Recovery of Dexterous Motor Control via Non-Monosynaptic Corticospinal Pathways.

Fine motor control of the human arm is assumed to depend on monosynaptic connections between the motor cortex and spinal motoneurons. We report that people with post-stroke hemiparesis could regain dexterous control using non-monosynaptic corticospinal tract (CST) projections during epidural cervical spinal cord stimulation (SCS). Participants in our pilot clinical study demonstrated the ability to improve strength, reaching smoothness, and fine force control of the arm and hand while receiving continuous stimulation. Detailed electrophysiology of corticospinal connectivity with transcranial m

[A change in the reflex excitability of spinal motoneurons during daytime sleep in newborn infants].

Studies have been made on monosynaptic reflexes elicited by the electrical stimulation of the ulnar and peroneal nerves (H-reflexes) in 5-7-day infants during sleep and in the awake state. During the orthodox stage of sleep, which is characterized by slow oscillations in EEG and by synchronous respiratory movements of the thorax and abdomen, the amplitude of H-refelexes was the highest, variation coefficient being the lowest. During paradoxical stage of sleep, characterized by desynchronized EEG, the presence of rapid movements of the eyes and by asynchronous respiratory movements of the thora

Opioid antagonists and spinal reflexes in the anaesthetized cat.

In barbiturate-anaesthetized cats, intravenous naloxone (0.025-0.10 mg/kg) increased the amplitude of monosynaptic reflexes produced by electrical stimulation of the nerves to the biceps-semitendinosus and gastrocnemius muscles and the complex reflexes to electrical stimulation of myelinated afferents of the sural and tibial nerves and reflexes to electrical stimulation of unmyelinated primary afferents of the tibial nerve. Increases in reflexes were also produced by the (-)- but not the (+)-isomer of the opiate antagonist N- furylmethylnormetazocine (both isomers being given in the dose range

Reflexes in prior polio and their relation to weakness and anterior horn cell loss.

The aim of this study was to evaluate the reflex pattern in patients with prior polio and to relate these findings to the degree of anterior horn cell (AHC) involvement and loss of muscle force. Twenty-five prior polio subjects were investigated with electromyography (EMG), force testing and reflex studies, which included the patellar and Achilles reflex, H-reflex, T-response and interlimb reflex (ILR). The clinical reflexes, H/M-ratio and T-response amplitude at rest were positively correlated with force and negatively correlated with the degree of AHC loss. The H/M-ratio was decreased compar

Central effects of the neurotropic mycotoxin fumitremorgin A in the rabbit (I). Effects on the spinal cord.

Effects of a potent neurotropic mycotoxin, fumitremorgin A (FTA), on the spinal cord were studied, using rabbits lightly anesthetized with urethane and chloralose. Spontaneous discharges of L7 spinal ventral roots and common peroneal nerves were increased after intravenous injection of 100-200 micrograms/kg of FTA. Their abnormal discharge pattern represented the convulsive effect of FTA. Spinal monosynaptic reflexes became irregular in amplitude, with a slight predominance of smaller reflexes. Polysynaptic reflexes were inhibited in many cases. These FTA-induced changes in ventral root discha

[Effects of mecamylamine and pempidine, secondary and tertiary amines, on the spinal reflex of cats (author's transl)].

The ganglionic blocking effects of the secondary and tertiary amines, mecamylamine and pempidine, on the spinal reflex of cats of both sexes were investigated. These blocking effects were then compared with findings in the quaternary ammonium compounds such as tetraethylammonium (TEA) and decamethonium (C10). Mecamylamine (5 mg/kg) and pempidine (1 mg/kg) inhibited spinal reflex potentials such as the monosynaptic reflex (MSR), the polysynaptic reflex (PSR) and the dorsal root reflex (DRR). Maximal inhibition occurred 40 min after intravenous administration these drugs. In the case of mecamyla

[Subclinical forms of spinal disorders in discogenic lumbar radiculitis].

In patients with discogenic lumbar radiculitis with a unilateral affection of L5 or S1 root motor and central responses of the medial group of plantar muscles were recorded. In S1 root pathology the central response consisted in 25% of the cases in repeated antidromal discharges, which testified to an increased excitability of the corresponding alpha-motoneurons. In patients with L5-radiculitis a predominance of the reflex component in the response was typical. Since this phenomenon was lacking in combined L5 and S1 root lesions, it was interpreted as a sign of a decreasing function of the seg

[A study of the interaction between the peripheral motor neurons of muscle antagonists by the H-reflex method under normal conditions and in spastic hemiparesis].

