A Handheld Ultrasound Device Can Predict Constipation with Rectal Fecal Retention in a Palliative Care Setting.

Although handheld ultrasound devices (HUDs) are commonplace, their ability to diagnose fecal retention (FR) remains unclear. This prospective observational study examined HUDs' usefulness in diagnosing FR in patients with constipation in a palliative care setting. Between 10 December 2020 and 30 June 2022, we compared rectal ultrasonographic findings obtained using HUDs with clinical manifestations in 64 males and 70 females (48%, 52%, 68 ± 11 years old) with constipation who had undergone computed tomography (CT). FR was diagnosed using a HUD and compared with CT and digital rectal examinatio

Integrating Buprenorphine Treatment for Opioid Use with Primary Care is Associated with Greater Retention in Treatment.

BACKGROUND: Medication-assisted treatment (MAT) for opioid use disorder (MAT) is encouraged for primary care use, but evidence for effectiveness when fully integrated with primary care versus provided separately is limited. METHOD: We conducted a retrospective cohort study of 257 MAT patients in our family medicine clinic to assess retention in treatment for patients receiving MAT integrated with primary care compared with our MAT patients who received their primary care elsewhere. RESULTS: At 1 year after enrollment, 60% of patients receiving MAT integrated with primary care remained in trea

Buprenorphine treatment retention and comorbidities among patients with opioid use disorder in a primary care setting.

BACKGROUND AND OBJECTIVES: More information is needed about comorbidities among patients receiving buprenorphine maintenance treatment and their relationship with retention. METHODS: Retrospective electronic health record data over a 5-year period from primary care patients receiving buprenorphine for the treatment of opioid use disorder were examined (N = 899). The present analysis determined the prevalence of comorbidities and examined associations with treatment retention as defined by cumulative duration of buprenorphine prescription. RESULTS: Tobacco use and comorbidities including hypert

Understanding HIV care delays in the US South and the role of the social-level in HIV care engagement/retention: a qualitative study.

INTRODUCTION: In a significant geographical shift in the distribution of HIV infection, the US South--comprising 17 states--now has the greatest number of adults and adolescents with HIV (PLHIV) in the nation. More than 60% of PLHIV are not in HIV care in Alabama and Mississippi, contrasted with a national figure of 25%. Poorer HIV outcomes raise concerns about HIV-related inequities for southern PLHIV, which warrant further study. This qualitative study sought to understand experiences of low-income PLHIV on the AIDS Drug Assistance Program in engagement and retention in continuous HIV care i

Longitudinal Patterns and Predictors of Racial Disparities of HIV Retention in Care: A Statewide Cohort Analysis.

Racial disparities have historically existed regarding HIV care outcomes, including retention in care (RIC). However, data regarding the longitudinal patterns and predictors of such disparities among different racial groups and various geolocations are limited. This study aims to explore the contribution of contextual features (e.g., socioeconomic and structural environmental factors) to the changes in RIC outcome disparity at the county level. All adult people with HIV who received a HIV diagnosis in South Carolina (SC) between 2013 and 2020 were included in this study, and their de-identifie

Correlates of retention in HIV care after release from jail: results from a multi-site study.

Retention in care is key to effective HIV treatment, but half of PLWHA in the US are continuously engaged in care. Incarcerated individuals are an especially challenging population to retain, and empiric data specific to jail detainees is lacking. We prospectively evaluated correlates of retention in care for 867 HIV-infected jail detainees enrolled in a 10-site demonstration project. Sustained retention in care was defined as having a clinic visit during each quarter in the 6 month post-release period. The following were independently associated with retention: being male (AOR = 2.10, p ≤ 0.0

Contextual Drivers of HIV Vulnerability and Retention in Care in the Central African Republic: An Ecological Analysis.

HIV transmission is shaped by specific contextual factors that are best understood through ecological and geospatial analysis. This study aimed to investigate three key drivers of HIV infection and vulnerability in the Central African Republic (CAR), a country affected by a long-standing socio-political crisis. To do so, we used sentinel surveillance data from antenatal HIV screening between 2019 and 2021 to estimate the spatial distribution of HIV and conducted an ecological analysis to examine its association with (1) forced population displacement and the presence of internally displaced pe

HIV Care Retention and Cost Analysis in Multi-Month ART Dispensing: A Randomized Controlled Trial in China.

INTRODUCTION: Multi-month dispensing (MMD), as a differentiated service delivery model, can reduce the frequency of clinic visits, waiting time and travel costs for clinically stable people living with HIV. This study aimed to evaluate the impact of 6-month MMD of antiretroviral therapy (ART) on retention and conduct a cost analysis in China. METHODS: We conducted a randomized, non-blind, non-inferiority study from December 2022 to March 2023 at The First Hospital of China Medical University. Eligible participants were randomly assigned to a 3-month dispensing group (n = 789) or a 6-month dis

Retention in HIV care and associated factors among youths aged 15-24 years in rural southwestern Uganda.

