[Interventional radiology in pediatrics].

A survey of therapeutic possibilities offered by pediatric interventional radiology is given. Authors emphasize that the percutaneous interventional radiology procedures are performed either as an adjuvant treatment facilitating other (surgical) methods or as definite therapy replacing traditional treatment modalities. It is also stressed that interventional procedures are cost effective and relative less expensive than surgery, and they are not so cumbersome to the patients and can shorten the time for hospitalization significantly. Authors present 5 illustrative cases including locoregional

Managing and Preventing Placement-Based Fatigue in Radiography and Allied Health Students: A Narrative Review.

Clinical placement plays a key role in the education of allied health students by providing real-world experiences required to develop professional competence and confidence. For radiography students, extensive unpaid clinical placements form a substantial component of professional training. Despite their educational value, clinical placements expose students to physical, emotional, and financial stressors that may contribute to fatigue. This narrative review aimed to investigate current and potential support systems, alternatives to placement, and healthy coping strategies available to radiog

Difficult biopsy and drainage: just say yes.

Abdominal radiologists are often asked to perform difficult percutaneous chest, abdomen, and pelvis biopsies and drainages with imaging guidance. Many of these procedures involve small target lesions far from the skin surface, in close proximity to critical structures. Organ location is changeable due to respiration, peristalsis, and pulsation, further complicating the planning process. High-level three-dimensional spatial awareness is critical to mastery of complex image-guided procedures. A comprehensive grasp of anatomy and expected changes can be exploited in certain cases to target lesion

A bullet-sired bone cyst.

Random gunfire deposited a bullet in the proximal tibial metaphysis of a 9-year-old girl. The wound was not incapacitating and was treated conservatively. Within 17 months, soreness developed in the proximal leg, and radiography revealed a large unicameral cyst within which the bullet freely tumbled. Eventually, fear of impending fracture prompted further radiography, computed tomography, surgical intervention and pathological examination of the cyst wall. We believe this is only the second description in the English-language literature of this rare sequence of events.

Splenic imaging and non-vascular intervention: when to intervene on the spleen and how to do it safely.

Cross-sectional interventional radiology (CSIR) for the spleen has evolved over the past two decades. Contemporary data show that CT- and US-guided splenic procedures are safer than once assumed, allowing for biopsy and drainage when clinically appropriate. Prior to intervention, however, close imaging evaluation of splenic lesions is required in order to recognize definitively benign, "do not touch" lesions as well as suspicious features that might warrant biopsy. This manuscript will first focus on imaging workup and decision-making for splenic lesions, and then discuss modern CSIR technique

The current status of interventional radiology in Canada: results of a survey by the Canadian Interventional Radiology Association.

OBJECTIVE: To evaluate the current status of interventional radiology in Canada. METHODS: A questionnaire was sent to 28 Canadian interventional radiologists (defined as a physician who performs any type of interventional procedure, including biopsies, but excluding interventional neuroradiology) practising in both tertiary and community hospitals in the major centres in all provinces except Prince Edward Island. RESULTS: Twenty-two (79%) of 28 surveys were completed and returned, providing data about 86 interventional radiologists (IRs). IRs were performing almost all of the following proce

Management of Bone Marrow Edema with Interventional Radiology: When and How.

Bone marrow edema, a typical and exclusive finding of magnetic resonance imaging, is a characteristic of multiple pathologies. There is still no consensus about its origin and histologic characteristics. In this article, we describe six causes of bone edema based on pathophysiologic characteristics: traumatic, infectious, inflammatory, degenerative, tumoral, and neuromediated. For each of them, we explain both the diagnostic and therapeutic role of interventional radiology. The fundamental importance of bone biopsy in characterizing the lesions associated with bone edema is also reviewed.

Imaging the neonatal heart--essentials for the radiologist.

Modern radiological imaging provides precise diagnosis in congenital heart disease (CHD). The most important and readily available radiological examination is still chest radiography. The diagnostically most important imaging method is echocardiography. Magnetic resonance imaging and computed tomography have gained much ground over the more invasive cardiac catheter angiography, which is still needed in more complex conditions and for interventional procedures, which are performed more frequently. This article is focused on imaging of the neonatal heart. Basically, characteristics of the chest

Bedside Ultrasound for Early Diagnosis of Necrotizing Enterocolitis: A Pilot Study.

OBJECTIVE: This study aimed to assess whether bedside ultrasound (BUS) as the first imaging modality allows an earlier diagnosis of necrotizing enterocolitis (NEC) compared with abdominal radiography. STUDY DESIGN: A before-after controlled study in preterm infants with suspected NEC. The intervention group (October 2019-October 2021) received BUS as the first imaging modality and was managed accordingly to BUS findings. The control group (October 2015-September 2019) received radiography as the first imaging modality. The main outcome was NEC confirmation at the time of initial imaging. Seco

[A mallet finger, and then?].

