Pattern of Changes during Treatment: A Comparison between a Positive Psychology Intervention and a Cognitive Behavioral Treatment for Clinical Depression.

Research on psychotherapy has traditionally focused on analyzing changes between the beginning and the end of a treatment. Few studies have addressed the pattern of therapeutic change during treatment. The aim of this study was to examine the pattern of changes in clinical and well-being variables during a cognitive behavioral therapy (CBT) program compared with an integrative positive psychology interventions program for clinical depression IPPI-D. 128 women with a diagnosis of major depression or dysthymia were assigned to the CBT or PPI group. A measure of depressive symptoms (i.e., Beck De

Substance use among women: associations with pregnancy, parenting, and race/ethnicity.

OBJECTIVE: To examine the association of pregnancy and parenting (0- to 2-year-old child) statuses with past 30-day use of alcohol, cigarettes and marijuana, psychotherapeutics and cocaine among American women aged 18-44, overall and by race/ethnicity. METHODS: Five years of data (2002-2006) from the National Survey on Drug Use and Health (NSDUH) were pooled. Binary logistic regression analyses stratified by race/ethnicity were performed to examine the relationships adjusting for age, marital status, education and family income. RESULTS: Overall, past 30-day alcohol, cigarette, marijuana, ps

Enhancing cognitive accessibility in existential -psychotherapy for adults with brain tumors: A patient-informed approach to intervention adaptation.

Individuals with primary or secondary brain tumors experience double vulnerability-significant emotional distress alongside cognitive decline. These cognitive changes often prohibit their inclusion in psychotherapeutic interventions due to restrictive eligibility criteria; however, simple accommodations including appointment reminders, therapy topic refreshers, prior session summaries, and additional time for processing information may support cognitive accessibility and enable engagement in interventions that promote well-being. Implementing such accommodations is vital to promoting neuro-onc

The Narrative-Emotion Process Coding System 2.0: A multi-methodological approach to identifying and assessing narrative-emotion process markers in psychotherapy.

OBJECTIVE: Recent studies suggest that it is not simply the expression of emotion or emotional arousal in session that is important, but rather it is the reflective processing of emergent, adaptive emotions, arising in the context of personal storytelling and/or Emotion-Focused Therapy (EFT) interventions, that is associated with change. METHOD: To enhance narrative-emotion integration specifically in EFT, Angus and Greenberg originally identified a set of eight clinically derived narrative-emotion integration markers were originally identified for the implementation of process-guiding therap

Process Rounds to Elevate Psychological Skills in Palliative Care.

BACKGROUND: A half-day workshop improved palliative care clinicians' ability to integrate psychological concepts into serious illness communication but created demand for longitudinal learning. OBJECTIVE: To pilot "Process Rounds," a four-session, case-based, adapted psychotherapeutic supervision group reinforcing formulation, countertransference, and mindful intervention. METHODS: Workshop graduates from four cohorts were invited; 25/143 enrolled. Each cohort met for four 60-minute sessions over 6-8 weeks. Eight-week postworkshop surveys included specific items for Process Rounds participan

Change during psychotherapy: the perspective of children and adolescents who have been sexually abused.

The aim of this research was to identify the meanings of psychotherapeutic change of children and adolescents who have suffered sexual abuse and were in psychotherapy. In order to do this, a qualitative study was carried out in which in-depth interviews complemented with drawings were conducted with 10 children and adolescents, aged between 6 and 16 years, who were taking part in psychotherapy due to sexual abuse. The interviews took place between 6 and 10 months after the start of the therapy and before it ended. Thematic narrative analysis was used for the interviews and visual narrative ana

Mindfulness and progressive muscle relaxation as standardized session-introduction in individual therapy: A randomized controlled trial.

OBJECTIVE: There is scarce research on the effects of mindfulness in individual therapy. As many practitioners integrate mindfulness exercises into individual therapy, empirical evidence is of high clinical relevance. METHOD: We investigated the effects of a session-introducing intervention with mindfulness elements (SIIME) in a randomized, controlled design. The effects of SIIME on therapeutic alliance and symptomatic outcome were compared with progressive muscle relaxation (PMR) and treatment-as-usual (TAU) control conditions. The sample comprised 162 patients with anxiety and depression.

Mindfulness as a transtheoretical clinical process.

The use of mindfulness in psychotherapy has garnered the attention of both researchers and therapists over recent years. Based on established research, use of mindfulness with clients is recommended to improve awareness during sessions, reduce ruminative thinking patterns, and increase self-compassion regardless of theoretical orientation. In this article, de-identified clinical material is used to illustrate both informal and formal mindfulness training in session. Further, we provide illustrations of presession and within-session therapist mindfulness, recommending that therapists develop th

[Adaptation of psychodrama in psychotherapy of patients with anorexia nervosa and bulimia nervosa].

The aim of the article was an attempt to present selected theoretical motifs and moreover self experience in the adaptation of elements of psychodrama by Moreno in psychodynamic psychotherapy (individual and group psychotherapy) in a group of people with anorexia and bulimia nervosa. Psychodrama through own creativity, spontaneity and taking action on the "here and now" stage helps to attain and intensify therapeutic aims which concern the consciousness of inner conflict of persons with anorexia and bulimia nervosa, which is translocated on their body.

Hysteria and mourning--a psychosomatic case.

Freud early in the history of analysis viewed hysteria in relation to trauma. This became a subject of great interest to Ferenczi particularly in his clinical activities in trying to find the balance between fantasy and trauma. The enactment in the body, as a psychosomatic symptom, is a way of unconsciously drawing attention to that which cannot be processed emotionally. A young man with a long-standing obsessional character had a sudden onset of motor disturbance. A recent set of family calamities triggered a psychosomatic defence against his having to be affectively in touch with a breakdown

A case illustration of resistance from a cognitive perspective.

