Predictors of bacterial infections among HCV-negative injection drug users in Rhode Island.

The prevalence and risk factors for treated bacterial infections (i.e., skin abscess or cellulitis, osteomyelitis, or endocarditis) were investigated among a community sample of drug users with a history of injection drug use (IDU) who tested negative for Hepatitis C (HCV). Participants were IDUs in an HCV reduction intervention trial followed for 24 months. Among 109 participants, 9.2% reported a bacterial infection during follow-up. Non-Caucasian participants and those who had injected for longer periods at baseline were less likely to experience a bacterial infection at follow-up. IDUs with

Correlates of partner-specific condom use intentions among incarcerated women in Rhode Island.

CONTEXT: Few studies of incarcerated women have examined potential associations between risky sexual behavior and relationship context factors; thus, little is known about the correlates of intentions to use condoms with main and casual partners in this underserved population. METHODS: A sample of 221 women incarcerated in a Rhode Island Department of Corrections facility in 2002-2003 were interviewed. Multiple linear regression analysis was performed to assess associations between selected demographic, psychosocial and behavioral variables and participants' reported intentions to use condoms

Costing analysis of a point-of-care drug checking program in Rhode Island.

BACKGROUND: Drug checking is a harm reduction strategy that provides greater awareness and information about the drug supply to the community. While fentanyl test strips are low-cost and available in most parts of the U.S., community-based organizations are considering using more sophisticated technologies, such as Fourier-transform infrared (FTIR) spectroscopy to test drugs. FTIR can detect multiple substances in a non-destructive manner that can be rapidly communicated to the program client by a trained technician, however implementation costs in community-based settings have not been assess

A survey of syphilis knowledge among medical providers and students in Rhode Island.

BACKGROUND: In the United States, syphilis cases have increased dramatically over the last decade. Recognition and timely diagnosis by medical providers are essential to treating syphilis and preventing further transmission. METHODS: From 2016 to 2017, a cross-sectional survey was performed among medical students, residents, fellows, and attending physicians in Rhode Island. Topics included demographics, level of medical training, experience diagnosing and treating syphilis, and familiarity with the reverse testing algorithm. Participants were asked 25 true/false questions to assess basic kno

HIV infection in refugees: a case-control analysis of refugees in Rhode Island.

OBJECTIVES: The number of HIV-infected refugees entering the USA is increasing. There is little data describing the HIV-infected refugee population and the challenges encountered when caring for them. We performed a retrospective case-control analysis of HIV-infected refugees in order to characterize their co-morbidities, baseline HIV characteristics, and longitudinal care compared to HIV-infected non-refugees. METHODS: A retrospective chart review was performed of HIV-infected refugees and non-refugees who were matched for gender, age, and time of establishment of initial HIV care. RESULTS:

Assessing Public Health and Preventative Health Needs of Underserved Communities in Rhode Island.

BACKGROUND: In the United States, significant disparities exist in preventative health among underserved communities, including African American/Black (AA/B), Hispanic/Latino (H/L) and lesbian/gay/bisexual/transgender/ queer (LGBTQ+) individuals. We explored the influence of social determinants of health (SDOH) on preventative health outcomes in these groups. METHODS: We conducted a comprehensive cross-sectional needs assessment at safety-net health clinics in Providence, Rhode Island, from June to September 2022. RESULTS: Of N=1,017 individuals, 67% identified as H/L, 16% identified as AA/B

Transitioning Young Adult Patients from Pediatric to Adult Rheumatology Practices in Rhode Island.

BACKGROUND: Young adults with rheumatic conditions encounter challenges when transitioning to an adult rheumatologist. At the Hasbro Pediatric Rheumatology Clinic, patients aged 18 and older were historically provided a list of adult rheumatologists; however, no formal transition process existed. We assessed the feasibility and acceptability of a pilot model to systematically transition pediatric patients to an adult rheumatologist using aspects of the Six Core Elements of Health Care Transition (HCT) from Got Transition®. METHODS: A multidisciplinary team facilitated the program. Eligibility

Stroke systems of care: evaluating disparities after stroke legislation implementation in Rhode Island.

INTRODUCTION: The Rhode Island Stroke Prevention and Treatment Act aimed to establish a standardised system of care for stroke patients through designated stroke centres, revise emergency medical services (EMS) protocols, and establish a centralised stroke database. While Stroke Systems of Care (SSOC) laws have proven effective in reducing overall stroke mortality, their benefits among racial minorities and economically disadvantaged groups remain unclear. This study investigates the impact of the Act and its amendment on stroke mortality rates across different racial groups. METHODS: Data fr

Cholangiocarcinoma in Rhode Island: Incidence Trends and Risk Profile Over the Last Decade.

The incidence of cholangiocarcinoma, a fatal disease of bile ducts, is increasing at unsettling rates in the Northeast United States, including Rhode Island. The cause of this region-specific increase in incidence of cholangiocarcinoma is unknown. This is a review of the literature on cholangiocarcinoma in conjunction with cancer data from the 1995-2018 Rhode Island Cancer Registry. The goal of this paper is to discuss the potential etiologies of the increased incidence in cholangiocarcinoma and identify populations in Rhode Island most at risk. Rhode Island residents have specific environment

Evaluating a Care Management Program for Dual-Eligible Beneficiaries: Evidence from Rhode Island.

