Conflicts of interest in clinical practice: lessons learned from cardiovascular medicine.

Cardiovascular diseases represent a major burden worldwide, and clinical trials are critical to define treatment improvements. Since various conflicts of interest (COIs) may influence trials at multiple levels, cardiovascular research represents a paradigmatic example to analyze their effects and manage them effectively to re-establish the centrality of evidence-based medicine.Despite the manifest role of industry, COIs may differently affect both sponsored and non-sponsored studies in many ways. COIs influence may start from the research question, data collection and adjudication, up to resul

Allocation of National Institutes of Health Funding by Disease Category in 2008 and 2019.

IMPORTANCE: Prior research suggests an association between burden of disease and National Institutes of Health (NIH) funding. The allocation of NIH funding should reflect, to some extent, the health needs of the population, along with other factors. OBJECTIVE: To examine the factors associated with NIH funding in 2019 for 46 diseases. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used disability-adjusted life-years to measure the 2008 and 2019 US burden of disease and compared them with NIH categoric funding for 46 diseases. EXPOSURES: Disability-adjusted life-years to measure the 20

Alzheimer's disease drug development: translational neuroscience strategies.

Alzheimer's disease (AD) is an urgent public health challenge that is rapidly approaching epidemic proportions. New therapies that defer or prevent the onset, delay the decline, or improve the symptoms are urgently needed. All phase 3 drug development programs for disease-modifying agents have failed thus far. New approaches to drug development are needed. Translational neuroscience focuses on the linkages between basic neuroscience and the development of new diagnostic and therapeutic products that will improve the lives of patients or prevent the occurrence of brain disorders. Industry Sponsorship and Research Outcomes in A Sample of Landmark Ophthalmology Trials.

PURPOSE: To evaluate ophthalmology trials, with versus without industry sponsorship, on the reporting of outcomes, conclusions, and risk of bias, to evaluate whether they favour the investigational product. DESIGN: Retrospective cohort study. METHODS: We identified a sample of landmark ophthalmology trials (with any publication year) defined as those indexed by 1 of several publicly available sources, including EyeWiki, 1 textbook, the National Institutes of Health, and ophthalmologist-led online sources. A sponsor-favoured intervention was defined as: a) the active treatment in placebo-cont

A review of UK publicly funded non-inferiority trials: is the design more inferior than it should be?

BACKGROUND: The number of non-inferiority (NI) trials, those aiming to show a new treatment is no worse than a comparator, is increasing. However, their added complexity over superiority trials can create confusion. Most guidance and reviews to date have an industry focus with research suggesting these trials may differ from publicly funded NI trials. The aim of this work is to review the design and reporting characteristics of UK publicly funded NI trials. This assessment will show how well recommendations from industry are translating to publicly funded trials. METHODS: The International St

Gambling Research and Industry Funding.

This paper discusses the relationship between investigative credibility and the sources of funding associated with gambling research. Some researchers argue against accepting funding from gambling industry sources; similarly, they decline to participate in activities directly or indirectly sponsored by gambling industry sources. In contrast, these anti-industry investigators evidence less resistance toward accepting funds from sources other than industry, for example, governments, because they believe that they have greater independence, reliability, and validity, and less undue influence and/

National Cancer Institute-Supported Obesity and Cancer Research, 2015-2022.

IMPORTANCE: The National Cancer Institute's (NCI) interest in obesity and cancer reflects associations of obesity with cancer incidence, as well as potential impacts of obesity on cancer treatment, survivorship, recurrence, and survival. Given the growing prevalence of obesity and its implications for cancer burden, understanding the historical scope and direction of research in this area informs future needs. OBJECTIVE: To describe trends in obesity and cancer research supported by the NCI over an 8-year period preceding the widespread uptake of glucagon-like peptide-1 (GLP-1) receptor agoni

Does Current United Kingdom Childhood Cancer Research Address the James Lind Alliance Children's Cancer Priority Setting Partnership Priorities? A Grant Mapping Study.

INTRODUCTION: The James Lind Alliance Children's Cancer Priority Setting Partnership identified 23 key research topics, as defined by children, survivors, families, carers, and professionals. Funders have expressed commitment to ensuring all priorities are addressed. The aim of our current project was to map UK-funded childhood cancer research against the 23 priorities to highlight gaps and inform future funding, policy and research directions. METHODS: A retrospective grant mapping study. Potential UK funders of childhood cancer research were contacted for details of projects funded January

Public and philanthropic research funding, publications, and research networks for cancer in the Commonwealth and globally between 2016 and 2023: a comparative analysis.

This Review presents a comprehensive analysis of the amounts and distribution of public and philanthropic global cancer research funding between 2016 and 2023, including patterns of international collaboration and downstream research output, with an emphasis on the Commonwealth. We show that annual investment decreased globally each year, apart from a rise in 2021. Network analysis revealed that grant and publication collaborations between the Commonwealth, the USA, and the EU are facilitated by linkages through a core group of Commonwealth countries, including the UK, Australia, and Canada. Funding and collaboration inequalities in infectious disease research - why does it matter?

To face the current and emerging challenges due to infectious diseases, global research communities must comprehend the challenges faced by scientists in low-income and lower-middle-income countries. Here we address pertinent issues regarding funding and collaboration inequalities that could potentially cripple economic and healthcare systems, escalating to bigger global threats.

Comparing research investment to United Kingdom institutions and published outputs for tuberculosis, HIV and malaria: a systematic analysis across 1997-2013.

BACKGROUND: The "Unfinished Agenda" of infectious diseases is of great importance to policymakers and research funding agencies that require ongoing research evidence on their effective management. Journal publications help effectively share and disseminate research results to inform policy and practice. We assess research investments to United Kingdom institutions in HIV, tuberculosis and malaria, and analyse these by numbers of publications and citations and by disease and type of science. METHODS: Information on infection-related research investments awarded to United Kingdom institutions

Ebola research funding: a systematic analysis, 1997-2015.

BACKGROUND: The latest outbreak of Ebola in West Africa overwhelmed the affected countries, with the impact on health extending far beyond Ebola-related deaths that have exceeded 11 000. The need to promptly mobilise resources to control emerging infections is widely recognized. Yet, data on research funding for emerging infections remains inadequately documented. METHODS: We defined research investment as all funding flows for Ebola and/or Marburg virus from 1997 to April 2015 whose primary purpose was to advance knowledge and new technologies to prevent or cure disease. We sourced data dire

Funding community collaboration to develop effective therapies for neurofibromatosis type 1 tumors.

The time from identifying a drug target to a new drug approval is often measured in decades and can take even longer for therapies to treat rare diseases. In fact, 95% of rare diseases do not have a specific therapy approved at all. Coordinated efforts to augment the drug development pipeline along with long-term and comprehensive support that enable scientific breakthroughs for rare diseases are possible, but it requires integration across multiple stakeholders. This article analyzes the coordinated funding efforts of four federal and philanthropic organizations to advance drug development fo

The lifecycle of a grant: Award acceptance to dissemination.

Securing grant funding is an important achievement for nurse practitioners and academic faculty, yet the postaward phase introduces complex administrative, regulatory, and scholarly responsibilities essential to project success. This article is the second in a two-part series examining the lifecycle of a grant. Building on part 1, which focused on preaward planning and proposal development, this manuscript addresses postaward activities from award acceptance through dissemination. Drawing on the authors' shared experience as principal investigators and project directors, the article provides p
