Should PNPO Deficiency Be Treated In Utero? Clinical Findings From Prenatal Pyridoxine Therapy.

Pyridox(am)ine-5'-phosphate oxidase (PNPO) deficiency is characterized by early-onset epileptic encephalopathy refractory to standard antiseizure medications. It is caused by variants in the PNPO gene, resulting in deficient PNPO enzyme activity, which normally converts pyridoxine-5'-phosphate and pyridoxamine-5'-phosphate (two vitamers of vitamin B6) into the active cofactor, pyridoxal-5'-phosphate (PLP). Treatment relies on PLP or pyridoxine in some cases. Long-term outcomes remain suboptimal. We describe two cases of unrelated children treated with vitamin B6 (pyridoxine) in utero: one with

The in vitro and crystallographic studies reveal the inhibitory potential of vitamin B6 analogues against a serine protease trypsin.

Trypsin is a representative member of serine protease family of peptidases. Its premature activation leads to develop several ailments, such as pancreatitis and colorectal cancer. The available therapies are few in numbers and possessed several side effects. Therefore, searching of new trypsin inhibitors has a great importance in drug discovery. In the current study, we have discovered the inhibitory potential of vitamin B6 and its commercially available analogues; pyridoxine (1), pyridoxal (2), pyridoxamine (3) and pyridoxal phosphate (4), against trypsin through in vitro and crystallographic

A new branch of mammalian vitamin B6 metabolism: AKR1C-mediated conversion of pyridoxal to pyridoxine and 4-pyridoxolactone.

Pyridoxal 5'-phosphate (PLP), the coenzyme form of vitamin B6, is indispensable for diverse metabolic processes, especially amino acid metabolism. In mammals, PLP is primarily synthesized via a salvage pathway involving pyridoxal kinase (PLK), pyridoxine/pyridoxamine 5'-phosphate oxidase (PNPO), and pyridoxal phosphate phosphatase (PLPP). However, recent evidence suggests the presence of additional, yet unidentified, enzymatic contributors to this pathway. Here, we identify aldo-keto reductase family 1 member C (AKR1C) isozymes as previously unrecognized enzymes involved in vitamin B6 metaboli

Glycation-Driven Mitochondrial and ER Stress Underlies Iodoacetic Acid-Induced Apoptosis in Porcine Uterus and Oviduct Epithelial Cells.

Iodoacetic acid (IAA), a highly cytotoxic disinfection byproduct commonly detected in drinking water, poses a potential risk to female reproductive health. The direct molecular mechanisms underlying its effects on the reproductive system epithelium remain unclear. This study demonstrates that IAA induces glycational stress in primary porcine uterine (UECs) and oviduct epithelial cells (OECs), representing an early event contributing to extensive cellular toxicity. IAA exposure inhibited Glyceraldehyde-3-Phosphate Dehydrogenase (GAPDH) enzymatic activity and promoted the accumulation of advance

Accelerated remission of the alcoholic organic brain syndrome with EMD 21657. Double-blind clinical and psychometric trials.

The efficacy of EMD 21657--a derivative of a pyritinol metabolite--on the alcoholic orangic brain syndrome (OBS)) was investigated in a double-blind placebo-controlled study. 19 patients received 3X300 mg EMD, 21 patients 3X1 dragée placebo throughout 6 weeks. The two groups did not differ as to age, sex, weight, height, alcohol anamnesis and IQ. Investigations were carried out before and at the end of the 2nd, 4th and 6th weeks of drug treatment. While an overall evaluation by doctor and patient at the end of the therapy did not reveal any signficant differences, it was found by means of the

Control of 5-aminolaevulinate synthetase activity in Rhodopseudomonas spheroides. Binding of pyridoxal phosphate to 5-aminolaevulinate synthetase.

1. Pyridoxal 5'-phosphate is a cofactor essential for the enzymic activity of aminolaevulinate synthetase from Rhodopseudomonas spheroides. It also aids activation of the low-activity enzyme by trisulphides such as cystine trisulphide, whereas inactivation of enzyme is facilitated by its absence. 2. The fluorescence spectrum of purified high-activity enzyme is that expected for a pyridoxal phosphate--Schiff base, but the firmly bound cofactor does not appear to be at the active centre. In dilute solutions of enzyme this grouping is inaccessible to nucleophiles such as glycine, hydroxylamine, b

Metabolic Rewiring of the Shikimate Pathway Enables Rose-Scented Rice.

