Nailfold capillaroscopy improves cardiovascular risk stratification in SSc: an adjustment of the SCORE2 algorithm.

OBJECTIVE: SSc is characterized by micro- and macrovascular damage, increasing cardiovascular (CV) risk. The SCORE2 algorithm underestimates CV risk in systemic autoimmune diseases. Nailfold capillaroscopy (NFC) is used to assess SSc-related microvascular damage. We aimed to evaluate whether incorporating NFC findings into SCORE2/SCORE2-OP improves CV risk stratification in SSc. METHODS: Retrospective multicentre study including 276 patients with SSc. Baseline 10-year CV risk was estimated using SCORE2 or SCORE2-OP. NFC at diagnosis was assessed. Cox regression identified NFC variables associ

After Risk-Adjustment Change, Dementia Diagnoses Increased In Medicare Advantage Relative To Traditional Medicare.

In 2020, the Centers for Medicare and Medicaid Services reintroduced Alzheimer's disease and related dementias to its risk-adjustment payment model for Medicare Advantage (MA) plans. Using 2017-20 data for 100 percent of community-dwelling beneficiaries enrolled in Medicare, we evaluated how the reintroduction of dementia to the risk-adjustment model affected rates of new (incident) dementia diagnoses among beneficiaries enrolled in MA relative to those enrolled in traditional Medicare. In response to the payment change, annual incident dementia diagnosis rates in MA increased by 11.5 percent

Effect of disability, homelessness, and neighborhood marginalization on risk adjustment for hospital performance measurement.

It is not known how disability, homelessness, or neighborhood marginalization influence risk-adjusted hospital performance measurement in a universal health care system. In this study, we evaluated the effect of including these equity-related factors in risk-adjustment models for in-hospital mortality, and 7- and 30-day readmission in 28 hospitals in Ontario, Canada. We compared risk adjustment with commonly used clinical factors to models that also included homelessness, disability, and neighborhood indices of marginalization. We evaluated models using historical data using internal-external

Assessment of the Medicare Advantage Risk Adjustment Model for Measuring Veterans Affairs Hospital Performance.

IMPORTANCE: Policymakers and consumers are eager to compare hospitals on performance metrics, such as surgical complications or unplanned readmissions, measured from administrative data. Fair comparisons depend on risk adjustment algorithms that control for differences in case mix. OBJECTIVE: To examine whether the Medicare Advantage risk adjustment system version 21 (V21) adequately risk adjusts performance metrics for Veterans Affairs (VA) hospitals. DESIGN, SETTING, AND PARTICIPANTS: This cohort analysis of administrative data from all 5.5 million veterans who received VA care or VA-purch

Intraoperative Adverse Events: Risk Adjustment for Procedure Complexity and Presence of Adhesions Is Crucial.

BACKGROUND: Benchmarking the quality of intraoperative care by comparing the rates of intraoperative adverse events (iAEs) necessitates adequate risk adjustment. We sought to identify the patient- and procedure-related risk factors for iAEs. STUDY DESIGN: Our 2007 to 2012 institutional American College of Surgeons NSQIP and administrative databases were linked and then screened for iAEs using the Patient Safety Indicator "Accidental Puncture/Laceration." Intraoperative adverse events were confirmed by systematic review of medical records. Comorbidities were assessed using American College of

Risk-adjustment of cesarean delivery rates: a practical method for use in quality improvement.

Risk-adjustment of cesarean birthrates has been hampered by inadequacies in the existing secondary data sources or by the need for extensive chart review. This study presents an efficient risk-adjustment model for cesarean birth, based on easily retrievable ICD-9 codes and clinical risk factors least influenced by physician practice style. Data are presented for mothers undergoing 7322 deliveries from 1997-1998 at a large academic medical center with a cesarean birth rate of 15.9%. Multiple logistic regression was used to predict the likelihood of cesarean delivery controlled for maternal age,

[Family characteristics, organic risk factors, psychopathological picture and premorbid adjustment of hospitalized adolescent patients].

The relation of some family characteristics such as cohesion and adaptability with organic risk factors, developmental psychopathology, clinical picture and premorbid adjustment was assessed in the group of 100 hospitalized adolescent patients and families. We found correlation between: some of organic risk factors (pathology of neonatal period, pathology of early childhood), some of indicators of developmental psychopathology (eating disorders, conduct disorders), some of clinical signs (mannerism, grandiosity, hostility, suspciousness, disturbances of content of thinking), premorbid adjustme

Use of mechanistic information to derive chemical-specific adjustment factors - Refinement of risk assessment.

When extrapolating data from animal toxicological studies a default factor (dUF) of 100 is applied to derive a heath based guidance value. The UF takes into account the interspecies differences (ID) and the intraspecies variability (IV). When re-evaluating the safety of phosphates used as food additives nephrocalcinosis was identified as the critical endpoint. The underlying mechanism for nephrocalcinosis was attributed to the precipitation of calcium phosphate in the kidney, depending on its solubility, irrespective of the species and the population. Based on the mechanism, the volume of prim

Risk and Adversity, Parenting Quality, and Children's Social-Emotional Adjustment in Families Experiencing Homelessness.

A multimethod, multi-informant design was used to examine links among sociodemographic risk, family adversity, parenting quality, and child adjustment in families experiencing homelessness. Participants were 245 homeless parents (Mage  = 31.0, 63.6% African American) and their 4- to 6-year-old children (48.6% male). Path analyses revealed unique associations by risk domain: Higher sociodemographic risk predicted more externalizing behavior and poorer teacher-child relationships, whereas higher family adversity predicted more internalizing behavior. Parenting quality was positively associated w

A pilot study of risk adjustment for benchmarking antibiotic use between hospitals in Sweden.

