AIM OF THE STUDY: Hepatic changes have been described during the refeeding syndrome due to increase in enzymes and hepatomegaly; however, they have not been properly described. Thus, the objective of this study was to investigate the hepatic histological characteristics and biochemical markers of hepatic steatosis in Wistar rats with refeeding syndrome.
MATERIAL AND METHODS: Thirty male Wistar rats were allocated to one of three groups: C, F or R. The animals from group C received an AIN-93 diet for 96 hours, and were then sacrificed. Animals allocated to group F were fasted for 48 hours and

Aging is linked to physiological and pathophysiological changes. In this context, elderly patients often are frail, which strongly correlates with negative health outcomes and disability. Elderly patients are often malnourished, which again is an independent risk factor for both frailty and adverse clinical outcomes. Malnutrition and resulting frailty can be prevented by adequate nutritional interventions. Yet, use of nutritional therapy can also have negative consequences, including a potentially life-threatening metabolic alteration called refeeding syndrome (RFS) in high-risk patients. RFS

The refeeding syndrome is increasingly recognised. It is a serious change in electrolytes when nutrition is reintroduced to malnourished patients. Alcohol dependence is a risk factor for the refeeding syndrome. We report a prospective cohort study of 36 alcoholics hospitalised for withdrawal management. We found no evidence of refeeding syndrome in any patient after 3 days of hospitalisation, despite hypomagnesaemia, a risk factor for the refeeding syndrome being prevalent (44% of subjects). Low thiamine levels were infrequent affecting 3/29 (10%). We recommend that in alcoholics admitted for

Refeeding syndrome (RS) is a serious and potentially fatal disorder. It is caused by a shift of fluids, sodium, potassium, magnesium and phosphorus as well changes in the metabolism of glucose, protein, fat and vitamins following the refeeding of malnourished patients, whether enterally or parenterally. RS has rarely been reported in patients with advanced kidney disease probably due to the pre-existing hyperphosphatemia, hypermagnesemia and hyperkalemia in these patients. In the following report, we present a patient with nephronophthisis type 1 deletion syndrome in whom her main previous nut

OBJECTIVES: The aim of this study was to describe the clinicopathological findings, management and outcome of cats with refeeding syndrome (RS) following prolonged starvation.
METHODS: Records from four referral hospitals were searched between May 2013 and November 2019 and retrospectively evaluated. Inclusion criteria were the presence of a risk factor for RS, such as severe weight loss or emaciation following a period of presumed starvation, hypophosphataemia or a delta phosphorous exceeding 30% reduction following refeeding, being treated on the basis of a clinical diagnosis of RS and one

PURPOSE: Anorexia nervosa (AN) is a complex medical condition affecting mainly adolescents and young adults. To monitor and prevent refeeding syndrome, current guidelines recommend daily laboratory testing in the first week of hospitalization and 2-3 times/week for the following 3 weeks. The aims of this study were to determine the proportion of abnormal results of the blood tests done during the first week of nutritional rehabilitation in adolescents with AN, the proportion of test having led to supplementation and the cost of all these tests.
METHOD: A retrospective chart review of admissio

Overzealous refeeding in chronically malnourished anorexia nervosa patients may cause life-threatening complications. We describe a 14-year-old girl with anorexia nervosa who had a decrease in body weight from 45 kg to 25.5 kg over an 18-month period. She received 40 kcal.kg-1.d-1 carbohydrate-rich nutrition via enteral and parenteral routes. Her serum phosphate concentration dropped from a baseline of 1.39 mmol/L (4.3 mg/dL) to 0.19 mmol/L (0.6 mg/dL) on Day 4 of refeeding. Concurrent with the development of hypophosphatemia, she became drowsy and developed generalized muscle weakness, impair

OBJECTIVE: Refeeding syndrome can occur in several contexts of relative malnutrition in which an overaggressive nutritional support is started. The consequences are life threatening with multiorgan impairment, and severe electrolyte imbalances. During refeeding, glucose-involved insulin secretion causes abrupt reverse of lipolysis and a switch from catabolism to anabolism. This creates a sudden cellular demand for electrolytes (phosphate, potassium, and magnesium) necessary for synthesis of adenosine triphosphate, glucose transport, and other synthesis reactions, resulting in decreased serum l