BACKGROUND: Retention in HIV care contributes to antiretroviral therapy adherence, which is a key factor for improved treatment outcomes and prevention of drug resistance. However, HIV treatment among the youths is characterized by loss to follow up, poor adherence to ART, risk of treatment failure and high mortality rates compared to young children and adults. There is limited information about factors associated with retention of youths in HIV care in rural settings in Uganda. We aimed to determine retention in HIV care and associated factors among youths aged 15-24 years in rural southweste

Effect of Baseline Symptom Manifestations on Retention in Care and Treatment among HIV-Infected Patients in Nigeria.

BACKGROUND: Symptom management is an important component of HIV care. But symptom patterns and how they affect engagement with HIV care and treatment services have not been adequately explored in the era of increased HIV treatment scale-up. We investigated the relationship between symptom patterns among people living with HIV (PLHIV) and 12 months retention in care, within the context of other clinical and demographic characteristics. METHODS: Retrospective cohort analysis of 5114 PLHIV receiving care within a large HIV treatment program in Nigeria. We assessed the prevalence and burden of ba

Barriers to maternal retention in HIV care in Ghana: key differences during pregnancy and the postpartum period.

BACKGROUND: Maternal retention in HIV care is lower for women in the postpartum period than during pregnancy, but the reasons are poorly understood. We examined key differences in barriers to retention in HIV care during and after pregnancy. METHODS: We conducted semi-structured, in-depth interviews with 30 postpartum women living with HIV. Participants were recruited from two tertiary facilities implementing Option B+ for prevention of mother-to-child HIV transmission in Accra, Ghana. We collected data from mothers who had disengaged from HIV care and those who were still engaged in care. Th

The CARES Program: Improving Viral Suppression and Retention in Care Through a Comprehensive Team-Based Approach to Care for People with HIV and Complex Psychosocial Needs.

Retention in HIV care and viral suppression rates remain suboptimal, especially among people with HIV (PWH) facing complex barriers to care such as mental health conditions, substance use disorders, and housing insecurity. The Center for Adherence, Retention, and Engagement Support (CARES) program utilizes an interdisciplinary team that delivers integrated services in a drop-in setting to provide individualized care to PWH with complex psychosocial needs. We describe the CARES program and evaluate its effectiveness in retaining patients in care to achieve virological suppression. We characteri

Comprehensive patient navigation is associated with increased active treatment retention in lung cancer care in rural Appalachia.

BACKGROUND: Lung cancer mortality is disproportionately concentrated in rural Appalachia. Patient navigation may improve access, but evidence supporting comprehensive lung cancer navigation across all diagnostic pathways remains limited. We evaluated an all-pathway navigation program at a community cancer center serving a high-burden, predominantly publicly insured Appalachian population. METHODS: We conducted a retrospective cohort study of 470 patients diagnosed with lung cancer at a single Commission on Cancer (CoC)-accredited community center between 2018 and 2022. Cost-Effectiveness of Interventions to Improve Retention in Care for Chronic Hepatitis B in the United States.

Monitoring and retention in care remain suboptimal for patients diagnosed with chronic hepatitis B (CHB) infection with only a minority receiving consistent follow-up. This study evaluates the cost-effectiveness of interventions aimed at improving long-term monitoring and retention in care among diagnosed CHB. A Markov model simulated outcomes for a cohort of 100,000 adults, comparing current practice to three interventions: EHR-based provider reminders, patient navigators and a combined strategy. Costs and quality-adjusted life years (QALYs) were evaluated from a healthcare perspective. All i

Evaluation of the impact of PEPFAR transition on retention in care in South Africa's Western Cape Province.

BACKGROUND: Research on the impact of the US President's Emergency Plan for AIDS Relief (PEPFAR) transition in South Africa (SA) in 2012 found varying results in retention in care (RIC) of people living with HIV (PLWH). OBJECTIVES: To investigate the factors that impacted RIC during the PEPFAR transition in Western Cape Province, SA, in 2012. METHODS: We used aggregate data from 61 facilities supported by four non-governmental organisations from 2007 to 2015. The main outcome was RIC 12 months after antiretroviral therapy initiation for two periods - during PEPFAR and post PEPFAR. We used ad

Reproductive health syndemics impact retention in care among women living with HIV in Rio de Janeiro, Brazil.

Syndemic psychosocial and reproductive factors affecting women's retention in HIV care remain understudied. We analyzed correlates of non-retention in a cohort of women with HIV in Brazil from 2000‒2015. Participants self-reported exposure to physical/sexual violence, illicit drug use, adolescent pregnancy, or induced abortion. Lifetime history of these psychosocial stressors were used to create a syndemic score based on the presence or absence of these conditions. All dichotomous variables were summed (range 0 to 4), with greater scores indicating more syndemic factors experienced. Logistic r

Retention in care and viral suppression in the PMTCT continuum at a large referral facility in western Kenya.

Medical records of pregnant and postpartum women living with HIV and their infants attending a large referral facility in Kenya from 2015 to 2019 were analyzed to identify characteristics associated with retention in care and viral suppression. Women were stratified based on the timing of HIV care enrollment: known HIV-positive (KHP; enrolled pre-pregnancy) and newly HIV-positive (NHP; enrolled during pregnancy). Associations with retention at 18 months postpartum and viral suppression (< 1000 copies/mL) were determined. Among 856 women (20% NHP), retention was 83% for KHPs and 53% for NHPs. V
