BACKGROUND: A protective intra-operative lung ventilation strategy has been widely recommended for laparoscopic surgery. However, there is no consensus regarding the optimal level of positive end-expiratory pressure (PEEP) and its effects during pneumoperitoneum. Electrical impedance tomography (EIT) has recently been introduced as a bedside tool to monitor lung ventilation in real-time.
OBJECTIVE: We hypothesised that individually titrated EIT-PEEP adjusted to the surgical intervention would improve respiratory mechanics during and after surgery.
DESIGN: Randomised controlled trial.

BACKGROUND: The energy transferred from the mechanical ventilator to the respiratory system over time, so-called mechanical power, may cause lung injury even with protective ventilation. This study hypothesized that protective ventilation aiming at moderate lung expansion results in less mechanical power and lung injury than aiming at permissive atelectasis or maximum lung expansion.
METHODS: Twenty-four anesthetized pigs with acute lung injury induced by saline lung lavage were randomly assigned to ventilation according to either the Acute Respiratory Distress Syndrome Clinical Network's low

INTRODUCTION: Spontaneous pneumothorax in dogs is predominantly caused by the rupture of air-filled lesions, such as bullae or blebs. The efficacy of Computed Tomography (CT) in detecting these lesions has been deemed limited due to its reportedly low sensitivity. This retrospective, cross-sectional study investigates the utility of CT in eight dogs diagnosed with recurrent pneumothorax, all of which had surgical confirmation of the cause of the pneumothorax.
MATERIALS AND METHODS: Thoracic radiographs were obtained before and the day following the CT studies. Initially, a CT study was conduc

BACKGROUND: Clinicians often administer 100% inspired oxygen (fraction of inspired oxygen [Fio2] = 1) for washout of volatile anaesthetics before tracheal extubation, potentially aggravating atelectasis formation and compromising oxygenation. We evaluated whether lower Fio2 during emergence from anaesthesia reduces atelectasis and improves oxygenation.
METHODS: We conducted a single-centre, randomised controlled trial (NCT06538740) in 48 adult participants undergoing elective surgery under general anaesthesia. Participants were randomised to receive either pure oxygen (100%) or lower (70% or

Rounded atelectasis is an uncommon form of pulmonary collapse that presents as an asymptomatic lung mass, and thus, can appear very similar to a pulmonary neoplasm. One feature that has been used to differentiate rounded atelectasis from a pulmonary malignancy is the lack of growth observed in cases of rounded atelectasis. The case presented here demonstrates that it is possible for rounded atelectasis to increase in size. This is the first documented case of enlargement of rounded atelectasis and shows that this disorder should be added to the list of benign causes of an enlarging pulmonary m

Veterinary radiology & ultrasound : the official journal of the American College of Veterinary Radiology and the International Veterinary Radiology Association2013eng
Discrete discoid or linear areas of increased soft opacity have been observed within the pulmonary parenchyma in thoracic radiographs of dogs and cats. Similar radiographic findings have been described in humans and termed plate-like atelectasis. The purpose of this retrospective study was to describe locations and characteristics of presumed plate-like atelectasis, presence of concurrent thoracic disease(s), and presence of persistent pulmonary changes on recheck thoracic radiographic studies in a cohort of dogs and cats. Hospital records between 2004 and 2011 were searched and a total of 90

BACKGROUND: Pulmonary atelectasis is a significant complication of metabolic and bariatric surgery (MBS). Sugammadex enables rapid neuromuscular blockade reversal compared to neostigmine, but its impact on atelectasis prevention remains unclear.
METHODS: This retrospective multicenter study used the TriNetX database to analyze adult patients who underwent MBS between 2010 and 2024. Patients receiving rocuronium were divided into sugammadex and control (i.e., neostigmine use) groups. After propensity score matching, the outcomes were assessed at the 30-day follow-up. The primary outcome was th

In mechanically ventilated neonates it is not uncommon to observe obstructive atelectasis from various causes. However it is extremely rare to see mucous plugging and massive pulmonary atelectasis in the absence of infection, aspiration, and respiratory distress syndrome in the first couple of days of life. In this report we describe a neonate born with cystic fibrosis (CF) who presented to us with hypoxic respiratory failure, pulmonary hypertension, and hypercarbia without lactic acedemia from sticky mucous plugging and massive lung collapse. Neonatal respiratory distress and wide spread pulm

Pulmonary gas exchange is regularly impaired during general anaesthesia with mechanical ventilation. This results in decreased oxygenation of blood. A major cause is collapse of lung tissue (atelectasis), which can be demonstrated by computed tomography but not by conventional chest x-ray. Collapsed lung tissue is present in 90% of all subjects, both during spontaneous breathing and after muscle paralysis, and whether intravenous or inhalational anaesthetics are used. There is a correlation between the amount of atelectasis and pulmonary shunt. Shunt does not increase with age. In obese patien