With the aid of the reflex method the authors studied the functional condition and interconnection of peripheral motoneurons of the muscles-antogonists in normal conditions and in spastic hemiparesis. In patients with spastic hemiparesis there was an increase in the inhibition latent period of the H-reflex following a conditioning stimula of the fibular nerve and a nonmonotonous restitution in the amplitude of the H-reflex during the periods of "late inhibition". These data permit to assume that in patients with pyramidal insufficiency the changes in the system of reciprocal inhibitive interac

Intrathecal baclofen and the H-reflex.

Baclofen was given intrathecally to six patients with severe lower limb spasticity due to traumatic spinal cord injury. The effects of the drug on spasticity and the ratio between the maximum amplitude of the H reflex and the M response from the soleus (Hmax/Mmax ratio) were assessed. In each patient, spasticity was reduced following intrathecal baclofen and in four patients there was a reduction in the amplitude of the H reflex and Hmax/Mmax ratio. These results suggest that the Hmax/Mmax ratio may be helpful in establishing optimum drug dosage, particularly when the drug is used on a chronic

Local anesthetic activity of 1-(morpholino or piperidino)-2-propanol derivatives.

The local anesthetic activities of 1-(morpholino or piperidino)-2-propanol derivatives were investigated. These activities were observed with nerve trunk anesthesia, rabbit's corneal anesthesia and intradermal injection anesthesia. Among these propanol derivatives, No. 24 and No. 25 which possess thioether-type sulfur were the most active and the effective potency was enhanced by increasing the dissociation constant (pKa) value in a series of the compounds. Effects on cat spinal reflex were also observed. These propanol derivatives revealed no selective depression of presynaptic depression or

[Neurophysiology of parkinsonian syndromes].

Neurophysiological investigations may contribute to establish the diagnosis of different parkinsonian syndromes such as multiple system atrophy, progressive supranuclear palsy and corticobasal degeneration, which are often suspected in case of atypical parkinsonian syndrome. They provide a means of objective analysis of certain symptoms (myoclonus for example) and identify the pathophysiological processes underlying these parkinsonian syndromes. This article reviews especially the following electrophysiological tests: autonomic nervous system analysis (sympathetic sudomotor skin response, R-R

Spino-spinal mechanisms in the chloralose anaesthetized cat.

1. In chloralose anaesthetized cats the events in the lumbosacral cord that follow stimulation of a forepaw or neck nerve have been examined.2. Four events take place in the lumbosacral cord after forepaw or neck muscle nerve stimulation. These are: (1) a brief period of generalized enhanced motoneurone excitability, (2) a prolonged and as yet unexplained enhancement of monosynaptic reflexes, (3) a period of presynaptic inhibition in muscle and skin afferent fibres, and (4) a prolonged inhibition of spino-bulbo-spinal reflexes.3. A variable and sometimes selective reduction of one or another d

Reciprocal Ia inhibition in spastic paralysis in man.

Reciprocal reflex connections were studied in capsular hemiplegia and spastic paresis with spinal cord lesions, using Lloyd's technique. Effects of conditioning stimulation of the tibial or peroneal nerve on the H reflex in the antagonists were examined. Stimulus intensity was controlled with reference to the threshold of the M wave. Weaker stimulation than this threshold was regarded as stimulation of group I afferents. It aroused no subjection sensation in intact subjects. Early and strong inhibition, comparable to Ia inhibition in the cat (Lloyd 1946), was observed from weak stimulation of

Facilitation of the spinal H-reflex by auditory stimulation in dyslexic adults.

Dyslexic subjects show a variety of mild sensory and motor deficits that have been assumed to reflect dysfunction of the large-diameter 'magnocells' in different parts of the brain. Hearing as a warning sense relies on rapidly-conducting fibers, and on the basis of the magnocellular deficit theory, we wondered whether auditory alerting would be weakened in dyslexic adults. We quantified the strength of sound-induced spinal facilitation in seven dyslexic and eight normal-reading adults by measuring the amplitudes of H-reflex, a monosynaptic spinal reflex, after loud binaural sounds. The audiosp

Inhibitory effects of combined agonist and antagonist muscle vibration on H-reflex in man.

Vibration alters human sensory motor performance. Changes in the excitability of spinal reflex mechanisms may be responsible for the majority of the observed alterations. We studied the differential effects of vibration locally applied to gastrocnemius soleus and tibialis anterior muscles separately and to both muscles simultaneously. From the results, it is deduced that combined agonist and antagonist muscle vibration may lead to summative interaction between pre- and postsynaptic inhibition at motoneuronal level. Whole-body vibration is taken to mean a combination of synchronous vibrations a
