Exercise headcount: what do you do with 800 scientists who cannot return to the building they have just evacuated?

Annual fire drills serve a valuable purpose in ensuring familiarity with evacuation procedures. However, the limited duration of these exercises may be far from the reality of a real incident. In this situation, the ability to return back into a building may be restricted for hours or days. As an emergency manager, this raises a number of questions, including what to do with the evacuated colleagues, how to provide welfare support and how to identify those people who require the most immediate assistance. This paper focuses on these questions through a case study of 'Exercise Headcount'. Devel

From Combat to Search and Rescue: The Modern Air Force Pararescue Medical Experience.

INTRODUCTION: This manuscript provides a comprehensive overview of modern Pararescue (PJ) medical experiences and capabilities, focusing on the period from 2012 to 2024. Pararescues, unique within the Department of Defense, are specifically trained for personnel recovery and provide rapid medical response in adverse conditions. Their extensive training pipeline equips them to handle a wide range of medical emergencies as advanced-scope paramedics that are also equipped with a broad scope of rescue skills. MATERIALS AND METHODS: This retrospective review analyzes data from the Air Combat Comma

Shelters in Alpine Rescue: Can They Create a Comfortable Zone at the Deployment Site?

IntroductionLightweight shelters made of water- and windproof polyester are frequently used by mountain rescue, but information on physical and psychological stress during their use is scarce.MethodsA prospective observational study was conducted with 48 experienced rescuers who performed cardiopulmonary resuscitation training on a manikin in a shelter in alpine terrain. The objective parameters of air temperature, moisture, and concentrations of carbon dioxide and oxygen were measured inside the shelter. The subjective experience of the rescuers regarding stress, moisture, noise, air draft, c

The impact of various rehydration volumes for firefighters wearing protective clothing in warm environments.

This study examined different fluid replacement quantities during intermittent work while wearing firefighting protective clothing and self-contained breathing apparatus in the heat (35 degrees C, 50% relative humidity). Twelve firefighters walked at 4.5 km per h with 0% elevation on an intermittent work (50 min) and rest (30 min) schedule until they reached a rectal temperature of 39.5 degrees C during work periods and 40.0 degrees C during rest, heart rates of 95% of maximum and/or exhaustion. During the heat-stress trials subjects received one of four fluid replacement quantities, high (H),

Lung cancer incidence among world trade center rescue and recovery workers.

BACKGROUND: Many World Trade Center disaster (WTC) rescue and recovery workers (WTC RRWV) were exposed to toxic inhalable particles. The impact of WTC exposures on lung cancer risk is unclear. METHODS: Data from the WTC Health Program General Responders Cohort (WTCGRC) were linked to health information from a large New York City health system to identify incident lung cancer cases. Incidence rates for lung cancer were then calculated. As a comparison group, we created a microsimulation model that generated expected lung cancer incidence rates for a WTC- and occupationally-unexposed cohort wit

Searching for Survivors through Random Human-Body Movement Outdoors by Continuous-Wave Radar Array.

It is a major challenge to search for survivors after chemical or nuclear leakage or explosions. At present, biological radar can be used to achieve this goal by detecting the survivor's respiration signal. However, owing to the random posture of an injured person at a rescue site, the radar wave may directly irradiate the person's head or feet, in which it is difficult to detect the respiration signal. This paper describes a multichannel-based antenna array technology, which forms an omnidirectional detection system via 24-GHz Doppler biological radar, to address the random positioning relati

A unique hospital physician disaster response system for a nonemployed medical staff.

Private hospitals with nonemployed, volunteer medical staffs face a special challenge in meeting the patient-care needs posed by a mass casualty incident (MCI). Although most disaster response systems focus on emergency department and trauma management, such systems often do not provide for the need to triage existing inpatients to create room for incoming casualties, for continuity of physician care for those patients, as well as for MCI victims in case of major disaster. Such systems must also provide a mechanism for ethical and appropriate rationing of limited resources during a MCI. Commun

Are rescue workers still at risk? A meta-regression analysis of the worldwide prevalence of post-traumatic stress disorder and risk factors.

Rescue workers (policemen, firefighters, emergency medical staff, etc.) experience intense stress due to rescuing and helping victims of accidents, terrorist attacks, violent crimes, and natural disasters. Overexposure and ineffective coping with such stressful events may lead to developing Post-Traumatic Stress Disorder (PTSD). Meta-regression procedures were applied to examine moderators such as the sample sex composition, age, working experience, occupation, country, or type of PTSD evaluation. The 9.8% PTSD prevalence found here was virtually the same compared with earlier findings from 10

Cancer risk among World Trade Center rescue and recovery workers: A review.

Twenty years after the September 11th, 2001 terrorist attacks, the association between exposures present at the World Trade Center (WTC) site and the risk of several specific types of cancer has been reported among rescue and recovery workers. The authors' objective was to conduct an updated review of these data. Most studies have found elevated rates of both prostate and thyroid cancers compared with rates in the general population, and some have reported statistically significant differences for the rates of all cancers as well. Studies including a larger combined cohort of WTC-exposed rescu

"Medic 7, Rescue 1, Engine 45, respond to an accident with entrapment." Medic 7 arrives, establishes command and sizes up the scene. It's a two-car head-on collision with one unresponsive driver entrapped by a jammed door. The driver of the other vehicle is walking around and appears uninjured. There's heavy damage to both vehicles, but most of the vehicle doors open and their side-curtain and front-end air bags have deployed. Both patients are triaged. The unresponsive driver is triaged "red" and the walking driver is triaged "green." A second ambulance is requested for the "green" patient an

Use of respiratory protection among responders at the World Trade Center site--New York City, September 2001.

The terrorist attacks on the World Trade Center (WTC) on September 11,2001, created an occupational health and safety challenge for New York City (NYC) firefighters and rescue workers responding to the disaster. Immediate respiratory hazards included explosions, fire, falling debris, and dust clouds containing particulate matter comprised of pulverized building materials. Ongoing risks included lingering particulate matter in the air and intermittent combustion products from initial and persistent fires beneath the rubble pile. Because the nature and extent of exposures in disaster situations

Point-of-care ultrasound (POCUS) use in mountain emergency medicine: results from an international online survey.

BACKGROUND: The extent to which point-of-care ultrasound (POCUS) is currently being used in international mountain emergency medicine has not yet been investigated. The aim of this study was to determine the use of POCUS in mountain rescue organizations which are members of the International Commission for Alpine Rescue (ICAR). METHODS: We asked 92 ICAR member organizations to complete an online survey to gather information about staffing, ultrasound equipment, indications, perceived utility, and aspects of education and clinical supervision. RESULTS: Our survey had a response rate of 42% (n =

Urban search and rescue (USAR) dogs are valuable members of their teams and play key roles in performing successful missions. A pair of dogs can do the work of dozens of people, the dogs are able to quickly sniff around collapsed structures and zip through constricted hallways with far greater accuracy than their plodding human counterparts. While in contaminated areas, their human counterparts are afforded the benefit of personal protective equipment (PPE) to keep exposures to chemical, biological and radiological substances to a minimum; USAR dogs, on the other hand, are not.
