Renal artery reconstruction has traditionally been limited to cases of documented renal artery hypertension. We report our experience with renal artery reconstruction on 32 arteries in 23 patients over a three-year period. These procedures were done for renovascular hypertension in nine patients, for renal preservation in seven patients, and concomitantly with aortic reconstruction in seven patients. Aortorenal bypass was the primary method of reconstruction, with thromboendarterectomy, primary reanastomosis, and splenorenal bypass being used less frequently. Significant improvement in hyperte

Cases with concurrent pheochromocytoma and renal artery stenosis, two "surgically curable" hypertensive disorders, are rare. The stenosis is generally secondary to direct compression or stretching of the renal artery by the tumor. Five such cases are reported. Sometimes the tumor and the renal artery stenosis occur independently giving rise to some pathogenetic queries. Diagnosis and therapy are briefly discussed.

CONTEXT: Renal artery aneurysm (RAA) is uncommon and usually asymptomatic, but complications like rupture or thromboembolism of the aneurysm can occur, with consequent renal infarction. Most of the clinical findings are found incidentally through imaging examinations, in investigating other diseases. Renal autotransplantation (RAT) is an alternative treatment for complex RAA, with satisfactory results described in the literature.
CASE REPORT: The patient was a 48-year-old man with a history of systemic arterial hypertension, thrombocytopenia and advanced hepatosplenic schistosomiasis. He comp

PURPOSE: To describe a case of improved renal function and renovascular hypertension after percutaneous treatment of a renal artery aneurysm.
CASE REPORT: A 72-year-old hypertensive woman with a hilar saccular aneurysm of the right renal artery underwent percutaneous coil embolization under local anesthesia. Four individual, soft, detachable platinum embolization coils were successfully inserted in the aneurysm with incomplete aneurysm exclusion. Nonetheless, the patient's blood pressure and renal function returned to normal. The arteriogram at 3 months demonstrated complete exclusion of the

BACKGROUND: The increasing experience with renal allotransplantation has led to continuing development in vascular surgical techniques. These improvements have enabled complex ex vivo renal artery surgery and renal autotransplantation to be performed. The aims of the present study were to describe the results achieved with renal autotransplantation and ex vivo renal artery reconstruction (RAR) at the Newcastle Transplant Unit, John Hunter Hospital, and to review the current indications for such surgery.
METHODS: A retrospective review was performed of patients who required renal autotransplan

The NIDDM patient, willingly with high blood pressure and atheroma, has frequently an abnormal renal function. Must a renal artery stenosis (RAS) be searched as a determining or favorising cause? We have searched RAS by color duplex scan, in 60 consecutive NIDDM patients with altered renal function (creatinine clearance < or = 60 mL/min). Metabolic blood pressure (ABPM), cardiovascular and renal investigations have been realised. The population was composed of 22F/38M with middle age: 70.7 +/- 6.2 yrs, diabetic duration: 11.6 +/- 8 yrs, the plasma creatinine was: 161 +/- 78 mumol/L and clearan

Atherosclerotic renal artery stenosis is the most common cause of secondary hypertension and may cause progressive renal disease and cardiac destabilization syndromes. Guideline-directed medical therapy is advised in all patients. Patients with refractory symptoms and hemodynamically significant stenoses are more likely to benefit from renal artery stent placement. Chronic mesenteric ischemia (CMI) is an infrequent and difficult to diagnose illness. Due to robust collateralization, clinical symptoms from mesenteric artery stenosis or occlusion is uncommon. Atherosclerosis is the most common et

The predominant cause of renal artery stenosis (RAS) is atherosclerosis. Clinical manifestations of atherosclerotic RAS are both direct (hypertension and kidney dysfunction) and indirect (increased cardiovascular events and mortality). However, in many cases, atherosclerotic RAS seems to be an incidental finding with no discernable effects. Antihypertensive medications such as renin-angiotensin-aldosterone system inhibitors, along with statins and aspirin, have significantly improved the medical treatment of atherosclerotic RAS. However, revascularization is still advocated in a variety of cli

