From fragmentation to integration: building a globally harmonised biodosimetry framework for radiological emergency preparedness.

Escalating geopolitical instability and the persistent risk of radiological or nuclear incidents highlight an urgent need for resilient global radiation emergency preparedness. In such crises, rapid and reliable biological dose assessment is indispensable for triage, therapeutic stratification, and long-term health surveillance. While the dicentric chromosome assay remains the gold standard for biodosimetry, its labour-intensive nature, limited throughput, and variability in implementation constrain large-scale responsiveness. Emerging approaches, including DNA-DSB repair foci analysis and G0-

Adult Medical Countermeasures: Antidotes and Cytokines for Radiological and Nuclear Incidents and Terrorism.

The war in Ukraine raises concerns for potential hazards of radiological incidents and their impact on humans, especially families. Preparedness and response to radiological and nuclear incidents necessitates familiarity with pharmaceutical countermeasures, including antidotes and cytokines. Searches found no published study comparing adult indications and dosing among standard references. This study addresses this gap by collecting, tabulating, and disseminating information to health care professionals. Expert consensus chose the following references to compare adult indications and dosing of

OFF-SITE RADIOLOGICAL DOSE ASSESSMENT IN A SEVERE NUCLEAR ACCIDENT OF APR1400.

Shin Kori unit 3 of Korea Hydro and Nuclear Power Co. is the pressurized water reactor of advanced power reactor 1400 (APR1400) in the Republic of Korea. A hypothetical accident was assumed for Shin Kori unit 3 to perform the time-dependent dose calculations along with the release rates of radionuclides to the environment. In order to assess the accident scenarios, Radiological Assessment System for Consequence Analysis code was used, which is a set of tools for emergency response applications developed by US Nuclear Regulatory Commission. A straight-line Gaussian model was used for the near-f

Radioactive Games? Radiation Hazard Assessment of the Tokyo Olympic Summer Games.

We conducted a comprehensive radiation hazard assessment of the Tokyo Olympic Games (Tokyo 2020, postponed to 2021). Our combined experimental and literature study focused on both external and internal exposure to ionizing radiation for athletes and visitors of the Games. The effective dose for a visit of 2 weeks ranges from 57 to 310 μSv (including flight dose). The main contributors to the dose are cosmic radiation during the flights (approximately 10-81%), inhalation of natural radon (approximately 9-47%), and external exposure (approximately 8-42%). In this complex exposure, anthropogenic

Comparative study of 131I released during a nuclear accident.

Radioiodine-131 (131I) is the most significant environmental contaminant during a short-term nuclear accident. When present in the atmosphere, it concentrates mainly in the thyroid gland through inhalation or ingestion. This can lead to partial or total destruction of thyroid cells, potentially causing hypothyroidism. It therefore negatively affects the thyroid in exposed populations. At high doses, radiation can also cause cellular mutations and increase the risk of thyroid cancer, particularly in children under five years old and those exposed during pregnancy. In such cases, nonradioactive

An improved method of nasal swab analysis for assessing alpha internal radioactive contaminants.

When responding to nuclear and radiological emergencies, rapid and on-site detections of possible internal radioactive contaminants are required for early dose estimation and medical triage. Nasal swab analysis is an effective method to provide valuable information for early and fast estimates of alpha radionuclide inhalation intakes and resultant doses. In this study, to improve the quality of nasal swab measurements, a specialised double-detector alpha counter was designed. Various parameters including swab materials, sample pre-preparation, angle-dependence and vacuum dependence were invest

Regional radiation risk and vulnerability assessment by integration of mathematical modelling and GIS analysis.

The Kola Peninsula, Russian Arctic exceeds all other regions in the world in the number of nuclear reactors. The study was aimed at estimating possible radiation risks to the population in the Nordic countries in case of a severe accident in the Kola Peninsula. Two approaches were tested: (1) probabilistic analysis of modelled possible pathways of radionuclide transport and precipitation and (2) deterministic approach (case studies) for most possible or worst-case scenarios of modelled transport and deposition of radionuclides. For the general population, Finland is at most risk with respect t

[An estimation of the effect of low doses of irradiation on the population in the area of the eastern Urals radioactive trace on the health status of offspring].

High morbidity among the children whose parents and/or grandparents who resided in the areas with relatively low pollution levels (0.2-2.0 Ci/km2) after the 1957 radiation accident at the production association "Mayak" is mainly formed under the influence of combined social, hygienic, and biomedical factors. The very residence of any child's ancestor in the low polluted areas produce no substantial impact on the nonspecific resistance of children.

[Health effects of plutonium pollution--assessment after the Thule disaster in 1968].

On 21 January 1968 a B-52 airplane with four nuclear weapons crashed on the ice close to the Thule air base in northern Greenland. Approximately six kilograms of plutonium was dispersed in the surroundings. Since 1968, the possible health impacts among the Danish civilian workers who assisted at the clean-up have been discussed regularly. A Nordic expert group was appointed by the Danish government to examine the evidence. The group concluded in May 1995 that no conclusive evidence of damage to the health of the worker as a result of plutonium pollution had been reported. However, they recomme

European Approach for Acute Radiation Syndrome's Management Facing Radiological and Nuclear Threats: A Call for Arms.

