Effect of ipratropium bromide on respiratory mechanics in infants with acute bronchiolitis.

The effect of nebulized ipratropium bromide in 14 infants (mean age: 20 weeks, range: 4-41) with acute respiratory syncytial virus bronchiolitis was examined. A modified rapid chest compression technique was used to obtain partial expiratory flow-volume curves and maximum flow at functional residual capacity. Passive respiratory mechanics were assessed by brief occlusion at end inspiration. Thoracic gas volume was measured in a body plethysmograph. No significant difference was found in forced and passive respiratory mechanics pre- and post-ipratropium bromide. No subgroups could be identified

Characterisation of respiratory mechanics in adults with CF using dynamic chest radiography.

BACKGROUND: As the lifespan of people with CF (pwCF) improves, sensitive markers of lung health are needed. Dynamic chest radiography (DCR) is a low ionising radiation dose cineradiographic imaging system that provides real-time information on moving thoracic structures. We examined whether DCR provides quantitative motion analysis that correlates with pulmonary function, anthropometric and clinical variables in pwCF, to explore DCR as a marker of lung health. METHODS: In this prospective, observational, single-centre, non-controlled study, participantsunderwent DCR and spirometry. We measure

Drug class effects on respiratory mechanics in animal models: access and applications.

Assessment of respiratory mechanics extends from basic research and animal modeling to clinical applications in humans. However, to employ the applications in human models, it is desirable and sometimes mandatory to study non-human animals first. To acquire further precise and controlled signals and parameters, the animals studied must be further distant from their spontaneous ventilation. The majority of respiratory mechanics studies use positive pressure ventilation to model the respiratory system. In this scenario, a few drug categories become relevant: anesthetics, muscle blockers, broncho

Changes in respiratory mechanics at birth in preterm infants: A pilot study.

OBJECTIVE: We aimed to measure lung mechanics at birth by the forced oscillation technique (FOT) for assessment of the initial degree of lung aeration and the short-term aeration changes after applying different respiratory support strategies. METHODS: Eighteen preterm infants (gestational age = 29-36 week) were randomized to receive either continuous positive airway pressure (CPAP) at 5 cmH2 O only or combined with a sustained inflation (SI; 15 seconds at 25 cmH2 O after 5 seconds of CPAP) at birth. We assessed the respiratory system reactance at 5 Hz (X5; increases with lung volume recruitm

Fluid mechanics based classification of the respiratory efficiency of several nasal cavities.

The flow in the human nasal cavity is of great importance to understand rhinologic pathologies like impaired respiration or heating capabilities, a diminished sense of taste and smell, and the presence of dry mucous membranes. To numerically analyze this flow problem a highly efficient and scalable Thermal Lattice-BGK (TLBGK) solver is used, which is very well suited for flows in intricate geometries. The generation of the computational mesh is completely automatic and highly parallelized such that it can be executed efficiently on High Performance Computers (HPCs). An evaluation of the functi

Evaluation of respiratory system mechanics in mice using the forced oscillation technique.

The forced oscillation technique (FOT) is a powerful, integrative and translational tool permitting the experimental assessment of lung function in mice in a comprehensive, detailed, precise and reproducible manner. It provides measurements of respiratory system mechanics through the analysis of pressure and volume signals acquired in reaction to predefined, small amplitude, oscillatory airflow waveforms, which are typically applied at the subject's airway opening. The present protocol details the steps required to adequately execute forced oscillation measurements in mice using a computer-con

Effect of obesity on respiratory mechanics during rest and exercise in COPD.

The presence of obesity in COPD appears not to be a disadvantage with respect to dyspnea and weight-supported cycle exercise performance. We hypothesized that one explanation for this might be that the volume-reducing effects of obesity convey mechanical and respiratory muscle function advantages. Twelve obese chronic obstructive pulmonary disease (COPD) (OB) [forced expiratory volume in 1 s (FEV(1)) = 60%predicted; body mass index (BMI) = 32 ± 1 kg/m(2); mean ± SD] and 12 age-matched, normal-weight COPD (NW) (FEV(1) = 59%predicted; BMI = 23 ± 2 kg/m(2)) subjects were compared at rest and duri

Flutter valve improves respiratory mechanics and sputum production in patients with bronchiectasis.

BACKGROUND AND PURPOSE: Although the application of airway clearance techniques is considered an important component in the treatment of several obstructive pulmonary diseases, there is no scientific evidence supporting the use of Flutter Valve™ in the management of patients with bronchiectasis. Moreover, the consequences of respiratory physiotherapy techniques on respiratory mechanics have not been fully studied. Therefore, we investigated the acute, short-term effects of Flutter Valve™ on respiratory mechanics and sputum production in bronchiectatic patients. METHODS: EIGHT patients were ev

Effects of acute blood volume expansion on respiratory mechanics in the rat.

BACKGROUNDS: The effects of acute blood volume expansion (BVE) on the respiratory mechanics of normal animals have been not extensively studied. The subject is of both theoretical and practical interest since BVE is a frequent medical intervention, and the associated increase in cardiac output may occur in different physiopathological situations. OBJECTIVES: To describe the changes in the parameters of respiratory mechanics occurring as an effect of acute BVE and the related increase in cardiac output. METHODS: We applied the end-inflation occlusion method in normal, positive pressure-ventil

Respiratory mechanics and morphometric changes during anesthesia with ketamine in normal rats.

Ketamine is believed to reduce airway and pulmonary tissue resistance. The aim of the present study was to determine the effects of ketamine on the resistive, elastic and viscoelastic/inhomogeneous mechanical properties of the respiratory system, lungs and chest wall, and to relate the mechanical data to findings from histological lung analysis in normal animals. Fifteen adult male Wistar rats were assigned randomly to two groups: control (N = 7) and ketamine (N = 8). All animals were sedated (diazepam, 5 mg, ip) and anesthetized with pentobarbital sodium (20 mg/kg, ip) or ketamine (30 mg/kg,

Effect of supine posture on respiratory mechanics in chronic left ventricular failure.