As health systems attempt to contain utilization and costs, care management programs are proliferating. However, there are mixed findings on their impact. In 2018, Rhode Island initiated a care management program for dually eligible Medicare and Medicaid beneficiaries at high risk of hospitalization or institutionalization. The objective of this study is to evaluate the association between health care utilization and costs and care management for dual-eligible participants (n = 169). The authors employed an interrupted time series analysis of administrative claims data using the Rhode Island A

Increase in Drug Overdose Deaths Involving Fentanyl-Rhode Island, January 2012-March 2014.

OBJECTIVE: This study identified sociodemographic, substance use, and multiple opioid prescriber and dispenser risk factors among drug overdose decedents in Rhode Island, in response to an increase in overdose deaths (ODs) involving fentanyl. METHODS: This cross-sectional investigation comprised all ODs reviewed by Rhode Island's Office of the State Medical Examiners (OSME) during January 2012 to March 2014. Data for 536 decedents were abstracted from OSME's charts, death certificates, toxicology reports, and Prescription Monitoring Program (PMP) databases. Decedents whose cause of death invo

Temporal and spatial behavior of pharmaceuticals in Narragansett Bay, Rhode Island, United States.

The behavior and fate of pharmaceutical ingredients in coastal marine ecosystems are not well understood. To address this, the spatial and temporal distribution of 15 high-volume pharmaceuticals were measured over a 1-yr period in Narragansett Bay (RI, USA) to elucidate factors and processes regulating their concentration and distribution. Dissolved concentrations ranged from below detection to 313 ng/L, with 4 pharmaceuticals present at all sites and sampling periods. Eight pharmaceuticals were present in suspended particulate material, ranging in concentration from below detection to 44 ng/g

Reported experiences during syringe purchases in Providence, Rhode Island: implications for HIV prevention.

Use of injection drugs remains a public health concern, with injection drug users (IDUs) experiencing an elevated risk of morbidity and mortality. While the sale of non-prescription syringes in pharmacies is legal in most states, some pharmacists choose not to sell non-prescription syringes, or opt to sell syringes in quantities too large for purchase by many IDUs. Thus, we sought to understand the interpersonal experience of syringe sale/ purchase, the implications of policies regarding syringe sales, and pharmacy staff and IDU (mis)perceptions about one another. We conducted in-depth, semi-s

Newly Diagnosed Individuals in Molecular HIV-1 Clusters in Rhode Island Over 3 Decades.

BACKGROUND: Characterizing clustering rates of people with HIV in high-risk populations can offer insights on the HIV epidemic, enhancing efforts to control its spread. METHODS: We investigated longitudinal dynamics of clustering rates among individuals newly diagnosed with HIV-1. Data were extracted from the medical records of all people with HIV in Rhode Island with available viral sequences. Partial pol sequences were grouped by HIV diagnosis year, and clusters were identified in annual phylogenies. Clustering trends were estimated within 11 sociodemographic variables with the Mann-Kendall

The Behavioral Health Harms of Youth Exposure to Gun Violence: A Rhode Island Example.

BACKGROUND: Youth exposure to gun violence is increasing and we must better understand its impact on mental health and substance use disorders in young adults. METHODS: The 2024 Rhode Island Young Adult Survey recruited n=1,008 young adults. Multivariable logistic regressions were used for mental illness and substance use disorder outcomes on exposure to gun violence in childhood while controlling for sexual and gender identity, race/ethnicity, age, and social ladder. RESULTS: Exposure to gun violence in childhood is highly prevalent (24.7%) and associated with higher odds of depression (AOR

Fentanyl test strip use and homelessness among people who use drugs in Rhode Island.

BACKGROUND: People experiencing homelessness have been disproportionately affected by the overdose crisis in the United States. We assessed whether an association exists between homelessness and fentanyl test strip (FTS) use to avoid drug overdose among people who use drugs in Rhode Island. METHODS: We recruited 505 participants aged 18-65 from September 2020 to February 2023 as part of the Rhode Island Prescription Illicit Drug Study (RAPIDS). Baseline data assessed correlates of past-month FTS use through bivariate and multivariable analyses. We used generalized estimating equations (GEE) t

Neighborhood-level association between release from incarceration and fatal overdose, Rhode Island, 2016-2020.

The association between recent release from incarceration and dramatically increased risk of fatal overdose is well-established at the individual level. Fatal overdose and. arrest/release are spatially clustered, suggesting that this association may persist at the neighborhood level. We analyzed multicomponent data from Rhode Island, 2016-2020, and observed a modest association at the census tract level between rates of release per 1000 population and fatal overdose per 100,000 person-years, adjusting for spatial autocorrelation in both the exposure and outcome. Our results suggest that for ea
