Background: Retrobulbar block is a popular regional anesthetic technique in modern eye surgery due to its excellent anesthetic properties and the provision of globe akinesia. Severe complications including inadvertent subarachnoidal injection, expulsive retrobulbar hemorrhage, and intoxication with local anesthetic, are very rare. However, most reports date back several decades, mechanisms of action are not fully understood, and in recent years the procedure has changed towards facilitating the retrobulbar injection by a brief analgosedation. We therefore describe a case with inadvertent brain

BACKGROUND: The ophthalmic manifestation of neurosarcoidosis is varied. The complication of optic neuropathy and central retinal vein occlusion (CRVO) is rare in sarcoidosis.
CASE REPORT: The patient was a 55-year-old female with systemic sarcoidosis suffering from visual loss as hand motion in her left eye. A fundus examination showed severe optic disc head edema and hyperemia, and a central retinal vein occlusion phenotype including engorgement of all branches of the central retinal vein, dot, and flame-shaped hemorrhages. Brain magnetic resonance imaging (MRI) revealed irregular hypertroph

Traumatic asphyxia is a rare condition that occurs after compressive thoracoabdominal trauma, which is characterized by subconjunctival hemorrhage, cervicofacial cyanosis, edema and petechiae. Serious life-threatening thoracic and abdominal injuries may coexist. After conservatory treatment in most cases complete recovery is achieved, but in isolated cases permanent neurological lesions may occur. We present the case of the 39-year-old male patient who suffered a compressive thoracoabdominal trauma. The physical examination showed the characteristic "ecchymotic mask". After surgical treatment

BACKGROUND: Retrobulbar filler injection has recently been considered an ideal method for orbital volume enhancement due to its nontoxic, easily reversible, and noninvasive characteristics. This study determined the arterial distribution in the orbit with the aim of defining a safety zone for retrobulbar filler injections used to enhance the orbital volume.
METHODS: Twenty-seven orbits of 24 formalin-embalmed cadavers were dissected. The orbital arteries were identified after removal of the eyeball, extraocular muscles, and connective tissues. The course of each orbital artery was then record

BACKGROUND This report describes a case in which a 2-stage approach and minimally invasive vitrectomy successfully treated a giant retrobulbar hematoma caused by an ocular perforation injury. CASE REPORT A 22-year-old man sustained an ocular perforation injury from an iron wire. Emergency debridement and suturing of an anterior scleral laceration were performed. B-scan ultrasonography revealed a giant retrobulbar hematoma and posterior scleral laceration after the initial suturing procedure. Ten days later, minimally invasive vitrectomy was performed. During this procedure, intraocular hemorrh

Background. Spontaneous orbital haemorrhage is a very rare condition and vision-threatening event. It may occur due to trauma, orbital surgery/injections, orbital vascular anomalies, and a variety of systemic predisposing factors. Signs of retrobulbar hemorrhage include proptosis, ophthalmoplegia, increased intraocular pressure, loss of pupillary reflexes, and optic disc or retinal pallor. Both Computed Tomography scan and Magnetic Resonance Imaging may be performed in the diagnosis. Case Report. A 31-year-old woman was referred to our hospital with a complaint of headache and blurred vision f

PURPOSE: A 17-year-old male presented with persistent visual loss and extensive orbital wall fractures following severe head and orbital trauma sustained in a motor vehicle accident.
OBSERVATIONS: He was found to have a full-thickness peripapillary retinochoroidoscleral rupture associated with a retrobulbar cyst, possibly related to post-traumatic communication through the retinochoroidoscleral defect. Management consisted of barrier laser photocoagulation, lateral orbitotomy with cyst drainage, and primary scleral repair. Postoperatively, the patient developed a submacular hemorrhage, which

PURPOSE: To evaluate the efficacy of retro-peribulbar and subconjunctival anesthesia associated with intravenous sedation in patients undergoing evisceration and orbital implant placement.
METHODS: The charts of 217 patients who underwent evisceration with trans-scleral implant placement were reviewed. Midazolam and fentanyl were used for intravenous sedation. For local anesthesia, a combination of lidocaine with epinephrine and bupivacaine was injected into the retrobulbar, upper peribulbar, and subconjunctival areas. The intraoperative pain and need for supplemental anesthetic injection wer

BACKGROUND: Ophthalmic procedures are increasingly being performed under regional anesthesia techniques such as peribulbar and incisionless sub-Tenon's blocks. The aim is to compare peribulbar block with incisionless sub-Tenon's block in terms of perioperative complications in patients who underwent cataract and vitreoretinal surgeries.
METHODS: The patients who underwent cataract or vitroretinal surgery under peribulbar block or incisionless sub-Tenon's block were included in the study. Two groups were compared each other in terms of anesthesia-related complications.
RESULTS: A total of 125

CASE PRESENTATION: We describe a 62-year-old man with disseminated melanoma (initially staged pT2aN0M0, AJCC stage IB, 8th edition AJCC) who developed multiple brain metastases with hemorrhagic transformation, pulmonary embolism, retrobulbar progression with eyeball herniation, and deep vein thrombosis (DVT). After cerebral edema improved with corticosteroid tapering and the patient became asymptomatic, he started combination immunotherapy with ipilimumab (3 mg/kg q3w × 4) and nivolumab (1 mg/kg q3w × 4). Despite initial benefit, progression led to radiotherapy for retrobulbar lesion and subse

A 6 1/2 years old boy got completely blind within 3 days. Ophthalmoscopically regular findings suggested a retrobulbar opticus infiltration in a pediatric-diagnosed acute myeloic leukemia FAB-M2. A quick diagnosis and prompt cytostatic therapy combined with irradiation of the retrobulbar space opened the way for complete remission with bilateral optic atrophy. The ophthalmoscopic examination of 12 other leukemic children showed once more the involvement of the central nervous system with bilateral papilloedema, 2 times retinal hemorrhages and one vitreous bleeding; 8 children had no ocular sym

OBJECTIVE: To describe the clinical findings and management of a case of retrobulbar hematoma formation secondary to performance of a maxillary nerve block in a dog.
CASE SUMMARY: An 11-year-old dog was presented for a routine dental procedure including dental extraction to be performed under general anesthesia. After premedication with intramuscular methadone, anesthesia was induced with intravenous alfaxalone until depth of anesthesia was sufficient to allow orotracheal intubation. Anesthesia was maintained with isoflurane delivered in 100% oxygen. A bilateral maxillary nerve block was perf

OBJECTIVES: Retrobulbar hemorrhage is a rare condition often necessitating immediate lateral canthotomy for preservation of vision. It is performed infrequently in emergency departments (EDs); therefore, a laboratory-based curriculum using a swine model was developed to teach emergency medicine (EM) residents and pediatric emergency medicine (PEM) fellows the proper technique of lateral canthotomy and to provide them with hands-on training.
METHODS: Anesthetized adult swine are used due to similarity with human anatomy and availability from other concurrent procedure laboratories. Fifteen to
