Brief interventions for chronic pain: Approaches and evidence.

Various countries have published national guidance supporting the integration of behavioral approaches into chronic pain treatment. Yet multiple barriers prevent broad patient access. Brief treatment formats may address universal shortcomings of therapists and resources and offer patients expanded access to care through lower costs and treatment burdens. This article summarizes published evidence for eight identified therapist delivered brief behavioral pain interventions (operationalized as 1-4 treatment sessions or ≤8 h total treatment time) for adults with chronic pain (≥18 years of age) in

Adjuvant psychological therapy for patients with cancer.

Cancer-related psychosocial morbidity has been documented in a substantial minority of patients. In some patients, such morbidity persists for many years even when they are free of disease. Consequently, methods of reducing psychosocial morbidity by psychological treatment should be investigated. We describe Adjuvant Psychological Therapy (APT)--a brief treatment programme designed specifically for patients with cancer. APT is currently being evaluated in a randomized, controlled study at The Royal Marsden Hospital.

Rumination in a 7-year-old child.

Regurgitation of food by rumination has rarely been reported in latency-age children. A 7-year-old boy had symptoms of rumination, gagging, bruxism, and enuresis. During the oppositional stage of development many unresolved conflicts had developed between the patient and his parents. Short-term psychotherapy which focused on resolution of the power struggles produced rapid remission of the symptoms. A two-year follow-up showed no recurrence of difficulty.

Protein extract as a surrogate mother.

Brief dynamic psychotherapy, carried out by an resident in training, can bring significant improvement to a patient who does not necessarily meet ideal selection criteria. S. had experienced early childhood deprivation resulting in adult psychosomatic manifestations of feeling hypoglycemic under pressure of responsibility or during fear of abandonment. He soothed himself with the aid of a bottle of "protein extract," which he carried at all times on his belt. The 21-week therapy resulted in his increased ability to differentiate the sources of his anxiety and allowed him to find new and more m

The Masters and Johnson treatment program for dissatisfied homosexual men.

Certain individuals who want to change their homosexual preference can be helped by a short-term intensive intervention adapted from the Masters and Johnson model for treating heterosexual disorders. Pretreatment counseling aims at improving the patient's dating, sexual, and intimacy skills; creative problem solving; and stress management. Then for 2 weeks the patient and an opposite-sex partner, living in social isolation, meet daily with a therapy team to address fears and anxieties about intimacy as they arise. The directive psychotherapy format alternates between confrontation of maladapti

The effect of psychotherapy in improving physical and psychiatric symptoms in patients with functional dyspepsia.

OBJECTIVE: Functional Dyspepsia (FD) is a common symptom of upper gastrointestinal discomfort. Few data are available on the role of psychotherapy in the treatment of dyspeptic syndromes. This study assesses whether brief core conflictual relationship theme (CCRT) psychoanalytic psychotherapy improves gastrointestinal and psychiatric symptoms in patients with functional dyspepsia. METHODS: A randomized, controlled trial was planned in two educational hospitals in city of Babol. Forty-nine patients with FD were randomly assigned to receive standard medication treatment with CCRT psychotherapy

Psychological interventions for co-occurring depression and substance use disorders.

BACKGROUND: Comorbid depression and substance use disorders are common and have poorer outcomes than either disorder alone. While effective psychological treatments for depression or substance use disorders are available, relatively few randomised controlled trials (RCTs) have examined the efficacy of these treatments in people with these comorbid disorders. OBJECTIVES: To assess the efficacy of psychological interventions delivered alone or in combination with pharmacotherapy for people diagnosed with comorbid depression and substance use disorders. SEARCH METHODS: We searched the following

Comment on "Avoidant Personality Traits and Avoidant Coping in Cognitive-Behavioral Therapy vs. Short-Term Psychodynamic Psychotherapy for Adult Depression".

We critically appraise the study by de Bruin et al., which examined whether avoidant personality traits and avoidant coping predict differential outcomes of cognitive-behavioral therapy (CBT) versus short-term psychodynamic psychotherapy (STPP) in adult depression. Although the authors address an important clinical question regarding dimensional personality assessment in treatment selection, several methodological issues may limit the strength of their conclusions. First, avoidant personality traits were operationalized using a composite score derived from subtracting extraversion from neuroti

One minute of grief: emotional processing in short-term dynamic psychotherapy for adjustment disorder.

