Vestibulo-Ocular Reflex Function in Adolescents With Sport-Related Concussion: Preliminary Results.

BACKGROUND: Oculomotor impairments, dizziness, and imbalance are common after sports-related concussion (SRC) in adolescents and suggest a relationship between SRC and vestibular system dysfunction. However, it is not clear whether the source of these problems is attributable to the peripheral or central vestibular system. HYPOTHESIS: The video Head Impulse Test (vHIT), which assesses peripheral vestibular function, will show differences in gain between adolescents with and without SRC. Furthermore, there will be an association between vHIT and clinical balance and vestibular/oculomotor testi

Relationship between motion sickness susceptibility and vestibulo-ocular reflex gain and phase.

BACKGROUND: It has been proposed that individual susceptibility to motion sickness is related to the vestibulo-ocular reflex (VOR) through the activation of the velocity storage mechanism. OBJECTIVE: We investigated whether motion sickness level was related to the gain and phase of the VOR. METHODS: VOR gain and phase were measured in 214 subjects while they rotated in yaw at 0.01 Hz, 0.02 Hz, 0.04 Hz, 0.08 Hz, and 0.16 Hz in darkness, and results were compared to the severity of symptoms the subjects experienced during subsequent tests to provoke motion sickness. These tests included cross-

Effect of common antivertiginous agents on the high velocity vestibulo-ocular reflex.

OBJECTIVE: It has long been suggested that antivertiginous medications exert their symptomatic effect through inhibition of the vestibulo-ocular reflex (VOR). We tested this hypothesis by directly measuring the VOR after administration of three agents from different substance classes: an antihistamine, a benzodiazepine and a calcium channel antagonist. METHODS: The gain and the variability of the high velocity VOR was assessed using video head impulses (vHIT) under the following conditions: baseline, after dimenhydrinate, after diazepam and after cinnarizine. RESULTS: We found that all three

Click-evoked vestibulo-ocular reflex: stimulus-response properties in superior canal dehiscence.

BACKGROUND: An enlarged, low-threshold click-evoked vestibulo-ocular reflex (VOR) can be averaged from the vertical electro-oculogram in a superior canal dehiscence (SCD), a temporal bone defect between the superior semicircular canal and middle cranial fossa. OBJECTIVE: To determine the origin and quantitative stimulus-response properties of the click-evoked VOR. METHODS: Three-dimensional, binocular eye movements evoked by air-conducted 100-microsecond clicks (110 dB normal hearing level, 145 dB sound pressure level, 2 Hz) were measured with dual-search coils in 11 healthy subjects and 19

The cerebellar nodulus and ventral uvula control the torsional vestibulo-ocular reflex.

1. The vestibulo-ocular reflex (VOR) was investigated in rhesus monkeys before and after surgical ablation of the cerebellar nodulus and ventral uvula. The lesion resulted in an alteration of the torsional VOR: compensatory eye movements were poor in the low frequency range and the time constant was reduced to values comparable to those of primary semicircular canal afferents. In addition, animals permanently lost their ability to generate torsional optokinetic nystagmus (OKN). 2. The effects of the lesion on the torsional VOR differed from those observed in the horizontal and vertical vestibu

Visual vestibular interaction: vestibulo-ocular reflex suppression with head-fixed target fixation.

In order to maintain clear vision, the images on the retina must remain reasonably stable. Head movements are generally dealt with successfully by counterrotation of the eyes induced by the combined actions of the vestibulo-ocular reflex (VOR) and the opto-kinetic reflex. We have studied how, in humans, the VOR gain (VORG) is modulated to provide appropriate eye movements in two situations: 1. fixation of a stationary object of the visual space while the head moves. This requires a visuo-vestibulo-ocular reaction to induce eye movements opposite in direction, and equal in velocity to head move

The vestibulo-ocular reflex and its possible roles in space motion sickness.

Prolonged exposure to an inappropriate vestibulo-ocular reflex (VOR) will usually lead to motion sickness, and it has been predicted on theoretical grounds that VOR gain may be decreased in weightlessness. While experiments during parabolic flight in aircraft tend to confirm this prediction, experiments during orbital spaceflight have led to apparently contradictory results. It is suggested that VOR gain is reduced initially, but that rapid compensatory mechanisms restore it to normal within minutes of reaching weightlessness. However, even though this process may lead to the rapid return of f

Cancellation of the vestibulo-ocular reflex during smooth pursuit in patients with maculopathy.

INTRODUCTION: Macular degeneration is associated with loss of central vision, including the fovea. Central visual field loss is associated with impaired smooth pursuit eye movements, and the associated scotoma can lead to target disappearance which may or may not be transient. While smooth pursuit in and of itself does not require a foveal position signal, suppression of the vestibuloocular reflex (VOR) needed during combined eye and head pursuit relies on the fovea for appropriate calibration. Prior work has shown that target extinction during, or even at the beginning of the trial does not a

The Vestibulo-Ocular Reflex is Associated With Visuospatial Dysfunction in Patients With Parkinson's Disease.

