Critical bandwidth in patients with acoustic neuroma.

Eleven patients with verified acoustic neuroma had critical band estimation performed by the method of loudness summation using noise bands centered around 1 kHz. The normal loudness difference between broad band noise and narrow band noise was reduced at all levels except the highest. Judged as single individuals, 9 of the 11 patients had a normal critical band. The pooled data indicated a normal critical band, both in patients with hearing loss less than 50 dB HL and in patients with hearing loss greater than or equal to 50 dB HL. The results are similar to those obtained in patients with Mé

Hearing loss in Pompe disease revisited: results from a study of 24 children.

Little information is available regarding the auditory function in Pompe patients. Hearing loss has been reported in classic infantile patients, but it is still unknown whether central nervous system involvement interferes with auditory function and whether enzyme replacement therapy can improve hearing. Auditory function has not been studied in children with milder forms of the disease. We analyzed repetitive auditory brainstem response measurements and pure tone audiometry in 24 children with Pompe disease. Only 1 of 13 patients with milder phenotypes showed recurrent conductive hearing loss

Auditory evoked potentials in children and adolescents with multiple sclerosis and neuromyelitis optica spectrum disorders.

BACKGROUND: In children, an acute demyelinating disease may evolve as a multiphasic disease with multiple relapses, such as multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD). The Brainstem Auditory Evoked Potentials (BAEP) and Long-Latency Auditory Evoked Potentials (LLAEP) contribute to the identification of either retrocochlear changes or other central auditory nervous system (CANS) changes. OBJECTIVES: To characterize BAEP and LLAEP in children and adolescents with MS and NMOSD and verify the diagnostic values of these potentials in each of the demyelinating diseas

Treatment of vertigo.

Vertigo is the illusion of motion, usually rotational motion. As patients age, vertigo becomes an increasingly common presenting complaint. The most common causes of this condition are benign paroxysmal positional vertigo, acute vestibular neuronitis or labyrinthitis, Ménière's disease, migraine, and anxiety disorders. Less common causes include vertebrobasilar ischemia and retrocochlear tumors. The distinction between peripheral and central vertigo usually can be made clinically and guides management decisions. Most patients with vertigo do not require extensive diagnostic testing and can be

Clinical study of chronic arachnoiditis in the posterior fossa.

A series of 12 cases of chronic arachnoiditis in the posterior fossa is reported. The diagnosis in six patients was established at suboccipital craniectomy whch brought about marked improvement in symptoms. The other six were diagnosed as having arachnoiditis in the posterior fossa from the facts that improvement in clinical symptoms was achieved by lumbar air insufflation, which has often been said to be effective in arachnoiditis, and no abnormal findings were revealed by pneumoencephalo-roulette tomography. By investigating the clinical features of these cases, we came to the following conc

Audiological findings of prolonged Menière's disease.

As Ménière's disease progresses, the fluctuations becomes less marked and the hearing level gradually declines. In the more established and advanced stages of the disease, a permanent and progressive hearing loss develops. It has been reported, however, that hearing acuity in some patients with Ménière's disease significantly improved after having had the disorder for many years. Such observation suggests that the inner ear pathology in late stages of Ménière's disease may not be necessarily irreversible. The purpose of this investigation was to assess whether or not various audiological evalu

Intensity of gadolinium enhancement on MRI is useful in differentiation of intracochlear inflammation from tumor.

OBJECTIVE: Magnetic resonance imaging is the gold standard in retrocochlear imaging; however, its role in management of inner ear diseases remains poorly defined. In this study, we investigate the utility of MRI in defining the nature of intracochlear pathology. STUDY DESIGN: A retrospective review of 11 patients with inner ear abnormalities on MRI between 2010 and 2013. Patients' diagnosis included cholesteatoma, labyrinthitis, meningitis, and intralabyrinthine schwannoma. SETTING: Tertiary care hospital. Neurologic features of chronic minamata disease (organic mercury poisoning) and incidence of complications with aging.

To elucidate the neurologic features of chronic Minamata disease, and the incidence of complications with aging, we studied 80 patients with documented Minamata disease (organic mercury poisoning) from 1986 to 1994 (mean age: 63 years). Of the cardinal neurologic findings, sensory impairment was seen with highest frequency in 98.8% of patients limited to the extremities in 86.3%. Impairment of lower extremity coordination was observed in 60%, constriction of the visual field in 51.9%, and retrocochlear hearing loss in 41%. To assess age-related complications, patients were separated into three

Cochlear Implant Outcomes in Patients with Neurosarcoidosis.

INTRODUCTION: Audiovestibular symptoms are rare in sarcoidosis, but they may also be the first manifestation of the disease. Sudden or progressive bilateral hearing loss is usually associated with vestibular impairment. The mechanism of hearing loss remains unclear, but clinical presentation and magnetic resonance imaging suggest a retrocochlear site for the lesion in most patients. Several cases of hearing recovery after corticosteroid treatment have been reported. In patients with severe or profound hearing loss, the benefit of cochlear implantation is challenging to predict in the case of a

Tinnitus suppression by electrical promontory stimulation in sensorineural deaf patients.

