The retinal pigment epithelium (RPE) is a highly specialised pigmented monolayer sandwiched between the choroid and the photoreceptors in the retina. Key functions of the RPE include transport of nutrients to the neural retina, removal of waste products and water from the retina to the blood, recycling of retinal chromophores, absorption of scattered light and phagocytosis of the tips of the photoreceptor outer segments. We present a case of a 57-year-old woman who reported bilateral visual impairment since 2 weeks. She had a medical history of congenital, cyanotic heart failure. Retinal pigment epithelial tears are most commonly found in the context of aged related macular degeneration with associated choroidal neovascular membrane or retinal pigment epithelial atrophy. Combined hamartomas of the retinal pigment epithelium (RPE) and retina are rare, benign lesions usually occurring in individuals with no underlying systemic abnormalities. As it is known that chlorpromazine (CPZ) can bind to melanins as well as cause ocular phototoxicity, we investigated the cytotoxic effects of UV-visible irradiation of melanotic and amelanotic retinal pigment epithelial (RPE) cells in the presence of CPZ. Twenty-one patients under age 55 had idiopathic serous detachments of the retinal pigment epithelium (rpe) one-fifth disc diameter or larger in size. We studied tapeto-retinal degeneration in the offspring of Royal College of Surgeons (RCS) and Wistar albino rats. Normal ERG values vary according to skin pigmentation: thresholds and implicit times are different. We believe the best experimental material is to be found in the affected pigmented offspring. Evolution of the degeneration over more than 9 months has been observed before extinction and the expressivity of the disease varies from one individual to another. This animal model closely resembles the ERG findings observed clinically in certain pigmentary degenerations of the retina.

Six patients had hemorrhagic detachment of the peripheral retinal pigment epithelium. Abnormal neovascular nets in older individuals, under the retinal pigment epithelium and originating from the choriocapillaris, may well be the cause of such hemorrhagic detachments. This lesion must be differentiated from choroidal tumors, choroidal detachment, and uveal effusion.

Melanin pigment has a significant role in ocular pharmacokinetics, because many drugs bind at high extent to melanin in the retinal pigment epithelial cells.