Rice fragrance is a key determinant of grain quality, yet the aroma profile of cultivated rice is highly uniform and largely dominated by the popcorn-like compound 2-acetyl-1-pyrroline. Here we expanded the aromatic repertoire of rice by engineering a rose-like fragrance through reconstruction of the phenylalanine-derived 2-phenylethanol (2-PE) pathway in the endosperm. To increase phenylalanine precursor supply, we co-expressed an engineered G211R/G212S variant of the rice 3-deoxy-D-arabino-heptulosonate 7-phosphate synthase isoform OsDHS2 (LOC_Os08g37790; mOsDHS2) with heterologous phenylace

Studies on the mechanism of acetaldehyde-mediated inhibition of aspartate aminotransferase (GOT) in human erythrocytes.

The mechanism of acetaldehyde-mediated GOT inhibition was studied in human red cells. In the GOT assay mix, acetaldehyde competitively inhibits activation of apoGOT by pyridoxal phosphate and pyridoxamine phosphate. However, Ki values are 100-1000 times greater than Km values for these B6 vitamers. Moreover, incubation of undiluted lysates with acetaldehyde at 37 degrees inhibits GOT activity without increasing apoGOT levels and without altering affinity of apoGOT for either B6 coenzyme. In undiluted lysates, inhibition is not prevented by disulfiram. However, incubation at 4 degrees prevents

Effect of respiratory infection on tissue riboflavin and flavin enzymes in mice.

The effects of Klebsiella pneumoniae infection on red blood cell and liver riboflavin levels and a few flavin enzymes were examined to verify our earlier hypothesis based on human experience that upper respiratory infections result in mobilization of riboflavin from tissues into blood, resulting in a rise in red blood cell riboflavin and an increased saturation of erythrocyte glutathione reductase (EGR) with its coenzyme flavin adenine dinucleotide (FAD; reduction in EGR activation coefficient, AC, values). Thirty-six-day-old male mice fed a diet marginally sufficient in riboflavin were inject

Molecular characterization of pyridoxine 5'-phosphate oxidase and its pathogenic forms associated with neonatal epileptic encephalopathy.

Defects of vitamin B6 metabolism are responsible for severe neurological disorders, such as pyridoxamine 5'-phosphate oxidase deficiency (PNPOD; OMIM: 610090), an autosomal recessive inborn error of metabolism that usually manifests with neonatal-onset severe seizures and subsequent encephalopathy. At present, 27 pathogenic mutations of the gene encoding human PNPO are known, 13 of which are homozygous missense mutations; however, only 3 of them have been characterised with respect to the molecular and functional properties of the variant enzyme forms. Moreover, studies on wild type and varian

Vitamin B6 Modification of Bovine Pericardial Bioprosthetic Heart Valve Leaflets Promotes Blood Outgrowth Endothelial Cell Adhesion.

Bioprosthetic heart valves (BHV), typically fabricated from glutaraldehyde-fixed bovine pericardium (BP), are the most common type of replacement for patients with severe heart valve disease. Despite being preferred, BHV are limited in durability due to structural valve degeneration (SVD), which occurs due to leaflet calcification and non-calcific mechanisms including advanced glycation end product (AGE) accumulation and serum protein infiltration. Another limitation of BHV is characteristically they do not endothelialize, even after years of implantation. Previously we showed pretreating BP w

Pyridoxine supply in human development.

Vitamin B(6) has an important role in the function of the human nervous system. Experimental data are not generally available on the role in human development, but significant conclusions may be made from studies of the effect of disorders of B(6) vitamer metabolism. Vitamin B(6) comprises seven compounds - pyridoxal, pyridoxine, pyridoxamine and their respective 5' phosphates. The common active form in human tissue is the 5'-phosphate form of pyridoxal (PLP) most of which is found in muscle bound to phosphorylase. Like many vitamins, B(6) can function both as a co-enzyme and as a chaperone. P

Formation of Nepsilon-(succinyl)lysine in vivo: a novel marker for docosahexaenoic acid-derived protein modification.

