BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) is increasingly used in adults with refractory cardiac arrest. However, post-resuscitation care in this population remains heterogeneous, evidence-based recommendations are lacking, and most evidence is derived from conventional cardiopulmonary resuscitation. We conducted a scoping review to describe the existing evidence on post-resuscitation care after ECPR.
METHODS: We systematically searched PubMed and Embase from inception to May 19, 2025, for studies reporting on post-resuscitation care in adults treated with ECPR following

The goal of resuscitation pharmacology is to restart the heart as quickly as possible while preserving vital organ function during chest compression. Unfortunately, the application of advanced life support to pediatric cardiac arrest patients is often unsuccessful. The goal of this paper is to review the scientific rationale and educational considerations used to derive the guidelines for medication use in the pediatric patient during CPR. The first step in drug delivery during CPR is to achieve vascular access. The endotracheal route and intraosseous route may be used, although the former is

BACKGROUND: The management of cardiac arrest patients receiving cardiopulmonary resuscitation (CPR) is an essential aspect of emergency medicine (EM) training. At our institution, we have a 1-month Resuscitation Rotation designed to augment resident training in managing critical patients. The objective of this study is to compare 30-day mortality between cardiac arrest patients with resuscitation resident (RR) involvement versus patients without. Our secondary outcome is to determine if RR involvement altered rates of initiating targeted temperature management (TTM).
METHODS: This study was c

ADULTS: Administer chest compressions (minimum 100/min, minimum 5 cm depth) at a ratio of 30:2 with ventilation (tidal volume 500-600 ml, inspiration time 1 s, F(I)O₂ if possible 1.0). Avoid any interruptions in chest compressions. After every single defibrillation attempt (initially biphasic 120-200 J, monophasic 360 J, subsequently with the respective highest energy), chest compressions are initiated again immediately for 2 min independent of the ECG rhythm. Tracheal intubation is the optimal method for securing the airway during resuscitation but should be performed only by experienced airw

There are no national or international guidelines for the resuscitation of conjoined twins. We have described how the U.S. Neonatal Resuscitation Program algorithm can be modified for delivery room resuscitation of omphaloischiopagus conjoined twins. In planning for the delivery and resuscitation of these patients, we considered the challenges of providing cardiopulmonary support to preterm conjoined twins in face-to-face orientation and with shared circulation via a fused liver and single umbilical cord. We also demonstrate how in situ simulation can be used to prepare a large, multidisciplin

Traumatic injuries represent a major health problem all over the world. In recent years we have witnessed profound changes in the paradigm of severe trauma patient resuscitation, new concepts regarding acute coagulopathy in trauma have been proposed, and there has been an expansion of specific commercial products related to hemostasis, among other aspects. New strategies in severe trauma management include the early identification of those injuries that are life threatening and require surgical hemostasis, tolerance of moderate hypotension, rational intravascular volume replacement, prevention

BACKGROUND: The Neonatal Resuscitation Program (NRP) is an effective tool in decreasing mortality and morbidity due to birth asphyxia. The aim of the study was to assess the skill and knowledge level of pediatric residents in a teaching hospital and the effects of NRP training.
METHODS: Subjects consisted of pediatric residents of Dr Behcet Uz Hospital, Izmir, Turkey. They were assessed on practice exam scenarios and NRP provider course flow charts. Teams with two members were formed randomly. Each resident was evaluated on a 100 point scale covering all resuscitation steps and interventions.

An early prediction score (EPS) is constructed as the sum of five events: the type of cardiac arrest is ventricular fibrillation; the type of respiratory arrest is gasping; pupil reaction is unequal, slow or normal, but present; swallowing activity is present and the cardiac arrest has been witnessed. Presence of any of these events contributes one point to the score, while absence contributes nothing to it. EPS during resuscitation results in a comparable amount of information, whether used to predict success, alive and conscious 14 days post-CPR or no-success. EPS early (10 min) after initia