BACKGROUND: Visuospatial impairment is one of the most frequent cognitive deficits in patients with Parkinson's disease (PD). It remains unknown whether the vestibulo-ocular reflex (VOR) function affects visuospatial perception and memory in patients with PD. OBJECTIVE: To delineate the relationship between VOR and visuospatial function in patients with PD. METHODS: We prospectively evaluated video head-impulse tests in 151 patients with PD (mean age ± $\; \pm \;$ standard deviation, 68 ± $\; \pm \;$ 9 years; 77 male). All patients conducted the Rey Complex Figure test (RCFT). RESUL

Effect of Sound Source Location and Spatial Hearing on the Vestibulo-Ocular Reflex.

BACKGROUND: There have been conflicting results on the effect of auditory stimulation on the vestibulo-ocular reflex (VOR) with some studies indicating suppression, enhancement, or no effect. No studies to date have assessed the effect of sound source location on VOR gain or the relationship between spatial hearing ability and VOR gain. PURPOSE: The purpose of the present study was to determine if VOR gain was affected by moving the location of the sound source within participants and to determine if these effects were related to spatial hearing ability. RESEARCH DESIGN: A between subjects r

Discordant vestibulo-ocular reflex function according to the frequency and mode of stimulation.

This study aimed to determine the incidence, pattern, and etiology of dissociated vestibulo-ocular reflex (VOR) function according to the stimulus frequency in dizzy patients. We retrospectively evaluated the results of bithermal caloric tests and video-head impulse tests (vHITs) in 1022 patients with dizziness or vertigo between July 2016 and April 2021. Patients were classified into concordant group (normal or abnormal results on both tests) and discordant group (dissociated results between two tests). Of 1022 patients, discordant group had 159 (16%), comprising abnormal horizontal vHITs wit

Stimulation of the nodulus and uvula discharges velocity storage in the vestibulo-ocular reflex.

The nodulus and sublobule d of the uvula of rhesus and cynomolgus monkeys were electrically stimulated with short trains of pulses to study changes in horizontal slow-phase eye velocity. Nodulus and uvula stimulation produced a rapid decline in horizontal slow phase velocity, one aspect of the spatial reorientation of the axis of eye rotation that occurs when the head is tilted with regard to gravity during per- and post-rotatory nystagmus and optokinetic after-nystagmus (OKAN). Nodulus and uvula stimulation also reproduced the reduction of the horizontal time constant of post-rotatory nystagm

Vestibulo-ocular reflexes in peripheral labyrinthine lesions: III. Bilateral dysfunction.

Measurements were made of the lesion-induced changes in vestibulo-ocular reflexes (VOR) to rotatory stimuli in a group of patients with bilateral peripheral labyrinthine lesions. All the patients had caloric responses that were below the confidence levels used for normal responses in our laboratory. Responses to rotatory stimuli were greatly decreased or absent at low frequencies of stimulation, but present to a much greater degree at the higher frequencies. Phase measurements showed an increase in relationship to the velocity of the stimulus; there was a corresponding shortening of the time c

Vestibulo-ocular reflexes in peripheral labyrinthine lesions: II. Caloric testing.

In a group of patients with unilateral labyrinthine dysfunction, the maximal velocity of the slow component response to caloric stimulation was compared with that to horizontal vestibular rotatory stimulation by a low-frequency stimulus. It was found that the larger the magnitude of the caloric response, the larger were the gain and sensitivity of the vestibulo-ocular reflex induced by physiologic stimuli. Rotary stimulation of patients with only one functioning inner ear provides information equivalent to that obtained from caloric stimulation and allows more rigorous quantification of the st

Identification of the location of vestibular lesions on the basis of vestibulo-ocular reflex measurements.

Quantitative analysis of electro-oculographic recordings of eye movement in response to precise visual and vestibular stimuli makes possible the differentiation of three categories of vestibular syndromes due to pathological changes in three different parts of the visual vestibulo-ocular reflex arc: (1) decreased vestibulo-ocular reflex gain (e.g., decrease in slow component velocity), but normal fast components and visual-vestibular interaction (labyrinth and eighth nerve); (2) normal slow component velocity but abnormal fast components to all stimuli (pontine or medullar reticular formation)

Catch-Up Saccades in Vestibulo-Ocular Reflex Deficit: Contribution of Visual Information?

OBJECTIVES: Catch-up saccades help to compensate for loss of gaze stabilization during rapid head rotation in case of vestibular deficit. While overt saccades observed after head rotation are obviously visually guided, some of these catch-up saccades occur with shorter latency while the head is still moving, anticipating the needed final eye position. These covert saccades seem to be generated based on the integration of multisensory inputs. Vision could be one of these inputs, but the known delay for triggering visually guided saccades questions this possibility. The main objective of this st

Head shaking does not alter vestibulo ocular reflex gain in vestibular migraine.

Vestibular Migraine (VM) is the most common cause of non-positional episodic vestibular symptoms. Patients with VM commonly report increased motion sensitivity, suggesting that vestibular responses to head movement may identify changes specific to VM patients. Here we explore whether the vestibulo-ocular reflex (VOR) gain alters in response to a clinical "headshake" maneuver in patients with VM. Thirty patients with VM in the inter-ictal phase, 16 patients with Benign Positional Paroxysmal Vertigo (BPPV) and 15 healthy controls were recruited. Patients responded to the question "Do you feel si
