[Acute myocardial infarction in patients with ST-segment elevation myocardial infarction : ESC guidelines 2017].

This article gives an update on the management of acute ST-segment elevation myocardial infarction (STEMI) according to the recently released European Society of Cardiology guidelines 2017 and the modifications are compared to the previous STEMI guidelines from 2012. Primary percutaneous coronary intervention (PCI) remains the preferred reperfusion strategy. New guideline recommendations relate to the access site with a clear preference for the radial artery, use of drug-eluting stents over bare metal stents, complete revascularization during the index hospitalization, and avoidance of routine

ST-Elevation Myocardial Infarction from Spontaneous Coronary Artery Dissection.

Spontaneous coronary artery dissection (SCAD) is a rare but life-threatening condition which occurs due to non-traumatic separation of the coronary artery wall. It is more common in women, with an unclear, non-atherosclerotic mechanism. We report a unique case of spontaneous coronary artery dissection presenting as ST-elevation myocardial infarction (STEMI). A 54-year-old woman presented with fever and recurrent abscess. On presentation, she was tachycardic, tachypneic and hypoxic, requiring nasal cannula. Physical exam was notable for healing a wound on the right lower back, status post incis

Cocaine-Associated ST-Elevation Myocardial Infarction: Different Pathophysiological Mechanisms.

There are many underlying mechanisms for cocaine-associated myocardial infarction, and the culprit must be elucidated for appropriate therapeutic management. Optical coherence tomography provides unique insights when angiography alone has limited diagnostic value; it also aids in the decision between conservative management and revascularization strategy and guides coronary interventions.

Mon2 predicts poor outcome in ST-elevation myocardial infarction.

AIMS: There are limited data on the role of human monocyte subsets in ST-elevation myocardial infarction (STEMI). The study aimed to establish the relationship between monocyte subsets, their phagocytic and nuclear factor κB (NFκB) activity and outcomes in STEMI. METHODS: Monocyte subsets and their phagocytic activity and intracellular levels of inhibitory κB kinase β (IKKβ, marker of NFκB activity) were measured by flow cytometry in 245 patients with STEMI, median follow-up of 46 months. RESULTS: Mon2 (CD14++CD16+CCR2+) counts were independently predictive of major adverse cardiovascular ev

Rabies Encephalitis With Myocarditis Mimicking ST-Elevation Myocardial Infarction.

Rabies virus infection remains a significant public health threat particularly in developing countries without effective canine vaccination programs. After symptoms develop, there is no effective treatment and mortality approaches 100%. In countries such as the United States where human rabies remains rare, initial diagnosis is often delayed. In this study, we describe a case of rabies encephalitis presenting with concern for acute ST-elevation myocardial infarction (STEMI) based on electrocardiogram, laboratory, and exam findings and briefly review the known literature on cardiac involvement

Causes of Prehospital Misinterpretations of ST Elevation Myocardial Infarction.

OBJECTIVES: To determine the causes of software misinterpretation of ST elevation myocardial infarction (STEMI) compared to clinically identified STEMI to identify opportunities to improve prehospital STEMI identification. METHODS: We compared ECGs acquired from July 2011 through June 2012 using the LIFEPAK 15 on adult patients transported by the Los Angeles Fire Department. Cases included patients ≥18 years who received a prehospital ECG. Software interpretation of the ECG (STEMI or not) was compared with data in the regional EMS registry to classify the interpretation as true positive (TP),

Chameleons: Electrocardiogram Imitators of ST-Segment Elevation Myocardial Infarction.

The imperative for timely reperfusion therapy for patients presenting with ST-segment elevation myocardial infarction (STEMI) underscores the need for clinicians to have an understanding of how to distinguish patterns of STEMI from its imitators. These imitating diagnoses may confound an evaluation, potentially delaying necessary therapy. Although numerous diagnoses may mimic STEMI, several morphologic clues may allow the physician to determine if the pattern is concerning for either STEMI or a mimicking diagnosis. Furthermore, obtaining a satisfactory history, comparing previous electrocardio

ST-elevation myocardial infarction following systemic inflammatory response syndrome.

Systemic inflammatory response syndrome (SIRS) complicated with ST-elevation myocardial infarction has rarely been reported, and the precise mechanisms of myocardial injury remain unclear. Here, we present a case involving a 45-year-old man who developed SIRS secondary to diabetes-induced infection, and who ultimately developed ST-elevation myocardial infarction with acute heart failure, fulminant diabetes, acute liver dysfunction, acute kidney dysfunction and rhabdomyolysis. The patient eventually recovered due to early detection, correct diagnosis and powerful treatment. Clinicians should be

Recurrent ST Elevation Myocardial Infarction: What is the Aetiology?

