Evaluating the causal effects of life-course adiposity on jaw anomalies.

BACKGROUND: Observational studies indicate that obesity correlates with jaw development and remodeling; however, causality remains unclear. This study aimed to examine the potential causal relationship between life-course adiposity and jaw anomalies. METHODS: Utilizing summary statistics from genome-wide association studies predominantly of European ancestry, we conducted univariable and multivariable Mendelian randomization (MR) to estimate overall and independent effects of six obesity traits (birth weight, childhood body size, childhood body mass index [BMI], adult BMI, adult body fat perc

Combined Fronto-orbital Contouring and Orthognathic Surgery in Craniometaphyseal Dysplasia.

Craniometaphyseal dysplasia is a rare congenital sclerosing skeletal dysplasia that presents with facial dimorphism and is clinically described by prominent supraorbital bridges, severe retrognathia, and respiratory problems. Fronto-orbital cranioplasty is necessary to achieve satisfactory aesthetic outcomes. The supraorbital arches were exposed to the nasal bone through coronal access, with the help of a saw and a wear drill, and osteotomy of the frontal and orbital regions with hyperostosis was performed. Our case report describes a rare and difficult to diagnose case with a few publications

Bilateral temporomandibular joint reconstructions with stock alloplastic prostheses on a distracted mandible - A case report.

Background: The management of patients with a triad of temporomandibular joint (TMJ) ankylosis, retrognathia, and obstructive sleep apnea (OSA) has long been a matter of debate. Even though consensus favors distraction osteogenesis (DO) in the 1st phase of treatment to relieve the OSA, various treatment approaches for TMJ reconstruction after ankylosis release have been put forth. Most of the reconstructive modality has been focused on autogenous materials or customized TMJ prostheses after mandibular distraction.Case Presentation: The authors present a case of a patient with the triad of TMJ

How does changing the vector of transport disc distraction affect the outcomes of surgery in patients of temporomandibular joint ankylosis with obstructive sleep apnea?

PURPOSE: Temporomandibular joint ankylosis (TMJA) in children is associated with retrognathia, reduction in pharyngeal airway volume (PAV), and obstructive sleep apnea (OSA). Distraction-osteogenesis (DO) is the proven method in the management of OSA. There is paucity in literature about the effect of distraction vector on PAV. It can be expected that an oblique vector would improve PAV and relieve OSA. Thus, the study was designed to explore the feasibility, advantages, and disadvantages of this modified technique for managing TMJA and OSA simultaneously. MATERIALS AND METHOD: The investigat

Recurrent agnathia-otocephaly caused by DNA replication slippage in PRRX1.

Agnathia-otocephaly is a rare craniofacial malformation complex that is caused by de novo heterozygous and biallelic mutations in PRRX1 in two unrelated babies, respectively. We studied the PRRX1 gene in a non-consanguineous Indonesian female infant who was diagnosed prenatally with severe retrognathia (bilateral Pruzansky type III). Her older affected brother died shortly after birth and had agnathia-otocephaly. A c.266_269dupAAAA frameshift mutation in the poly A tract in PRRX1 was identified in the proband while her father only had an inframe duplication (c.267_269dupAAA) of the adenosine t

Anterior maxillary distraction using a tooth-borne device for hypoplastic cleft maxillas-a pilot study.

PURPOSE: The management of maxillary retrusion in a patient with cleft lip and palate is performed using Le Fort I advancement or distraction osteogenesis using an external or an internal device. Distraction mostly involves movement of the entire maxilla at the Le Fort I level and is characterized by a higher relapse rate and a hampering of speech. Hence, distraction of the anterior maxilla was performed using a tooth-borne palatal distractor similar to the one proposed by Gunaseelan et al (J Oral Maxillofac Surg 65:1044, 2007). However, the technique was modified by placing the distractor pre

Pierre Robin sequence in a patient with ectrodactyly-ectodermal dysplasia-clefting syndrome: a case report and review of the literature.

Ectrodactyly-ectodermal dysplasia-clefting (EEC) syndrome is a rare condition first identified in 1970. The majority of cases are reported to be autosomal dominant with variable expression and penetrance. The most common clinical feature is ectodermal dysplasia. Ectrodactyly, cleft lip with or with out cleft palate and urogenital anomalies are also main clinical features of EEC syndrome. Pierre Robin (PR) sequence has the clinical findings of micro-retrognathia, retroposed tongue (glossoptosis), cleft of the secondary palate, and upper airway obstruction. Etiologically PR sequence is heterogen

Functional therapy and adenotonsillectomy clinical trial for class II malocclusion (FACT-II): protocol for a randomised controlled trial.

INTRODUCTION: Class II malocclusion with mandibular retrognathia is a common complication of paediatric obstructive sleep apnoea (OSA), often accompanied by transverse maxillary deficiency. In early orthodontic treatment, a twin block (TB) is a regular functional appliance for correcting this malocclusion. For paediatric OSA, the most common risk factor is adenotonsillar hypertrophy (AHT). Untreated AHT may lead to the persistence and worsening of obstructive sleep-disordered breathing traits, including habitual mouth breathing. Additionally, the clockwise mandibular rotation associated with A

Surgical correction of neonatal obstructive sleep apnea due to a temporomandibular joint ankylosis.

UNLABELLED: In growing children, temporomandibular joint (TMJ) ankylosis and septic arthritis are uncommon. Retrognathia and micrognathia affect airway patency and can cause obstructive sleep apnea (OSA). No unified diagnostic criteria have been established for the management of this pathology. We describe the first case of treatment for pediatric TMJ ankylosis and severe OSA due to neonatal group B streptococcal septic TMJ arthritis. Untreated pathological changes in the TMJ will eventually lead to ankylosis. Among children, this will include facial growth disturbances leading to mandibular r

Orthodontic Management of Skeletal Class II Malocclusion with the Invisalign Mandibular Advancement Feature Appliance: A Case Report and Review of the Literature.

The treatment of Class II malocclusion due to mandibular retrognathia is one of the most common challenges met in orthodontic practice. When it comes to a growing patient, functional appliances are the optimal way to achieve growth modification by enhancing mandibular growth. Clear aligners have been part of the orthodontic treatment for several decades but until recently they were only used to correct mild malocclusions. In 2017, Align Technology introduced the Invisalign with Mandibular Advancement Feature (IMAF) which replicates the action of functional appliances. As this device is new to
