Quadruplets are a set of four offspring born at one birth which can be fraternal (multizygotic), identical (monozygotic) or a combination of both. Ovarian hyperstimulation syndrome (OHSS) commonly occurs as a complication of ovarian stimulation with gonadotrophins. Spontaneous OHSS is an extremely rare event, but can occur as a result of stimulation with pregnancy-derived hCG. We herein report a case of quadruplet pregnancy complicated by OHSS with spontaneous ovulation. The patient had previously undergone ovarian stimulation with clomiphene citrate plus FSH. BACKGROUND: Multifetal pregnancies are high-risk pregnancies. Coexistence of conjoint twins with multifetal pregnancies further increases the risk factor, which is already considerably high with multiple births. Assisted reproductive techniques lead to an increase in multifetal pregnancies, especially monozygotic pregnancies, which in turn lead to an increase in the rate of conjoined twins.

Data from 14 triplet and 2 quadruplet pregnancies (50 infants) during the period 1974-1988 were analysed. The perinatal mortality rate was only 6%, despite a preterm delivery rate of 88%. Preterm delivery was more common in young (less than or equal to 29 years) multiparous women. Perinatal complications were, as expected, strongly associated with immaturity. Maternal serum alpha-fetoprotein (AFP) levels were higher in six triplet pregnancies and three quadruplet pregnancies than in control singleton pregnancies matched for maternal age, parity and time of gestation at which the serum sample was taken. Between 12 and 23 weeks of pregnancy, the average AFP levels among the triplet pregnancies was three times that found in the singleton pregnancies, and the level in the quadruplet pregnancies was even higher. Maternal serum AFP levels appear to be associated with the number of fetuses in utero.

Prescribing clomiphene citrate for infertility patients has been taken very lightly. High order pregnancy can happen with this treatment. In addition to high perinatal mortality and morbidity of multiple pregnancy, the economic and social strains on the national health system and the family is great. The case presented showed that although the outcome of pregnancy was successful but the cost to the national health was tremendous. It is therefore recommended that measures should be taken to avoid high order births. Moreover, delaying the delivery, if possible, will be cost effective.

OBJECTIVE: We report a case of conjoined twins in a trichorionic quadruplet pregnancy after ovulation induction with clomiphene citrate for anovulation.
METHODS: The diagnosis of a trichorionic quadruplet pregnancy was made in routine ultrasound at 11 weeks' gestation.
RESULTS: Conjoined twinning was thoracopagus and presented with increased nuchal translucency. Selective termination of the conjoined twins was performed with potassium chloride injection. The patient delivered 2 healthy infants after an uneventful follow-up.

Multifetal pregnancy reduction was performed in a patient with quadruplet pregnancy at the 12th week of gestation. Reduction of the fetuses to two was carried out by injecting potassium chloride solution into the fetal chest under transabdominal ultrasound guidance. No complications were encountered and the woman was delivered of healthy twins at 37 weeks of gestation.
