Perforator vessel recipient options in the lower extremity: an anatomically based approach to safer limb salvage.

When free tissue transfer is employed for defects of the lower third of the leg, recipient anastomoses are typically performed to major vessels. The aim of this study was to assess soleal perforators located in the distal half of the leg as potential vessels for free flap recipient vessels. Six fresh cadavers (12 limbs) were dissected. Perforators of adequate size (>or=1 mm) were documented as was the location and ease of dissection. Lower extremity magnetic resonance angiograms (MRAs) of 18 extremities were retrospectively reviewed. Two free tissue transfers to lower extremity perforators wer

Graft vessel wall pathology in a case of hepatic artery pseudo-aneurysm in a liver transplant recipient.

Pseudo-aneurysms (PAs) of the hepatic artery are rare complications of liver transplantation, which are characterized by a high mortality rate. The majority occur within the first 2 months after orthotopic liver transplantation. They become clinically manifest with sudden hypotension, gastrointestinal bleeding, and abnormal liver function test results. Early diagnosis and treatment are essential to prevent life-threatening hemorrhage. Conventional treatment consists of surgical resection and vascular reconstruction, but a feasible treatment option involves an angiographic approach with the pos

Recipient vessel selection for microvascular chest wall reconstruction: A narrative review and decision support framework.

BACKGROUND: Recipient vessel selection is a critical determinant of success in microvascular chest wall reconstruction. While the internal mammary and thoracodorsal vessels are most frequently used, prior surgery, radiotherapy, or flap design may necessitate alternative or salvage options. Despite its importance, no unified framework currently guides systematic vessel selection across reconstructive scenarios. METHODS: A narrative review of the literature was conducted across PubMed, Medline, Embase, and Web of Science from database inception to August 2025. Studies reporting clinical outcome

Anterior Tibial Vessel Turnover as an Alternative Recipient Strategy in Lower Extremity Free Flap Reconstruction.

Background/Objectives: Reconstruction of complex lower extremity soft tissue defects remains challenging, particularly in the proximal and middle tibial regions, including the knee, where suitable recipient vessels are often limited due to prior trauma, infection, or surgical intervention. This study aimed to evaluate the feasibility and clinical applicability of anterior tibial vessel turnover as an alternative recipient vessel strategy in free flap reconstruction. Methods: A retrospective review was conducted of seven patients who underwent free flap reconstruction using anterior tibial vess

Recipient Vessel Selection in Free Flap Phalloplasty: A Systematic Review and Single-Arm Meta-Analysis.

BACKGROUND: Selection of recipient vessels in phalloplasty is challenging due to the absence of adequate vessels in the immediate proximity. There is no consensus on the appropriate vessel selection. We hypothesized that recipient vessel selection in phalloplasty is associated with differences in complications and flap survival. METHODS: A literature search following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines across Ovid MEDLINE/PubMed, Embase, and CENTRAL databases identified 46 articles. Collected data included demographics, surgical details, dono

Outcomes for recipient vessel selection in head and neck free flap reconstruction: a retrospective cohort study.

Microvascular free flap reconstruction is essential in head and neck reconstruction. Cervical recipient vessels are traditionally preferred, whereas temporal recipient vessels are increasingly used in selected cases. This study aimed to compare outcomes of temporally versus cervically anastomosed free flaps. A retrospective cohort study including 158 free flap reconstructions was conducted at a single center. Seventy-nine flaps were anastomosed to temporal recipient vessels and compared with a cohort of cervically anastomosed flaps during the same period. Baseline characteristics, perioperativ

Extension of the jejunum in the reconstruction of cervical oesophagus with free jejunum transfer using the thoracoacrominal vessels as recipients.

BACKGROUNDS: The degree to which the jejunum can reach upward is a significant consideration in cervical oesophagus reconstruction with vascularised free jejunum transfer using the thoracoacrominal vessels as recipient vessels. The present study aims to elucidate this issue. MATERIALS AND METHODS: In 30 fresh cadavers, the thoracoacrominal vessels were dissected, and the jejunums were harvested, carrying the second branches of the superior mesenteric arteries and veins as their pedicles. After the mesenteric vessels were anastomosed to the thoracoacrominal vessels, the jejunums were advanced

Usefulness of Pulsed Wave Doppler Ultrasonography for Evaluating Recipient Vessels of Free Flap Reconstruction in Patients With Severe Extremity Trauma: A Preliminary Report.

BACKGROUND: In cases of severe extremity trauma treated with the 'internal fixation for fractures and flap' technique, successful soft tissue reconstruction requires selecting recipient vessels that avoid the zone of injury. Pulsed wave Doppler ultrasonography has been proposed as a useful tool for the preoperative evaluation of recipient vessels in plastic surgeries. This report aimed to present the preliminary experience of pulsed wave Doppler ultrasonography in traumatic disease. METHODS: This report included 14 free flaps from 13 patients with severe extremity trauma. The flaps included n

Determination of the recipient vessels in the head and neck using multislice spiral computed tomography angiography before free flap surgery: a preliminary study.