There is no established standard for comparing overall antibiotic use between hospitals taking patient characteristics into account. The objective of this study was to investigate whether there is a correlation between surrogate markers for patient morbidity, namely case mix index (CMI), mean length of hospital stay (LoS) and mean cost per admission, and antibiotic use in a sample of Swedish hospitals. All primary and secondary hospitals in three counties with high and three counties with low consumption of antibiotics were selected. Data from 16 hospitals were included. A regression analysis

Risk adjustment for hospital use using social security data: cross sectional small area analysis.

OBJECTIVES: To identify demographic and socioeconomic determinants of need for acute hospital treatment at small area level. To establish whether there is a relation between poverty and use of inpatient services. To devise a risk adjustment formula for distributing public funds for hospital services using, as far as possible, variables that can be updated between censuses. DESIGN: Cross sectional analysis. Spatial interactive modelling was used to quantify the proximity of the population to health service facilities. Two stage weighted least squares regression was used to model use against su

Spatial heterogeneity in risk-adjusted return of spontaneous circulation after out-of-hospital cardiac arrest: the urbanization-adjusted RACA model.

BACKGROUND: Registry-based benchmarking of return of spontaneous circulation (ROSC) requires well-calibrated risk adjustment across Emergency Medical Services (EMS) systems. Existing risk-adjustment models, however, do not account for urbanization-related heterogeneity, which may contribute to systematic urbanization-stratified miscalibration, bias cross-system benchmarking, and misdirect quality improvement. We developed and validated an urbanization-adjusted ROSC after cardiac arrest (URACA) by incorporating Eurostat's Degree of Urbanisation (DEGURBA) into RACA and assessed whether it improv

Risk-Adjusted Prevention. Perspectives on the Governance of Entitlements to Benefits in the Case of Genetic (Breast Cancer) Risks.

This article is a revised version of our proposal for the establishment of the legal concept of risk-adjusted prevention in the German healthcare system to regulate access to risk-reduction measures for persons at high and moderate genetic cancer risk (Meier et al. Risikoadaptierte Prävention'. Governance Perspective für Leistungsansprüche bei genetischen (Brustkrebs-)Risiken, Springer, Wiesbaden, 2018). The German context specifics are summarized to enable the source text to be used for other country-specific healthcare systems. Establishing such a legal concept is relevant to all universal a

Risk-adjusted cesarean rates: what risk factors for cesarean delivery are important to practicing obstetricians?

OBJECTIVE: To determine which primary cesarean delivery risk factors are important to practicing physicians. STUDY DESIGN: A sample of current members of the American College of Obstetrician Gynecologists were surveyed about the risk factors for primary cesarean delivery that they thought were most important. Data on demographic and practice characteristics were also collected. Sample size was determined by theoretical saturation. RESULTS: Theoretical saturation was reach at 60 responses. A total of 290 surveys were returned. The 10 most common factors listed by physicians as preexisting pat

A Novel Machine Learning Algorithm for Creating Risk-Adjusted Payment Formulas.

IMPORTANCE: Models predicting health care spending and other outcomes from administrative records are widely used to manage and pay for health care, despite well-documented deficiencies. New methods are needed that can incorporate more than 70 000 diagnoses without creating undesirable coding incentives. OBJECTIVE: To develop a machine learning (ML) algorithm, building on Diagnostic Item (DXI) categories and Diagnostic Cost Group (DCG) methods, that automates development of clinically credible and transparent predictive models for policymakers and clinicians. Adjusting for BMI in analyses of volumetric mammographic density and breast cancer risk.

BACKGROUND: Fully automated assessment of mammographic density (MD), a biomarker of breast cancer risk, is being increasingly performed in screening settings. However, data on body mass index (BMI), a confounder of the MD-risk association, are not routinely collected at screening. We investigated whether the amount of fat in the breast, as captured by the amount of mammographic non-dense tissue seen on the mammographic image, can be used as a proxy for BMI when data on the latter are unavailable. METHODS: Data from a UK case control study (numbers of cases/controls: 414/685) and a Norwegian c

Multiplicity-Adjusted Confidence Limits in Risk Assessment with Quantal Response Data.

In risk assessment, it is often desired to make inferences on the risk at certain low doses or on the dose(s) at which a specific benchmark risk (BMR) is attained. At times, [Formula: see text] dose levels or BMRs are of interest, and some form of multiplicity adjustment is necessary to ensure a valid [Formula: see text] simultaneous inference. Bonferroni correction is often employed in practice for such purposes. Though relative simple to implement, the Bonferroni strategy can suffer from extreme conservatism (Nitcheva et al., 2005; Al-Saidy et al., 2003). Recently, Kerns (2017) proposed the

The impact of an individualized risk-adjusted approach on hypertension treatment in primary care.

Previous studies suggest that up to 60% of all patients with hypertension receive inappropriate treatment. Current 2013 European Society of Hypertension/European Society of Cardiology (ESH/ESC) guidelines recommend taking cardiovascular risk factors into account when assessing treatment for patients with hypertension. The authors hypothesize that this approach will reduce the proportion of patients receiving inappropriate treatment. In this cross-sectional study using electronic medical records of Swiss primary care patients, the authors estimate the proportion of patients receiving inappropri

Stability of risk-adjusted primary cesarean delivery rates over time.

OBJECTIVE: The purpose of this study was to compare how hospital rankings, based on risk-adjusted primary cesarean rates, change over time. STUDY DESIGN: Washington State Birth Events records data from 1995 through 1998 were used. Risk-adjustment models were built for each year. Risk-adjusted hospital rankings and outlier status (either "above" or "below" expected cesarean delivery rates) were calculated for each year. Hospitals that were identified as outliers for each year were compared with the use of kappa statistics. The top and bottom 25% of the hospital rank list from each year were al