Free radical-catalyzed peroxidation of docosahexaenoic acid (DHA, C22:6/omega-3) generates various lipid peroxidation products that covalently modify biomolecules such as proteins. Under a free radical-generating system, DHA significantly modified lysine residues in bovine serum albumin. Upon incubation of oxidized DHA with an amino-compound pyridoxamine or a lysine-containing peptide, N-propanoyl and N-succinyl adducts were determined to be the major modification products. The hydroperoxide levels in the oxidized DHA closely reflected the formation of the N(epsilon)-(succinyl)lysine (SUL) upo

Metabolic Profiling of the Cerebrospinal Fluid in Pediatric Epilepsy.

To characterize metabolic profiles within the central nervous system in epilepsy, we performed gas chromatography-tandem mass spectrometry (GC-MS/MS)-based metabolome analysis of the cerebrospinal fluid (CSF) in pediatric patients with and without epilepsy. The CSF samples obtained from 64 patients were analyzed by GC-MS/MS. Multivariate analyses were performed for two age groups, 0-5 years of age and 6-17 years of age, to elucidate the effects of epilepsy and antiepileptic drugs on the metabolites. In patients aged 0-5 years (22 patients with epilepsy, 13 without epilepsy), epilepsy patients

Lipid glycation and protein glycation in diabetes and atherosclerosis.

Recent instrumental analyses using a hybrid quadrupole/linear ion trap spectrometer in LC-MS/MS have demonstrated that the Maillard reaction progresses not only on proteins but also on amino residues of membrane lipids such as phosphatidylethanolamine (PE), thus forming Amadori-PE (deoxy-D: -fructosyl PE) as the principal products. The plasma Amadori-PE level is 0.08 mol% of the total PE in healthy subjects and 0.15-0.29 mol% in diabetic patients. Pyridoxal 5'-phosphate and pyridoxal are the most effective lipid glycation inhibitors, and the PE-pyridoxal 5'-phosphate adduct is detectable in hu

Podophyllotoxin induces testicular toxicity via the microbiota-gut-testis axis based on the toxicological evidence chain concept.

Podophyllotoxin (PPT), a natural lignan with well-established antitumor efficacy and well-documented ovarian toxicity, was first investigated for its potential testicular toxicity in male Sprague-Dawley rats, following the guidance of the Toxicological Evidence Chain (TEC) framework. Through an integrated approach combining physiological and pathological phenotyping, targeted metabolomics of intestinal contents, serum, and testicular tissues, testicular transcriptomics, and 16S rDNA sequencing of gut microbiota, we systematically delineated the injury evidence chain induced by PPT. Drug failure in diabetic kidney disease: translational barriers, target validation and trial de-risking strategies.

Diabetic kidney disease (DKD) remains a difficult therapeutic setting in nephrology, metabolism and clinical pharmacology. Standard care has moved from glucose- and blood-pressure-centred treatment toward disease-modifying therapy built around renin-angiotensin-aldosterone system (RAAS) blockade, sodium-glucose cotransporter 2 (SGLT2) inhibitors, glucagon-like peptide-1 receptor agonists and nonsteroidal mineralocorticoid receptor antagonism. Yet DKD drug development is also defined by repeated translational failure. Many agents with strong mechanistic rationale have failed to improve hard kid

A sensitive UPLC-MS/MS method for simultaneous quantification of one-carbon metabolites & co-factors in human plasma.

One-carbon metabolism is an important metabolic pathway involved in many diseases, such as congenital malformations, tumours, cardiovascular diseases, anaemia, depression, cognitive diseases and liver disease. However, the current methods have specific defects in detecting and qualifying the related compounds of one-carbon metabolism. In this study, a validated method was established to simultaneously quantify 22 one-carbon metabolites & co-factors in human plasma and applied to the study of correlation between one-carbon metabolism and colorectal cancer in human plasma samples, which were fro

A unique and promising combination of medications for the treatment of Alzheimer's disease.

At present there is no therapy for Alzheimer's Disease which completely stops the progressive dementia effecting late onset Alzheimer's Disease (AD) patients. It is felt that the main reason for this failure is that AD appears to be a disease caused by four major pathological processes. To date, efforts to develop treatments have addressed only one or another of these four etiologies. However, even a partially effective therapy against one cause allows the others, untreated, to continue their inexorable destruction of the neurons of the brain. It is suggested that a therapy is required which i