Coronary artery disease is a leading cause of morbidity and mortality in the western world. Plaque rupture in an atherosclerotic lesion is the most commonly encountered underlying pathophysiology. Spontaneous coronary dissection can have similar presentation, but we as a community of cardiologists may not be aware of uncommon aetiologies, such as vasculitis presenting as ST elevation myocardial infarction (STEMI). Here we present a case report of a lady, who presented with STEMI on three occasions within five days, due to underlying granulomotosis with polyangiitis (GPA).

Acute ST elevation myocardial infarction during intravenous immunoglobulin infusion.

Intravenous immunoglobulin (IVIG) preparations are increasingly being used in the treatment of autoimmune disorders. This treatment is regarded as generally safe, and most of the adverse effects associated with IVIG administration are mild and transient. This paper reports a 72-year-old patient with known ischemic heart disease admitted for a Guillain-Barré syndrome variant, who developed acute ST elevation myocardial infarction (MI) during the first hours of IVIG infusion. The literature on acute MI during IVIG treatment is reviewed. Evaluation of each patient for cardiovascular risk prior to

Acute ST-elevation myocardial infarction secondary to paradoxical embolism.

Acute ST-elevation myocardial infarction (STEMI) is a life threatening condition usually caused by rupture of an atheromatous plaque in the coronary arteries. Other causes, such as septic emboli, cholesterol emboli and cocaine abuse have been reported. We report a case of acute STEMI in a young female without any vascular risk factors due to a presumptive paradoxical embolism via a secundum atrial septal defect (ASD). Young patients without vascular risk factors presenting with acute myocardial infarction should be investigated to exclude an underlying cause of their myocardial infarction, suc

[Modern treatment of acute myocardial infarction without ST elevation].

The term unstable coronary syndromes represents a continuum of patients with unstable chest pain with or without small or large acute myocardial infarctions. There is a tendency towards an epidemiological shift to fewer large infarctions with ST elevation in the ECG (STEMI) to increased numbers of small infarctions without ST elevation (nSTEMI). Patients with unstable angina or nSTEMI should start antithrombotic medication with aspirin, heparin and clopidogrel upon arrival in hospital. Patients with medium or high risk of death or cardiac events will benefit from therapy with IIb/IIIa glycopro

Acute ST-segment elevation myocardial infarction: to be or not to be?

Calcium has a significant effect on cells in the myocardium, affecting conduction, intracellular signaling and contraction of muscle fibers. In fact, calcium levels could affect myocardial action potential and affect heart conduction. Hypocalcemia manifest on ECG with arrythmias or QT prolongation, in some extraordinary cases, it could present with variations in the ST segment, which requires the study of differential diagnoses of cardiac origin, such as acute myocardial infarction with ST elevation (STEMI) or pericarditis. We expose a case of a 28-year-old male patient who arrived at the emer

Clinical outcomes of ST-elevation myocardial infarction patients who present special forms of ST-segment elevation.

BACKGROUND: There are several patterns of special ST-segment elevation morphology in patients with ST-elevation myocardial infarction (STEMI), but their effect on prognosis is unclear. We, therefore, investigated the mortality of patients with these patterns by comparing them to patients with typical ST-segment morphologies. METHODS: This observational study assessed 1277 consecutive patients with STEMI. Their initial electrocardiograms were analyzed quantitatively and divided into four categories: typical (n = 1138), Tombstone (n = 62), Triangular (n = 39), and Lambda patterns (n = 38). The

Trend of Galectin-3 Levels in Patients with Non-ST-Elevation and ST-Elevation Myocardial Infarction.

Background and Objectives: Given the fact that galectin-3 has a predictive significance on the development of myocardial dysfunction after acute myocardial infarction, the aim of our study was to examine potential factors that could be important for the dynamics of the concentration of this biomarker in the early postinfarction period. Materials and Methods: This study included 89 patients with a diagnosis of stable angina pectoris (SAP) or the first non-ST elevation (NSTEMI) or ST-elevation (STEMI) myocardial infarction, who underwent percutaneous coronary intervention (PCI). The study group

Triglyceride-rich Lipoprotein Cholesterol, Lipid Profiles, and Inhospital Outcomes in Acute Myocardial Infarction: A Comparison of ST-segment Elevation Myocardial Infarction and Non-ST-segment Elevation Myocardial Infarction Patients.

INTRODUCTION: Triglyceride-rich lipoprotein cholesterol (TRL-C) appears as an emerging atherosclerotic risk factor. However, the comparative evidence between ST-segment elevation myocardial infarction (STEMI) and non-STEMI (NSTEMI) is limited, especially in Indian population. The aim of the study is to assess the TRL-C level, lipid profile, and inhospitalization outcome. METHODS: This hospital-based cross-sectional study included 125 myocardial infarction (MI) patients at a tertiary care hospital in Pune. The lipid profile test was performed in fasting conditions of patients, and estimation o
