Nanotechnology in reproductive medicine: from gamete engineering to precision therapeutics.

Infertility represents a significant global health burden, necessitating advanced therapeutic interventions. While Assisted Reproductive Technologies have revolutionized fertility treatment, they remain constrained by limited efficacy, off-target toxicity, and procedural complexity. Nanotechnology has emerged as a transformative frontier, offering precise targeting and controlled release capabilities to circumvent these limitations. This review systematically synthesizes nanotechnological applications in reproductive medicine, ranging from gamete quality enhancement-via antioxidant nanoparticl

Revitalizing reproductive health: innovations and future frontiers in restorative medicine.

BACKGROUND: Infertility affects around 17.5% of reproductive-aged individuals worldwide, posing significant personal and public health challenges. Although Medically Assisted Reproduction and Assisted Reproductive Technology (ART; e.g., in vitro fertilization) have advanced outcomes, many couples fail to conceive due to unaddressed pelvic, uterine, or systemic factors. OBJECTIVES: We aim to (1) define the current usage of Restorative Reproduction Medicine (RRM) in clinical practice, (2) compare RRM outcomes with conventional ART, and (3) propose an integrated model of RRM plus ART for optimal

[Optical introscopy is a new diagnostic technique in reproductive medicine].

The WHO classification's concept "infertility of unclear genesis" is due to a number of circumstances. On the one hand, this is a preponderance of the subtle forms of diseases, which are a cause of female infertility, including the subclinical forms of small pelvic inflammatory diseases (SPID). On the other hand, this is an imperfection of existing diagnostic methods. Laparoscopy considered to be the gold standard demonstrates a not very high efficiency in diagnosing SPID because of its low sensitivity. In practice, laparoscopic diagnosis of SPID is combined with ultrasound study, computed tom

Evaluating the medical literature in reproductive medicine: questions concerning prognosis.

To illustrate the recommended approach to questions concerning prognosis and the formulation of clinical recommendations, the principles of evidence-based medicine were applied to questions regarding the cardiovascular health risks associated with polycystic ovary syndrome (PCOS) and the effects of treatment on risk. A critical examination of the published literature reveals that the clinical diagnosis of PCOS does not, by itself, increase the risk of developing coronary heart disease (CHD) or adversely affect the prognosis in women with CHD. Rather, any increased risks can be attributed to ot

The effects of COVID-19 on pregnancy and implications for reproductive medicine.

COVID-19 was officially declared a pandemic in March 2020. Since then, our understanding of its effects on pregnancy have evolved rapidly. Emerging surveillance data and large cohort studies suggest that pregnancy is associated with an increased risk of intensive care unit hospitalization, invasive ventilation, and death. Pregnancies complicated by SARS-CoV-2 infection are associated with increased likelihood of cesarean delivery and preterm birth. Intrauterine transmission occurs, but seems to be rare. Critical gaps remain, and rigorous high-quality data are needed to better ascertain pregnan

The role of digital image analysis in reproductive biology and medicine.

The use of high-speed computers, direct digitization of video images, and new methods to probe soft tissues has revolutionized our view of biology. Many of these new techniques are now commonly used in several areas of medicine, but their use in reproductive biology has just begun. At present, digital image processing is limited to a few applications in reproductive medicine, including digital colposcopy and the laboratory analysis of spermatozoon motility and cellular morphology. Early implementations of digital image processing experienced problems typical of any newly introduced high techno

Medical microrobots in reproductive medicine from the bench to the clinic.

Medical microrobotics is an emerging field that aims at non-invasive diagnosis and therapy inside the human body through miniaturized sensors and actuators. Such microrobots can be tethered (e.g., smart microcatheters, microendoscopes) or untethered (e.g., cell-based drug delivery systems). Active motion and multiple functionalities, distinguishing microrobots from mere passive carriers and conventional nanomedicines, can be achieved through external control with physical fields such as magnetism or ultrasound. Here we give an overview of the key challenges in the field of assisted reproductio

Platelet-rich plasma activation: are there differential effects in reproductive medicine?