Preoperative assessment of the recipient vessels in free flap surgery directly affects the success rate of the operation by determining the flap type, pedicle length, orientation to the recipient site, and need for a vein graft. For this purpose, conventional angiographic methods are still being used with some disadvantages. The aims of this study were to evaluate the potential success of multislice computed tomography angiography in assessment of the recipient vessels before free flap surgery and to reveal if this may be an alternative to conventional angiography. The study was bilaterally ca

Utilization of the internal mammary perforators as the recipient vessels for microsurgical breast reconstruction: A systematic review and meta-analysis of the literature.

BACKGROUND: The selection of reliable recipient vessels is essential for successful free tissue transfer. The use of internal mammary intercostal perforators (IMAPs), instead of the internal mammary vessels as the recipient vessels, has been described in breast reconstruction. Debates exist regarding the reliability of these perforators as recipient vessels because of their variability in location and caliber. The aim of this paper was to conduct a systematic literature review and meta-analysis to determine the reliability of the IMAPs as recipient vessels. METHODS: A systematic literature re

[Reconstruction of Perineal and Pelvic Defects: the Inferior Epigastric Artery (DIEA) as a Recipient Vessel for Free Flap Reconstruction].

Extensive defects in the perineal or anterior/lateral pelvic region can occur as a result of tumour-related resections, infections, or postoperative complications. In cases where local or pedicled flap reconstructions are insufficient, microsurgical reconstruction using free flaps becomes necessary. Due to its location and calibre, the inferior epigastric artery (DIEA) is a suitable recipient vessel for microsurgical tissue transfer. In a retrospective case study conducted between June 2024 and March 2025, patients with perineal or pelvic defects underwent microsurgical reconstruction. Defect c

Mechanical Dilation of the Recipient Vessel with the DeBakey Vascular Dilator in Lower Extremity Reconstruction: A Report of Two Cases.

In lower extremity reconstruction, the recipient vessel often requires long-range mechanical dilation because of extensive vasospasm or plaque formation induced by concomitant atherosclerosis. While a forceps dilator can be used to manipulate and dilate vessels approximately 1 cm from their end, a DeBakey vascular dilator can dilate long-range vessels. The authors successfully performed free flap reconstruction of the lower extremity using the DeBakey vascular dilator. Of the two patients who underwent lower extremity reconstruction, one had extensive vasospasm, and the other had plaques in th

The Descending Genicular Artery as a Recipient Vessel in Chronic Limb-Threatening Ischemia-A Case Report and Literature Review.

In recent decades, the increasing complexity of arterial bypasses in the management of chronic limb-threatening ischemia has spurred the development of alternative techniques, such as revascularization of genicular arteries. Few publications on this technique can be found in the literature, and its use has been restricted to specialized vascular groups. This article describes the case of a patient with extensive femorotibial occlusive disease who received a collateral artery bypass, using the deep femoral artery as a donor, the cephalic vein as an alternative autogenous substitute, and the des

Anterior Interosseous Artery as a Recipient Vessel in Superficial Circumflex Iliac Artery Perforator Flap Transplantation to the Forearm.

BACKGROUND: When transplanting a superficial circumflex iliac artery perforator (SCIP) flap to the forearm, the small diameter of the superficial circumflex iliac artery (SCIA) complicates end-to-end anastomosis with the radial or ulnar artery. The small diameter of the anterior interosseous artery (AIA) may make it suitable for end-to-end anastomosis with the SCIA; however, no such cases have been reported to date. We assessed AIA diameter using computed tomography angiography (CTA) and analyzed six cases of SCIP flap transfer to the forearm using the AIA as the recipient vessel. METHODS: In

Medial sural artery as a salvage recipient vessel for complex post traumatic microvascular lower limb reconstruction.

BACKGROUND: Complex lower extremity trauma still poses a formidable challenge for micro vascular reconstruction. The extensive surrounding zone of trauma can make it difficult to find a suitable recipient pedicle for anastomosis. A need was felt for exploration of newer recipient vessels in lower extremity which could be harnessed for reliable and tension free micro anastomosis. The purpose of the present report is to highlight the possibility of using medial sural artery for safe micro vascular anastomosis in selected scenarios like vessel depleted extremity. METHODS: Between 2008 and 2013,

Reconstruction of Anterior and Posterior Tibial Arteries as Recipient Vessels for Free Flap Transfer in Gustilo-Anderson Classification Type IIIB Severe Open Tibial Fractures.

BACKGROUND: In severe lower leg trauma requiring soft tissue transfer, a limited number of run-off vessels is a risk factor for reoperation and flap necrosis. We hypothesized that early reconstruction of injured major arteries (the anterior tibial artery (ATA) or posterior tibial artery (PTA)) to prepare two recipient arterial systems could reduce anastomotic complications. This study aimed to evaluate the effectiveness of arterial reconstruction in patients with severe lower extremity trauma. METHODS: This study included patients with Gustilo-Anderson type IIIB open lower leg fractures and a
