Molecular epidemiology of HIV type 1 subtypes in Rwanda.

HIV-1 infection is characterized by genetic diversity, with multiple subtypes and recombinant variants circulating, particularly in sub-Saharan Africa. During the Rwandan genocide, many women experienced multiple rapes and some became HIV-1 infected. We studied plasma and peripheral blood mononuclear cells (PBMCs) from 30 infected women comprising two exposure groups: those with numerous contacts, raped multiple times, and women with one lifetime sexual partner and no history of rape. Population-based sequences from gag, pol, and env genes were analyzed to determine HIV-1 subtypes and intersub

[Foreign body aspiration in Kigali University Teaching Hospital, Rwanda].

We present the case of a 12-year-old girl referred to Kigali University Teaching Hospital (KUTH) for persistent cough, fever and haemoptysis. Respiratory symptoms started acutely with a stridor at age 4. Thereafter she developed a chronic cough with intermittent fever. She was treated ambulatory in the health care centre with oral antibiotics and finally referred to the district hospital at age 7. The chest X-ray then suggested tuberculosis for which a 6 month treatment was given with no improvement. The cough persisted and haemoptysis appeared so the patient was referred to the reference hosp

BCG vaccination and pediatric HIV infection--Rwanda, 1988-1990.

In Africa, tuberculosis (TB) is one of the most common severe diseases associated with human immunodeficiency virus (HIV) infection. For infants in Rwanda and other countries where the risk for TB infection is high, the World Health Organization (WHO) recommends vaccination against TB at birth with bacille Calmette-Guérin (BCG) vaccine (1). This report summarizes a study of the occurrence of immune responses and adverse reactions following BCG vaccination among a cohort of infants born to HIV-seropositive and HIV-seronegative mothers in Kigali, Rwanda, where the prevalence of HIV infection amo

In vitro antimicrobial sensitivity of Neisseria gonorrhoeae from Rwanda.

The in vitro sensitivity of 104 isolates of Neisseria gonorrhoeae to six antimicrobial agents was tested. More than 50% of the isolates produced penicillinase. Of those that did not produce penicillinase, 26% were resistant (minimum inhibitory concentration (MIC)) greater than or equal to 0.5 mg/l), and 68% showed a decreased sensitivity for penicillin G (0.06 less than or equal to MIC less than or equal to 0.25 mg/l). Twenty six per cent and 50% of the strains, respectively, showed a decreased sensitivity to thiamphenicol (MIC greater than or equal to 1 mg/l) and tetracycline (MIC greater tha

Risk factors associated with prevalent HIV-1 infection among pregnant women in Rwanda. National University of Rwanda-Johns Hopkins University AIDS Research Team.

This study evaluated risk factors associated with prevalent HIV-1 infection among pregnant women in a semi-rural but densely populated area surrounding the town of Butare in Rwanda. Overall seroprevalence was 9.3% in 5690 pregnant women who sought antenatal care at one of five health centres. Factors associated with higher seroprevalence of HIV-1 included history of multiple sexual partners, history of at least one sexually transmitted disease (STD), relatively high socioeconomic status (SES), being unmarried, young age at first pregnancy, and low gravidity. Women who had used oral contracepti

Exploring the lived socioeconomic experiences of teen mothers in Rwanda.

Teenage pregnancy is one of the major public health problems worldwide, with the issue more severe in low-and middle-income countries. While poverty is a main risk factor, the negative consequences of teenage pregnancy further perpetuate the cycle of poverty. To support teen mothers, many support programs were created; however, the benefits which reached them were limited. Since social norms vary from community to community, the types of challenges teen mothers face influence the type of support they need. This study explored the firsthand socio-economic experiences of women who underwent teen

Reflections on a global partnership for perinatal quality improvement in Rwanda.

Between 2017 and 2023, the Royal College of Paediatrics and Child Health and the Rwanda Pediatric Association (its peer institution in Rwanda) collaborated with UNICEF, the Rwandan Ministry of Health and the Rwanda Biomedical Center to design and deliver a perinatal programme to improve clinical care and outcomes across multiple hospitals and selected primary health centres. Over the period, the programme showed measurable positive impact on quality of care and clinical outcomes. However, the persistence of preventable or avoidable mortality, in particular among newborns in hospital and health

The impacts of COVID-19 on routine immunization for children in Rwanda.

INTRODUCTION: The Coronavirus Disease 2019 (COVID-19) pandemic had a direct impediment to the provision of critical health services worldwide, and one of the most affected areas was routine immunisation programmes. This caused a significant drop in child immunisation rates, especially in the early stages of the pandemic. This research aimed to investigate the socio-demographic predictors of the continued persistence of routine immunisation services in Rwanda during the COVID-19 pandemic. METHODS: The survey was done between January 3 and March 31, 2022 among mothers living in five districts in

Implementation of a Large-Scale Ebola Vaccination Campaign in Rwanda.