OBJECTIVE: Depth of emotional processing has shown to be related to outcome across approaches to psychotherapy. Moreover, a specific emotional sequence has been postulated and tested in several studies on experiential psychotherapy (Pascual-Leone & Greenberg, 2007). This process-outcome study aims at reproducing the sequential model of emotional processing in psychodynamic psychotherapy for adjustment disorder and linking these variables with ultimate therapeutic outcome. METHOD: In this study, 32 patients underwent short-term dynamic psychotherapy. On the basis of reliable clinical change st

The use of a group meal in the brief group therapy of bulimia nervosa.

The group meal is a technique that can be easily incorporated into traditional forms of short-term group therapy for bulimia nervosa. It is a modification of the exposure and response-prevention model that has been used in the treatment of this population. Patients plan and eat a meal together with the group therapist during one of the group therapy sessions. Over the course of the meal, patients discuss thoughts and feelings as they experience them, allowing for greater depth of therapeutic work than when simply recalling events that occurred between sessions. A case example is described to i

[Decoupling or ultra-short therapy of torticollis].

S. Rentrop's survey of literature on the history of the psychotherapeutic treatment of torticollis (76 titles) last year has encouraged us to contribute two examples of treatment to the discussion. The large number of theories, explanations, and treatment methods reveals the wide variety of increasingly effective treatment techniques, but it also makes apparent the dilemma involved in the search for a clear and comprehensive theory. On the basis of two case studies, we would like to show how torticollis symptoms could be eliminated with the help of a one-time focus on a largely unconscious tra

[1 case of anorexia nervosa].

After a brief survey on etiopathogenesis, clinic, paraclinic and differential diagnosis of the disease, the authors describe a patient, who at the age of 18 developed Anorexia nervosa, with a progressive body weight reduction from 70 to 30 kg. After the consultation with a psychiatrist the patient was admitted to a psychoneurological dispensary for treatment. Attention is drawn to the diagnostic difficulties and the internist was underlined, has to think and diagnose that, even though rare disease.

Treatment of a transvestic fetishist with cognitive-behavioral therapy and supportive psychotherapy: case report.

Transvestic fetishism is a paraphilia marked by recurrent, intense sexually arousing fantasies, sexual urges, or behavior involving cross-dressing, in a heterosexual male. There are many explanations of the pathogenesis, but none are conclusive. Different treatments have been applied, but they generally remain obscure and disappointing. Transvestic fetishists rarely seek psychotherapy, because of their dynamic balance between perversion and intrapsychic disintegration. There are few studies, either qualitative or quantitative, associated with transvestic fetishism in Taiwan. This case report d

Solution-Focused Brief Therapy for Stuttering in the Public Schools: Children Solve Their Own Stuttering Problems in This Case Study.

PURPOSE: This clinical focus article follows the case studies of three school-age children who stutter in solution-focused brief therapy (SFBT), highlighting treatment features and demonstrating positive outcomes. Empowerment and self-agency are emphasized as desired characteristics. Children searched within themselves and acted to influence therapy results. Techniques such as self-disclosure and fluency shaping were incorporated into this approach. METHOD: In a public school district, participants with moderate-to-severe stuttering used the Clinical Use of Self-Reports to measure their perce

[Significance of brief interventions in the healthcare supply chain of eating disorders: a narrative review].

So far there is no comprehensive overview on brief outpatient interventions in eating disorders. The specific relevance of psychotherapeutic brief interventions for Anorexia Nervosa, Bulimia Nervosa and Binge Eating Disorder is presented against the background of current healthcare supply chains. This review is based on a literature search that evaluated relevant publications in applicable literature databases. The articles were excerpted and are presented in a narrative overview. In summary, the literature shows a marginal expansion of healthcare provision towards personnel-efficient and cost

Cognitive-behavioral group psychotherapy of bulimia nervosa: importance of logistical variables.

Although much of the psychotherapy for psychiatric disorders is conducted on a weekly basis, several researchers in the field of bulimia nervosa have utilized a more intensive approach as a means to strengthen treatment effects. A second issue concerns the amount of emphasis that should be placed on encouraging the interruption of bulimic symptoms early in treatment. In the current study we systematically studied these two issues. Subjects were randomly assigned to one of four forms of cognitive-behavioral group psychotherapy, the four cells differing on the variables of intensity and emphasis