The aim of this in vitro short report was to determine whether activation of platelet-rich plasma (PRP) results in differential effects on growth factor release and cell proliferation in reproductive medicine. This study involved PRP from five donors, activated with either CaCl2 (exogenous activation) or type I collagen (mimicking endogenous activation). The release of growth factors (EGF and PDGF-AB) and the proliferative response of human ovarian fibroblasts were analysed. PRP activated with CaCl2 formed stable clots and released statistically significant higher levels of EGF (871 ± 426 pg/m

Survey of U.S. reproductive medicine clinicians' attitudes on polygenic embryo screening.

Polygenic embryo screening (PES) is used to screen embryos for their genetic likelihood of developing complex conditions and traits. We surveyed 152 U.S. reproductive endocrinology and infertility specialists (REIs) on their views of PES. While most respondents (97%) were at least slightly familiar with PES, general approval of PES was low (12%), with the majority expressing disapproval (46%) or uncertainty (42%). A majority (58%) believed risks outweigh benefits, while only 16% felt benefits outweigh risks. Most clinicians (85-77%) were very or extremely concerned about low accuracy, confusio

Influencing ovarian aging in reproductive medicine: promise, evidence, and unresolved questions.

Ovarian aging is a fundamental biological constraint on female fertility, driven by depletion of the primordial follicle pool and progressive alterations in the ovarian microenvironment. In recent years, a range of intraovarian interventions aimed at modifying ovarian aging, including platelet-rich plasma, autologous stem cell-based approaches, and mitochondrial transfer, have gained clinical and commercial attention. These strategies are supported by biologic hypotheses and early changes in surrogate markers such as antimüllerian hormone and antral follicle count, yet their clinical significa

Advanced three-dimensional culture technologies in reproductive medicine: A narrative review.

Recent advancements in 3-dimensional (3D) in vitro culture systems for follicles, oocytes, and embryos represent a paradigm shift in assisted reproductive technologies and reproductive biology. By mimicking the native physiological microenvironment more faithfully than conventional 2-dimensional systems, these innovative platforms directly address critical limitations of current in vitro embryo production protocols, including low blastocyst development rates, compromised embryo quality, and concerning epigenetic aberrations. This comprehensive review examines a spectrum of emerging 3D strategi

Advancements in three-dimensional bioprinting for reproductive medicine: a systematic review.

Reproductive failure due to age, genetics and disease necessitates innovative solutions. While reproductive tissue transplantation has advanced, ongoing research seeks superior approaches. Biomaterials, bioengineering and additive manufacturing, such as three-dimensional (3D) bioprinting, are harnessed to restore reproductive function. 3D bioprinting uses materials, cells and growth factors to mimic natural tissues, proving popular for tissue engineering, notably in complex scaffold creation with cell distribution. The versatility which is brought to reproductive medicine by 3D bioprinting all

Male contraception - Part of gender medicine and reproductive rights of men.

A pregnancy is in general the result of the sexual interaction of two individuals, with either different or same genders (male, female, non-binary etc). Preventing an unwanted pregnancy is therefore basically a "joint preventive behavior", which can be achieved by gender specific behaviors with gender specific methods in a specific sociocultural environment. This sociocultural environment (circumstances) determines the frame of gender specific behaviors (reproductive rights) and the available gender specific contraceptive methods (contraceptive technology). In the sociocultural history of cont

The medieval biological clock? Gendered reproductive aging in medieval western medicine.

This paper examines discussions of women's and men's reproductive aging in a series of western European medical texts written in the period 1100-1300. It uses the modern image of the biological clock to explore how far physicians in earlier periods understood reproductive aging to be a process of slow decline before a final age at which fertility ended (menopause for women, or a less defined 'old age' for men), and how far they viewed women's reproductive aging as different from men's. The article argues that, in contrast to modern medical and popular understandings, medieval physicians assume
