of both are excited, is still a matter of debate. Galvanic vestibular stimulation has been in use since the late 18th century, but despite the long-known and well-do

Music for epilepsy: impact of temporal music structure on intervention outcomes.

Previous music interventions for epilepsy have reportedly associated favorable patient outcomes with exposure to Mozart's Sonata for Two Pianos in D Major (K. 448). Other songs have reportedly replicated this phenomenon, raising the possibility that music reportedly associated with favorable outcomes might contain similar properties. This exploratory study investigated this possibility, which could inform a conceptual foundation for prospective works and future hypotheses. Music was assigned to Group A (literature-reported favorable intervention outcomes) or Group B (literature-reported as not

Exploring Objective Correlates to Perceived Severity in the Covarying Breathy, Rough, and Strained Voice Qualities of Dysphonic Voices.

OBJECTIVE: While many research studies evaluate a single voice quality dimension to improve measurement precision, this approach often fails to capture the covariance among more than one voice quality. The goal of the current study was to investigate the relative utility of objective acoustic and bioinspired psychoacoustic-based measurement methods in a set of dysphonic voices that covary in severity from mild to severe along three voice quality dimensions. METHODS: Stimuli included 26 voices that varied across three levels of dysphonic severity (mild, moderate, and severe) in three covarying

Research on sound quality prediction of vehicle interior noise using the human-ear physiological model.

In order to improve the prediction accuracy of the sound quality of vehicle interior noise, a novel sound quality prediction model was proposed based on the physiological response predicted metrics, i.e., loudness, sharpness, and roughness. First, a human-ear sound transmission model was constructed by combining the outer and middle ear finite element model with the cochlear transmission line model. This model converted external input noise into cochlear basilar membrane response. Second, the physiological perception models of loudness, sharpness, and roughness were constructed by transforming

Exploring the role of feedback-based auditory reflexes in forward masking by schroeder-phase complexes.

Several studies have postulated that psychoacoustic measures of auditory perception are influenced by efferent-induced changes in cochlear responses, but these postulations have generally remained untested. This study measured the effect of stimulus phase curvature and temporal envelope modulation on the medial olivocochlear reflex (MOCR) and on the middle-ear muscle reflex (MEMR). The role of the MOCR was tested by measuring changes in the ear-canal pressure at 6 kHz in the presence and absence of a band-limited harmonic complex tone with various phase curvatures, centered either at (on-frequ

Speech perception in children with specific reading difficulties (dyslexia).

Many experimental studies over the last two decades have suggested that groups of children who suffer significant delay in reading also show a weakness in phoneme discrimination and identification. In order to look further at the relation between type of reading deficit, auditory acuity, and speech discrimination, a group of 13 children with specific reading difficulty (SRD), 12 chronological-age controls, and 12 reading-age controls were tested on a battery of speech-perceptual, psychoacoustic, and reading tests. A sub-group of children with Specific Reading Difficulty (SRD) were poor at spee

[Time resolution ability in dichotic hearing experiments (author's transl)].

Concerning auditory analysis of speech signals time resolution is one of the most important ability of the hearing system. In psychoacoustic experiments with dichotic represented bursts of noise the detection and time resolution of short changes of interaural time delay and noise coherence are investigated. The results of normal and pathological binaural hearing are discussed and compared to other auditory time analysis experiments.

Habilitation beyond the Bionic Metaphor: Producing Deafnesses of the Future.

In this article, we travel back to the early days of experimental use of cochlear implants (CIs) in the 1970s, when unsettled expectations of the device and broad investigations of its effects began to settle and center on speech outcomes. We describe how this attention to speech outcomes coalesced into specific understandings of what CIs do, and how implicit or explicit understandings of CIs as bionic devices that normalize hearing influenced research on and expectations of CIs into the present. We conclude that accumulated evidence about what is known and unknown about experiences and materi

[Pulmonary crackles, what does the clinician hear?].

INTRODUCTION: The overall duration of a pulmonary crackle is usually less than 20-30 ms but psychoacoustics demonstrates that an acoustical event with a duration of less than 20-40 ms cannot be estimated in terms of pitch and duration. We pose the hypothesis that the main resonant information is contained into the breath sounds following the crackle. METHODS: Eight patients with COPD, viral pneumonia, bronchiectasis, congestive heart failure, hypoproteinemia and fibrosing alveolitis were recruited for this study. Thirty-six crackles were analyzed in time and frequency domains; 12 in each cate

Contextual effects on loudness judgments for sounds with continuous changes of intensity are reflected in nonauditory areas.

Psychoacoustic research suggests that judgments of perceived loudness change differ significantly between sounds with continuous increases and decreases of acoustic intensity, often referred to as "up-ramps" and "down-ramps." The magnitude and direction of this difference, in turn, appears to depend on focused attention and the specific task performed by the listeners. This has led to the suspicion that cognitive processes play an important role in the development of the observed context effects. The present study addressed this issue by exploring neural correlates of context-dependent loudnes