Brian, a 36-year-old, single, white male, entered cognitive therapy in response to a depressive episode precipitated by the loss of a job. In addition to his Major Depression, the client met diagnostic criteria for Dysthymia and Personality Disorder Not Otherwise Specified. The first three sessions focused on Brian's unemployment crisis and related dysphoria, as well as his passive-avoidant approach to life. Brian collaborated with the therapist in formulating a treatment plan and quickly found a new job, whereupon he abruptly withdrew from therapy. Shortly thereafter, Brian contacted the ther

[Modular psychotherapy-Toward personalized psychotherapy].

Globally, nearly 1 billion people are affected by mental disorders. Psychotherapy, despite its demonstrated efficacy, often shows only limited effects. Current estimates suggest that fewer than half of the patients achieve a clinically relevant symptom reduction. Against this background, innovative psychotherapeutic approaches are gaining importance, particularly personalized treatments, such as modular psychotherapy. Modular psychotherapy refers to treatment programs composed of distinct, functionally defined units (so-called modules) that are independently effective and can be flexibly combi

Fostering coping as a mechanism of symptom change in cognitive behavioural therapy for psychosis.

INTRODUCTION: Cognitive behavioural therapy for psychosis (CBTp) has been repeatedly shown to ameliorate psychotic symptoms. However, so far we have little understanding of the mechanisms of change in CBTp. In this study, we tested whether improved cognitive and behavioural efforts to manage taxing external or internal demands (=coping) constitute a mechanism of change in CBTp. METHODS: Using data from a published trial for a secondary analysis, we tested whether the continuous symptomatic improvement of patients (n = 57) who received up to 45 sessions of CBTp and completed weekly self-assess

Disentangling the impact of resistance and ambivalence on therapy outcomes in cognitive behavioural therapy for generalized anxiety disorder.

Resistance and ambivalence about change are increasingly recognized as important determinants of treatment outcomes. Moreover, resistance and ambivalence are thought to be theoretically related in that clients who are more ambivalent about change are more likely to demonstrate resistance to the process and tasks of treatment. In the context of cognitive behavioural therapy (CBT) for generalized anxiety disorder, the present study simultaneously examined early resistance and ambivalence using two observer-based coding systems in order to determine their inter-relationship and, importantly, to i

Cohesion and outcome in short-term psychodynamic groups for complicated grief.

This study used two measures of cohesion for the process analysis of 12 short-term, time-limited groups for complicated grief. The measures had similar theoretical definitions but differed in terms of rater source (member vs. observer), measurement score (mean of items vs. global rating), and rating unit (individual vs. group). We examined the relationship between the measures, assessed the development of cohesion over the life of the group, and evaluated each measure's relationship to outcome. A principal components analysis with each measure yielded one cohesion component, which supported a

[Modular acute psychotherapy-Background, concept and implications for psychiatric and psychotherapeutic training : A consensus paper of the German Society for Psychiatry and Psychotherapy, Psychosomatics and Neurology (DGPPN)].

BACKGROUND: The application of disorder-specific psychotherapy often requires a minimum level of joint treatment, clarification of goals and a therapeutic alliance. In acute psychiatric treatment these prerequisites are not always initially present: admissions can be unplanned or involuntary, diagnostic classification can be uncertain and shared goals and a viable therapeutic alliance first need to be established. OBJECTIVE: The modular acute psychotherapy (MAPT) is presented as an integrative psychotherapeutic concept for the specific requirements of acute psychiatric treatment and its impli

Development of a nursing-led psychotherapeutic programme to promote coping in adults living with cancer: a qualitative programme development study.

BACKGROUND: Emotional distress is highly prevalent among adults living with cancer and may compromise psychological adaptation. Although psychosocial interventions are widely implemented, there is limited evidence regarding the structured development of nursing-led psychotherapeutic programmes grounded in nursing theory. AIMS: To develop a structured, theory-informed, nursing-led psychotherapeutic programme aimed at promoting coping and psychological adaptation in adults living with cancer. METHODS: This qualitative study reports the final stages of a multi-phase programme development proces

Mediating Mechanisms in Cognitive Behavioral Therapy for Childhood OCD: The Role of Dysfunctional Beliefs.

Reframing cognitions is assumed to play an important role in treatment for obsessive-compulsive disorder (OCD). However, there hardly is any empirical support for this assumption, especially for children. The aim of this study was to examine if changing dysfunctional beliefs is a mediating mechanism of cognitive behavioral therapy (CBT) for childhood OCD. Fifty-eight children (8-18 years) with OCD received CBT. Dysfunctional beliefs (OBQ-CV) and OCD severity (CY-BOCS) were measured pre-treatment, mid-treatment, post-treatment, and at 16-week follow-up. Results showed that OCD severity and dysf

The clinical practice of Cognitive Behavioural Therapy for children and young people with a diagnosis of Asperger's Syndrome.

Children and young people diagnosed with Asperger's Syndrome (AS) have significant social-communication difficulties and impaired empathy and theory of mind skills. These difficulties place them at risk of developing mental health problems, particularly anxiety, depression and obsessive compulsive disorder. Although Cognitive Behavioural Therapy (CBT) is recognised as an effective intervention for these problems in both child and adult populations, little research has specifically looked at the use of CBT with children and young people with an AS diagnosis. However, limited evidence suggests t
