Retinoblastoma which developed in microphthalmia.

We report a rare case of retinoblastoma which developed in unilateral microphthalmia. A 4-year-old girl presented with a large, intraocular mass in microphthalmia without a family history of retinoblastoma. The eyeball was enucleated due to rapid growth of the intraocular tumor. Microscopically it proved to be a retinoblastoma with some Flexner-Wintersteiner rosettes. This unusual case shows that retinoblastoma should not be excluded just because the eye is small, particularly if the family history is not available or is incomplete.

[Spontaneous growth inhibition in retinoblastoma].

Report on 8 cases with spontaneous arrest of retinoblastoma. The incidence was 3.2% which is approximately 1000 times more than in other malignant tumors. Multifocal lesions suggesting that this form of "regression" may not be a regression at all. Spontaneous arrest of retinoblastoma is obviously a lesion of low malignant potential. The presence of a spontaneous arrest of retinoblastoma necessitates genetic counselling and examinations of the eyes in the offspring. Synonymous names are: 1. Spontane Rückbildung eines Retinoblastoms (spontaneous regression of retinoblastoma), 2. Retinoma, 3. Ret

Successful therapy for trilateral retinoblastoma.

Trilateral retinoblastoma, the intracranial malignancy associated with bilateral retinoblastoma, is an uncommon and clinically aggressive malignancy with a uniformly fatal outcome. Three children with newly diagnosed trilateral retinoblastoma were treated with systemic (cyclophosphamide and vincristine) and intrathecal (methotrexate, hydrocortisone, and cytarabine) chemotherapy, as well as craniospinal irradiation (one patient) in addition to therapy of the eye lesions. All three patients have had partial or complete response of the pineal tumors to chemotherapy, with no active disease eight o

Effectiveness of genetic counselling. Retinoblastoma.

The authors studied 32 families with 36 RB patients. Among them 24 were sporadic cases; four had recurrence of RB; two had RB plus bone tumour in relatives and two were RB propositi. Two evaluations with an interval from four to 13 years were made. In the first the recurrence risk was informed as high or low. The patients' decisions were evaluated at the second consultation. The authors considered that counselling was negative in more than 50% of cases. For normal parents, prognosis of RB was severe, whereas for RB patients it was mild.

Second primary tumour following retinoblastoma.

Reported is a child who at the age of 18 months had presented with a left retinoblastoma. The lesion had involved the optic nerve (Stage IV), and was successfully treated by enucleation and radiotherapy. The radiation field did not include the mandible or the submandibular triangle. He was followed up until he left the area aged 11 years. He represented aged 12 years with a progressive swelling of the left submandibular gland. Histology of the surgically excised submandibular gland revealed a mucoepidermoid carcinoma of the submandibular gland. This case highlights the need for continual surve

Seven cases of trilateral retinoblastoma.

Of 245 consecutive children with retinoblastoma referred to the Oncology Service at the Wills Eye Hospital between January 1974 and August 1988 and followed up for three months to 15 years, seven (3%) developed midline intracranial malignancies consistent with the diagnosis of trilateral retinoblastoma. Six of the seven had bilateral retinoblastoma, and four had a family history of retinoblastoma. The time of diagnosis of retinoblastoma varied between five months after the intracranial tumor was diagnosed and four years before the intracranial tumor was diagnosed. The midline intracranial mali

[Chromosome disorders in retinoblastoma cells].

The karyotype of 7 unilateral retinoblastomas was studied. Loss or a gain of chromosomes and structural chromosomal changes were observed in all tumors. Modal chromosome number was pseudodiploid in four and near-diploid in three tumors. There were no sex chromosomes (X or Y) in 3 tumors. Four tumors were characterized by the presence of an identical marker chromosome--i(6p). Two other markers--lp+ and 17q+ (probably, not quite identical)--were present in 4 and 3 tumors, respectively. The cells of two retinoblastomas contained both i (6p) and 17q+ markers.

Nonocular cancer in retinoblastoma survivors.

From a review of the records of 2,302 patients with retinoblastoma collated from CPMC and AFIP, it was found that retinoblastoma patients who survived their original eye cancer have a high incidence of second nonocular malignancies. The second neoplasms occur almost exclusively (97.5%) in patients who have had bilateral retinoblastoma, although retinoblastoma is much more commonly unilateral. The second neoplasms have appeared between 1 and 42 years after the successful treatment of retinoblastoma and have been fatal in approximately 85% of cases. Seventy-one percent of patients develop tumors

Retinoblastoma presenting as congenital glaucoma.

An infant girl was referred to our paediatric ophthalmology service as a case of congenital glaucoma not responding to topical medication, presenting with asymmetrical globe enlargement, epiphora, corneal oedema and right eye elevated intraocular pressure of 29 mm Hg. Examination under anaesthesia of the right eye revealed iris neovascularisation, corneal oedema and a retrolental vascularised dome-shaped mass. Examination of the left eye was normal. B-scan ultrasonography demonstrated a bi-lobulated retinal lesion with calcifications extending into the anterior hyaloid to the lens. Since the f

Cavitary retinoblastoma with MYCN amplification.

