Spontaneous middle cerebral artery occlusion leading to moyamoya phenomenon and aneurysm formation on collateral arteries.

BACKGROUND: Spontaneous middle cerebral artery occlusion associated with moyamoya phenomenon is distinct from moyamoya disease. The hemodynamic stress on the collateral channel occasionally leads to aneurysm formation, which may manifest as hemorrhage. The etiology of this disease has not been fully understood. CASE DESCRIPTION: A 63-year-old woman presented with left putaminal hemorrhage. The cerebral angiogram revealed a significant stenosis in the proximal segment of the left middle cerebral artery. Collateral arteries originating from the horizontal segment of the ipsilateral anterior cer…

Silent microbleeds are associated with volume of primary intracerebral hemorrhage.

The authors performed a correlative radiologic study on the micro-bleeds and volume of intracerebral hemorrhage in the supratentorial ICH patients. In the patients with lobar or putaminal hemorrhage, the hemorrhage volumes increased more than twofold or threefold in the patients with micro-bleeds. Moreover, the presence of microbleeds was an independent risk factor for large-sized hemorrhage. These data show that microbleeds may be associated with a larger ICH volume.

Short- and long-latency median nerve somatosensory evoked potentials in stroke patients.

Short- and long-latency somatosensory evoked potentials (SEPs) were elicited by stimulation of the median nerve in 104 stroke patients, 59 men and 45 women. Their ages ranged from 25 to 90 years, with a mean age of 68 years. All of the patients presented with acute unilateral motosensory or sensory deficits. Based on CT findings, the patients were divided into four groups: thalamic hemorrhage (52 cases), thalamic infarct (21 cases), putaminal hemorrhage or infarct (13 cases) and infarction in the corona radiata or subcortical white matter (18 cases). The criteria for abnormal SEP responses wer…

Location of acute brain hemorrhage in patients undergoing antithrombotic therapy.

INTRODUCTION: The relationship between antithrombotic therapy and the anatomical location of acute brain hematoma remains disputed. The curr … cteristics in patients with thalamic, cerebellar, or lobar hemorrhage were compared with those with putaminal hemorrhage. A multivariate analysis using the logistic regression model showed that antithrombotic therapy was an independent factor for cerebellar hemorrhage (OR 3.66, 95%CI 1.31-10.18), lobar hemorrhage (OR 2.27, 95%CI 1.12-4.57), and thalamic hemorrhage (OR …

Primary intracerebral hemorrhage in Izumo City, Japan: incidence rates and outcome in relation to the site of hemorrhage.

OBJECTIVE: The aim of this community-based study was to investigate the incidence rates and outcome of primary intracerebral hemorrhage (ICH) in relation to the site of hemorrhage. METHODS: The subjects were 350 patients with primary first-ever ICH who were treated during the 8-year period 1991 to 1998 in Izumo City, Japan. RESULTS: The crude and age- and sex-adjusted incidence rates for all types of ICH were 52 and 47 per 100,000 population, respectively, for all ages. The most common site of ICH was the putamen (120 patients, 34%), followed by the thalamus (115, 33%), lobar areas (53, 15%)

Lentiform fork sign: a magnetic resonance finding in a case of acute metabolic acidosis.

We report a 33 year-old woman addicted to chronic unspecified solvents abuse with stupor, respiratory disorders, tetraplegia and severe metabolic acidosis. On admission an unenhanced cranial CT scan showed symmetrical hypodensities of both lentiform nuclei. MR imaging performed 12 hours after stupor demonstrates bilateral putaminal hemorrhagic necrosis, bilateral external capsule, corona radiata and deep cerebellar hyperintensities with right cingulate cortex involvement. DWI reflected bilateral putaminal hyperintensities with restricted water diffusion as to citotoxic edema and development of

The diagnosis of hypertensive intracerebral hemorrhage: the contribution of computed tomography.

Sixty-eight cases of hypertensive intracerebral hemorrhage were reviewed and the CT scans studied. The CT scan was accurate in localizing and measuring intracerebral hemorrhage. There were three cases of pontine hemorrhage which did not appear on the CT scan. Hemorrhages at other sites were always seen on the CT scan. The size of putaminal and thalamic hemorrhage as calculated from CT scan allowed accurate prognosis to be made at the onset. CT scanning has become the diagnostic test or first choice when intracerebral hemorrhage is suspected.

Idiopathic cerebral hemorrhage during delivery in normotensive women--case reports.

Two normotensive pregnant women who developed cerebral hemorrhage during delivery are reported. The hematoma was small and subcortical in one and large and putaminal in the other. They had no history of hypertension, cerebrovascular disorders, or eclampsia. Cerebral angiography studied in one case revealed no abnormalities. Idiopathic cerebral hemorrhage during delivery without any evidence of vascular risk factors is discussed.

Intracranial hemorrhage from undiagnosed metastatic brain tumor during general anesthesia.