Thrombolytic therapy has long been considered a relative contraindication during cardiopulmonary resuscitation (CPR), mainly due to concerns about fatal bleeding complications. However, in patients with acute myocardial infarction (AMI), untreated coronary thrombosis remains a major cause of unsuccessful resuscitation. We report the case of a 66-year-old man with a history of hypertension and cerebral infarction who presented with chest pain, subsequently developed cardiac arrest, and was successfully resuscitated after concurrent intravenous thrombolysis during CPR. The patient collapsed afte

Pediatric cardiac arrest is a relatively rare but devastating presentation in infants and children. In contrast to adult patients, in whom a primary cardiac dysrhythmia is the most likely cause of cardiac arrest, pediatric patients experience cardiovascular collapse most frequently after an initial respiratory arrest. Aggressive treatment in the precardiac arrest state should be initiated to prevent deterioration and should focus on support of oxygenation, ventilation, and hemodynamics, regardless of the presumed cause. Unfortunately, outcomes for pediatric cardiac arrest, whether in hospital

PURPOSE OF REVIEW: Intravenous and enteral fluid resuscitation are frequently used therapies in the management of pediatric patients in emergency departments and critical care settings. Any state of intravascular fluid deficit, ranging from mild dehydration due to gastroenteritis to fulminant septic shock, requires careful assessment and early restoration of hemodynamic stability. Rapid fluid resuscitation has gained increased recognition since the most recent pediatric shock management guidelines. We sought to review the evidence for rapid fluid resuscitation and to outline its clinical indic

Intrauterine resuscitative measures are commonly initiated during labor when the fetal heart rate (FHR) pattern is indeterminate or abnormal. The most effective use of these measures is directed at the presumed underlying cause. However, some FHR patterns are nonspecific, while others are such that intrauterine resuscitation will not remedy the situation. The goals of intrauterine resuscitation during labor are, at its best, to reverse any hypoxia that might lead to further deterioration, and at the very least to avoid prolonged periods of indeterminate or abnormal FHR patterns, which may caus

PURPOSE OF REVIEW: To summarize recent advances in pediatric cardiopulmonary arrest prevention, resuscitation and postresuscitation management.
RECENT FINDINGS: Pediatric cardiac arrest has traditionally been considered a futile medical condition with dismal outcomes. Data in the 21st century indicate that more than 25% of children treated for in-hospital cardiac arrests survive to hospital discharge and more than 10% of children older than 1 year treated for out-of-hospital cardiac arrests survive to hospital discharge. These data establish that children are more likely to survive to hospita

PURPOSE OF REVIEW: To summarize current opinion and advances in pediatric cardiopulmonary resuscitation, including etiology, pathophysiology, rationale for interventions, and postresuscitation management.
RECENT FINDINGS: Cardiac arrest and ventricular fibrillation in children are not as uncommon as previously reported. Out-of-hospital cardiac arrests occur in 8-20 children/100,000/year, and in-hospital arrests occur in 2-6% admitted to a pediatric intensive care unit. Most pediatric arrests are precipitated by asphyxia or circulatory shock, but approximately 10% are precipitated by ventricul

Ten percent of all newborns require resuscitation at birth. The Neonatal Resuscitation Program establishes the authoritative technique of newborn resuscitation. Errors continue to occur that are related to the use of unskilled resuscitators; intubation; inadequate suctioning of meconium; and the postresuscitation problems of hypoglycemia, hypocarbia, and hypotension. Specific recommendations are offered to avoid these pitfalls of neonatal resuscitation.

Timing of diagnosis and intervention, together with understanding of physiology and knowledge of anatomy, are essential for success in the practice of surgery in general, and emergency surgery in particular. Either excess of treatment, for example fluid overload in tamponated haemorrhage such as a ruptured retroperitoneal Abdominal Aortic Aneurysm, or defect of treatment such as delayed arrest of life threatening bleeding from a transected Superficial Femoral Artery above the knee, can both lead to disastrous outcome or death. Proximal control of damaged arteries is an obligatory step before r

Pediatricians and other practitioners who attend deliveries maintain resuscitation skills throughout their careers. NRP certification and continuing education programs (PCEP) have done much to standardize the care a newborn receives in the first minutes after birth. Neonates have benefited tremendously from strong national programs (NRP and PCEP) with great local execution of training modules.