In 62 patients, whose hearing acuity was normal or profoundly impaired, tinnitus suppression was attempted through electrical promontory stimulation (EPS) with a Promontory Stimulator (Cochlear Co.). The causes of tinnitus were idiopathic sudden deafness, ototoxicity, noise induced hearing loss, labyrinthitis, Meniere's disease, acoustic neuroma and unknown origin. Reduction of tinnitus was achieved in 46 of 68 ears (67.6%). Of these, EPS was most effective in cases of noise induced hearing loss (100.0%; 3/3), followed by those of idiopathic sudden deafness (87.5%; 14/16), Meniere's disease (8

Auditory brainstem response: An overview of neurophysiological implications and clinical applications -A Narrative Review.

Evoked Potentials are electrical potentials that occur in a group of neurons in response to stimulation of a sensory organ which can be recorded by surface electrodes. Testing evoked potentials is useful in assessing the integrity of neuronal pathways both at sensory and motor levels of neural control. Early auditory evoked potentials include cochlear and brainstem auditory-evoked potentials, popularly known as electrocochleogram, and auditory brainstem response. Evoked potential audiometry is a neurophysiogical test to assess auditory pathway function in response to auditory stimuli. Auditory

Auditory and Olfactory Deficits in Essential Tremor - Review of the Current Evidence.

BACKGROUND: Essential tremor (ET) is the most common adult movement disorder, characterized by several motor and increasingly well recognized non-motor symptoms. Sensory deficits, such as hearing impairment and olfactory dysfunction, are amongst them. This review analyzes the available evidence of these sensory deficits and their possible mechanistic basis in patients with ET. METHOD: A PubMed literature search on the topic was performed in the May 2019 database. RESULTS: Nineteen articles on hearing impairment and olfactory dysfunction in ET patients were identified. The prevalence of heari

Steroid responsive fluctuating sensorineural hearing loss due to juvenile pilocytic astrocytoma involving the cerebellopontine angle.

Tumors of the cerebellopontine angle (CPA) are common and represent up to 10% of all intracranial tumors. Rarely, intrinsic brainstem tumors can involve the CPA and present with auditory symptoms typical of CPA tumors such as hearing loss, vertigo and tinnitus. We report on a rare case of an intrinsic brainstem neoplasm presenting with steroid responsive fluctuating sensorineural hearing loss in a child. The patient initially presented with an acute worsening of an unilateral sensorineural hearing loss, without additional symptoms, that responded to oral steroids. Otoacoustic emission testing

Topodiagnostic assessment of occupational noise-induced hearing loss using distortion-product otoacoustic emissions compared to the short increment sensitivity index test.

Because the efforts needed to produce an "automatic audiogram" by means of distortion-product otoacoustic emissions (DPOAE) are still experimental, their capacity to produce objective topodiagnostic information must still be defined. In the present study results of the short increment sensitivity index testing (SISI) test in 97 patients with noise-induced hearing loss (NIHL) were compared with those of a DP-gram. Pure tone, speech and impedance audiometry, SISI tests and DPOAE measurements were performed on all patients. SISI was measured in both ears of 93 patients and unilaterally in 4 patie

Sensitivity of auditory brainstem response in acoustic neuroma screening.

Auditory brainstem response (ABR) is the reference screening technique for acoustic neuromas, but because of a few false negatives and the increasing performance of magnetic resonance imaging (MRI), its role as the standard method has been questioned. We assessed sensitivity of screening tests in 89 patients with surgically proven acoustic neuromas. Sensitivity of ABR was 92%; 94% for extracanalicular neuromas and 77% for intracanalicular neuromas. For stapedius reflex (SR), sensitivity was 84% and for caloric vestibular response (CVR) 86%. The combined sensitivity of ABR + SR was 97% and of A

The stapedius reflex tests in retrocochlear hearing disorders.

The commonly used parameters of the stapedius reflex responses in the diagnosis of sensorineural hearing disorders are the reflex threshold level and the persistence of the reflex response on prolonged stimulation, i.e., the reflex decay test. With the object of establishing the sensitivity of these reflex parameters to identify tumours affecting the eighth nerve in the early stages, a series of 97 such cases were examined. The mean hearing loss of the affected ears was 32 dB; 28 of the cases still had normal thresholds of hearing; 95 of the ears demonstrated elevated reflex thresholds and/or

Reflex "delay" in sensorineural hearing-loss.

Delay of the acoustic reflex is concerning the latency and the rise-time of the response. The magnitude of these parameters is correlated with the stimulus characteristics but it is also modified by the instrumentation. Normative data obtained with commercial instruments demonstrate statistically meaningful differences of latency depending on age groups, ipsi-or contralateral recording, monophasic or biphasic deflection, which is also a normal response in about 12% of ears. The value are comprised within 150 ms for ipsilateral recording and 200 ms for the contralateral recording. The interaura

[Trimetazidine versus betahistine in vestibular vertigo. A double blind study].

The efficacy of trimetazidine (60 mg/day) in vertigo was compared with that of betahistine (24 mg/day) in a three-month double-blind study. Included in the study were only patients with peripheral vertigo associated or not with tinnitus or hearing loss, and excluded were those presenting with symptoms related to retrocochlear or central disease. Out of the 40 patients enrolled, 20 suffered from Meniere's disease; 4 patients either dropped out of the study or were non-compliant to therapy and could not be taken into account in the final analysis, which bore on 36 patients (18 treated by trimeta
