A normal diffusing capacity of the lungs for carbon monoxide is rare in incidental pulmonary arterial hypertension in systemic sclerosis: Data from the Pulmonary Hypertension Assessment and Recognition of Outcomes in Scleroderma cohort.

OBJECTIVE: Our purpose was to determine the frequency of normal diffusing capacity for carbon monoxide defined as ⩾70% predicted, in those diagnosed with pulmonary arterial hypertension in the Pulmonary Hypertension Assessment and Recognition of Outcomes in Scleroderma cohort. We compared those with normal diffusing capacity for carbon monoxide to those with reduced diffusing capacity for carbon monoxide <70% in order to better clarify the role of pulmonary function testing as a screening test for pulmonary arterial hypertension and to better understand this population. METHODS: Entry criteri

Lung diffusion capacity correlates with pre-implant pulmonary hypertension and predicts outcome after LVAD implantation.

AIMS: Diffusing capacity of the lung for carbon monoxide (DLCO ) reduction is common in heart failure (HF) and is associated with a worse prognosis. Correlations between DLCO and pulmonary hypertension (PH) are unclear, and published data are conflicting; it has been shown that DLCO impairment may persist or even worsen after normalization of pulmonary pressures following left ventricle assist device (LVAD) implantation, maybe reflecting persistent pulmonary damage. We aimed to investigate the impact of pre-implant DLCO and central haemodynamics on outcome in patients with advanced HF implante

Lung clearance index and diffusion capacity for CO to detect early functional pulmonary impairment in children with rheumatic diseases.

BACKGROUND: In adults with rheumatic diseases pulmonary complications are relevant contributors to morbidity and mortality. In these patients diffusion capacity for CO (DLCO) is an established method to detect early pulmonary impairment. Pilot studies using DLCO indicate that early functional pulmonary impairment is present even in children with rheumatic disease albeit not detectable by spirometry and without clinical signs of pulmonary disease. Since the lung clearance index (LCI) is also a non-invasive, feasible and established method to detect early functional pulmonary impairment especial

Relation of pulmonary diffusing capacity changes to HRCT chest and V/Q SPECT findings at short-term and intermediate follow-up after COVID-19: a prospective cohort study (The secure study).

BACKGROUND: Several patients exhibit a severity-dependent reduced pulmonary diffusing capacity (DLCOc) following coronavirus disease 2019 (COVID-19) infection. This has been attributed to fibrosis-like restrictive lung disease, as shown by chest high-resolution computed tomography (HRCT), and concurrent ventilatory disturbances observed by ventilation-perfusion single-photon emission computed tomography (V/Q SPECT) imaging. The aim of this study was to investigate whether reductions in DL,COc at short- and intermediate-term follow-up were associated with initial severity of COVID-19, and to wh

Zone of z-scores uncertainty in pulmonary function interpretation: A proof-of-concept study from CO and NO lung diffusing capacities.

BACKGROUND: The probabilistic interpretation of unusually low lung diffusing capacity (DL) has been recently standardized but the impact of different reactive gases, reference equations and structural abnormalities on definition of lower limits of normal (LLN) is not well established. METHODS AND RESULTS: To assess the uncertainty zone of LLN (z-scores) interpretation in computed tomography (CT)-determined interstitial pulmonary fibrosis (IPF) and pulmonary emphysema (PE), we have measured the single-breath DL for carbon monoxide (DLCO) and nitric oxide (DLNO) in 120 adults, 66 of them with I

Evaluation of Carbonmonoxide, Diffusion Capacity, Respiratory Muscle Strength Values, and Pulmonary Volume in Smoking Men over 40 Years Old.

OBJECTIVE: To determine the effects of tobacco use on the lungs before respiratory symptoms or basic functional disorders occur. MATERIAL AND METHODS: Forty-six active smokers between June 2018 and June 2019 who did not have any respiratory complaints, had no lung or chronic disease affecting the respiratory system, and consumed at least 20 packs/year were prospectively evaluated. In addition, a control group consisting of 50 non-smokers was formed. After confirming that spirometry and chest radiographs were normal, volunteers were taken to measure carboxymetry, plethysmography, respiratory m

Beyond Diffusion Capacity: Continuous Exercise Oximetry Reveals Phenotype-Related Cardiopulmonary Signals in Idiopathic Pulmonary Fibrosis and Progressive Pulmonary Fibrosis-A eurILDreg Pilot Study.