The increasing concern related to radiological and nuclear threats within the European Union (EU) has intensified the regional preparedness needs. The anticipated number of injured, contaminated, and irradiated individuals may range from a few to hundreds of thousands, necessitating immediate emergency medical care and hospitalization. Given this potential influx of casualties, the medical teams responsible for initial management may become rapidly overwhelmed, facing immense pressure to formulate appropriate response strategies and treatment options. Therefore, it is critical to establish app

Collaborative activities in a biological dosimetry network for radiation emergencies in South Korea.

PURPOSE: Biological dosimetry is an essential analytic method to estimate the absorbed radiation dose in the human body by measuring changes in biomolecules after radiation exposure. Joint response in a network to mass-casualty radiation incidents is one way to overcome the limitations of biological dosimetry, sharing the workload among laboratories. This study aimed to investigate the current performance, collaborative activities and technical advances of the Korea biodosimetry network (K-BioDos), and suggest the future directions toward successful joint response. MATERIALS AND METHODS: A su

Findings and Recommendations From a Series of Workshops on Hospital Emergency Responses to an Improvised Nuclear Device Detonation.

The New York City Department of Health and Mental Hygiene and the Greater New York Hospital Association held 3 workshops and 2 follow-up meetings with hospital emergency managers and colleagues to determine hospitals' response actions to a scenario of a 10-kiloton improvised nuclear device detonation. The scenario incorporated 3 zones of damage (moderate, light, and beyond damage zones) and covered the period of 0 to 72 hours postdetonation divided into 3 24-hour operational periods. The Joint Commission's critical emergency areas were used to determine the objectives and response actions that

Suit Up: A Systematic Review of the Personal Protective Equipment (PPE) Recommended and Utilized by Various Classes of Responders to Nuclear Radiological Disasters at Nuclear Power Plants.

INTRODUCTION: Interest in nuclear power as a cleaner and alternative energy source is increasing in many countries. Despite the relative safety of nuclear power, large-scale disasters such as the Fukushima Daiichi (Japan) and Chernobyl (Ukraine) meltdowns are a reminder that emergency preparedness and safety should be a priority. In an emergency situation, there is a need to balance the tension between a rapid response, preventing harm, protecting communities, and safeguarding workers and responders. The first line of defense for workers and responders is personal protective equipment (PPE), b

The convergence approach may be critical to improving early situational awareness in hostile radioactive environments.

This study explores the impact of a simulated radiological dispersal device (RDD) event in an urban area on young adults around 20 years old. The RDD releases radioactive Cs-137 (7.0E+3 Ci), a common industrial sterilization source. The study aims to demonstrate that combining computational codes and epidemiological models can produce valuable data to guide initial actions when confronting a hostile radioactive environment. The HotSpot Health Physics and RESRAD-RDD codes were used in the simulation to evaluate the event's initial phase. The codes were executed together, and the HotSpot output

RABiT-II-DCA: A Fully-automated Dicentric Chromosome Assay in Multiwell Plates.

We developed a fully-automated dicentric chromosome assay (DCA) in multiwell plates. All operations, from sample loading to chromosome scoring, are performed, without human intervention, by the second-generation Rapid Automated Biodosimetry Tool II (RABiT-II) robotic system, a plate imager and custom software, FluorQuantDic. The system requires small volumes of blood (30 µl per individual) to determine radiation dose received as a result of a radiation accident or terrorist attack. To visualize dicentrics in multiwell plates, we implemented a non-classical protocol for centromere FISH staining

Detection of partial-body exposure to ionizing radiation by the automatic detection of dicentrics.

In accidental exposure to ionizing radiation, it is essential to estimate the dose received by the victims. Currently dicentric scoring is the best biological indicator of exposure. The standard biological dosimetry procedure (500 metaphases scored manually) is suitable for a few dose estimations, but the time needed for analysis can be problematic in the case of a large-scale accident. Recently, a new methodology using automatic detection of dicentrics has greatly decreased the time needed for dose estimation and preserves the accuracy of the estimation. However, the capability to detect nonh

Strategy for population triage based on dicentric analysis.

After large-scale accidental overexposure to ionizing radiation, a rapid triage of the exposed population can be performed by scoring dicentrics and ring chromosomes among 50 metaphases. This is rapid but is not accurate because the sensitivity is around 0.5 Gy. After the triage step, dose can be estimated by scoring 500 metaphases. This is lengthy but very accurate because the sensitivity is between 0.1 and 0.2 Gy. To improve the methodology, we propose the use of software for automatic dicentric scoring that was tested on victims of an accident in Dakar. Manual scoring of 50 metaphases was c

[The function of the digestive system organs in those who worked in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station (a review of the literature)].

There is an increase in the number of liquidators suffering from digestive diseases, as per the literary survey submitted. Most prevalent during the early postirradiation period are chronic gastritis, duodenitis, cholecystitis, and peptic ulcer, while later there prevail chronic hepatitis. Several distinguishing features are noted, that make for a graver course of the above-named illnesses, such as, in the first place, pathological processes in digestive organs, accompanied by profound morphofunctional disturbances in them, presenting with immunological as well as biochemical shifts. A grave c