A patient with endometrial cancer presented with intracranial hemorrhage from an undiagnosed metastatic brain tumor during abdominal radical hysterectomy. Since she was neurologically intact, a systematic examination for brain metastasis had not been performed preoperatively. After the surgery, she had delayed recovery from general anesthesia with right hemiplegia and aphasia. Computed tomography and magnetic resonance imaging showed left putaminal hemorrhage from brain metastasis.

Aphasia after left hemispheric intracerebral hemorrhage.

The function of subcortical nuclear structures in language is uncertain, and language disorders after injury to these structures are described incompletely. We report 15 patients with left putaminal or thalamic hemorrhage, describe the range of language and behavioral disorders produced, and review the potential mechanisms of these disorders. Clinicoanatomic correlations revealed no definite differences between aphasia after hemorrhage in putamen or in thalamus.

[Somatosensory evoked potentials over the unaffected hemisphere in patients with unilateral intracerebral hematoma].

Somatosensory evoked potentials (SEPs) recorded from the unaffected hemispheres were studied in patients of less than one month after the onset of unilateral intracerebral hematoma. We examined 66 SEPs obtained from 49 patients with putaminal hemorrhage (midline shift 0-15 mm), 38 SEPs from 25 patients with thalamic hemorrhage (midline shift 0-8 mm), and 14 SEPs from 10 patients with subcortical hemorrhage (midline shift 0-12 mm). These examinations were made after electrical stimulation of the median nerve at the wrist by using square waves of 0.2 msec in duration delivered at a rate of 3 Hz.…

Magnetoencephalography and positron emission tomography studies of a patient with auditory agnosia caused by bilateral lesions confined to the auditory radiations.

The aim of this study was to investigate auditory cortex function in the context of auditory stimuli in a patient with auditory agnosia due to bilateral lesions confined to the auditory radiations. A male patient experienced mild left temporal hemiplegia because of right putaminal hemorrhage at the age of 43 years. Thereafter he recovered completely but hypertension persisted. When he was 53 years old, he suffered left putaminal hemorrhage and went into a coma. After recovering from the coma and right hemiplegia he could hear but could not discriminate speech sounds. Brain CT and MRI demonstra…

Angiographically occult vascular malformations causing intracranial hemorrhage.

In 5 patients, angiographically occult vascular malformations led to intracerebral or intracerebellar hemorrhages. Angiographic findings were not suggestive of vascular abnormalities, but signs of a mass were present in 4 patients. Computed tomography (CT) scanning suggested the presence of cryptic vascular lesions accompanied by intraparenchymal hemorrhage in 2 patients. In all patients surgical treatment was successful. Histological examination confirmed these lesions to be venous malformations in the cerebellum in 2 patients and in the left frontal region in 1, and arteriovenous malformatio

[Lacunar syndromes due to intracerebral hemorrhage].

Nine cases (seven men and two women, mean age 64.5 years) of classical lacunar syndromes due to intracerebral hemorrhage are reported. Three patients presented with pure motor hemiparesis (two putaminal hematomas with proportional weakness and one cortical hemorrhage with brachio-crural hemiparesis). Four patients presented with sensorimotor stroke due to thalamo-capsular hemorrhage. The last two patients had thalamic hemorrhage causing ataxic hemiparesis or dysarthria-clumsy hand syndrome. Four subjects had arterial hypertension, one was diabetic, and two were treated with anti-vitamin K. Abr

Clinico-epidemiologic study of stroke in Akita, Japan.

An analysis was performed of 2,168 consecutive stroke patients who were examined by computed tomography and entered into a hospital-based stroke registry in Akita Prefecture, Japan. The occurrence of cerebral hemorrhage, cerebral infarction, and subarachnoid hemorrhage was 30, 55, and 14%, respectively. Age-specific rates of subarachnoid hemorrhage were higher in women than men; other types of stroke showed a preponderance in men. Total strokes increased in the winter; this seasonal difference was confined to cerebral hemorrhage. Putaminal hemorrhages predominated in the younger age groups; th…

Movement-induced focal tonic-clonic seizure-like movements after ipsilateral basal ganglia hemorrhage.

We present a 46-year-old man who developed repetitive focal tonic-clonic seizure-like involuntary movement in the left lower extremity after a left basal ganglia (BG) hemorrhage. His symptoms were induced only by voluntary movement of the left lower extremity and were followed by transient paralysis. Electroencephalography during an attack showed no epileptiform discharges. Carbamazepine completely abolished his symptoms. This report implies that damage to the BG and/or its connections might cause seizure-like involuntary movement.

Aneurysm of the lenticulostriate artery--report of four cases.

Four rare cases of aneurysm of the lenticulostriate artery (LSA) are presented. LSA aneurysms were located at the origin in three patients and distally in one. Two cases were of multiple aneurysms, one was associated with hypertensive intracerebral hematoma (putaminal hemorrhage), and the other with moyamoya disease. Two patients were successfully treated by microsurgical procedures. The occurrence of LSA aneurysm suggests that aneurysm formation and growth are accentuated by hemodynamic alteration and stress.
