A laboratory study comparing two methods for removing plastic carrier obturators from severely curved root canals.

BACKGROUND: Removing plastic carrier obturators during root canal retreatment is challenging and often takes a long time. The aim of this laboratory investigation is to compare a recently described indirect, two-stage technique for removing plastic carriers from curved canals and evaluate its efficiency, safety and efficacy in comparison to a conventional direct technique. METHODS: Thirty-six extracted mandibular premolars were chosen and root filled with plastic carrier obturators and endodontic cement. The teeth were allocated randomly into two different retreatment techniques: Group 0 (Dir

Minimally Invasive Endodontic Treatment through the Labial/Buccal Cervical Approach - Protocol and Case Series.

INTRODUCTION: Traditional access cavity preparation involves removing the roof of the pulp chamber and smoothing the dentin bulges at the root canal orifice, thereby creating straight-line access. However, this may damage more healthy dental tissue and reduce the tooth's fracture resistance. This case series presents a novel minimally invasive endodontic protocol for 1 maxillary canine and 4 mandibular premolars, which required root canal therapy, due to labial/buccal cervical decay that caused pulpitis or periapical periodontitis. METHODS: Our treatment protocol includes the following steps:

Influence of Access Cavity Design on Root Canal Detection, Instrumentation Efficacy, and Fracture Resistance Assessed in Maxillary Molars.

INTRODUCTION: The aim of this study was to assess the influence of contracted endodontic cavities (CECs) on root canal detection, instrumentation efficacy, and fracture resistance assessed in maxillary molars. Traditional endodontic cavities (TECs) were used as a reference for comparison. METHODS: Thirty extracted intact maxillary first molars were scanned with micro-computed tomographic imaging at a resolution of 21 μm, assigned to the CEC or TEC group (n = 15/group), and accessed accordingly. Root canal detection was performed in 3 stages: (1) no magnification, (2) under an operating micros

Conservative shaping combined with three-dimensional cleaning can be a powerful tool: Case series.

Conservative endodontics has been introduced about a decade ago. Since then, it has been demonstrated that less canal preparations lead to more dentin preservation resulted in decreased stress on tooth structure, mainly in the coronal third of the root, and potentially a higher resistance to fracture. In addition, smaller and larger canal preparations were comparable with regard to the cleanliness of the root canal. The purpose of this case series was to report on the outcome of root canal treatments following a conservative canal preparation, followed by three-dimensional cleaning technique (

Accuracy of electronic apex locators in the determination of the working length in teeth with natural apical root resorption in the presence of different irrigation solutions.

BACKGROUND: Root resorption (RR) is usually a consequence of dental trauma, pulpal infection, primary occlusal pressure, or orthodontic tooth movement, leading to the loss of anatomical root formation. As a result of apical RR, the apical constriction is destroyed, and the determination of the working length (WL) may become difficult. OBJECTIVES: The purpose of the present study was to evaluate the accuracy of 4 different electronic apex locators (EALs) - ProPex® II, Propex Pixi®, DentaPort ZX, and DTE DPEX V - in the determination of WL in teeth with natural apical RR in the presence of diff

Examination of Irrigant Flow on a Tooth With Internal Root Resorption by Using a Computational Fluid Dynamics Model.

OBJECTIVE: This study investigated the flow of an endodontic irrigant in a single-rooted tooth with internal root resorption (IRR). METHODS: A simulation of a prepared central incisor with internal root resorption was created and irrigation with a 30-G needle was performed. The fluid pattern of the irrigant was evaluated using a Computational Fluid Dynamics model. In addition, the effects of the needle-insertion depth in the root canal and the size of root resorption on the fluid flow and the wall shear stress (WSS) values were assessed. The IRR was placed immediately below the canal orifice.

[Confusion and solution for root canal working length determination].

Pulpitis and periapical periodontitis is a type of bacterial infectious disease, and bacteria frequently plant in the entire root canal system during the terminal stage. Main clinical treatment strategy of the disease requires root canal therapy, a key and core procedure for the successful treatment by thoroughly removing the root canal infection. The premise and guarantee of thoroughly removing root canal infection is by determining the accurate root canal working length. However, introduction of the complexity of the apical root anatomical structure, the confusion in determining the position

Treatment of a Maxillary Second Molar with One Buccal and Two Palatal Roots Confirmed with Cone-Beam Computed Tomography.

Root canal configuration is an important subject in endodontic practice and dentists should be familiar with all possible types of root canal configuration. A forty-three year old male was referred for root canal treatment of his maxillary left second molar tooth with symptomatic irreversible pulpitis. Pre-operative radiographs showed a three rooted molar. However, after access cavity preparation two palatal and one buccal orifices were detected. The patient was informed of the unusual root canal anatomy and cone-beam computed tomography (CBCT) was ordered for precise evaluation of the anatomy

Endodontic-related facial paresthesia: systematic review.

Paresthesia is a neurosensitivity disorder caused by injury to the neural tissue. It is characterized by a burning or twinging sensation or by partial loss of local sensitivity. Paresthesia related to endodontic treatment can occur because of extravasation of filling material or the intracanal dressing, as a consequence of periapical surgery or because of periapical infection. A literature review of paresthesia in endodontics was undertaken, with a view to identifying and discussing the most commonly affected nerves, the diagnostic process and the treatment options. Among reported cases, the m