MYCN amplification within the tumor has emerged as an important factor in the oncogenesis of a subset of retinoblastomas, most of which are caused by pathogenic variants in the RB1 gene. We present the case of a 3-month-old girl with unilateral cavitary retinoblastoma that initially responded well to intra-arterial chemotherapy but later recurred. After enucleation, the tumor was found to exhibit MYCN amplification, but there were no RB1 variants. Retinoblastoma tumors with MYCN amplification are known to carry a poor prognosis and often require enucleation. On the contrary, cavitary retinobla

Retinoblastoma in Dandy-Walker Syndrome.

Dandy-Walker syndrome (DWS), also referred to as Dandy-Walker malformation, is a rare congenital developmental anomaly characterized by enlargement of the posterior fossa, dilatation of the fourth ventricle, and cerebellar hypoplasia with upward rotation. Retinoblastoma is the most common primary intraocular malignancy in children and typically presents in the pediatric age group, with leukocoria and strabismus being common early signs. Although DWS and retinoblastoma are individually rare, their simultaneous occurrence is exceptionally uncommon. The etiologies of both disorders have been link

Multidisciplinary Approach for Retinoblastoma Management.

We read with keen interest the article by Shah et al "Pathologic Risk Factor in Retinoblastoma: An Institutional Experience Based on Analysis of Enucleated Eyes"(Shah A et al, 2012). They have highlighted high-risk histopathological features in Retinoblastoma with retrolaminar optic nerve invasion being the most common and also showed statistically significant correlation of high risk histological features with tumor size and AJCC stage of tumor. Their aim to assess the frequency and spectrum of high risk histopathological features in enucleated specimens of retinoblastoma was successful which

Retinoblastoma presenting with orbital cellulitis.

PURPOSE: To study the effectiveness of pre-enucleation steroids in reducing inflammation in patients with retinoblastoma presenting as orbital cellulitis. METHODS: Medical records of consecutive retinoblastoma patients presenting at a single tertiary eye care center during a period of 3 years were retrospectively reviewed. For those who presented with orbital cellulitis, clinical, radiological, and histopathological variables were assessed. The effect of pre-enucleation steroids was noted in this group of patients. RESULTS: Of 260 retinoblastoma cases reviewed, 14 had retinoblastoma-associat

Diffuse anterior retinoblastoma: A review.

Diffuse anterior retinoblastoma is a rare variant of diffuse infiltrating retinoblastoma which occurs in up to 1-2% of cases of retinoblastoma. In diffuse anterior retinoblastoma there is a small focus of tumor in the peripheral retina from which free tumor cells enter the aqueous humor and implant on the ciliary body, iris, lens and trabecular meshwork. Patients most commonly present with pseudouveitis, pseudohypopyon and increased intraocular pressure. The differential diagnosis is broad and all of the reported cases relied upon aspirates from the aqueous humor in order to make the diagnosis

Vitreoretinal complications of retinoblastoma treatment.

PURPOSE: To describe complications of the retina and vitreous occurring in children undergoing treatment for retinoblastoma and their clinical management. METHODS: Retrospective analysis of Ret-Cam images of 206 patients with retinoblastoma treated at one center between 1996 and 2003. Images were studied for vitreoretinal features other than tumor persistence or recurrence. Specifically, complications such as vitreous, retinal, or choroidal hemorrhage, retinal detachment, retinal fold, vascular obstruction, and preretinal or subretinal proliferation were sought. RESULTS: Vitreoretinal compli

Visual fields in retinoblastoma survivors.

OBJECTIVE: To describe the visual field defects in retinoblastoma survivors and relate those defects to characteristics such as tumor size, tumor location, and treatment modality. METHODS: Thirty-one patients treated for retinoblastoma were included in this study. Humphrey visual fields were determined in 33 eyes. RESULTS: Twenty-seven patients (29 eyes, 68 tumors) had sufficient diagnosis and treatment data available for further analysis. Twenty-six of the 27 patients had both absolute and relative visual field defects. Four types of visual field defects were observed and correlated with lo

Retinoblastoma incidence and sunlight exposure.

To evaluate positive findings from an earlier report, we studied the relation between retinoblastoma incidence and ultraviolet (UV-B) radiation levels in the Surveillance, Epidemiology, and End Results (SEER) programme areas of the USA using weighted regression, as well as in international data after adjusting for race, economic development, and climate. The association was not statistically significant within the USA (P > 0.20). At an international level, the relation was significant overall and after adjusting for economic development, but it was not significant after adjusting for race and

Constitutional deletions predisposing to retinoblastoma.

Patients with the heritable form of retinoblastoma carry a constitutional mutation in the retinoblastoma locus in heterozygous form. The majority of such cases are the result of new mutations, which may be inherited by their offspring. We have identified such constitutional mutations within the retinoblastoma locus in 3 out of 66 investigated unrelated gene carriers, using Southern blot analysis and Rb-gene cDNA-probes. The identified mutations were found to be located in different regions of the gene. These analyses may be used to identify or exclude close relatives at risk for the disease. I
