Feasibility of non-coplanar tomotherapy for lung cancer stereotactic body radiation therapy.

To quantify the dosimetric gains from non-coplanar helical tomotherapy (HT) arcs for stereotactic body radiation therapy (SBRT) of lung cancer, we created oblique helical arcs by rotating patient's CT images. Ten, 20 and 30 degrees of yaws were introduced in the treatment planning for a patient with a hypothetical lung tumor at the upper, middle and lower portion of the right lung, and the upper and middle left lung. The planning target volume (PTV) was 43 cm(3). 60 Gy was prescribed to the PTV. Dose to organs at risk (OARs), which included the lungs, heart, spinal cord and chest wall, was opt

An on-line replanning method for head and neck adaptive radiotherapy.

Daily setup for head and neck (HN) radiotherapy (RT) can vary randomly due to neck rotation and anatomy change. These differences cannot be totally corrected by the current practice of image guided RT with translational repositioning. The authors present a novel rapid correction scheme that can be used on-line to correct both interfractional setup variation and anatomy change for HN RT. The scheme consists of two major steps: (1) Segment aperture morphing (SAM) and (2) segment weight optimization (SWO). SAM is accomplished by applying the spatial relationship between the apertures and the cont

Atlas selection methods for multi-atlas-based segmentation in breast cancer radiotherapy planning.

We evaluated atlas selection methods for multi-atlas-based segmentation (MABS) in breast cancer radiotherapy planning. Forty-five patients were divided into 30 atlas and 15 test cases. The 30 atlases were stratified into three groups based on breast separation, height, and volume. Firstly, MABS was performed on each of the 30 atlas cases using the remaining 29 atlases. Secondly, MABS was performed on 15 test cases using the 30 atlases. The Dice similarity coefficient (DSC) was calculated to assess the agreement between MABS and manual segmentation. The DSC was found to increase as more atlases

Automated Tumor Segmentation in Radiotherapy.

Autosegmentation of gross tumor volumes holds promise to decrease clinical demand and to provide consistency across clinicians and institutions for radiation treatment planning. Additionally, autosegmentation can enable imaging analyses such as radiomics to construct and deploy large studies with thousands of patients. Here, we review modern results that utilize deep learning approaches to segment tumors in 5 major clinical sites: brain, head and neck, thorax, abdomen, and pelvis. We focus on approaches that inch closer to clinical adoption, highlighting winning entries in international compet

A research on the improved rotational robustness for thoracic organ delineation by using joint learning of segmenting spatially-correlated organs: A U-net based comparison.

PURPOSE: To study the improved rotational robustness by using joint learning of spatially-correlated organ segmentation (SCOS) for thoracic organ delineation. The network structure is not our point. METHODS: The SCOS was implemented in a U-net-like model (abbr. SCOS-net) and evaluated on unseen rotated test sets. Two hundred sixty-seven patients with thoracic tumors (232 without rotation and 35 with rotation) were enrolled. The training and validation images came from 61 randomly chosen unrotated patients. The test data included two sets. One consisted of 3000 slices from the rest 171 unrotat

Automated planning architectures for online adaptive radiotherapy: A narrative review of the ethos and unity/Monaco platforms.

Online adaptive radiotherapy (OART) modifies treatment plans using anatomy-of-the-day imaging, but successful clinical implementation depends on automated planning strategies that can generate high-quality plans within the constraints of an online workflow. This narrative review compares the automated planning architectures of the Varian Ethos and Elekta Unity/Monaco systems using a targeted literature search through July 2026. Ethos generates a daily adapted plan through goal-prioritized supervisory optimization governed by an unchanged clinical directive. Unity/Monaco provides Adapt-to-Posit

High-precision localization of radiation isocenter using Winston-Lutz test: Impact of collimator angle, phantom position, and field size.

PURPOSE: This study aimed to evaluate the impact of collimator angle, ball bearing (BB) phantom position, and field size on the accuracy of Winston-Lutz (WL) test-derived radiation isocenters. METHODS: WL tests were performed on four TrueBeam linear accelerators. Fifty-six images (eight gantry angles multiplied by seven collimator angles) were acquired for each WL test. Images with different sets of collimator angles were used to compute the radiation isocenters. The resulting radiation isocenters were correlated with the collimator angles. Then, the BB position and radiation field size were

[Dosimetric planning in the radiation therapy of patients with cancer of the corpus uteri].

The paper deals with application of computerized procedures for dose planning in intracavitary irradiation of endometrial carcinoma from the Agat-B source. Computer calculations of dosage measured at some basic points of reference as well as dose distribution in different schedules of multipositional irradiation of carcinoma are presented. Lateral position of single-source applicators and central position of a 4-5 source applicator proved to offer most advantage. Radiobiologic aspects of planning radiation treatment for endometrial carcinoma versus dose rate in application of three-position te

Conebeam CT-guided 3D MLC-based spatially fractionated radiation therapy for bulky masses.