OBJECTIVE: Determine the incidence of refeeding syndrome, defined by the presence of hypophosphatemia in very-low-birth-weight (VLBW) infants with intrauterine growth restriction (IUGR) compared with those without IUGR.
STUDY DESIGN: In this retrospective cohort study, VLBW infants admitted over a 10-year period (271 IUGR and 1982 non-IUGR) were evaluated for specific electrolyte abnormalities in the first postnatal week.
RESULT: IUGR infants were significantly more likely to have hypophosphatemia (41% vs 8.9%, relative risk (95% confidence interval: 7.25 (5.45, 9.65)) and severe hypophospha

Commentary is provided on the pivotal paper by Weinsier and Krumdieck from 1981 describing 2 patients who developed profound and fatal refeeding syndrome following initiation of aggressive total parenteral nutrition. This classic description was among the first to describe the overwhelming cardiovascular and pulmonary manifestations that can accompany parenteral refeeding with carbohydrate in chronically malnourished patients. The syndrome has also been described with oral and enteral nutrition. One of the hallmarks of the syndrome is hypophosphatemia. Since 1981, dosing schemes for addressing

Although cachectic patients are relatively well adapted to their calorically deprived state, they are prone to acute metabolic imbalances when infused with hypertonic solutions of dextrose and amino acids. Of particular concern is hypophosphatemia and its associated disorders of cardiac, pulmonary, hematological, and neuromuscular functions. This report describes two chronically malnourished but stable patients who were given aggressive total parenteral nutrition support, which was rapidly followed by acute cardiopulmonary decompensation associated with severe hypophosphatemia and other metabo

Refeeding syndrome (RFS) is a potentially life-threatening metabolic disorder that occurs when nutrition is rapidly reintroduced after prolonged starvation or malnutrition. It is characterized by insulin-driven shifts of phosphate, potassium, and magnesium into cells, leading to severe hypophosphatemia, hypokalemia, and hypomagnesemia. These electrolyte derangements impair myocardial energy metabolism, contractility, and conduction, resulting in arrhythmias, heart failure, and sudden cardiac arrest. Thiamin deficiency and fluid overload further exacerbate cardiac instability, making cardiovasc

Wernicke encephalopathy is caused by thiamine deficiency, and treatment with high-dose thiamine is recommended for its management. We herein report the case of an older adult woman who developed refeeding syndrome after receiving intravenous fursultiamine. Although the patient's oral food intake was restricted to prevent refeeding syndrome, the total caloric load, including glucose contained in fursultiamine, exceeded the recommended amount. This case demonstrates that the caloric contribution of glucose present in fursultiamine should not be overlooked when treating Wernicke encephalopathy wi

Refeeding syndrome (RFS) is a rare, potentially life-threatening, condition seen in malnourished patients starting refeeding. RFS may provoke seizures and acute encephalopathy and can be considered an internal severe neurotrauma in need of specific treatment. The objective was to describe course of disease, treatment and, for the first time, multimodal monitoring output in a comatose patient suffering RFS. After gastric-banding and severe weight loss, the patient initiated self-starving and was transferred to our intensive care unit (ICU) following rapid refeeding. At arrival, seizures, decrea

We present the case of a young male patient who presented with paralysing muscle weakness due to severe hypokalaemia and hypophosphataemia. The initial patient history evaluations could not establish the aetiology. Only after we reviewed the patient's history did he reveal that he had been following a severe calorie-restricted regime, the human chorionic gonadotropin diet, which had ended 2 days prior to developing symptoms. This information then allowed us to diagnose severe refeeding syndrome. As a further complication, the patient developed rhabdomyolysis. After correction of serum electrol

Rheumatic diseases can lead to a state of malnutrition via a variety of mechanisms. Malnutrition is defined as an insufficient availability of energy, proteins, electrolytes and other nutrients compared to the requirements of a healthy body. After such a catabolic phase, a sudden resupply of the body's full caloric needs can cause life-threatening complications due to an acute paucity of electrolytes and micronutrients. Such metabolic disturbances occurring after the reconstitution of nutrition are termed refeeding syndrome. With sufficient background knowledge about the refeeding syndrome, ph

Refeeding syndrome (RS) is defined as fluid and electrolyte shifts as a result of aggressive nutritional rehabilitation and these electrolyte imbalance can result in several cardiac complications. We aimed to evaluate the incidences of RS and hypercholesterolemia in children with anorexia nervosa (AN) and related factors for that. We retrospectively evaluated the medical records of 51 patients aged 10-18 years diagnosed with AN between January 2015 and May 2020. RS and hypercholesterolemia were seen in 21 (41.2%) and 39 (76.5%) of patients, respectively. Patients with RS had an older mean age
