Clinical Impact of Specific Extraocular Muscle Manipulation and the Oculocardiac Reflex on Postoperative Vomiting in Pediatric Strabismus Surgery: A Multicenter, Observational Study.

BACKGROUND: Strabismus surgery, which is commonly performed in children, poses a high risk of postoperative vomiting. The current anesthesia guidelines for the prevention of postoperative vomiting in children are based on heterogeneous populations involving different types of surgery, and risk factors for postoperative vomiting in, specifically, the pediatric strabismus surgery population are unclear. Moreover, the effects of manipulating the deeply attached extraocular muscles and the oculocardiac reflex on this risk remain inconclusive. AIM: To evaluate the associations among inferior obliq

Effect of sub-anesthetic dose of esketamine on oculocardiac reflex of pediatric strabismus surgery in total intravenous anesthesia: study protocol for a randomized controlled trial.

BACKGROUND: The oculocardiac reflex (OCR) is the most common complication in pediatric strabismus surgery, and its main clinical manifestation is bradycardia. Esketamine can activate the sympathetic nervous system, leading to an indirect cardiovascular stimulation effect, which results in increased blood pressure and a faster heart rate. The aim of this study is to explore whether a sub-anesthetic dose of esketamine can effectively inhibit the oculocardiac reflex during strabismus surgery. METHOD: This prospective, single-center, randomized, double-blinded trial will enroll 204 pediatric pati

Comparing Effectiveness and Safety of Intravenous Atropine with Topical Tetracaine in Preventing and Relieving Oculocardiac Reflex in Patients Undergoing Strabismus Surgery: A Randomized Clinical Trial.

BACKGROUND: Oculocardiac reflex (OCR) is one of the serious complications following surgical therapeutic procedures for strabismus. Various medications have been tested to prevent or mitigate this complication. We aimed to compare the effect of intravenous atropine and topical tetracaine on the incidence and severity of OCR in strabismus surgery. MATERIALS AND METHODS: In this triple-blind randomized clinical trial study, 120 patients who were candidates for strabismus surgery were randomly assigned to receive intravenous atropine, topical tetracaine, or artificial tears as the control. Marcus Gunn Syndrome associated with an unusual oculo-cardiac reflex.

The Marcus Gunn Syndrome is discussed. It is postulated that an atypical oculocardiac reflex is easily initiated in these patients so that precautions must be taken for its prevention and early detection. Marcus Gunn first described the syndrome in 1883. His patient had a congenital ptosis of the left upper eyelid. Paradoxical lid retraction was evoked by movement of the jaw. Consequently the syndrome also became known as the 'jaw-winking' syndrome. Other varieties of the Marcus Gunn syndrome have also been described. The pathogenesis of this syndrome seems to be the presence of abnormal centr

The human heart rate response profiles to five vagal maneuvers.

Healthy teens and adults performed four vagotonic maneuvers. A large series of strabismus surgery patients had deliberately quantified tension on extraocular rectus muscles during general anesthesia. The mean bradycardia was greatest for diving response (apneic facial exposure to cold) and Valsalva maneuver and least for pressure on the globe and carotid sinus massage. Bradycardia occurred for every subject for the non-surgical maneuvers, however, extraocular muscle tension frequently caused no change in heart rate or even tachycardia. The inter-subject variance in percent heart rate change wa

The preventive effect of dexmedetomidine on anesthesia complications in strabismus surgery: a systematic review and meta-analysis.

OBJECTIVE: Dexmedetomidine is a medication that has analgesic, sedative, and anti-anxiety properties. In the clinical, it is often used to prevent common complications associated with strabismus surgery, including postoperative delirium, postoperative nausea and vomiting, postoperative pain, and oculocardiac reflex. However, its effectiveness and side effects of the present studies are different. The sample sizes of the present studies on the prevention of complications of dexmedetomidine are small. Therefore, this study evaluates the efficacy of dexmedetomidine in preventing anesthesia-relate

Sinus arrest induced by trivial nasal stimulation during alfentanil-nitrous oxide anaesthesia.

