Public health and human rights: questions from an epidemic in Prato.

This article uses Carlo Cipolla's account of the plague epidemic in Prato in 1630-1631 to identify several key questions in public health. The article goes on to observe that the underlying problems posed for public health by the plague epidemic in Prato are similar, in important respects, to those encountered in addressing contemporary public health concerns. Questions of inequalities in health and socio-economic differentials in mortality; the relationship between economic wellbeing and health; the relevance of access to effective interventions; the significance of knowledge concerning disea

Can human rights discourse improve the health of Indigenous Australians?

OBJECTIVE: Recognition of the poor health outcomes of Indigenous Australians has led to an interest in using human rights discourse as a framework for arguing that the Australian Government has an international obligation to improve Indigenous health. METHOD: This paper explores two potential directions for human rights discourse in this context. The first is the development and elaboration of an asserted 'human right to health'. The second focuses on developing an understanding of the interactions between health and human rights, particularly the underlying social determinants of health, and

Is breast cancer survival correlated with indicators of the right to health? An interdisciplinary study in 58 countries.

BACKGROUND: Women with breast cancer have different chances of surviving their disease, depending on where they live. When breast cancer is diagnosed and treated early, the chances of longer-term survival improve. Inequalities in survival are deemed unfair, because high survival for breast cancer can be achieved. We enquired whether adoption of a human rights-based approach to breast cancer care and management is correlated with breast cancer survival. METHOD: We reviewed the law literature on key concepts of the human right to health and performed a scoping review of the public health litera

The nexus of mental health, human rights and sustainability: interdisciplinary approach.

This paper explores the direct and indirect effects of climate change on mental health. It then goes on to describe recent judgements in human rights courts globally on climate change and its mental health implications, holding national governments to account for the mental health implications of climate change. It then explores ways in which psychiatrists can start to influence this nexus of mental health, sustainability and human rights in their clinical practice, in research and education as well as in policy making. This influence can be at the level of the individual patient and at the lo

Reproductive health rights and survival: The voices of mothers experiencing homelessness.

Women experiencing homelessness report higher rates of reproductive health-related traumas, including unplanned pregnancy, miscarriage, and abortion than their non-homeless peers. Using phenomenological hermeneutic methods, we sought to understand the reproductive health histories of women currently experiencing homelessness (N = 20, 25-61 years old, Mage = 38.33, SDage = 9.33) analyzing data collected between June 2014 and July 2015 in north central Texas. Three key themes highlight the essence of the women's experiences: (1) unexpected pregnancy-pregnancy just happened, (2) loss of reproduct

Protecting public health in the age of bioterrorism surveillance: is the price right?

Millions of dollars have been spent improving the bioterrorism surveillance capabilities of the public health system. Yet relatively little attention has been paid to the benefits that such expenditures yield. To assess the impact of an aerosol release of Bacillus anthracis, this article collects the available evidence on the potential benefits of environmental detection relative to the costs of a bioterrorist attack like the one in 2001, which occurred in the absence of any such detection. The cost-benefit model shows that biological surveillance that reduces time to treatment to 48 hours yie

Country-level right-to-health and health-system indicators and five-year net survival from adult brain tumours: An ecological analysis using CONCORD-3 data.

BACKGROUND: Country-level right-to-health and health-system indicators, mapped to availability, accessibility, acceptability and quality (AAAQ) domains, are used to evaluate equitable cancer outcomes, but their disease-specific validity is uncertain. Adult brain tumours are a stringent test case: no validated screening exists, glioblastoma drives much brain-tumour mortality, and survival depends on neurosurgical and radiotherapy access, neuropathology and molecular classification. METHODS: Ecological analysis of five-year net survival from CONCORD-3 (adult C71 brain tumours, diagnosed 2010-14

I Just Wish They'd Ask the Right Questions About Sex: Working to Improve Sexual Health Communication With Older Adults.

BACKGROUND AND AIMS: Sexual health among older adults (ages 50 and above) remains under-addressed in clinical settings, often due to ageism and misconceptions about older adults being asexual. Despite high rates of sexual activity in this age group, healthcare providers rarely initiate discussions about sexual wellness, particularly related to HIV and STIs. This project aimed to explore how health communication tools, specifically film and reflexive exercises, can challenge implicit bias and improve provider comfort with sexual health conversations across the lifespan. METHODS: A 24-min docum

Leveraging National Data and Regional Innovations to Right-Size Tobacco Treatment Policy for Behavioral Health in a Midwestern State.

BACKGROUND: People with mental health (MH) and substance use disorders (SUD) have high rates of tobacco use and tobacco-related mortality. They want to stop smoking and studies have shown they can quit, but few behavioral health facilities provide tobacco treatment. The purpose of this paper is to describe how a midwestern statewide behavioral health collaboration used regional data to pinpoint strengths and weaknesses in tobacco treatment trends, identified policies in neighboring states that were associated with high rates of tobacco treatment, and worked with state leaders to implement thes

Health Through a Human Right Lens at the US-Mexico Border: Increasing Access to Healthcare for Central American Immigrants.

The number of immigrants seeking entry into the U.S. through asylum requests or through irregular means is increasing, and most come from the Northern Triangle of El Salvador, Guatemala, and Honduras. Immigrants come fleeing extreme poverty, violence, health and social inequities, and drastic climate changes. Most had limited access to healthcare at home, and even more limited care along the journey. Those that are allowed entry into the U.S., are confronted with feeling unwelcome in many communities, having to navigate an array of local, state, and federal laws that regulate access to healthc

Infertility in the Pacific: A crucial component of the sexual and reproductive health and rights agenda.

Across Pacific Island countries, women and men are disproportionately affected by several risk factors for infertility, including sexually transmissible infections, complications from unsafe abortions, postpartum sepsis, obesity, diabetes, tobacco smoking and excessive alcohol consumption. Despite this, little is known about community awareness of infertility, behavioural risk factors, the lived experiences of infertile couples or the contexts in which they access fertility care. In this opinion piece we discuss the current evidence and gaps in evidence regarding infertility in Pacific Island

Land Rights and Health Outcomes in American Indian/Alaska Native Children.

American Indian and Alaska Native (AI/AN) land rights, sovereignty conflicts, and health outcomes have been significantly influenced by settler colonialism. This principle has driven the numerous relocations and forced assimilation of AI/AN children as well as the claiming of AI/AN lands across the United States. As tribes across the country begin to reclaim these lands and others continue to struggle for sovereignty, it is imperative to recognize that land rights are a determinant of health in AI/AN children. Aside from the demonstrated biological risks of environmental health injustices incl