Increasing utilization of diagnostic imaging studies has provided newer insight into our current knowledge of visceral artery aneurysms. Because many are "silent," an appreciation of the true incidence and natural history of these nonaortic arterial aneurysms is only recently being understood. Historical data suggest that renal artery aneurysms (RAAs) occur in approximately 0.1% of the general population. Recognized complications associated with RAAs include renovascular hypertension, renal artery thrombosis, infarction from distal embolization, arteriovenous fistula formation, and, the most d

The attitude to treatment of renal artery stenosis has recently been modified from an active to a more expectant strategy based on informations from randomised studies. The primary treatment should be antihypertensive agents. Revascularisation should be considered in patients with refractory hypertension, recurrent pulmonary oedema, bilateral renal artery stenosis or progressive azotaemia, and in patients with a narrow stenosis to a single kidney. The treatment, i.e. surgery or PTA with or without stent, should be selected on an overall view of the patients' health using a combination of clini

Renal artery stenosis is the cause of progressive ischemic nephropathy and of renovascular hypertension. Due to the invasiveness of arteriography, which is claimed to be the gold standard at the present time, several noninvasive imaging techniques are available. Colour Doppler sonography is cost-effective, but magnetic resonance angiography and computer tomography are more expensive; however, both are potential candidates for the definition of a new gold standard. Evaluation of renal vasculature by means of Doppler sonography includes intra- and extrarenal scanning as well as power Doppler and

Renovascular hypertension caused by renal artery stenosis was a coincidental discovery in 5 of the 6 patients presented. They did not have any comlaint and it was by routine bloodpressure measurement that hypertension was diagnosed. In one patient without hypertension stenosis of both renal arteries was seen on the aortography for an infrarenal aortic occlusion. Depending on the extent and localisation of the stenosis unilateral renal artery stenosis was treated by a saphenous vein graft interposition, or by an aortorenal vein patch. In a man with bilateral renal artery stenosis a venous bridg

BACKGROUND: Renal artery intramural haematoma (IMH) is a rare cause of renal artery obstruction after stenting. Diagnosis and treatment are difficult as there are only a few cases reported.
CASE SUMMARY: We present the case of sudden-onset abdominal pain and non-functional kidney 3 days after renal artery stent implantation. Subacute luminal narrowing of the renal artery was initially diagnosed using computed tomography angiography and renal artery angiography, and a final diagnosis of subacute renal artery IMH was made using intravascular ultrasound (IVUS). Subsequently, the patient was trea

PURPOSE: To investigate the effect of renal denervation (RDN) on blood pressure and renal function in refractory hypertension after renal artery recanalization and optimal medical therapy.
METHODS: Ten patients (6 women; mean age 70.0±5.1 years) with an office systolic blood pressure >160 mmHg despite taking ≥3 antihypertensive drugs and uni- or bilateral renal artery stenting were treated with RDN. Radiofrequency (RF) energy was delivered to the native segment of the artery keeping a 5-mm safe distance from the stented segments. Standardized office (OBP) and ambulatory (ABP) blood pressure m

Atherosclerotic renal artery stenosis (ARAS) is frequently associated with concomitant coronary and peripheral arterial disease with a significant impact on cardiovascular morbidity and mortality. Renal angioplasty of ARAS is more challenging because of increased incidence of technical failures, complications, and restenosis; while there is barely perceptible control of hypertension and only marginal improvement in renal function. This is because most of the patient population in recent randomized trials had unmanifested or clinically silent renovascular disease. Manifestations of RAS should b

We report a case of ruptured renal artery aneurysm into the renal pelvis. A 48-year-old woman presented with complaints of gross hematuria and right back pain. Abdominal ultrasonography, computed tomography (CT) and magnetic resonance imaging (MRI) revealed the aneurysm, which was 5 x 5 cm in diameter. Enhansed CT revealed blood flow from the renal artery aneurysm into the renal pelvis. Radical nephrectomy was performed. Rupture of renal artery aneurysm into the renal pelvis is the 3rd case in the Japanese literatures. Pathogenesis and management of this rare condition are discussed.