A case is reported of bradycardia and sinus arrest induced by insertion of a nasal temperature probe. Other possible causes of bradycardia and sinus arrest under anaesthesia are reviewed briefly. Evidence for the neurological basis of a nasocardiac reflex, similar to the oculocardiac reflex, is presented. A minor, trivial stimulus may elicit this reflex.

Bradycardia and asystole during temporomandibular joint reconstruction.

Anesthesia providers are familiar with the oculocardiac reflex, one type of trigeminocardiac reflex. While less common, arrhythmias associated with manipulation of other trigeminal nerve branches can occur. We report the presentation and management of bradycardia and asystole from stimulation of the mandibular branch of the trigeminal nerve during temporomandibular joint reconstruction.

Maxillomandibulocardiac reflex in a dog.

BACKGROUND: The trigeminocardiac reflex (TCR) is a brainstem reflex that may be observed in anaesthesia during surgical procedures stimulating the intracranial or peripheral portion of the trigeminal nerve. The peripheral TCR is divided into the oculocardiac reflex and the maxillomandibulocardiac reflex based on the affected sensory branches of the trigeminal nerve. In veterinary medicine the oculocardiac reflex has been described, however the maxillomandibulocardiac reflex has never been reported. CASE PRESENTATION: A 5-year-old male Epagneul Breton was presented for surgical management of a

Trigeminocardiac reflex in the Postanesthesia care unit.

Bradycardia caused by the oculocardiac reflex is an anticipated occurrence during certain surgeries. The afferent pathway involves the trigeminal nerve's ophthalmic division. Reflex bradycardia from the trigeminocardiac reflex, via stimulation of maxillary or mandibular divisions of cranial nerve V, although less well known, has also been reported intraoperatively. Unstable bradycardia associated with stimulation of the mandibular division of cranial nerve V during trigeminal neuralgia pain episodes in the Postanesthesia Care Unit is presented.

Influence of Pre- and Intraoperative Local Anaesthetic in Strabismus Surgery under General Anaesthesia in Adults.

BACKGROUND: As studies have shown a reduction in the occurrence of the oculocardiac reflex with the addition of local anaesthesia, we changed our care regime accordingly a few years ago. To promote and establish better patient care, we retrospectively analysed the files of our patients who underwent strabismus surgery from 2013 to 2021 in order to compare strabismus surgery under general anaesthesia with and without local anaesthetics in a routine clinical setting. PATIENTS AND METHODS: Data from 238 adult patients who had undergone strabismus surgery could be extracted from the files: G1: n

[Advances in the development of ocular cardiac reflex in extraocular muscle surgery].

Ocular cardiac reflex (OCR) usually occurs in ophthalmic surgery, especially in extraocular muscle(s) surgery. OCR generally can cause sinus bradycardia, arrhythmia, reduced atrial pressure, ventricular tachycardia, ventricular fibrillation, ventricular doublet, dizziness, nausea and other symptoms. Severe cases will appear cardiac arrest, respiratory arrest, etc. Recent studies on the mechanism of OCR and the relationship between surgical operation and anaesthesia in extraocular muscle surgery and the occurrence of OCR are reviewed in this paper in order to reduce the occurrence of OCR and tr

Bradycardia and the trigeminal nerve.

A review of the literature suggests that the oculocardiac reflex is part of a wider phenomenon involving any structure supplied by the trigeminal nerve. This can lead to potentially fatal complications in maxillofacial surgery. A review of the literature is discussed and two further cases are reported.

Reflex bradycardia in facial surgery.

Traction on, or stretching of, any structure innervated by the trigeminal nerve can cause reflex slowing of the pulse, inappropriately known as the "oculocardiac reflex" because it is well-known to result from traction on the extra-ocular muscles. This phenomenon can cause problems in surgery of the face and cranium as well as the orbit. An awareness of its management can obviate morbidity and mortality. An account of the phenomenon, its treatment and illustrative case histories are presented.