BACKGROUND/OBJECTIVES: Ebola Virus Disease (EVD) remains a public health threat in sub-Saharan Africa. The 10th Ebola outbreak in the Democratic Republic of the Congo (DRC) in 2018-2020 led the Rwanda Ministry of Health to launch a large-scale Ebola vaccination campaign using the two-dose Ad26.ZEBOV and MVA-BN-Filo regimen. The campaign was implemented by local organizations, the Center for Family Health Research and Rinda Ubuzima, in partnership with the Rwanda Biomedical Center. METHODS: The campaign targeted those who live near or routinely cross the Rwanda/DRC border and unvaccinated firs

Resource Allocation in the Pediatric Intensive Care Unit in Rwanda.

Background: Children born in low‑ and middle‑income countries are 14 times more likely to die before reaching the age of five compared to children in high‑income countries. Pediatric Intensive Care Units (PICUs) with specialized equipment and advanced medications managed by trained clinicians have reduced mortality of children worldwide, yet countries with limited funds and scarce resources strain to meet needs of critically ill children. Objectives: The aim of the study was to identify the disease burden of patients entering the PICU at the Central Hospital in Kigali, Rwanda, and the relation

A 17-Year Experience of Valvular Heart Surgery in Rwanda.

BACKGROUND: The advanced presentation of rheumatic heart disease in Rwanda often necessitates surgical intervention. We summarize the outcomes of valvular heart operations in Rwanda between 2006 and 2023. METHODS: The study included 366 patients in the Rwandan cardiac surgery registry who underwent valvular surgery. We examined surgical details, perioperative outcomes, and long-term outcomes. Cox multivariable analyses were conducted to assess factors predictive of survival outcomes. Additionally, subgroup analyses compared outcomes between mechanical valve replacement and bioprosthetic valve

A handmade trap for malaria mosquito surveillance by citizens in Rwanda.

For effective sampling of mosquitoes in malaria surveillance programmes, it is essential to include attractive cues in traps. With the aim of implementing a citizen science project on malaria vectors in rural Rwanda, a handmade plastic bottle trap was designed and tested in the field to determine its effectiveness in capturing adult Anopheles gambiae sensu lato, the main malaria vector, and other mosquito species. Carbon dioxide (CO2) and light were used as attractive cues. CO2 was produced by inoculating sugar with yeast and water. Light was emitted from a torch by light-emitting diodes (LEDs

The impact of the Fundamental Critical Course on knowledge acquisition in Rwanda.

BACKGROUND: Emerging critical care systems have gained little attention in low- and middle-income countries. In sub-Saharan Africa, only 4% of the healthcare workforce is trained in critical care, and mortality rates are unacceptably high in this patient population. OBJECTIVES: We sought to retrospectively describe the knowledge acquisition and confidence improvement of practitioners who attend the Fundamental Critical Care Support (FCCS) course in Rwanda. METHODS: We conducted a retrospective study in which we assessed survey data and multiple-choice question data that were collected before

Effect of Delay of Care for Patients with Craniomaxillofacial Trauma in Rwanda.

OBJECTIVES: Craniomaxillofacial (CMF) trauma represents a significant proportion of global surgical disease burden, disproportionally affecting low- and middle-income countries where care is often delayed. We investigated risk factors for delays to care for patients with CMF trauma presenting to the highest-volume trauma hospital in Rwanda and the impact on complication rates. STUDY DESIGN: This prospective cohort study comprised all patients with CMF trauma presenting to the University Teaching Hospital of Kigali, Rwanda, between June 1 and October 1, 2020. Challenges and opportunities of smallholder pig production systems in Rwanda.

To decipher the dynamics, challenges, and opportunities of smallholder pig farming in Rwanda, a household survey was conducted in ten districts of the country. A total of 900 households selected randomly across four provinces and one city, 12 districts, 28 sectors, and villages were interviewed using a pretested printed questionnaire. Most of the respondents were heads of household: 70.9% for western, 63.5% in eastern, 54.1% in northern, and 52.5% in southern province, and their mean age was 46.7 years and had widely varying education levels. Crossbred pig genotypes were the most preferred due

Controlling hepatitis C in Rwanda: a framework for a national response.

With the introduction of direct-acting antiviral drugs, treatment of hepatitis C is both highly effective and tolerable. Access to treatment for patients, however, remains limited in low- and middle-income countries due to the lack of supportive health infrastructure and the high cost of treatment. Poorer countries are being encouraged by international bodies to organize public health responses that would facilitate the roll-out of care and treatment on a national scale. Yet few countries have documented formal plans and policies. Here, we outline the approach taken in Rwanda to a public healt