INTRODUCTION: Pulmonary atelectasis is the least studied pulmonary complication in cystic fibrosis (CF). The following objectives have been proposed: 1) to determine whether allergic bronchopulmonary aspergillosis (ABPA) acts as a factor associated with a poor prognosis in patients who subsequently developed atelectasis (decline in lung function, exacerbations, transplantation, lung surgery and death), 2) to identify factors associated with the onset of APBA prior to the diagnosis of atelectasis, 3) therapeutic management, and 4) the clinical and radiological course of both conditions in combi

OBJECTIVE: To describe sudden onset of generalized pulmonary atelectasis following thoracic lavage in 3 dogs.
SERIES SUMMARY: Thoracic lavage was performed following ligation of a patent ductus arteriosus in case 1, prior to closure of a large traumatic full thickness wound in the chest wall in case 2, and during investigation of an idiopathic spontaneous pneumothorax in case 3. In each case anesthesia and surgery were uneventful until thoracic lavage was performed, after which sudden generalized pulmonary atelectasis was observed. The atelectasis was visualized and was associated with oxyhem

BACKGROUND: Pulmonary complications can be caused by intraoperative mechanical ventilation. In particular, prolonged mechanical ventilation is associated with a high mortality rate, a risk of pulmonary complications, prolonged hospitalization, and an unfavorable discharge destination. Pre- and postoperative rehabilitation are important for the resolution of pulmonary complications in acute cases. However, there has been a lack of studies on interventions for pulmonary rehabilitation of patients with chronic pulmonary complications caused by prolonged mechanical ventilation. Accordingly, we des

In-situ thrombosis of the pulmonary artery is a filling defect in a pulmonary artery and is often mislabeled as the most common cause of a pulmonary artery filling defect, an embolism. Thrombus in-situ has been associated with chronic pleural effusions, pulmonary resections, infections and other souces of chronic inflammation, it has not been described as occuring with rounded atelectasis. Herein, we present a rare case of a pulmonary artery thrombus in-situ adjacent to rounded atelectasis; this case underscores the necessity for careful assessment and advocates for heightened awareness of in-

Atelectasis is a complication of different pulmonary diseases; however, neonatal compression atelectasis due to pneumothorax is rarely reported in the literature. Recently, we encountered a typical case of atelectasis. A preterm infant was admitted to the neonatal intensive care unit owing to severe respiratory distress. Lung ultrasound examination confirmed severe pneumothorax and large area of atelectasis. Lung re-expansion occurred when the air was drained from the pleural cavity.

Pulmonary lymphoma is rare, accounting for < 1% of primary lung cancers. Most primary pulmonary lymphomas (PPL) are low-grade mucosa-associated lymphoid tissue (MALT)-type, and among PPL, diffuse large B-cell lymphoma (DLBCL) is extremely rare. In contrast, there has been an increase in the incidence of DLBCL among patients with autoimmune disorders and recurrent or chronic bacterial infection. A subset of DLBCL has been reported to develop through transformation of preexisting or concurrent MALT. The respiratory symptoms are non-specific, and the chest X-ray findings demonstrate the presence

BACKGROUND Common variable immunodeficiency (CVID) is an inborn error of immunity characterized by impaired antibody production, poor vaccine responses, and reduced serum immunoglobulin levels, resulting in respiratory manifestations that can progress to pulmonary complications. Although vitamin D has been associated with pulmonary outcomes in other populations, the association between serum 25-hydroxyvitamin D [25(OH)D] status and pulmonary manifestations, particularly atelectasis, in CVID remains unclear. MATERIAL AND METHODS We conducted a retrospective cross-sectional study of 48 adult pat

INTRODUCTION: Pulmonary atelectasis is a significant respiratory problem in children. This study aimed to investigate the underlying causes of atelectasis and evaluate the outcomes of flexible fiberoptic bronchoscopy (FFB) in pediatric patients with this condition.
MATERIALS AND METHODS: Clinical data and outcomes of 110 children who underwent FFB were retrospectively analyzed based on radiological imaging obtained at three and six months following the procedure, and the patients were categorized into three groups: Group 1 (complete resolution of atelectasis), Group 2 (partial resolution), an

OBJECTIVE: To test the hypothesis that extended postoperative antibiotic cover would reduce the incidence of pulmonary complications in patients undergoing major head and neck surgery with tracheostomy.
DESIGN: A prospective, randomized, controlled trial was carried out to determine the efficacy of an extended course (5 days) of intravenous amoxicillin-clavulanic acid in reducing the rate of atelectasis and pulmonary infections postoperatively. Other possible risk factors that might predispose to pulmonary complications were also evaluated.
SETTING: Tertiary referral center for head and neck

UNLABELLED: Inspired Oxygenation in Surgical Patients During General Anesthesia With Controlled Ventilation: A Concept of Atelectasis. By Bendixen HH, Hedley-Whyte J, and Laver MB. New Engl J Med 1963; 269:991-996. Reprinted with permission.
ABSTRACT: The purpose of this study was to determine if the pattern of ventilation, by itself, influences oxygenation during anesthesia and surgery and examine the hypothesis that progressive pulmonary atelectasis may occur during constant ventilation whenever periodic hyperventilation is lacking, but is reversible by passive hyperinflation of the lungs.