For fast, safe, and effective management of large and bulky (≥8 cm) non-resectable tumors, we have developed a conebeam CT-guided three-dimensional (3D)-conformal MLC-based spatially fractionated radiation therapy (SFRT) treatment. Using an in-house MLC-fitting algorithm, Millennium 120 leaves were fitted to the gross tumor volume (GTV) generating 1-cm diameter holes at 2-cm center-to-center distance at isocenter. SFRT plans of 15 Gy were generated using four to six coplanar crossfire gantry angles 60° apart with a 90° collimator, differentially weighted with 6- or 10-MV beams. A dose was calc

Radiation therapy treatment planning in supratentorial glioblastoma multiforme: an analysis based on post mortem topographic anatomy with CT correlations.

In a previous study of the brains of 15 adults with glioblastoma multiforme who received minimal or no radiotherapy we determined the topographic distribution of tumor cells. All 15 brains had been fixed and then cut in the coronal or horizontal plane. The distribution of neoplastic cells was determined and entered onto tracings of the whole mount histologic sections. The last CT scans obtained prior to death of 11 of the patients were reviewed independently by a neuroradiologist who traced, on the CT scans, the outer edge of both the contrast-enhancing area and the peritumoral low density "ed

A semi-supervised domain adaptation method with scale-aware and global-local fusion for abdominal multi-organ segmentation.

BACKGROUND: Abdominal multi-organ segmentation remains a challenging task. Semi-supervised domain adaptation (SSDA) has emerged as an innovative solution. However, SSDA frameworks based on UNet struggle to capture multi-scale and global information. PURPOSE: Our work aimed to propose a novel SSDA method to achieve more accurate abdominal multi-organ segmentation with limited labeled target domain data, which has a superior ability to capture the multi-scale features and integrate local and global information effectively. METHODS: The proposed network is based on UNet. In the encoder part, a

Dosimetric investigation of small fields in radiotherapy measurements using Monte Carlo simulations, CC04 ionization chamber, and razor diode.

The motivation of this study is to check the dosimetry of small field sizes used in various treatment techniques using different methods (Monte Carlo simulations and detectors). We created two Monte Carlo models for Elekta Versa HD linear accelerators using EGSnrc (BEAMnrc-DOSXYZnrc) codes. Previous studies led us to define one model with an ideal symmetry full-width-half-maximum (FWHM) of 0.15 cm in the x and y directions for the Gaussian distribution of the primary electron source and redefine the other with a larger asymmetry FWHM of 0.35 cm in the X and 0.6 cm in the Y directions. We calcu

[Creation and evaluation of a concave dose distribution by combining multiple arc fields].

To create a concave dose distribution, a partial shielding radiation technique or intensity-modulated radiation therapy (IMRT) is usually required. However, in the present study we focused on how to create a concave dose distribution using conventional irradiation techniques. A treatment plan was experimentally created using planning CT scans of the neck. Two target volumes were predefined: planning target volume (PTV) 1, which included macroscopic tumor volume, tonsil, and bilateral retropharyngeal node, and PTV2, which included macroscopic and microscopic tumor volume. The prescribed doses f

Toxicity, normal tissue and dose-volume planning parameters for radiotherapy in soft tissue sarcoma of the extremities: A systematic review of the literature.

BACKGROUND: Patients with soft tissue sarcoma of the extremities (STSE) are left with high incidence of toxicities after Radiotherapy (RT). Understanding the normal tissue dose relationship with the development of long-term toxicities may enable better RT planning in order to reduce treatment toxicities for STSE. This systematic review of the literature aims at reporting the incidence of acute and late toxicities and identifying RT delineation guidance the normal tissues structures and dose-volume parameters for STSE. AI-assisted software development for brachytherapy seed inventory management.

PURPOSE: Medical physics departments frequently rely on in-house software to address niche clinical needs, yet these tools are often built by physicists without formal software engineering training, resulting in legacy applications that are difficult to maintain, poorly documented, and vulnerable to obsolescence from operating system upgrades. This study investigates the use of AI-driven "vibe coding", i.e. software development through natural language prompting, to enable a clinical physicist with no web development experience to replace an obsolete MATLAB-based radioactive seed inventory sys

Compressed radiotherapy treatment planning: A new paradigm for rapid and high-quality treatment planning optimization.

BACKGROUND: Radiotherapy treatment planning involves solving large-scale optimization problems that are often approximated and solved sub-optimally due to time constraints. Central to these problems is the dose influence matrix-also known as the dij matrix or dose deposition matrix-which quantifies the radiation dose delivered from each beamlet to each voxel. Our findings demonstrate that this matrix is highly compressible, enabling a compact representation of the optimization problems and allowing them to be solved more efficiently and accurately. PURPOSE: To develop a compressed radiotherap