The mechanisms of orthopnea and the role of changes in respiratory mechanics in left ventricular failure (LVF) are poorly understood. We have measured total respiratory airflow resistance (Rrs) using forced oscillation in the sitting and supine positions in 10 patients with chronic LVF (NYHA II-III) shortly after recovery from acute LVF and in 10 matched control subjects (CON). Seated, the patients with LVF had small lung volumes but no evidence of airway obstruction (mean FEV(1)/FVC, 81%). Mean Rrs at 6 Hz was only slightly higher in LVF (3.4 cm H(2)O. L(-1). s) than in CON (2.6 cm H(2)O. L(-

Respiratory drive and pulmonary mechanics during haemodialysis with ultrafiltration in ventilated patients.

The improvements of respiratory drive and pulmonary mechanics which follow haemodialysis with ultrafiltration in mechanically ventilated renal failure patients seem predictable but have not been studied before. In this study, 14 renal failure patients with stable haemodynamics mechanically ventilated with pressure support ventilation (PSV) were enrolled. Respiratory drive (represented as P0.1), pulmonary mechanics, breathing pattern, arterial blood gas and haemodynamics were measured according to the time schedule: pre-dialysis (Time 0), and at 60, 120, 180, 240 minutes thereafter. Respiratory mechanics and control of the ventilatory cycle in the garter snake.

We measured respiratory airflow and intrapulmonary pressure in spontaneously breathing garter snakes (Thamnophis sirtalis) before and after bilateral vagotomy. Each breathing cycle consisted of expiration, inspiration, and a breath-holding period with the glottis closed, as previously reported. Respiratory system compliance was very high compared with that of mammals (0.042 ml/cm H2O per g), but was considerably reduced when the animal was coiled. Resistance to airflow averaged 6.35 cm H2O per ml/sec. Lung volume during the breath-hold was usually above the relaxed volume of the system. Analys

Anaphylaxis in the monkey: respiratory mechanics, acid-base status and blood gases.

Aggregate anaphylaxis was induced by intravenous injection of the specific antigen in eight ovalbumin-sensitized monkeys. Changes in respiratory mechanics, acid-base status and blood gases were studied during the following half hour. Within 1 minute after challenge, a short period of respiratory depression, probably reflex-mediated, was observed. This was followed by hyperventilation, and arterial PCO2 decreased. There was a rapid increase in pulmonary resistance (Rpulm) and a concomitant decrease in pulmonary dynamic compliance (Cdyn), suggesting constriction of smooth muscles in the lung. Rp

Intra-breath measures of respiratory mechanics in healthy African infants detect risk of respiratory illness in early life.

Lower respiratory tract illness (LRTI) is a leading cause of mortality and morbidity in children. Sensitive and noninvasive infant lung function techniques are needed to measure risk for and impact of LRTI on lung health. The objective of this study was to investigate whether lung function derived from the intra-breath forced oscillation technique (FOT) was able to identify healthy infants at risk of LRTI in the first year of life.Lung function was measured with the novel intra-breath FOT, in 6-week-old infants in a South African birth cohort (Drakenstein Child Health Study). LRTI during the f

High Mechanical Power and Driving Pressures are Associated With Postoperative Respiratory Failure Independent From Patients' Respiratory System Mechanics.

OBJECTIVES: High mechanical power and driving pressure (ΔP) have been associated with postoperative respiratory failure (PRF) and may be important parameters guiding mechanical ventilation. However, it remains unclear whether high mechanical power and ΔP merely reflect patients with poor respiratory system mechanics at risk of PRF. We investigated the effect of mechanical power and ΔP on PRF in cohorts after exact matching by patients' baseline respiratory system compliance. DESIGN: Hospital registry study. SETTING: Academic hospital in New England. PATIENTS: Adult patients undergoing gener

AI-Driven Lumped-Element Modeling of Human Respiratory System for Studying Voice Mechanics.

A predictive physics-based model of human respiratory, phonatory, and articulatory subsystems is developed to simulate voice production. Representing lungs, compressible airways, and vocal folds as spring-damper-mass controlled piston-cylinder systems, our mathematical model robustly captures the intricate dynamics of airways during phonation. The nonlinear viscoelastic properties of lung tissues and compressible airways were investigated, yielding a responsive and expressive baseline respiratory model with the capability to further extend into a patient-specific model for both respiration and

A novel method to quantify breathing effort from respiratory mechanics and esophageal pressure.

Breathing effort is important to quantify to understand mechanisms underlying central and obstructive sleep apnea, respiratory-related arousals, and the timing and effectiveness of invasive or noninvasive mechanically assisted ventilation. Current quantitative methods to evaluate breathing effort rely on inspiratory esophageal or epiglottic pressure swings or changes in diaphragm electromyographic (EMG) activity, where units are problematic to interpret and compare between individuals and to measured ventilation. This paper derives a novel method to quantify breathing effort in units directly

Changes in respiratory mechanics in response to crystalloid infusions in extremely premature infants.

Extremely premature infants are at a higher risk of developing respiratory distress syndrome and circulatory impairments in the first few weeks of life. Administration of normal saline boluses to manage hypotension is a common practice in preterm infants. As a crystalloid, a substantial proportion might leak into the interstitium; most consequently the lungs in the preterm cohorts, putatively affecting ventilation. We downloaded and analyzed ventilator mechanics data in infants managed by conventional mechanical ventilation and administered normal saline bolus for clinical reasons. Data were d
