Meta-analysis of recurrent laryngeal nerve injury in thyroid surgery with or without intraoperative nerve monitoring.

Intraoperative nerve monitoring (IONM) aimed at reducing the injuries of recurrent laryngeal nerve during thyroidectomy is controversial. We conducted a meta-analysis to assess the incidence of nerve injuries with or without IONM. Studies published from January 1994 to February 2012 in English language on humans were identified. Heterogeneity of studies was checked by the Higgins test. Summary estimates of predictive values of injury were made using the Mantel-Haenszel test based on the fixed-effects model. Publication bias was assessed by a funnel plot and Egger's method. Eight articles were

Refining postoperative monitoring of recurrent laryngeal nerve injury in esophagectomy patients through transcutaneous laryngeal ultrasonography.

BACKGROUND: Recurrent laryngeal nerve injury (RLNI) leading to vocal cord paralysis (VCP) is a significant complication following minimally invasive esophagectomy (MIE) with upper mediastinal lymphadenectomy. Transcutaneous laryngeal ultrasonography (TLUSG) has emerged as a non-invasive alternative to endoscopic examination for evaluating vocal cord function. Our study aimed to assess the diagnostic value of TLUSG in detecting RLNI by evaluating vocal cord movement after MIE. METHODS: This retrospective study examined 96 patients with esophageal cancer who underwent MIE between January 2021 a

Intraoperative electromyogram monitoring of the recurrent laryngeal nerve: experience with an intralaryngeal surface electrode. A method to reduce the risk of recurrent laryngeal nerve injury during thyroid surgery.

INTRODUCTION: A clinical method to localize the recurrent laryngeal nerve intraoperatively in order to minimize the risk of accidental injury is presented. METHODS AND RESULTS: By means of an electrode, the nerve was stimulated by a pulsed contact current. The resulting muscle potential was detected using an electrode placed in the larynx. We applied this technique during the time period between 1 January 1997 and 31 December 1998. In 96 cases of primary thyroid resection, the recurrent nerve was identified 167 times. The patients were operated on for nodular thyroid (n=85), Grave's disease (

Injury mechanisms and electromyographic changes after injury of the recurrent laryngeal nerve: Experiments in a porcine model.

BACKGROUND: Recurrent laryngeal nerve (RLN) injury during surgery may reveal differences in electromyographic (EMG) changes after sustained compression or traction. METHODS: In 20 pigs with the NIM-FLEX EMG-endotracheal tube, EMG was recorded at baseline, during sustained RLN compression, or traction until 70% amplitude decrease and during 30 minutes of recovery. RESULTS: Seventy percent amplitude decrease from baseline was reached after 110 ± 98 seconds (compression group) and 2034 ± 2108 seconds (traction group). Traction induced a pronounced latency increase, peaking at 122 ± 8% in contra

Paradoxical movement of rat vocal folds following recurrent laryngeal nerve injury.

OBJECTIVE: To establish a rat model with paradoxical vocal fold movement to understand the detailed etiology and physiology of laryngeal synkinesis by evaluating vocal fold movement and by electromyography. METHODS: Adult Wistar rats were used. The recurrent laryngeal nerve was transected, anastomosed, and the anastomotic portion was placed in a silicone tube. At 2, 4, and 10 weeks after the treatment (n = 30), we performed laryngoscopy and electromyography. The vocal fold movement was recorded, the hemiglottal area was measured, and the Δarea was calculated by subtracting the area during exp

Mediastinal traction injury to the recurrent laryngeal nerve: An unusual cause for a hoarse voice.

BACKGROUND: Chest wall trauma is a frequent cause for emergency department presentations, with traumatic pneumathoraces a relatively common occurance. Vocal cord paralysis however, is uncommon. CASE REPORT: A 44 year old man presented with a traumatic pneumothorax following a fall. A weak and hoarse voice was noted and vocal cord palsy demonstrated on indirect laryngoscopy. CT chest was unremarkable for mediastinal lesions. CONCLUSIONS: Clinicians need be mindful of traction injuries to mediastinal structures when examining those with chest wall injuries.

Recurrent laryngeal nerve injury: a complication of central venous catheterization-a case report.

This case report highlights the emergence of new complications associated with old procedures as the frequency of their use increases. A 29-year-old man with aplastic anemia was admitted for a bone marrow transplant. Before this procedure, a Hickman line was inserted. This was followed by hoarseness of voice. An examination revealed an immobile left vocal cord. Further investigations did not show any cause for this complication. Therefore, the hoarseness and vocal cord paralysis was attributed to trauma associated with the insertion of the Hickman line.

Novel Biomolecule-Infused Gelatin Injectable for Treatment of Recurrent Laryngeal Nerve Injury.

OBJECTIVE: Unilateral vocal fold paralysis (UVFP) due to recurrent laryngeal nerve injury (RLN) is a major cause of voice disorders. We have recently identified three biomolecules (agrin, acetylcholine, and neuregulin) with the potential to promote reinnervation after RLN injury. This study aimed to determine if a gelatin injectable with the reinnervating biomolecules will induce site-specific amplification of neurotrophic factor release and reinnervation after RLN injury in unilateral vocal fold paralysis. METHODS: C57BL/6 mice underwent RLN transection with the following treatment allocatio