Mallet finger (hammer finger) predominantly results from trauma causing injury to the terminal extensor tendon at the level of the distal phalanx. It remains unclear whether radiography provides additional value in all patients with a mallet finger, which warrants further investigation. We recommend obtaining radiographs in primary care only for mallet fingers resulting from high-energy trauma. The purpose of radiography should not be to distinguish between tendinous and bony mallet fingers, but rather to assess whether surgical intervention may be indicated.

Challenging, Safe, and Effective Use of External Iliac Vein for Insertion of Tunneled Cuffed Hemodialysis Catheters: A Single-Center Prospective Study.

BACKGROUND: Providing well-functioning vascular access is crucial for patients undergoing chronic hemodialysis. Peripheral arteriovenous fistulas and grafts are the preferred accesses in hemodialysis patients. Patients with bilateral obstruction of internal jugular veins and subclavian veins require a suitable vascular access. Thus, the insertion of iliac vein tunneled cuffed catheters (TCCs) by interventional nephrologists may be a good option for these patients. We aimed to evaluate the outcomes of iliac vein TCCs in patients lacking other vascular options. METHODS: 80 tunneled cuffed hemod

Health human resources shortages in radiography and sustainable workforce development in Australia.

OBJECTIVES: Health Human Resources (HHR) are critical for the effective functioning of healthcare systems, yet significant shortages exist, particularly in radiography. The increasing demand for diagnostic radiography services, driven by advancements in medical technology, an aging population, and the prevalence of chronic diseases, exacerbates these shortages. The COVID-19 pandemic further highlighted workforce vulnerabilities, increasing workloads and burnout. This review examines HHR shortages in radiography in Australia and proposes strategies for sustainable workforce development. KEY FI

Radiology of ileal pouch-anal anastomosis: normal findings, examination pitfalls, and complications.

Ileal pouch-anal anastomosis (IPAA) is a procedure in which an ileal reservoir is constructed after total colectomy and anastomosed to the anus. IPAA is a well-established option for patients who require surgery for chronic ulcerative colitis or familial adenomatous polyposis. Although excellent functional results can be achieved with IPAA, the procedure is associated with an appreciable number of complications, including small bowel obstruction, pouch fistula, anastomotic separation, anastomotic leakage, pelvic infection and abscess, stricture, and pouchitis. However, most of these complicati

[Medical radiation exposure and justification at a large teaching hospital: comparison of radiation-related and disease-related risks].

PURPOSE: The medical X-ray exposure was determined in a 2400-bed hospital. The radiation-related risk was compared with the severity of disease (ICD) to verify the justification for X-ray procedures. A model to estimate radiation and disease-related "loss of lifetime" was applied. MATERIALS AND METHODS: X-ray exposure from radiography, fluoroscopy and CT was determined for diagnostic and interventional procedures during one hospital stay of 403 patients (0.5 % of all 80 000 patients/year). CTDI and DLP in CT, DAP in fluoroscopy or SED in radiography were used to calculate the effective dose (

Partial Venous Inflow Occlusion under Mild Hypothermia for Membranectomy in a Dog with Cor Triatriatum Dexter.

Cor triatriatum dexter (CTD) is an uncommon congenital cardiac anomaly in dogs. This case report describes successful membranectomy for CTD via partial venous inflow occlusion under mild hypothermia in a dog. A 7-month-old intact male mixed-breed dog weighing 20.5 kg presented with a history of abdominal distention, lethargy, and anorexia. Clinical examination, radiography, echocardiography, microbubble testing, and computed tomography revealed a remnant right atrium membrane obscuring the venous blood inflow from the vena cava. Considering the potential risk of re-stenosis following intervent

Laparoscopic distal pancreatectomy for hemosuccus pancreaticus due to an intraductal papillary mucinous neoplasm.

Hemosuccus pancreaticus (HP) is a condition that is defined as gastrointestinal hemorrhage which occurs through the pancreatic duct. We present the rare case of a 75-year-old woman who developed HP caused by intraductal papillary mucinous neoplasm (IPMN). She had been followed for sarcoidosis, splenic artery aneurysm and pancreatic cyst. The patient presented to the emergency room of our hospital with epigastric pain and bloody discharge. Computed tomography revealed hemorrhage from a splenic artery aneurysm in a cystic lesion of the pancreas. On angiography, extravasation from the splenic art