Background: Idiopathic pulmonary fibrosis (IPF) and progressive pulmonary fibrosis (PPF) are defined and monitored without definite exercise-based criterion, despite exertional desaturation being among the earliest functional signs of disease and an established independent predictor of mortality in IPF. The 6 min walk test (6MWT) provides robust prognostic information in IPF, yet whether it can kinetically distinguish IPF from PPF under sustained submaximal loading has not been systematically examined. We hypothesised that second-by-second oximetry during the 6MWT and the 1 min (min) sit-to-st

Association of abnormal pulmonary vasculature on CT scan for COVID-19 infection with decreased diffusion capacity in follow up: A retrospective cohort study.

BACKGROUND: Coronavirus Disease 2019 (COVID-19) is a respiratory viral illness causing pneumonia and systemic disease. Abnormalities in pulmonary function tests (PFT) after COVID-19 infection have been described. The determinants of these abnormalities are unclear. We hypothesized that inflammatory biomarkers and CT scan parameters at the time of infection would be associated with abnormal gas transfer at short term follow-up. METHODS: We retrospectively studied subjects who were hospitalized for COVID-19 pneumonia and discharged. Serum inflammatory biomarkers, CT scan and clinical characteri

Mild parenchymal lung disease and/or low diffusion capacity impacts survival and treatment response in patients diagnosed with idiopathic pulmonary arterial hypertension.

There are limited published data defining survival and treatment response in patients with mild lung disease and/or reduced gas transfer who fulfil diagnostic criteria for idiopathic pulmonary arterial hypertension (IPAH).Patients diagnosed with IPAH between 2001 and 2019 were identified in the ASPIRE (Assessing the Spectrum of Pulmonary Hypertension Identified at a Referral Centre) registry. Using prespecified criteria based on computed tomography (CT) imaging and spirometry, patients with a diagnosis of IPAH and no lung disease were termed IPAHno-LD (n=303), and those with minor/mild emphyse

Pulmonary function abnormalities in chronic severe cardiomyopathy preceding cardiac transplantation.

Pulmonary function data, including diffusing capacity, were evaluated in 56 patients with chronic severe cardiomyopathy before heart transplantation. Cardiac catheterization data were used to describe the relationship between cardiac and pulmonary function. Of 56 patients 44 had some abnormality in pulmonary function. The majority, 30 of 56, had a restrictive impairment alone. Of 28 patients in whom diffusing capacity was measured, 64% had a diffusion impairment. There was no association of pulmonary function impairment with type of cardiomyopathy or smoking history. Pulmonary capillary wedge

Diffusing capacity and forced vital capacity in 5,003 asbestos-exposed workers: relationships to interstitial fibrosis (ILO profusion score) and pleural thickening.

BACKGROUND: Asbestosis is an interstitial lung disease whose radiographic severity has long been graded by the International Labour Office (ILO) profusion score. Its effect on pulmonary function is further impacted by asbestos related pleural thickening. OBJECTIVES: This report aims to describe the relationships between radiographic grading of interstitial and pleural fibrosis and a key test of pulmonary function, the diffusing capacity, which measures gas exchange and has rarely been assessed in large groups, and to confirm the relationship to an independent test of pulmonary function, the v

Stereotactic intensity-modulation radiation therapy for vertebral body and paraspinal tumors.

The aim of the present study was to investigate the relationship between alterations in pulmonary function (i.e., diffusing capacity for carbon monoxide and pulmonary volumes) and the ventilatory response elicited during the cycle-run succession in triathletes. Ten well-trained triathletes performed three exercises: 1) 30 min cycling plus 20 min of running, termed the cycle-run succession; 2) 30 min cycling; and 3) 20 min running. Before and 10 min after each trial, the triathletes underwent pulmonary function testing, including spirometry and diffusing capacity for carbon monoxide. During all

Pulmonary adaptations to 12 wk of supervised high intensity interval training in COPD: a nonrandomized controlled pilot study.

This study investigates the change in diffusing capacity from rest to submaximal exercise in patients with mild to severe chronic obstructive pulmonary disease (COPD) compared with a healthy, age- and sex matched control group, and investigates if the diffusing capacity can be altered by high-intensity interval training (HIIT). Thirty-five patients with COPD and 15 healthy age- and sex matched controls were included. Pulmonary diffusing capacity was measured using the combined single-breath method using carbon monoxide and nitric oxide (DL,CO,NO), which were measured at rest and during 60% of

[Pulmonary hypertension screening in systemic scleroderma: a cohort study of 67 patients].

PURPOSE: Pulmonary hypertension is a severe complication of systemic sclerosis and has emerged as a major cause of morbidity and mortality in this condition. Treatment is all the more efficient as pulmonary hypertension is early diagnosed. A good knowledge of the clinical, biological and functional features of pulmonary hypertension in systemic sclerosis is therefore necessary to suspect and to diagnose pulmonary hypertension as early as possible. METHODS: Sixty seven patients with systemic sclerosis were retrospectively studied. We compared clinical, immunological, functional (spirometry) an
