Augmenting patient safety surveillance in radiation oncology with large language model-based root cause analysis: A proof-of-concept study.

We investigated the potential utility of large language models (LLMs) in supporting patient safety efforts. Specifically, we evaluated the reasoning capabilities of LLMs in performing root cause analysis (RCA) of radiation oncology incidents using narrative reports from the Radiation Oncology Incident Learning System (RO-ILS). We prompted four state-of-the-art LLMs, Gemini 2.5 Pro, GPT-4o, o3, and Grok 3, with the "Background and Incident Overview" sections from 19 publicly available RO-ILS cases. Each model was instructed to perform RCA and generate root causes, lessons learned, and suggested

Improving ICSI success rates following root cause analysis and use of system behaviour charts: the devil is in the detail!

A fertility clinic observed a reduction in its fresh intracytoplasmic sperm injection (ICSI) implantation rate key performance indicator (KPI) below benchmark threshold which was further monitored but did not improve. The clinic had been performing ICSI successfully for >16 years with good ICSI implantation rates meeting benchmark level. A root cause analysis (RCA) was conducted, including the input from an external observer, reviewing all systems and processes. A bundle of recommended changes was implemented as part of an improvement cycle with the aim to increase fresh ICSI implantation r

Root cause analysis of epidural spinal cord stimulator implant infections with resolution after implementation of an improved protocol for surgical placement.

BACKGROUND: Placing a spinal stimulator for the purpose of restoring paralysed function is a novel procedure; however, paralysis predisposes people to infection. Preventing surgical site infections is critical to benefit this population. OBJECTIVE: The objective of this study was to review the root cause analysis of postoperative wound infections by a hospital epidemiology team following implantation of epidural spinal cord neurostimulators in patients with chronic spinal cord injury. METHODS: A team was assembled to review the case of every individual who had been enrolled to receive a neur

Applying the Human Factors Analysis and Classification System within root cause analysis to prevent medical errors and enhancing patient safety culture: insights from a medical center.

INTRODUCTION: Enhancing patient safety and minimizing medical errors are crucial in healthcare. While root cause analysis (RCA) is commonly used to investigate adverse events, its lack of human factor integration limits its effectiveness. The Human Factors Analysis and Classification System (HFACS), adapted from aviation to healthcare, systematically identifies human and organizational factors. However, the integration of HFACS into RCA and the impact of HFACS-RCA implementation remain underexplored. Therefore, this study aims to provide a practical case of HFACS integration into RCA and explo

Root Cause Analysis for Outbreaks of Carbapenem-Resistant Acinetobacter baumannii and Pseudomonas aeruginosa in a Pediatric Intensive Care Unit: A Retrospective Study.

OBJECTIVE: Children in the pediatric intensive care unit (PICU) are inherently susceptible to multidrug-resistant (MDR) bacterial infections due to physiological vulnerabilities and clinical management-related factors. Such infections are prone to outbreaks and confer substantial harm. This study aims to investigate the etiologies of carbapenem-resistant Acinetobacter baumannii (CRAB) and carbapenem-resistant Pseudomonas aeruginosa (CRPA) infection outbreaks in the PICU and develop targeted prevention and control strategies using the root cause analysis (RCA) approach. METHODS: A retrospectiv

Risk factors for post-dural puncture headache following injury of the dural membrane: a root-cause analysis and nested case-control study.

BACKGROUND: Post-dural puncture headache following trauma to the dural membrane during neuraxial anaesthesia occurs in 0.13-6.5% of pregnant patients. Identifying factors beyond individual performance that contribute to this adverse event is crucial to developing improvement strategies. METHODS: We used a root cause analysis framework, in a nested case-control study, to identify associated factors. Cases were all patients who had a post-dural puncture headache requiring an epidural blood patch. These patients were matched to a random group of control patients without post-dural puncture heada

Root resorption caused by aligners, self-ligating appliances, and conventional fixed appliances: a CBCT-based meta-analysis.

BACKGROUND: Orthodontically induced inflammatory root resorption (OIIRR) is a common adverse effect of orthodontic treatments. Radiographs are routinely used to diagnose OIIRR; however, 3-dimensional cone beam computed tomography (CBCT) studies have recently been conducted to assess hard tissue loss more accurately. There is controversial evidence of differences between aligners and fixed appliances in terms of OIIRR. This meta-analysis aims to investigate the differences in OIIRR between fixed appliances and aligners based on recent CBCT-based studies. METHODS: A systematic review and meta-a

Quality review of an adverse incident reporting system and root cause analysis of serious adverse surgical incidents in a teaching hospital of Scotland.

BACKGROUND: A significant proportion of surgical patients are unintentionally harmed during their hospital stay. Root Cause Analysis (RCA) aims to determine the aetiology of adverse incidents that lead to patient harm and produce a series of recommendations, which would minimise the risk of recurrence of similar events, if appropriately applied to clinical practice. A review of the quality of the adverse incident reporting system and the RCA of serious adverse incidents at the Department of Surgery of Ninewells hospital, in Dundee, United Kingdom was performed. METHODS: The Adverse Incident M

Understanding root causes of homelessness in Marin County, CA and prevention strategies through semi-structured interviews and constant comparative analysis.

INTRODUCTION: Even with the increased prevalence of homelessness in the US, there is limited research that investigates circumstances that can lead to homelessness, especially in affluent communities in California. This lack of understanding, even when resources are available, perpetuates the inequities in health and wellbeing statewide, connection to basic necessities, effective prevention strategies, and meaningful policy changes. METHODS: This qualitative analysis consisted of a total of 24, semi-structured interviews conducted in the Spring of 2023 by public health researchers. Participan
